Reproductive tract infections (RTIs) such as vaginal candidiasis and bacterial vaginosis (BV) are common among sexually active women and can be both symptomatic or asymptomatic. The microbiota of the reproductive tract triggers immune response at the cervicovaginal interface resulting in secretion of cytokines during the course of these RTIs. The objective of this study was to evaluate the cytokine profile in cervicovaginal lavage of women having asymptomatic vaginal infections. Measurement of vaginal cytokines was done for various interleukins including IL-1β, IL-6, IL-8, IL-10, IL-12/IL23p40, IL-17A, tumour necrosis factor alpha (TNF-α) and interferon gamma (IFN-γ) by ProcartaPlex™ Multiplex Immunoassay. Non-communicable diseases (NCDs) and cancers of breast, oral cavity and cervix contribute to around 5.87 million (60%) deaths in India. Despite this, there is limited evidence on preparedness of the tribal health system in mitigating these conditions. This mixed-methods study aimed at identifying enablers and challenges using a multistakeholder approach for the screening of NCDs and common cancers in a tribal block of Maharashtra, India.This study was conducted in a tribal community of Dahanu taluka in Palghar district of Maharashtra. A total of nine focus group discussions (FGDs) among tribal women and accredited social health activists (ASHAs), 13 key informant interviews (KIIs) among auxiliary nurse midwives (ANMs) and community health officers (CHO) and facility surveys of five public health facilities were conducted. The FGDs and KIIs were conducted using guides, recorded digitally, transcribed, analyzed and triangulated to identify emerging themes.The tribal women had limited knowledge about NCDs and common cancers. Paucity of health facilities, out-of-pocket expenditure, misconceptions, belief on traditional healers and inability to prioritize health were identified as major challenges. The ASHAs were recognized as a key connecting link between health system and community while provision of culturally appropriate IEC materials and adequate training were recognized as critical enablers by healthcare providers in implementing screening for NCDs and common cancers.The study recommends incorporating socioculturally relevant strategies in the tribal population and strengthening health facilities in terms of infrastructure and training with involvement of ASHAs for successful implementation of the National Programme for Prevention and Control of Cancer, Diabetes, Cardiovascular Diseases and Stroke (NPCDCS) through health and wellness centres.

BACKGROUND The COVID-19 pandemic and the associated social restrictions may have disrupted the provision of essential services, including family planning (FP) and contraceptive services. This protocol is adapted from a generic study protocol titled “Health systems analysis and evaluations of the barriers to availability and readiness of sexual and reproductive health services in COVID-19 affected areas,” conducted by the World Health Organization (WHO) Department of Reproductive Health and Research. OBJECTIVE This study aims to assess the availability and use of FP and contraceptive services in primary health facilities during and after the COVID-19 pandemic; assess the risk perceptions of COVID-19 stigma, barriers to access, and quality of services from clients’ and providers’ perspectives in the COVID-19–affected areas; and assess the postpandemic recovery of the facilities in the provision of FP and contraceptive services. METHODS In-depth interviews will be conducted with clients—women in the reproductive age group and their male partners who visit the selected health facilities for FP and contraceptive services—and health providers (the most knowledgeable person on FP and contraceptive service provision) at the selected health facilities. Focus group discussions will be conducted with clients at the selected health facilities and in the community. The in-depth interviews and focus group discussions will help to understand clients’ and health service providers’ perspectives of FP and contraceptive service availability and readiness in COVID-19–affected areas. A cross-sectional health facility assessment will be conducted in all the selected health facilities to determine the health facility infrastructure’s ability and readiness to provide FP and contraceptive services and to capture the trends in FP and contraceptive services available during the COVID-19 pandemic. Scientific approval for this study is obtained from the WHO Research Project Review Panel, and the WHO Ethics Review Committee has given ethical approval in the 3 countries. RESULTS Using a standardized research protocol will ensure that the results from this study can be compared across regions and countries. The study was funded in March 2021. It received ethics approval from the WHO Ethics Review Committee in February 2022. We completed data collection in September 2022. We plan to complete the data analysis by March 2023. We plan to publish the study results by Summer 2023. CONCLUSIONS The findings from this study will provide a better understanding of the impact of the COVID-19 pandemic on FP and contraceptive services at the facility level, which will help policy makers and health managers develop and strengthen FP policies and services in health facilities to be more responsive to community needs. To study the genome wide alterations in sperm DNA methylation in male partners of idiopathic recurrent pregnancy loss cases and note regions as potential diagnostic markers DESIGN: Case-Control study and methylome analysis of human sperm SUBJECTS: Control group consists of apparently healthy fertile men having fathered a child within the last 2 years (n = 39); and Case group consists of male partners of idiopathic recurrent pregnancy loss cases having two or more consecutive 1None MAIN OUTCOME MEASURES: Sperm DNA samples of controls and cases were selected for Whole Genome Bisulfite Sequencing analysis based on the previously set thresholds of global methylation levels and methylation levels of imprinted genes (KvDMR and ZAC). Whole Genome Bisulfite Sequencing of selected sperm genomic DNA was performed to identify differentially methylated CpG sites of idiopathic recurrent pregnancy loss cases compared to fertile controls. Pathway analysis of all the differentially methylated genes was done by DAVID functional annotation tool and Kyoto Encyclopedia of Genes and Genomes tool. Differentially methylated CpGs within genes relevant to embryo and placenta development were selected to further validate their methylation levels in study population by pyrosequencing.A total of 9497 differentially methylated CpGs with highest enrichment in intronic regions were obtained. 5352 differentially methylated regions and 2087 differentially methylated genes were noted. Signaling pathways involved in development were enriched upon pathway analysis. Select CpGs within genes PPARG, KCNQ1, SETD2 and MAP3K4 showed distinct hypomethylated sub-populations within idiopathic recurrent pregnancy loss study population.Our study highlights the altered methylation landscape of idiopathic recurrent pregnancy loss sperm and their possible implications in pathways of embryo and placental development. The CpG sites that are hypomethylated specifically in sperm of idiopathic recurrent pregnancy loss sub-population can be further assessed as predictive biomarkers.

Globally the use of modern contraceptive methods has risen to 851 million. Use of these contraceptive methods does influence the vaginal health, which in turn affects the susceptibility towards acquiring RTI/STI. Methods to assess the vaginal health have also evolved over decades. The objective of this narrative review is to assess the influence of contraceptive methods on the vaginal health and also evaluate the methods used to assess the vaginal milieu. Suitable articles published in literature from 2007-2020 were identified from PubMed, Google Scholar using relevant keywords. Hormonal contraceptives included were combined oral contraceptive pills (COCP), Depot medroxyprogesterone acetate (DMPA)and Levonorgestrel IUCD(LNG-IUS). Non hormonal methods included were barrier methods, copper IUCD, diaphragm and vaginal sponge. Outcome parameter recorded were Nugent score, vaginal pH, bacterial vaginosis or defined microbiome profile.COCP have been shown to protect the vaginal ecosystem primarily due to their oestrogen component. The use of IUCD causes initial dysbiosis chiefly due to associated irregular bleeding but long term use of LNG IUS stabilizes the microbiome. Use of injectable DMPA though does not increase the susceptibility to HIV but can promote growth of anaerobic organisms. Literature regarding condom, diaphragm and sponge is very scanty to draw a meaningful conclusion. Hence contraceptive methods can affect the vaginal health. There is need to periodically assess the vaginal milieu using test which is appropriate as per available expertise, infrastructure and cost and treat vaginal dysbiosis in respective cohorts to prevent reproductive morbidity.

Cervicitis is an inflammatory condition of cervix, when presented along with vaginal discharge; it is termed as cervico-vaginitis. These can be infective, hence important to diagnose due to risk of spreading to upper genital tract. This cross-sectional study was undertaken to evaluate the microbiological spectrum in cervicitis or cervico-vaginitis among 100 sexually active women by Gram stain and Multiplex Real time polymerase chain reaction. Bacterial vaginosis 21(21%) was the most common RTI. Among STIs, genital mycoplasmas were the predominant infections hence further research is required to understand their pathogenesis.

Despite adequate treatment of reproductive tract infection, there is persistence of symptoms in some patients. This raises the possibility of existence of other silent microbes with pathogenic potential. Apart from the common sexually transmitted organisms such as Chlamydia trachomatis and Neisseria gonorrhoeae, there are other silent and emerging pathogens, like genital mycoplasma, which have been associated with cervicitis, pelvic inflammatory disease, infertility, and pregnancy-related complications in women. Although these organisms were identified decades ago, they are still overlooked or ignored. There is a need to understand the role played by these organisms in Asian populations and their susceptibility to the standard line of treatment. Data on genital mycoplasma infections in Indian women is heterogeneous, with limited evidence of pathogenicity. Although known for their wide spectrum of reproductive morbidities in western counterparts, these microorganisms are yet to gain the attention of Indian clinicians and microbiologists. There is paucity of adequate information in India regarding these infections, so Indian literature was compiled to get an overview of these pathogens, their association with reproductive morbidities, and their response to treatment. Thus, there is a need to explore genital mycoplasma infections in Indian women, especially in the arena of antimicrobial resistance among genital mycoplasma, which has the potential to become a major problem. A literature search with keywords focusing on "genital mycoplasma", "sexually transmitted infections India", "sexually transmitted mycoplasma", and "characteristic of mycoplasma" was carried out through computerized databases like PubMed, MEDLINE, Embase, and Google Scholar.

Female genital tract tuberculosis (FGTB) is a chronic disease with varied presentation. The diagnosis of FGTB for early institution of treatment remains a clinical challenge. Its laboratory diagnosis is difficult because of paucibacillary nature of the condition and limitation of available diagnostic tests. In view of the intricate problems in diagnosis of FGTB, physicians tend to over treat with empirical anti-tuberculosis drugs. Apart from concerns of drug toxicity, this may be a contributing factor in the increasing incidence of multidrug-resistant TB reported in India. The main goal for advances in TB diagnostics is to reduce delay in diagnosis and treatment. In addition, there should be reduced complexity, improving robustness, and improving accuracy of the laboratory test for diagnosis of Female genital tuberculosis. Objective This narrative review is written with the following objectives. 1) To get a comprehensive overview as well as recent advances in diagnostic test used in the detection of FGTB. 2) To understand the limitations as well as advantages of these laboratory diagnostic test. 3) To provide clinical guidance regarding the detection in susceptible women. Method The literature search was performed using electronic database of Pubmed, Medline, Embase and Google Scholar. Grey literature search was also done. Studies published in English were included. Following keywords were used for search – Tuberculosis, extra pulmonary tuberculosis, female genital tuberculosis, diagnosis of female genital tract tuberculosis. The personal knowledge and experience of authors in the field, helped in archiving the relevant articles. Result Studies suggest that though culture is an invaluable contributor in the diagnosis of FGTB, molecular tests like PCR, LAMP, Xpert MTB/RIF and line probe assays have shown potential and are now being explored to strengthen the diagnostic algorithm of FGTB. Conclusion The use of algorithm approach with combination of both rapid culture and newer molecular techniques will facilitate the accurate and timely diagnosis of FGTB.

Purpose: To develop a quasi steady state model for the time course concentration profile describing the oxygen diffusion and consumption in a multilayered corneal tissue and investigate the effect of various model parameters on the oxygen concentration for open and closed eyes. Method: A simple mathematical model for the oxygen transport in multilayered corneal tissue was developed using Fick’s law of diffusion and Michaelis-Menten kinetics of metabolism. A Crank-Nicoloson finite difference scheme of the equation describing the oxygen diffusion and consumption was written, in which spatial diffusive terms were approximated by central differences while the temporal terms were approximated by average of forward and backward time differences. A system of linear equations obtained from the Crank-Nicholoson finite differences schemes was solved by the Thomos Algorithm. Result: The model predict that oxygen tension without contact lens for an open and closed eye increases along the distance from the aqueous side in each of the layers and the partial pressure gradient in the stroma is higher than that in the epithelium and endothelium layers. It is also observed that the oxygen tension with contact lens in the steady and transient stares, in case of low oxygen permeability of lens decreases along the distance from the aqueous side to the stroma, whereas, at higher oxygen permeability of the lens it increases along the distance for open and closed eyes. Conclusion: Oxygen tension as observed in the cornea of an open eye with or without contact lens is higher than that in closed eye. Also at a high oxygen permeability of contact lens enhance the oxygen tension significantly than that of low oxygen permeability. Purpose The aim of the present work is the development of a simple mathematical model for the time course concentration profile of topically administered drugs in the anterior chamber aqueous humor and investigation of the effects of various model parameters on the aqueous humor concentration of lipophilic and hydrophilic drugs. Method A simple pharmacokinetic model for the transient drug transport in the anterior segment has been developed by using the conservation of mass in the precorneal tear film, Fick's law of diffusion and Michaelis–Menten kinetics of drug metabolism in cornea, and the conservation of mass in the anterior chamber. An analytical solution describing the drug concentration in the anterior chamber has been obtained. Result The model predicts that an increase in the drug metabolic (consumption) rate in the corneal epithelium reduces the drug concentration in the anterior chamber for both lipophilic and hydrophilic molecules. A decrease in the clearance rate and distribution volume of the drug in the anterior chamber raises the aqueous humor concentration significantly. It is also observed that decay rate of drug concentration in the anterior chamber is higher for lipophilic molecules than that for hydrophilic molecules. Conclusion The bioavailability of drugs applied topically to the eye may be improved by a rise in the precorneal tear volume, diffusion coefficient in corneal epithelium and distribution coefficient across the endothelium anterior chamber interface, and by reducing the drug metabolism, drug clearance rate and distribution volume in anterior chamber.
