Abstract

Imaging the brain vasculature can be critical for cerebral perfusion monitoring in the context of neurocritical care. Although ultrasensitive Doppler (UD) can provide good sensitivity to cerebral blood volume (CBV) in a large field of view, it remains difficult to perform through the skull. In this work, we investigate how a minimally invasive burr hole, performed for intracranial pressure (ICP) monitoring, could be used to map the entire brain vascular tree. We explored the use of a small motorized phased array probe with a non-implantable preclinical prototype in pigs. The scan duration (18 min) and coverage (62 $\pm$ 12 % of the brain) obtained allowed global CBV variations detection (relative in brain Dopplerdecrease =-3[-4-+16]% \& Dopplerincrease. = +1[-3-+15]%, n = 6 \& 5) and stroke detection (relative in core Dopplerstroke. =-25%, n = 1). This technology could one day be miniaturized to be implanted for brain perfusion monitoring in neurocritical care.

6. The SVD Beamformer: Physical Principles and Application to Ultrafast Adaptive Ultrasound

Abstract

A shift of paradigm is currently underway in biomedical ultrasound thanks to plane and diverging waves for ultrafast imaging. One remaining challenge consists in the correction of phase and amplitude aberrations induced during propagation through complex layers. Unlike conventional line-per-line imaging, ultrafast ultrasound provides for each transmission, backscattering information from the whole imaged area. Here, we take benefit from this feature and propose an efficient approach to perform fast aberration correction based on the Singular Value Decomposition of an ultrafast compound matrix built from backscattered data for several plane wave transmissions. First, we explain the physical signification of SVD and associated singular vectors within the ultrafast matrix formalism. We theoretically demonstrate that the spatial and angular variables separation, rendered by SVD on ultrafast data, provides an elegant and straightforward way to optimize angular coherence of backscattered data. In heterogeneous media with an aberrating phase screen approximation, we demonstrate that the first spatial and angular singular vectors retrieve on one side the non-aberrated image, and on the other, the phase and amplitude of the aberration law. In vitro results prove the efficiency of the image correction, but also the accuracy of the aberrator determination. Based on spatial and angular coherence, we introduce a complete methodology for adaptive beamforming of ultrafast data, performed on successive isoplanatism patches undergoing SVD beamforming. The simplicity of this method paves the way to real-time adaptive ultrafast ultrasound imaging and provides a theoretical framework for future quantitative ultrasound applications.

Abstract

The quest to decode the complex supraspinal mechanisms that integrate cutaneous thermal information in the central system is still ongoing. The dorsal horn of the spinal cord is the first hub that encodes thermal input which is then transmitted to brain regions via the spinothalamic and thalamocortical pathways. So far, our knowledge about the strength of the interplay between the brain regions during thermal processing is limited. To address this question, we imaged the brains of adult awake male mice in resting state using functional ultrasound imaging during plantar exposure to constant and varying temperatures. Our study reveals for the first time the following: (1) a dichotomy in the response of the somatomotor-cingulate cortices and the hypothalamus, which was never described before, due to the lack of appropriate tools to study such regions with both good spatial and temporal resolutions. (2) We infer that cingulate areas may be involved in the affective responses to temperature changes. (3) Colder temperatures (ramped down) reinforce the disconnection between the somatomotor-cingulate and hypothalamus networks. (4) Finally, we also confirm the existence in the mouse brain of a brain mode characterized by low cognitive strength present more frequently at resting neutral temperature. Abstract

Acute spinal cord injury (SCI) leads to severe damage to the microvascular network. The process of spontaneous repair is accompanied by formation of new blood vessels; their functionality, however, presumably very important for functional recovery, has never been clearly established, as most studies so far used fixed tissues. Here, combining ultrafast Doppler imaging and ultrasound localization microscopy (ULM) on the same animals, we proceeded at a detailed analysis of structural and functional vascular alterations associated with the establishment of chronic SCI, both at macroscopic and microscopic scales. Using a standardized animal model of SCI, our results demonstrate striking hemodynamic alterations in several subparts of the spinal cord: a reduced blood velocity in the lesion site, and an asymmetrical hypoperfusion caudal but not rostral to the lesion. In addition, the worsening of many evaluated parameters at later time points suggests that the neoformed vascular network is not yet fully operational, and reveals ULM as an efficient in vivo readout for spinal cord vascular alterations. Finally, we show statistical correlations between the diverse biomarkers of vascular dysfunction and SCI severity. The imaging modality developed here will allow evaluating recovery of vascular function over time in pre-clinical models of SCI. Also, used on SCI patients in combination with other quantitative markers of neural tissue damage, it may help classifying lesion severity and predict possible treatment outcomes in patients.
