1. A study to assess the effectiveness of audiovisual education on mirror therapy exercises among hospitalized stroke patients at a tertiary care hospital of south indian town

Abstract

Background: Hemiplegia, which means paralysis of one side of the body, is one of the most common and debilitating effects of a stroke. Stroke is a leading cause of permanent disability in adults. It is estimated that there will be 23 million new cases of strokes and 8 million stroke deaths in 2030. Mirror therapy (MT), in addition to a traditional rehabilitation program, was found to be useful in terms of motor recovery and upper limb function. Materials and Methods: The current study aimed at assessing the effect of audiovisual education on MT exercises among the hospitalized stroke patients. A quasi-experimental design was adopted in this study, where the audiovisual education was given to the experimental group, whereas routine treatment was given to the control group. Results: The result shows that in the experimental group, the majority (40%; n = 6) of the patients belonged to the age group of 61–70 years, whereas in the control group, majority (33.3%; n = 5) of the patients belonged to the age group of 51–60 years and 61–70 years. Majority (60%) of the patients in this study were male. Conclusion: MT exercises were found to be an effective, low-cost, and nonpharmacological method of lowering pain in stroke patients in the hospital.

2. A study to evaluate sensorimotor function of the upper and lower extremities of hospitalized cardiovascular accident patients by using mirror therapy as an intervention

Abstract

Background: Cerebrovascular accidents (CVAs) represent a substantial public health concern due to their high prevalence and associated mortality. Reports indicate that 85% of CVA survivors experience hemiplegia, with 69% suffering from limitations in their upper limb functionality. Despite the existing standard therapeutic approaches, achieving active recovery in the upper extremities remains a challenge. Mirror therapy is an uncomplicated, cost-effective, and patient-centered approach to stroke management. Materials and Methods: This study is a prospective observational study with a quasi-experimental design, with two groups, namely interventional and control groups. Patients who had been provisionally diagnosed and hospitalized with unilateral ischemic stroke and had upper and lower weakness or both on one side and were willing to participate in the study were included. Patients who are bedridden, ventilated, unconscious, and psychologically unstable are excluded from the study. A total of 60 patients were included in the study, 30 each in the case and control groups. Results: In total, 60 patients were included in the study, 30 each in the case and control groups. Most of the study participants belong to the age group of over 55 years (66.66% in the experiment group and 53.33% in the control group). Overall, 80% (n = 24) of the experiment group participants and 66.6% (n = 20) of the control group participants were males. Conclusion: The observations of this study depict a significant improvement in the sensorimotor function after receiving mirror therapy, which validates the findings of the previous studies and hypotheses.

3. Role of mirror therapy in management of stroke – Current and future perspectives

Abstract

Mirror therapy (MT) is a sort of therapy that makes use of vision for managing pain in persons with amputated limbs or weakness in stroke patients. Mirror treatment works by deceiving the brain by making it appear as though the missing limb or weaker limb is moving while the patient looks at the actual, functional limb in a mirror. By doing this, the brain conceals the fact that the amputated limb itself does not provide any signals of movement. Although very encouraging, MT has notable drawbacks that researchers are desiring and working hard to resolve so that it could be applied in treating more composite movements or pain of several types. Stroke has been the third biggest cause worldwide for the years of life lost (YLL); the age-standardized YLL grew by 12.9% (10.6–15.2) between 1990 and 2007 and by 12.1% (9.9–14.1) between 2007 and 2017. MT is a kind of rehabilitation method where the movement of an unaffected limb creates the illusion that the affected limb is also moving, for which a mirror is positioned amidst the legs or arms.

4. Demystifying Dhat Syndrome: A Two-Session Therapy Proposal

Abstract

Dhat syndrome is a common semen-loss related physical and psychological distress usually reported by young males. The treatment mainly centres around nonpharmacological strategies such as psychoeducation. Most of the patients present as outpatients to alternative medicine or nonpsychiatry branches and treatment is usually not delivered. Some of the reasons may include perceived need for longer duration of sessions with the patient, available literature with complex psychiatric jargon and barriers to treatment due to stigma attached to psychiatry. This article presents a two session therapy proposal for Dhat syndrome that could be useful as a brief intervention over tele-consultations, and in consultation liaison clinics by nonpsychiatrists as well.

Abstract

Lowering of intraocular pressure is currently the only therapeutic measure for Glaucoma management. Many longterm, randomized trials have shown the efficacy of lowering IOP, either by a percentage of baseline, or to a specified level. This has lead to the concept of 'Target” IOP, a range of IOP on therapy, that would stabilize the Glaucoma/prevent further visual field loss, without significantly affecting a patient's quality of life. A clinical staging of Glaucoma by optic nerve head evaluation and perimetric parameters, allows a patient's eye to be categorized as having – mild, moderate or severe Glaucomatous damage. An initial attempt should be made to achieve the following IOP range for both POAG or PACG after an iridotomy. In mild glaucoma the initial target IOP range could be kept as 15-17 mmHg, for moderate glaucoma 12-15 mmHg and in the severe stage of glaucomatous damage 10-12 mmHg. Factoring in baseline IOP, age, vascular perfusion parameters, and change on perimetry or imaging during follow up, this range may be reassessed over 6 months to a year. “Target” IOP requires further lowering when the patient continues to progress or develops a systemic disease such as a TIA. Conversely, in the event of a very elderly or sick patient with stable nerve and visual field over time, the target IOP could be raised and medications reduced. An appropriate use of medications/laser/surgery to achieve such a “Target” IOP range in POAG or PACG can maintain visual fields and quality of life, preventing Glaucoma blindness.

Abstract

Background: Risperidone is a widely used antipsychotic, known to cause secondary hyperprolactinaemia. Related problems include bone mineral density (BMD) and vitamin D deficiency. However, there is insufficient information about the extent, severity, and association between these side effects, particularly in the Asian population. Objectives: To estimate the prevalence of osteoporosis, and vitamin D deficiency in patients taking risperidone for more than one year. Also, to investigate whether erectile dysfunction (ED) or menstrual dysfunction (MD) can be used as a proxy indicator of BMD loss in such patients, replacing dual energy X-ray absorptiometry (DEXA) scan. Method: Sixty-five patients (mean age 29.6) receiving risperidone as the only prolactin raising medication for minimum period of one year were selected taking into consideration the socio-demographic and clinical variables. History of ED/ MD, DEXA measurement of their lumbar and hip bone, and endocrine variables were recorded. Results: The prevalence of hyperprolactinaemia in female was found to be 84.4% and in males 78.8%; females being 1.4 times more at risk than males. Abnormal BMD was found in more than 40% of the subjects. Furthermore, 30% had vitamin D deficiency and 60.8% had vitamin D insufficiency. A statistically significant association was observed between ED/ MD and BMD (odds ration [OR] 3.71, confidence interval [CI] 1.23-11.24, p=0.02), but this varied according to the gender. Conclusion: These results suggest that patients on long term risperidone are at high risk of developing hyperprolactinaemia, reduced BMD and Vitamin D, although multiple contributory factors or mechanisms can be suggested. Clinically, ED was more significantly associated with changes in BMD.

With a focus on the project’s two central actors, US architect-planner Albert Mayer and Prime Minister Jawaharlal Nehru, the essay argues that the Etawah Pilot program was a modernizing experiment in citizen assimilation that became a trans-national model for postwar development aid with the international architect-planner as the traveling technocrat set to develop expertise for newly independent nations.

Abstract

Background Optimizing preoperative anemia is a required component of the Joint Commission Patient Blood Management Certification and an important component of Enhanced Recovery After Surgery. Objective To describe a preoperative anemia protocol developed and implemented at the Kaiser Permanente San Jose Medical Center in California to facilitate preoperative identification and treatment of anemia. Methods The protocol included all operations at risk of causing substantial blood loss. It excluded emergent operations and those for which the patient had a normal last hemoglobin value within the prior 12 months unless newly developed anemia was suspected. Eligible patients were screened for laboratory evaluation, and those with anemia were treated for reversible causes. Consistency was ensured by physician, staff, and patient education, and by use of electronic health records. Administration of intravenous iron and erythropoietin and consultation with specialists were expedited as part of a management algorithm. Results Among 510 patients enrolled during 1 year, 442 (87%) received anemia screening laboratory tests. Half of those with laboratory results were eligible for further optimization: 207 had anemia and 21 had iron deficiency without anemia. Among the 228 patients eligible for optimization, 189 (83%) had anemia addressed preoperatively. Of 129 patients with iron deficiency anemia, 102 (79%) received intravenous iron preoperatively, with a mean preoperative increase in hemoglobin level by 0.98 g/dL (n = 79). Conclusion Integration of specialty services, optimization of technology, and consistency across practitioners were crucial for successful implementation and sustainability of a preoperative anemia protocol developed to expedite and enhance best practices.

Abstract

Background: Several studies have documented poor knowledge of schizophrenia among caregivers and its negative influence on clinical outcomes such as treatment response and prognosis. The benefits of group psychoeducation of caregivers of patients suffering from schizophrenia have been well established. However, very few studies have explored the effectiveness of the group psychoeducation program in the Indian setting. Aims: This study aims to evaluate the effects of a group psychoeducation program on the knowledge of schizophrenia among caregivers. Settings and Design: The study was conducted at the department of psychiatry at a tertiary care hospital. The study design is a quasi-experimental, two-group pretest posttest design. Methods: Sixty-eight caregivers fulfilling the inclusion criteria were recruited consecutively from the inpatient wards. Their knowledge was assessed with a Knowledge About Schizophrenia Interview (KASI) questionnaire, within 1 week of admission and again during the week of discharge. Controls were caregivers who received psychoeducation through “standard care” alone. Cases were caregivers who received the “group psychoeducation program” in addition to the “standard care” during the course of their stay at the hospital. Their pretest and posttest scores were compared. Results: Fifty caregivers successfully completed the study. There was statistical difference between cases and controls in education (P = 0.007) and duration of stay (P ≤ 0.001). Comparing pretest and posttest scores of all the participants showed that psychoeducation received through various sources (standard care or standard care with group psychoeducation) from admission until discharge produced a significant change in knowledge (P = 0.001). On controlling for education and duration of inpatient stay using multivariate analysis, group psychoeducation further produced a significant improvement in KASI scores in subsections of management (B = 0.839, confidence interval [CI] = 0.301–1.377, P = 0.002) and course and prognosis (B = 1.366, CI = 0.095–2.637, P = 0.035). Conclusions: Group psychoeducation in the Indian setting is an effective and feasible option to improve the knowledge of schizophrenia among caregivers, especially with regard to management, course, and prognosis.

Abstract

Lowering of intraocular pressure is currently the only therapeutic measure for Glaucoma management. Many longterm, randomized trials have shown the efficacy of lowering IOP, either by a percentage of baseline, or to a specified level. This has lead to the concept of 'Target” IOP, a range of IOP on therapy, that would stabilize the Glaucoma/prevent further visual field loss, without significantly affecting a patient's quality of life. A clinical staging of Glaucoma by optic nerve head evaluation and perimetric parameters, allows a patient's eye to be categorized as having – mild, moderate or severe Glaucomatous damage. An initial attempt should be made to achieve the following IOP range for both POAG or PACG after an iridotomy. In mild glaucoma the initial target IOP range could be kept as 15-17 mmHg, for moderate glaucoma 12-15 mmHg and in the severe stage of glaucomatous damage 10-12 mmHg. Factoring in baseline IOP, age, vascular perfusion parameters, and change on perimetry or imaging during follow up, this range may be reassessed over 6 months to a year. “Target” IOP requires further lowering when the patient continues to progress or develops a systemic disease such as a TIA. Conversely, in the event of a very elderly or sick patient with stable nerve and visual field over time, the target IOP could be raised and medications reduced. An appropriate use of medications/laser/surgery to achieve such a “Target” IOP range in POAG or PACG can maintain visual fields and quality of life, preventing Glaucoma blindness.

Abstract

Postischemic injury in 38 recipients of 7-day-old cadaveric renal allografts was classified into sustained ( n = 15) or recovering ( n = 23) acute renal failure (ARF) according to the prevailing inulin clearance. Recipients of long-standing allografts that functioned optimally ( n = 16) and living transplant donors undergoing nephrectomy ( n = 10) served as functional and structural controls, respectively. A combination of physiological and morphometric techniques were used to evaluate glomerular filtration rate and its determinants 1–3 h after reperfusion and again on day 7 to elucidate the mechanism for persistent hypofiltration in ARF that is sustained. Glomerular filtration rate in the sustained ARF group on day 7 was depressed by 90% (mean ± SD); the corresponding fall in renal plasma flow was proportionately less. Neither plasma oncotic pressure nor the single-nephron ultrafiltration coefficient differed between the sustained ARF and the control group, however. A model of glomerular ultrafiltration and a sensitivity analysis were used to compute the prevailing transcapillary hydraulic pressure gradient (ΔP), the only remaining determinant of ΔP. This revealed that ΔP varied between 27 and 28 mmHg in sustained ARF and 32–38 mmHg in recovering ARF on day 7 vs. 47–54 mmHg in controls. Sustained ARF was associated with persistent tubular dilatation. We conclude that depression of ΔP, perhaps due partially to elevated tubule pressure, is the predominant cause of hypofiltration in the maintenance stage of ARF that is sustained for 7 days.

Abstract

There are several challenges involved in the development of a production control system for a hybrid/alternative energy vehicle. Some of the challenges center around the new component technologies such as batteries, power electronics, electric machines or hydraulics that are an integral part of such a vehicle. Others center around the invention that is required to develop novel algorithms that utilize these component technologies in the most synergistic fashion with conventional technologies so as to optimize a cost function of interest (fuel economy for example). While these challenges are noteworthy in their own right, there are additional challenges that need to be overcome to actually bring them to production and to deliver a quality product that meets the OEM requirements for quality and robustness over the lifetime of the vehicle (10 years/150,000 miles for example). These hybrid/alternative energy production vehicles are expected to be exercised in the very same environments (e.g. hot, cold, high altitude, humid) that the consumer has been using conventional vehicles in. This paper describes several processes that enable the development of control systems that can not only “get things working”, but also meet the more rigorous quality and robustness demands imposed upon a production vehicle. While these processes are not limited in their usage to only hybrid/alternative energy vehicles, it is the newness of the technologies in these vehicles, and the need to respond rapidly to the needs of the marketplace that really necessitates the use of such processes on these types of vehicles.Copyright © 2010 by ASME

Background10 Hz electroencephalographic (EEG) alpha rhythms correlate with memory performance. Alpha and memory decline in older people. We wished to test if alpha-like EEG activity contributes to memory formation. Flicker can elicit alpha-like EEG activity. We tested if alpha-frequency flicker enhances memory in older people. Pariticpants aged 67–92 identified short words that followed 1 s of flicker at 9.0 Hz, 9.5 Hz, 10.0 Hz, 10.2 Hz, 10.5 Hz, 11.0 Hz, 11.5 Hz or 500 Hz. A few minutes later, we tested participants' recognition of the words (without flicker).ResultsFlicker frequencies close to 10 Hz (9.5–11.0 Hz) facilitated the identification of the test words in older participants. The same flicker frequencies increased recognition of the words more than other frequencies (9.0 Hz, 11.5 Hz and 500 Hz), irrespective of age.ConclusionThe frequency-specificity of flicker's effects in our participants paralleled the power spectrum of EEG alpha in the general population. This indicates that alpha-like EEG activity may subserve memory processes. Flicker may be able to help memory problems in older people.

Abstract

Ford Motor Company has recently implemented a Hardware-In-the-Loop (HIL) testing system for a new, highly complex, hybrid electric vehicle (HEV) Electronic Control Unit (ECU). The implementation of this HIL system has been quick and effective, since it is based on proven Commercial-Off-The-Shelf (COTS) automation tools for real-time that allow for a very flexible and intuitive design process. An overview of the HIL system implementation process and the derived development benefits will be shown in this paper. The initial concept for the use of this HIL system was a complete closedloop vehicle simulation environment for Vehicle System Controller testing, but the paper will show that this concept has evolved to allow for the use of the HIL system for many facets of the design process. Finally, the paper will demonstrate the benefits that have been achieved by using multiple HIL validation systems, to coordinate development with suppliers using the modeldriven process, and to simulate conditions for testing that are difficult to achieve in actual vehicles.

Abstract

We determined the effect of postischemic injury to the human renal allograft on p-aminohippurate (PAH) extraction (EPAH) and renal blood flow. We evaluated renal function in 44 allograft recipients on two occasions: 1–3 h after reperfusion ( day 0) and again on postoperative day 7. On day 0 subsets underwent intraoperative determination of renal blood flow ( n = 35) by Doppler flow meter and EPAH( n = 25) by renal venous assay. Blood flow was also determined in another subset of 16 recipients on postoperative day 7 by phase contrast-cine-magnetic resonance imaging, and EPAHwas computed from the simultaneous PAH clearance. Glomerular filtration rate (GFR) on day 7 was used to divide subjects into recovering ( n = 23) and sustained ( n = 21) acute renal failure (ARF) groups, respectively. Despite profound depression of GFR in the sustained ARF group, renal plasma flow was only slightly depressed, averaging 296 ± 162 ml ⋅ min−1⋅ 1.73 m−2on day 0 and 202 ± 72 ml ⋅ min−1⋅ 1.73 m−2on day 7, respectively. These values did not differ from corresponding values in the recovering ARF group: 252 ± 133 and 280 ± 109 ml ⋅ min−1⋅ 1.73 m−2, respectively. EPAHwas profoundly depressed on day 0, averaging 18 ± 14 and 10 ± 7% in recovering and sustained ARF groups, respectively, vs. 86 ± 6% in normal controls ( P < 0.001). Corresponding values on day 7remained significantly depressed at 65 ± 20 and 11 ± 22%, respectively. We conclude that postischemic injury to the renal allograft results in profound impairment of EPAHthat persists for at least 7 days, even after the onset of recovery. An ensuing reduction in urinary PAH clearance results in a gross underestimate of renal plasma flow, which is close to the normal range in the initiation, maintenance, and recovery stages of this injury.
