El objetivo de esta tesis está relacionado con tratar de minimizar el riesgo de longevidad mediante su adecuada modelización. Así, se va a desarrollar un modelo dinámico de mortalidad que está centrado en estudiar los cambios y/o variaciones que sufren las tasas de mortalidad de un año a otro. El modelo que aquí se presenta, asume que las variaciones que se producen en la curva de mortalidad están linealmente relacionadas con un número reducido de factores. Estos factores de riesgo se corresponden con una o varias tasas de mortalidad de una edad concreta, es decir, dicho factor o factores de riesgo se identifican con las variaciones de las tasas de mortalidad de lo que denominaremos “edades clave”. Una de las principales ventajas del modelo es que la tasa de mortalidad de la edad clave es totalmente observable y contrasta con el resto de los modelos alternativos que capturan la dinámica de la mortalidad a través de un parámetro que no es observable. Tras una primera propuesta de un nuevo modelo de mortalidad basado en la edad clave, se proponen varias mejoras a ese modelo, utilizar máxima verosimilitud en la estimación de los parámetros, estimar todos los parámetros en un único paso y reducir de manera significativa el número de parámetros a solo seis. También a modo de ejemplo, se ha calculado el V@R y CV@R, dos medidas desarrolladas para gestionar el riesgo de longevidad y que ponen de manifiesto la capacidad del modelo de cara a su utilización para la medición a largo plazo del riesgo de longevidad. Por último, se describe la utilidad y simplicidad de utilizar los métodos de remuestreo como herramienta para medir la capacidad predictiva de los modelos de mortalidad. Conviene señalar que representa una novedad la utilización de estos métodos para calibrar la capacidad predictiva de datos en forma de panel y en especial, para datos de mortalidad. Además, los resultados se van a mostrar mediante la utilización de los gráficos de radar, una herramienta muy interesante para resumir y ordenar la capacidad predictiva de diferentes modelos.

45 p., En este trabajo se desarrollará un modelo unifactorial para explicar la dinámica de las tasas de mortalidad y abordar el riesgo relacionado con ésta. El factor seleccionado para explicar el comportamiento de la curva de mortalidad será la tasa de mortalidad correspondiente a una edad clave, y, mediante un análisis empírico de las poblaciones masculina y femenina de Francia y España, se pondrá de manifiesto que este enfoque produce resultados, al menos tan buenos, como los logrados por otros modelos bastante más complejos. Este planteamiento, basado en la edad clave, presenta varias ventajas frente a otras alternativas de la literatura. En efecto, el factor de riesgo (la edad clave) es totalmente observable, y la metodología puede extenderse fácilmente mediante la incorporación de factores adicionales (más edades clave), el estudio del efecto cohorte, el análisis de causas específicas de mortalidad o la consideración de sólo algunos tramos específicos dentro de la curva de mortalidad. Por otro lado, debe señalarse que este modelo, basado en edades clave, se ha inspirado en estudios previos sobre la dinámica de la curva de tipos de interés, por lo que gran parte de las metodologías desarrolladas para tipos de interés serían fácilmente adaptables al estudio de problemas relacionados con el riesgo de longevidad., The paper deals with the mortality risk evolution and presents a one factor model explaining the dynamics of all of the mortality rates. The selected factor will be the mortality rate at the key age, and an empirical study involving males and females in France and Spain will reveal that the present approach is not outperformed by more complex factor models. The key age seems to reflect several advantages with respect to other factors available in the literature. Actually, it is totally observable, and the methodology may be easily extended so as to incorporate more factors (more key ages), a cohort effect, specific mortality causes or specific ages. Furthermore, the choice of a key age as an explanatory factor is inspired by former studies about the interest rates dynamics, which allows us to draw on the model in order to address some longevity risk linked problems. Indeed, one only has to slightly modify some interest rate linked methodologies.

El accidente cerebrovascular es una alteración brusca y repentina del flujo sanguíneo cerebral, con diferente clínica según la parte afectada. El incremento de los factores de riesgo cardiovascular, hace que aumente su incidencia. En el proceso agudo se desarrolla la mayoría de la actividad profesional, olvidando ciertos cuidados posteriores de gran importancia, por la variedad de secuelas que se producen tras el ictus, y que genera cambios en la vida de los pacientes. La metodología de formación “Entre Iguales” no está muy desarrollada en España, a pesar de la evidencia en su efectividad. Pretende potenciar el rol del paciente como principal responsable del cuidado de su salud, fomentar el autocuidado y mejorar su calidad de vida, empleando el apoyo “entre iguales”. Se propone elaborar un programa de educación grupal para fomentar conductas saludables que ayuden a afrontar a los pacientes su nueva vida tras un accidente cerebrovascular, basado en el método de paciente experto o formación entre iguales como herramienta de educación sanitaria. Se desarrolla un programa de educación para la salud siguiendo el método del “Paciente Experto” y elaboración de una guía de autocuidados para pacientes y familiares que complemente las sesiones de formación. El desarrollo del “Paciente Experto” en pacientes con ictus y sus familiares, fomentará un incremento en el autocuidado y por consiguiente en su calidad de vida, permitiendo que manejen su enfermedad y estado de salud., Grado en Enfermería

- Abstract
ObjectivesLow-molecular-weight heparins are widely used in clinical practice for the treatment or prophylaxis of venous thromboembolism (VTE). As these drugs are eliminated mainly by renal means, any renal function impairment may lead to higher plasma concentrations and increase the risk of bleeding. This study aims to evaluate whether in clinical practice there is an increase in the occurrence of bleeding in patients with renal insufficiency (RI) during treatment or prophylaxis with dalteparin, and to analyse the risk factors potentially influencing the appearance of such bleeding events.MethodsPatients were sampled from the Universitary Severo Ochoa Hospital, Leganés, Spain. This was a retrospective cohort study with a 1 year inclusion period, conducted at a Spanish university hospital with 400 beds, on patients undergoing treatment or prophylaxis for VTE with dalteparin for a minimum of 3 days. The main outcome measure was the number of patients who had bleeding events, independently of their severity, during dalteparin administration in patients with RI.Results367 patients were included in the study. Bleeding occurred in 17.9% of patients in the group with RI and in 7.3% of patients with normal renal function (NRF). Most haemorrhages in both cohorts were grade 2 on the WHO scale (64.7% in the RI group and 69.2% in the NRF group). Logistic regression analysis allowed the presence of RI (MDRD-4 (Modification of Diet in Renal Disease) <50 mL/min) to be identified as a risk factor.ConclusionPatients with RI treated with dalteparin face a higher risk of bleeding than those with NRF, which seems to make it necessary to monitor and seek new dosage adjustments for these patients.Impact on practice statements: This study yields new data on dalteparin in RI, which has not been widely studied before.

- Abstract
The aim of this study was to assess the effect of insemination timing on pregnancy rates in red deer (Cervus elaphus) when using sex-sorted sperm samples. Semen was collected by electroejaculation from 8 mature stags and processed to obtain: Conventional samples, following standard freezing procedures for commercial purposes; Control sorted samples, diluted and handled as per sorted samples but without being submitted to the sorter passage; and Y Sex Sorted (YSS) samples. Hinds were synchronized via intravaginal CIDR (Controlled Internal Drug Release) placement and given eCG (Folligon PMSG Serum Gonadotrophin) on day 12, upon CIDR removal. They were then inseminated with one of each sperm treatment, at the following post-eCG intervals: I_1, 55:01–55:30 h; I_2, 55:31–56:00 h; I_3, 56:01–56:30 h; or, I_4, 56:31–57:00 h. Pregnancy rates were assessed at parturition. Average pregnancy rates were highest (P < 0.05) for Conventional samples (77.6%), but similar between YSS (49.8%) and Control sorted (51.3%) samples. However, when insemination interval was taken into account, pregnancy rates within the YSS group, pregnancy rates were 80 and 83.1% for I_1 and I_2, respectively were obtained. Notably, these rates were similar (P > 0.05) to the average pregnancy rates obtained with Conventional samples (77.6%). As expected, YSS sperm yielded 94% male offspring contrasting with the 57% males obtained with Conventional and Control sorted samples. Our findings support the importance of developing specific insemination timing protocols to improve pregnancy rates when using frozen-thawed sex-sorted sperm. These findings provide the foundation for further investigations in order to determine why the YSS sperm are able to fertilize the oocyte in a shorter period of time than the conventional samples.

- Abstract
This study investigated the effect of sex-sorting and cryopreservation on post-thaw characteristics and fertility of red deer (Cervus elaphus) sperm for the first time. Semen was collected by electroejaculation from 10 mature stags during the breeding season, and each ejaculate split into four experimental groups: Bulk sorted spermatozoa, sorted but not sexed (BSS); sorted high purity X-spermatozoa (XSS); sorted high purity Y-spermatozoa (YSS); and, control non-sorted spermatozoa (NS). Following, all samples were frozen over liquid nitrogen. Two straws per stag and sample type were analyzed immediately post-thaw and following a 2-h incubation period at 37 °C. Post-thaw total motility (TM) as assessed by CASA was not different (P < 0.05) among NS, BSS and YSS sperm. For XSS, post-thaw TM was lower (39%, P < 0.05) than that for NS (54%) or BSS (50%), but similar (P > 0.05) to that of YSS (47%) sperm. The percentage of apoptotic spermatozoa as assessed by PI/YO-PRO-1 and flow cytometry analysis, was higher (17%, P ≤ 0.05) for XSS sperm than NS (12%), BSS (13%) and YSS (14%) sperm. Following incubation there were no differences (P > 0.05) in TM or percent apoptosis among treatments. Post-thaw chromatin stability calculated as the DNA fragmentation index (%DFI) was similar among treatments; following incubation %DFI increased in all except YSS, which displayed the lowest value (P < 0.05). Artificial insemination of synchronized hinds yielded 44, 52 and 62% delivery rates for YSS, NS and standard frozen-thawed sperm, respectively (P < 0.05). Notably, 93 and 55% of fawns born were males for the YSS and NS spermatozoa, respectively (P < 0.05). In summary, Y-sorted sperm displayed acceptable post-thaw sperm evaluation parameters and the expected offspring sex ratio. More studies are needed to understand the source of sperm damage that may compromise the fertility of Y-sorted red deer sperm.

- Abstract
The aims of this study were to assess the effects of the sex-sorting process on post-thaw sperm quality as well as on induced oxidative stress damage (HO 0 mm = H000; HO 50 mm = H050; HO 100 mm = H100) and the protective action of reduced glutathione (GSH) and Trolox, when comparing sorted (BSS) and non-sorted (NS) red deer spermatozoa incubated at 37°C. Sperm samples from three stags were collected by electroejaculation and frozen. Immediately after thawing, sperm motility was higher (p < 0.05) for NS (59% ± 3.3) than BSS (36.9% ± 5.8) sperm. Furthermore, the percentage of apoptotic sperm was higher (p < 0.05) for BSS (21.6% ± 5.0) than NS sperm (14.6% ± 1.2). The presence of HO increased DNA damage in NS (H000 = 4.1% ± 0.9; H050 = 9.3% ± 0.7; and H100 = 10.9% ± 2.3), but not in BSS sperm. However, in the presence of oxidant, GSH addition improved (p < 0.05) sperm motility in both groups of sperm samples as compared to their controls (NS: 44.5 ± 4.8 vs 21.1 ± 3.9 and BSS: 33.3 ± 8.1 vs 8.9 ± 1.8). These results demonstrate that the sperm-sorting process induces sublethal effects, albeit selecting a sperm population with a chromatin more resistant to oxidative stress than that in non-sorted sperm. Moreover, addition of GSH at 1 mm may be a good choice for maintaining the quality of stressed sperm samples, unlike Trolox, which inhibited sperm motility.

El presente trabajo analiza por qué educar ya es cosa de cuatro. Educar era cosa de tres. De los padres, de los educadores y/o maestros, y por supuesto de los niños/as. Pero ahora sabemos que muchos niños y niñas invierten un increíble número de horas a la semana viendo la TV o jugando a los videojuegos. Teniendo en cuenta que los programas que ven influyen claramente en los valores del niño/a, en su percepción del mundo y de la realidad social, tenemos que tener cuidado con el uso de este medio de comunicación. But now we know that a lot of children spend a huge number of hours watching TV or playing video games in front of a screen. Keeping in mind that the programs they watch have a clear influence on their values and on their perception of the world and of their social reality, parents and educators must be careful with the use of mass media. A fourth actor called television has appeared on the stage and it is necessary to take it into consideration as a potential ally, rather than as an enemy., El presente trabajo analiza por qué educar ya es cosa de cuatro. Mutations within the glucocorticoid receptor (GR) gene locus lead to glucocorticoid resistance which is characterized by several clinical symptoms such as adrenal gland hyperplasia and salt-sensitive hypertension, although the underlying mechanisms are still unknown. We studied GR haploinsufficient (GR
+/− ) Sprague Dawley rats which, on a standard diet, showed significantly increased plasma aldosterone and corticosterone levels and an adrenocortex hyperplasia accompanied by a normal systolic blood pressure. Following a high salt diet, these rats developed salt-sensitive hypertension and maintained elevated enzyme-soluble epoxide hydrolase (sEH) in adrenal glands, while sEH was significantly decreased in wild-type rats. Furthermore, GR+/− rats showed dysregulation of the equilibrated linoleic and arachidonic acid pathways, with a significant increase of less active metabolites such as 8,9-DiHETrE. In Sprague Dawley rats, GR haploinsufficiency induced steroid disturbances, which provoked hypertension only in combination with high salt intake, which was accompanied by disturbances in sEH and fatty acid metabolism. Our results suggest that sEH inhibition could be a potential target to treat hypertension in patients with GR haploinsufficiency. [ABSTRACT FROM AUTHOR]- Published

Currently, sperm reproductive biotechnologies such as sex sorting and cryopreservation are undoubtedly valuable tools for improving the economic and biological efficiency of red deer production systems. In this context, and because of the particular characteristics of this species (extensive exploitation typically far from laboratory facilities), a key goal is to optimize the design of an adequate handling protocol of sperm samples before samples are subjected to sex sorting and cryopreservation procedures to obtain better outputs from the application of these technologies. The main aim of this paper was to design an adequate protocol for Iberian red deer sperm handling before sex sorting by flow cytometry to obtain optimal yields when sex sorting is used in this species. Semen samples from 11 adult males were obtained by electroejaculation during the breeding season. In this study, we tested different protocols for the handling of Iberian red deer spermatozoa before sorting by using different concentrations of sperm (400 or 800 × 10 6 ) and adding or not Hoechst 33342 before the transport of samples to the sorting facilities. Based on the results, the most adequate method used to handle samples before sorting was transportation at a high sperm concentration (800 × 10 6 /mL) without Hoechst 33342. These transportation conditions in combination with Hoechst 33342 staining at 5.2 μL/mL once at the flow cytometry laboratory resulted in better (P < 0.05) sorting efficiency (99.9% of the samples showing split) than both, those samples transported at 400 × 10 6 sperm/mL (between 51.2 and 55.2% of the samples showing split) and those samples stained before transport at a sperm concentration of 400 × 10 6 sperm/mL (between 15.4 and 75.7% of the samples showing split). Sorting rates and sperm quality after sorting and cryopreservation was not affected (P > 0.05) by sperm handling before sorting. El objeto del presente trabajo es llevar a cabo un análisis sobre la capacidad predictiva de distintos modelos de mortalidad en el tramo de edades más interesante para las empresas aseguradoras, entre 65 y 99 años. Para ello, se pone de manifiesto la utilidad de los métodos de remuestreo como herramienta para medir dicha capacidad predictiva desde diferentes puntos de vista de los modelos de mortalidad. Los métodos de remuestreo no se han utilizado mucho en la literatura actuarial para testar la capacidad predictiva, sino que se han utilizado principalmente para construir intervalos de confianza gracias al Bootstrap. Conviene destacar la novedad de utilizar esta herramienta con datos en forma de panel, lo que requiere una adaptación de los modelos. Se hará un recorrido desde la teoría de los modelos y métodos hasta la aplicación práctica de ellos en el caso de España, tanto para hombres y mujeres, teniendo en cuenta la inclusión del efecto Covid., The object of the present work is to accomplish an analysis of the predictive capacity of different mortality models in the most interesting age bracket for insurance companies, between 65 and 99 years of age. For this purpose, the usefulness of resampling methods as a tool to measure this predictive capacity from different points of view of mortality models are shown. Resampling methods have not been widely used in the actuarial literature to test predictive ability but have mainly been used to construct confidence intervals thanks to the Bootstrap. It is worth noting the novelty of using this tool with panel data, which requires an adaptation of the models. El sector de los seguros está evolucionando en gran cantidad de aspectos, siendo uno de ellos la posibilidad de ofrecer a los clientes productos personalizados. En este contexto, resulta de interés para los seguros de vida el estudio de tablas de mortalidad para las distintas causas de muerte. Esta información permite realizar una oferta concreta donde el precio del riesgo a cubrir se puede cuantificar de forma más precisa a las necesidades de nuestra cartera. Para la estimación de las diversas tablas de mortalidad que serán utilizadas en este estudio, los datos de defunciones y censo de población han sido extraídos del Instituto Nacional de Estadística para el periodo 2020 y para ambos sexos. En base a las estimaciones realizadas a partir de estos datos, se ha llevado a cabo un análisis de las tablas de mortalidad elaboradas para cada causa de muerte. Además, se ha realizado una comparación entre las diferentes causas de muerte y las tablas que exige la dirección general de seguros en España, tabla de segundo orden general PASEM 2020, a través de diferentes indicadores de mortalidad y mediante la estimación de varios seguros de supervivencia., The insurance industry is changing in many aspects, and one of those is the possibility to offer customized products to clients. In this context, the study of mortality tables for different causes of death is relevant to life insurance. This information enables a precise offer to be made where the price of the risk to be covered can be accurately quantified for our client portfolio. In fact, to estimate the diverse mortality tables that will be used in this study, the mortality data; deaths and population census have been extracted from the National Institute of Statistics. Based on the estimates made using these data, it has been analyzed the mortality tables developed for each cause of death.
