In dieser Arbeit wurden die prozeduralen Charakteristika und intraprozeduralen Prädiktoren für eine Terminierung in eine Atriale Tachykardie unter Anwendung der „modifizierten stepwise approach“-Ablation bei insgesamt 436 Patienten mit persistierendem Vorhofflimmern untersucht. Es konnte eine statistische Signifikanz für die Prädiktoren CL nach PVI und CL nach CFAE gezeigt werden., In this work, the procedural characteristics and intraprocedural predictors of termination into atrial tachycardia using the "modified stepwise approach"-ablation were investigated in a total of 436 patients with persistent AF. In dieser Studie wurde ausgehend von der Hypothese, dass struktureller Umbau in den Vorhöfen zur Aufrechterhaltung von Vorhofflimmern beiträgt, bei den Patienten invasiv mit Hilfe des 3-D-Mapping System NaVX der Fibroseanteil des linken Atriums bestimmt, um diesen mit möglichen Fibrosemarkern (RAGE,ST2 und HGMB1) und anderen Risikofaktoren zu korrelieren. Es ergab sich kein Hinweis auf eine Korrelation zwischen FIbroseanteilen und den Biomarkern HMGB1, RAGE und ST2., In this study we hypothesize, that there are biomarkers that can be used as non-invasive predictors for the success rate of catheter ablation. The biomarkers HMGB1, RAGE and ST2 are fibrosis- biomarkers. We correlated them with low-voltage areas detected by 3-Dimensional-Mapping and other cardiovascular risk factors. In this study we found no correlation between HMGB1, RAGE and ST2 and the amount of low-voltage areas/fibrosis in the left atrium.

Das Vorhofflimmern (VHF) gilt als häufigste symptomatische Herzrhythmusstörung und stellt in der heutigen Welt eine ernstzunehmende Volkskrankheit dar. Besonders anfällig für die Entwicklung von VHF sind Patienten mit angeborenem Herzfehler (AHF). Obwohl durch den großen chirurgischen Fortschritt heutzutage vermehrt das Erwachsenenalter erreicht wird, stellt VHF vor allem bei dieser Patientengruppe eine bedeutende und nicht zu unterschätzende Herausforderung im klinischen Alltag dar. Die Katheterablation, welche heutzutage als Therapieoption kaum mehr wegzudenken ist und in der Normalbevölkerung schon lange als effiziente und sichere Methode eingesetzt wird, scheint auch für Erwachsene mit angeborenem Herzfehler (EMAH) eine vielversprechende Methode zu sein. Die vorliegende Arbeit untersucht die Sicherheit, Durchführbarkeit und Langzeiterfolgsraten der Katheterablation von VHF bei Erwachsenen mit angeborenem Herzfehler unter ununterbrochener oraler Antikoagulation., Atrial fibrillation (AF) is the most common symptomatic cardiac arrhythmia and is a widespread disease, which has to be taken seriously in today's world. Patients with congenital heart disease (CHD) are particularly susceptible to developing AF. As adults with CHD now increasingly reach adulthood due to the great surgical progress, AF is becoming a significant and not to be underestimated challenge in everyday clinical practice, especially for this group of patients.Catheter ablation, which is now an indispensable therapeutic approach and has long been used as an efficient and safe method in the general population, also seems to be a promising method for adults with CHD. The present work examines the safety, feasibility and long-term success rates of catheter ablation for AF in adults with CHD on uninterrupted oral anticoagulation.

Nach Ablation von persistierendem Vorhofflimmern kann eine Herzschrittmachtherapie notwendig werden. 40 (3.2 %) Patienten unseres 1234 Personen umfassenden Patientenkollektivs benötigten eine akute Schrittmachertherapie innerhalb einer Woche nach der Ablationsprozedur. Die Katheterablation stellt eine effektive Therapie für Patienten mit Vorhofflimmern dar. Die aktuelle Leitlinie empfiehlt die orale Antikoagulation auch nach erfolgreicher Ablation fortzuführen. Aktuell sind wenige Daten zur Inzidenz von zerebralen thromboembolischen Ereignissen sowie Blutungskomplikationen nach Ablation verfügbar. In dieser Studie zeigt das Beenden der oralen Antikoagulation nach erfolgreicher Ablation keine erhöhte Inzidenz von zerebralen thromboembolischen Ereignissen, jedoch signifikant mehr Blutungskomplikationen unter fortgeführter oralen Antikoagulation., Catheter ablation is an effective therapy of atrial fibrillation these days. Although little data is known on the incidence of cerebral thromboembolic events and hemorrhagic complication, current guidelines suggest continuation of oral anticoagulation after successful ablation. In this study the discontinuation of oral anticoagulation did not show a higher incidence of cerebral thromboembolic events. Hemorrhagic events were significantly more prevalent during continued oral anticoagulation.

In der vorliegenden Arbeit wurde der Einfluss von Narben- und Substratarealen auf das Ergebnis der Ablation bei persistierendem Vorhofflimmern (VHF) untersucht. VHF stellt mit weltweit 33,5 Millionen Betroffenen die häufigste Herzrhythmusstörung bei Erwachsenen dar (Go, Hylek et al. 2001, Chugh, Havmoeller et al. 2014, Calkins, Hindricks et al. 2017). Die Assoziation mit relevanter Morbidität, Mortalität und nicht zuletzt die Auswirkung auf Kosten im Gesundheitssystem führen zu steigendem Interesse an effektiven Therapieoptionen. Die Katheterablation ist bei medikamentös therapierefraktärem, symptomatischem VHF die beste aktuell verfügbare Form der Rhythmuskontrolle (Parkash, Tang et al. 2011). Bei paroxysmalem VHF können durch Ablation überzeugende Ergebnisse erreicht werden, während bei persistierendem VHF weiterhin nach effektiven Methoden der Ablation geforscht wird (Oral, Knight et al. 2002, Hocini, Sanders et al. 2004, O'Neill, Jais et al. 2006, Cappato, Calkins et al. 2010). Bereits 1995 konnte in Studien gezeigt werden, dass chronische oder wiederkehrende fibrillatorische Aktivität in den Vorhöfen progressives und strukturelles Remodelling induziert (Morillo, Klein et al. 1995, Wijffels, Kirchhof et al. 1995). Durch den Umbau des Vorhofmyokards entstehen unter anderem Substratareale, welche VHF aufrechterhalten können, beispielsweise CFAE-Areale (Nademanee, McKenzie et al. 2004). Schlussendlich resultiert der Umbauprozess in einem Verlust von Kardiomyozyten und damit einem erhöhten Kollagengehalt im Sinne einer Fibrose des Vorhofmyokards (Allessie, Ausma et al. 2002). Welchen Einfluss die spezifischen Veränderungen im Vorhofmyokard auf den Ablationserfolg haben ist nicht abschließend geklärt. In die Studie eingeschlossen wurden 70 Patienten, welche am Deutschen Herzzentrum München einer Erstablation bei persistierendem oder lang-anhaltendem VHF unterzogen wurden. With paroxysmal AF, convincing results can be achieved through ablation, while with persistent AF, research is still being conducted to identify effective methods of ablation (Oral, Knight et al. 2002, Hocini, Sanders et al. 2004, O'Neill, Jais et al. 2006, Cappato, Calkins et al. 2010). Studies as early as 1995 were able to show the induction of progressive and structural remodeling through chronic or recurring fibrillatory activity in the atria (Morillo, Klein et al. 1995, Wijffels, Kirchhof et al. 1995). The remodeling of the atrial myocardium creates substrate areas, for example CFAE areas, which can maintain AF (Nademanee, McKenzie et al. 2004). Ultimately, the remodeling process results in a loss of cardiomyocytes and thus an increased collagen content in the sense of fibrosis of the atrial myocardium (Allessie, Ausma et al. 2002). The impact of specific changes in the atrial myocardium on the success of the ablation remains indistinct. This study included 70 patients who underwent an initial ablation at the German Heart Center in Munich with persistent or long-term AF. Stepwise ablation (modified approach) according to electroanatomical mapping was performed in all patients. Electrical activity in the atrium was displayed using the Ensite-NavX® Viewer program.
