Introduction: Cancer patients are exposed to several types of treatments, including chemotherapy. In this context, patients experience several nursing diagnoses, including spiritual distress. The definition of the diagnosis of spiritual distress is grounded in lack of meaning and purpose in life, a sense of suffering, and a feeling of disconnected. Purpose: The aim of this study was to determine the prevalence, sensitivity, specificity, and predictors of the nursing diagnosis of spiritual distress of cancer patients undergoing chemotherapy. Designs The study used a longitudinal questionnaire design with quarterly data collection points over a 12‐month period. Participants were recruited through random sampling, in an outpatients' setting in one oncology day unit in Portugal. Findings The highest prevalence of spiritual distress was found at 3 months after patients started chemotherapy. The highest value of specificity was lack of meaning in life and express suffering, and the highest values of sensitivity concerned spiritual distress diagnosis. The predictors of spiritual distress were express suffering, alienation, questioning meaning in life, lack of serenity, questioning the meaning of suffering, hopelessness, and lack of meaning in life. Conclusions: Spiritual distress is a human response that is current in patients undergoing chemotherapy, and the highest prevalence seems to occur at 3 months after commencing chemotherapy. Express suffering and lack of meaning in life play the role not only of defining characteristics (DC) in this study, but also of predictors in the diagnosis of spiritual distress. Clinical Relevance: The identification of the prevalence, predictors, sensitivity, and specificity of the DC of the nursing diagnosis of spiritual distress in cancer patients undergoing chemotherapy may facilitate nurses' clinical reasoning and improve the planning of nursing care in clinical practice in order to improve spiritual well‐being in cancer patients. Purpose: To analyze the accuracy of the defining characteristics of the nursing diagnosis of sexual dysfunction in women undergoing pharmacological treatment for breast cancer. Methods: A cross‐sectional accuracy study with statistical analysis. Findings: Alteration in sexual satisfaction was the defining characteristic with the highest sensitivity value, while perceived sexual limitation was the defining characteristic with the highest value for both sensitivity and specificity. Conclusions: Perceived sexual limitation is a good clinical indicator to infer initial stages of sexual dysfunction and to confirm its presence. Implications For Nursing Practice: Accurate diagnostics allow devising an action plan directed to the patients' real needs, helping prescribe and deliver the appropriate nursing interventions. - Abstract
Aim: The aim of the study was to validate defining characteristics (according to NANDA international) of the nursing diagnoses “acute pain” (00123) and “chronic pain” (00133) and their meaning as well as the mostly subjective feelings for the patients with different wound and pain. Material and methods: The data were acquired from outpatient clinics for wound healing and from inpatient wards from various localities of the Czech Republic (n = 180 patients). The study fol lowed „Patient focus CDV model“. Results: The most frequent defining characteristics of “acute pain diagnose” was „looking for pain relieving position” (weighted score 0.93), and of “chronic pain diagnose” „fear of appearance of another wound” (weighted score 0.74). We have verified correlation between the age of the respondents and intensity of pain and the numeric value of the average defining characteristic score (p ≥ 0.05). Conclusion: Weighted score values above 0.8 were found in patients with a pain intensity ranging from 7- 10 points - severe pain (10-degree scale). Under these conditions, patients selected as the most important defining characteristics: „looking for pain relief position, protective behaviour, observed signs of pain and frequent pain“. Defining characteristics of the nursing diagnosis „chronic pain“ were lower in the weighted score but highlighting features typical of long-term is sues such as non-healing wounds: „fear of another wound formation, limited mobility at the site of pain, observed protective behaviour and persistent pain”. Aim: The aim of the study consisted in content validation of the defining characteristics, and related factors of the nursing diagnosis of Deficient fluid volume [Dehydration] (00027), from the perspective of neonatal nurses. Design: Content validation study. Methods: The defining characteristics of the nursing diagnosis of Deficient fluid volume [Dehydration] (00027) were evaluated with the help of the Fehring Diagnostic Content Validity Model. The set consisted of 125 experts/nurses. The significance of the defining characteristics was rated by the nurses on a Likert scale from 1 to 5. Data collection was carried out from March to September 2016. Results: seven characteristics from 32 were designated primary defining characteristics by the experts: decreased urine output, sunken fontanelle, change in skin turgor, dry mucous membranes, increased urine concentration, elevated haematocrit, and general appearance of exhaustion. 25 characteristics were designated secondary defining characteristics; 12 of which were characteristics from NANDA–I. Items such as: constipation, decreased pulse volume, cramps, absence of tears, and abdominal distension achieved a weighted score under 0.50. All from the group of related factors were defined as secondary by the experts. Conclusion: The set of experts regarded the primary significant characteristics for the determination of the nursing diagnosis of Deficient fluid volume [Dehydration] in newborns to consist in seven defining characteristics. All related factors were regarded to be secondary. [ABSTRACT FROM AUTHOR]

Purpose The main purpose of the study was to validate the defining characteristics and related factors of the nursing diagnosis 'chronic pain' in Slovakia and the Czech Republic. Methods This is a descriptive study. The validation process involved was based on Fehring's Diagnostic Content Validity Model. Findings Three defining characteristics (reports pain, altered ability to continue previous activities, and depression) were classified as major by Slovak nurses, and one defining characteristic (reports pain) was classified as major by Czech nurses. Conclusion The results of the study provide guidance in devising strategies of pain assessment and can aid in the formulation of accurate nursing diagnoses. Implications for Nursing Practice The defining characteristic 'reports pain' is important for arriving at the nursing diagnosis 'chronic pain.' [ABSTRACT FROM AUTHOR]

The idea that the cohomology of finite groups might be fruitfully approached via the cohomology of ambient semisimple algebraic groups was first shown to be viable in the papers [E. Cline, B. Parshall, and L. Scott, Cohomology of finite groups of Lie type, I, Publ. Math. Inst. Hautes Études Sci. 45 (1975), 169–191] and [E. Cline, B. Parshall, L. Scott and W. van der Kallen, Rational and generic cohomology, Invent. Math. 39 (1977), 143–163]. The second paper introduced, through a limiting process, the notion of generic cohomology, as an intermediary between finite Chevalley group and algebraic group cohomology. The present paper shows that, for irreducible modules as coefficients, the limits can be eliminated in all but finitely many cases. These exceptional cases depend only on the root system and cohomological degree. In fact, we show that, for sufficiently large $r$, depending only on the root system and $m$, and not on the prime $p$ or the irreducible module $L$, there are isomorphisms ${\mathrm{H} }^{m} (G({p}^{r} ), L)\cong {\mathrm{H} }^{m} (G({p}^{r} ), {L}^{\prime } )\cong { \mathrm{H} }_{\mathrm{gen} }^{m} (G, {L}^{\prime } )\cong {\mathrm{H} }^{m} (G, {L}^{\prime } )$, where the subscript ‘gen’ refers to generic cohomology and ${L}^{\prime } $ is a constructibly determined irreducible ‘shift’ of the (arbitrary) irreducible module $L$ for the finite Chevalley group $G({p}^{r} )$. By a famous theorem of Steinberg, both $L$ and ${L}^{\prime } $ extend to irreducible modules for the ambient algebraic group $G$ with ${p}^{r} $-restricted highest weights. This leads to the notion of a module or weight being ‘shifted $m$-generic’, and thus to the title of this paper. Our approach is based on questions raised by the third author in [D. I. Stewart, The second cohomology of simple ${\mathrm{SL} }_{3} $-modules, Comm. Algebra 40 (2012), 4702–4716], which we answer here in the cohomology cases. We obtain many additional results, often with formulations in the more general context of ${ \mathrm{Ext} }_{G({p}^{r} )}^{m} $ with irreducible coefficients. [ABSTRACT FROM AUTHOR]

Abstract: This paper is a contribution to the general program introduced by Isaacs, Malle and Navarro to prove the McKay conjecture in the representation theory of finite groups. We develop new methods for dealing with simple groups of Lie type in the defining characteristic case. Using a general argument based on the representation theory of connected reductive groups with disconnected center, we show that the inductive McKay condition holds if the Schur multiplier of the simple group has order 2. As a consequence, the simple groups and are “good” for and the simple groups are “good” for in the sense of Isaacs, Malle and Navarro. We also describe the action of the diagonal and field automorphisms on the semisimple and the regular characters. [Copyright &y& Elsevier]

14. Interpreting definitions: the problem of interpreting definitions of medical concepts.

- Abstract
During the last hundred years medical language has been influenced by scientific and technological progress. As a result uncertainty in medical communication is increasing. This may have serious consequences not only for health care delivery but also for medical science. Disease classification, assessment of the validity of epidemiological investigations and comparison of the results of various investigations are examples of what will become less secure. The purpose of the article is to emphasise a main source of uncertainty--the problem of interpreting definitions. Two issues of interpretation are explored. One concerns the logical status of a statement and the other the function of a statement. It is concluded that the security of medical language can be increased through the elucidation of the logical status and functions of statements making up medical concepts. [ABSTRACT FROM AUTHOR]

Objective: The objective of the present study was validation of the nursing diagnosis Chronic Pain 00133 in women with gynaecological cancers. Methods: To validate the nursing diagnosis, we used the Patient-Focused Clinical Diagnostic Validity (CDV) model. We developed a questionnaire-type measuring instrument containing six defining characteristics of the nursing diagnosis Chronic Pain 00133. We discarded three defining characteristics because they were intended for an objective assessment by nurses. The sample consisted of 40 patients hospitalised at the Faculty Hospital in Nitra, the Department of Radiology and Clinical Oncology. The patients rated the defining characteristics on the Likert-type scale from 1 (Not at all characteristic of me) to 5 (Very characteristic of me). Results: The patients rated five defining characteristics as minor: Alteration in ability to continue previous activities (WS 0.66); Alteration in sleep pattern (WS 0.57); Self-focused (WS 0.72); Self-report of pain characteristics using standardized pain instrument (Brief Pain Inventory) (WS 0.7); Self-report of intensity using standardized pain scale (visual analogue scale, and numeric rating scale) (WS 0.7). They discarded one defining characteristic – Anorexia (WS 0.48). Conclusion: The study results imply the need for a quality comprehensive evaluation of chronic pain. The results also suggest the use of pain assessment instruments in clinical practice. Výsledky; Pacientky zhodnotili päť určujúcich znakov ako vedľajších: Zmenená schopnosť pokračovať v predchádzajúcich aktivitách (VS 0,66); Zmenený vzorec spánku (VS 0,57); Zameranosť na seba (VS 0,72); Vlastné hlásenie o príznakoch bolesti pomocou štandardizovaného nástroja (krátky inventár bolesti – Brief Pain Inventory) (VS 0,7); Vlastné hlásenie o intenzite bolesti pomocou štandardizovaných škál bolesti (vizuálna analógová škála a numerická škála) (VS 0,7) a jeden určujúci znak vyradili – Anorexia (VS 0,48). Záver; Z výsledkov výskumu vyplýva potreba kvalitného komplexného hodnotenia chronickej bolesti. Výsledky tak isto poukazujú na využívanie nástrojov na hodnotenie bolesti v klinickej praxi.

Purpose: To document the prevalence of the nursing diagnosis of anxiety in hospital patients, based on its level of severity, defining characteristics (DCs), and other related factors, and to identify the key DCs that serve as predictors of clinically significant anxiety (CSA). Design: Cross-sectional study. We enrolled 116 consecutive adult patients hospitalized from October 10 to 16, 2016, in medical-surgical wards within the first 48 hr of admission. Methods: The potential DCs and related factors of anxiety were collected based on the NANDA International terminology. Anxiety was considered clinically significant when presenting at moderate, severe, or panic level. The differences in DC prevalence among patients having or not having CSA were analyzed by unpaired student's t-test. Multivariate analysis was used to examine the independent association between the DCs and CSA. Findings: The prevalence of CSA was 36.2% and was significantly higher in patients who were older, female, and taking anxiolytic drugs, and among those who had cancer. The most frequent related factor for CSA was major change in health status. In the logistic regression, the presence of the DCs helplessness, altered attention or concentration, and anguish independently increased the odds of having CSA, whereas subjects presenting with Diminished ability to problem-solve had about a 96% reduction in the likelihood to suffer from such a condition. Conclusions: A high prevalence of CSA among medical-surgical patients was shown. Furthermore, a critical cluster of DCs useful to identify CSA was found. Clinical Relevance: The ability to accurately diagnose CSA should help prescribe and deliver the appropriate nursing interventions.

The decomposition problem for finite groups asks, for a given prime p dividing the order of the group, how can the restrictions of the ordinary characters to p-regular classes be written in terms of irreducible Brauer characters? We follow the development of L. Chastkofsky and J. E. Humphreys to construct the Brauer Complex of Sp(4,q) and Sp(6,q). The complex is an open simplicial complex derived from the alcove geometry of the associated affine Weyl group together with Deligne-Lusztig character labels. The Deligne-Lusztig characters of Sp(4,q), odd q, were computed by B. Srinivasan and of Sp(6,q), even q, by F. Luebeck. As a consequence we gain information about the decomposition matrices for these groups in defining characteristic.
