1. Antidiarrheal Activities of the Methanol Leaf Extracts of Olinia rochetiana (Oliniaceae) Against Castor Oil-Induced Diarrhea in Mice

Therefore, this study aimed to investigate the acute toxicity and antidiarrheal effect of O. rochetiana leaf extracts.Methods: Cold maceration was used to extract plant leaf powder with 80% methanol. The extract’s antidiarrheal action was tested against a castor oil-induced diarrheal model, a charcoal meal test, and enteropooling tests at doses of 100, 200, and 400 mg/kg. Negative controls received the vehicle at 10 mL/kg, while positive controls received loperamide at 3 mg/kg.Results: From the study, no apparent toxicity was observed when a single dose of 2000 mg/kg was administered. In the castor oil-induced model, the extract delayed the onset of diarrhea, reduced stool frequency, and decreased wet feces weight and number in a dose-dependent manner at 200 mg/kg (p < 0.05) and 400 mg/kg (p < 0.01). The percent reduction in moist feces at 100, 200, and 400 mg/kg was 54.2, 23.97, and 18.26%, respectively, indicating a significant dose-dependent decrease. In a charcoal meal test, the extracts at 200 and 400 mg/kg revealed a peristaltic index of 65 and 46%, respectively, with considerable inhibition of charcoal transport at 23 and 39%. The weight and volume of intestinal contents dropped significantly at a dose of 400 mg/kg (p < 0.01), which is 0.43 mg/kg, in the enteropooling test when compared with the tested dose. The computed in vivo antidiarrheal index revealed diarrheal inhibition values of 46.06 and 71.06% at 200 and 400 mg/kg, respectively.Conclusion: In the current investigation, O. rochetiana showed significant antidiarrheal activity with no symptoms of toxicity in mice.Keywords: antidiarrheal activity, castor oil-induced diarrhea, gastrointestinal transit time, enteropooling, Olinia rochetiana

The current conventional anti-inflammatory drugs have many undesirable health effects and are in need of newer drugs. The purpose of this study was to evaluate the anti-inflammatory effects of an aqueous methanol crude extract of Premna schimperi leaves.Methods: Premna schimperi leaf was extracted with 80% methanol and concentrated; the concentrated extract was used to evaluate the acute toxicity and anti-inflammatory effects. For the acute toxicity study, a single dose of Premna schimperi extract at a dose of 2000 mg/kg was administered and observed for 14 days. Acute, sub-acute, and chronic anti-inflammatory models were employed to evaluate the anti-inflammatory effect of the extract compared to the standard drug. Data were analyzed with SPSS V. 27, and the significance was established with a one-way ANOVA followed by a post hoc Tukey’s test.Results: Acute oral toxicity testing at a dose of 2000 mg/kg did not show any sign of toxicity. According to the phytochemical study, the plants contained flavonoids, terpenoids, tannins, cardiac glycosides, steroids, phenolics, and anthraquinones. The extract doses of 200 mg/kg, 400 mg/kg, and 800 mg/kg of extracts effectively (p< 0.001) reduced paw edema in the acute and sub-acute models of inflammation. When compared to the negative control group, all tested doses in the chronic model significantly (p< 0.05) decreased the production of exudates and the amount of granuloma tissue.Conclusion: Premna schimperi displayed significant anti-inflammatory activity. As a result, identifying the determinants of unsuccessful outcomes of directly observed treatment, short-course (DOTS) is critical.Purpose: This study aimed to assess the determinants of unsuccessful TB treatment outcomes among patients with DOTS in Jimma town, southwest Ethiopia.Patients and Methods: An unmatched case-control study using secondary data was conducted among 118 cases and 472 controls from April 1 to June 1, 2021. Cases were TB cases with unsuccessful treatment courses whereas controls were those of successful treatment. Data were collected using a standardized tool. Statistical analysis was done using SPSS version 23 software. A binary logistic regression model was used to identify the determinants.Results: We found that the determinants including age > 35 years (AOR=1.9, 95% CI: 1.14– 3.2), lack of contact person (AOR= 27, 95% CI: 15.8– 46.3), being HIV positive (AOR=7.3, 95% CI: 3.7– 14.2)), and being malnourished (AOR=4.5, 95% CI: 1.5– 12.9) were significantly associated with unsuccessful tuberculosis treatment outcome.Conclusion: In this study, advanced age, being HIV positive, lack of contact person, and being malnourished were determinant factors for unsuccessful TB treatment outcomes. Even though there are numerous studies on the prevalence of sexual coercion among students, nurses, adolescents, and young pregnant women, studies on the prevalence of sexual coercion among female waiters are limited. Furthermore, there is no evidence existed that show a relationship between sexual coercion and HIV risk behavior in Ethiopia.Purpose: The purpose of this study was to examine the relationship between sexual coercion and HIV risk behavior among female waiters in Jimma, southwest Ethiopia.Patients and Methods: We conducted a cross-sectional survey from 1st April to 30, 2018, among 420 female waiters of reproductive age working in the licensed food and drinking establishments in Jimma town. A structured interviewer-administered questionnaire was used to collect data. Statistical analysis was conducted with SPSS version 21 statistical software. A binary logistic regression model was used to determine the association between independent variables and outcome variables.Results: The lifetime prevalence of sexual coercion among female waiters was 71.4% (95% confidence interval: 67.1– 76.8). More than two-thirds (71.6%) of female waiters engaged in HIV-related risk behaviors. Working in the bar (AOR 4.64, 95% CI: 2.15– 10.0), being a substance user (AOR 3.37, 95% CI: 1.7– 6.7), experiencing sexual coercion (AOR 7.6, 95% CI: 3.8– 15.3) were significantly associated with HIV risk behaviors.Conclusion: A significant number of female waiters experienced sexual coercion and engaged in HIV-risk behaviors. Workplace, substance use, and sexual coercion were significantly associated with HIV risk behavior. As a result, establishments, town health offices, and other stakeholders should work together to safeguard female waiters from the burdens of sexual coercion, HIV risk behavior, and sexually transmitted infections.Keywords: HIV-related risk behaviors, sexual violence, sexual coercion, female waiters, southwest Ethiopia

Media is crucial in reaching health messages to audiences, especially those who are distant and rural. Therefore, the study aimed to assess demands, access, and factors associated with access to health messages through mass media in the rural community of Kersa District of East Hararghe, Eastern Ethiopia.Methods: A mixed-methods study was conducted from October 15 to November 20, 2020. A quantitative cross-sectional and a qualitative phenomenological study design were applied. A total of 578 participants were included by using a systematic sampling technique. Collected data were entered into Epidata version 3.1 and analyzed using SPSS version 22.0. A multivariate logistic regression analysis model was used and reported using an adjusted odds ratio (AOR) with a 95% confidence interval (CI). Statistical significance was set at p < 0.05. For qualitative, six-focused group discussions (FGDs) were used and then analyzed thematically.Results: Overall, the demand of and access to health messages through mass media was 32.5% (95% CI=28.5– 34.2%) and 26.6% (95% CI=24.6– 28.7%), respectively. Factors such as having electric services (AOR=2.36, 95% CI=2.13– 5.41), having a mobile phone (AOR=4.56, 95% CI=4.32– 8.73), exposure to TV (AOR=4.73, 95% CI=1.03– 11.62), and exposure to social media and printed media (AOR=5.24, 95% CI=1.07– 15.63), a preference for programs such as news, current affairs, entertainment, health and educational were 2.37, 9.47, 4.75 and 7.55 times more likely to access health messages (AOR=2.37, 95% CI=1.00– 5.61; AOR=9.47, 95% CI=3.54– 25.34; AOR=4.75, 95% CI=1.23– 18.38; and AOR=7.55, 95% CI=3.12– 8.66, respectively). Qualitative findings, participants demand for health messages from health workers, radio, and the main source for accessing the message was the radio.Conclusion: Approximately one in every three and one in every four rural communities in the study area had demand, and access to health messages through mass media, respectively. As a result, all stakeholders should emphasize and strengthen expanding methods of reaching health messages using mass media.Keywords: health message, media, access, rural community, Eastern Ethiopia

Intra-medullary nailing (IM) has been an effective and cost-effective method of treating long bone shaft fractures, but it can lead to complications such as knee pain.Purpose: This study aimed to evaluate knee pain and its associated factors following retrograde intramedullary nailing for femur fractures.Patients and Methods: The study followed 110 patients diagnosed with femur fractures and treated with retrograde SIGN Standard Nail or Fin Nail from January 2020 to December 2022 at two hospitals in Ethiopia. The patients were followed up for at least 6 months, and data were collected from medical charts, patient interviews, and phone calls to patients who did not attend the follow-up appointment. Binary logistic regression analysis was used to identify factors associated with knee pain.Results: The study showed that 40 patients reported knee pain at 6-months follow-up, making a prevalence of 36.4%. Factors significantly associated with knee pain were a time of injury to nailing (AOR=4.23, 95% CI: 1.28– 13.92), use of a screw to the medial cortex (AOR=9.30, 95% CI: 2.90– 12.74), and fracture site (AOR= 2.67, 95% CI: 14.01– 7.03). Specifically, the longer the time from injury to nailing, the higher the risk of knee pain. The use of a longer screw to the medial cortex and a fracture site were also positively associated with knee pain.Conclusion: This study concludes that although retrograde intramedullary nail fixation is an effective method for femur fractures, it often results in knee pain. Approximately 4 of 10 patients suffered from knee pain in this study. Avoiding delayed surgical management and minimizing the use of prominent metalwork may reduce knee pain.Keywords: knee pain, intramedullary nailing, Ethiopia

Despite efforts to improve basic and comprehensive emergency obstetric treatment, women continue to suffer from disastrous maternal outcomes.Purpose: This study aimed to assess the survival status and predictors of mortality among women with uterine rupture at public hospitals in the Harari Region, Eastern Ethiopia.Patients and Methods: We conducted a retrospective cohort study among women with uterine rupture in public hospitals in Eastern Ethiopia. All women with uterine rupture were followed for 11 years retrospectively. Statistical analysis was conducted with STATA version 14.2. Kaplan–Meier curves together with a Log rank test were used to estimate the survival time and show the presence of differences among groups. Cox Proportion Hazard (CPH) model was used to determine the association between independent variables and survival status.Results: There were 57,006 deliveries in the study period. We found that 10.5% (95% CI: 6.8– 15.7) of women with uterine rupture have died. The median recovery and death time for women with uterine rupture were 8 and 3 days with interquartile range (IQR) of 7– 11 days and 2– 5 days, respectively. Antenatal care follow-up (AHR: 4.2, 95% CI: 1.8– 9.79), education status (AHR: 0.11; 95% CI: 0.02– 0.85), visiting health center (AHR: 4.89; 95% CI: 1.05– 22.88), and admission time (AHR: 4.4; 95% CI: 1.89– 10.18) were the predictors of survival status of women with uterine rupture.Conclusion: One out of ten study participants died due to uterine rupture. Factors including not having ANC follow-up, visiting health centers for treatment, and being admitted during the night time were predictors. Thus, a great emphasis has to be given to the prevention of uterine rupture and the linkage within health institutions has to be smooth to improve the survival of patients with uterine rupture with the help of different professionals, health institutions, health bureaus, and policymakers.Keywords: emergency obstetrics, uterine rupture, predictors, mortality, Ethiopia

Despite condom availability and promotion for use in STI prevention, consistent condom use remains too low in Ethiopia. A significant proportion of Ethiopian military personnel had multiple sexual partners, with lower rates of condom use with non-regular sexual partners. As a result, determining the pattern of condom use and the factors associated with it among military personnel is critical.Objective: The purpose of this study was to evaluate consistent condom use and associated factors among sexually active military personnel in Eastern Ethiopia from February 1 to 28, 2020.Methods: A cross-sectional study was conducted among 327 sexually active military personnel. To select the study participants, a systematic sampling technique was used. The data were entered into EpiData and exported to the Statistical Package for Social Sciences version 20 for analysis. To identify factors associated with condom use, bivariate and multivariable logistic regression analysis were used, and the strength of the association was measured using odds ratio and 95% confidence interval (CI), with P-values less than 0.05 considered statistically significant.Results: The overall prevalence of consistent condom uses over one year by the respondents was 59.9% (95% CI: 54, 65). Study participants with an age of 30– 35 years old (AOR = 3.12; 95% CI: 1.43, 11.38), above 35 years old (AOR = 2.42; 95% CI: 1.83, 9.30), college and above in educational status (AOR = 1.52; 95% CI: 1.20, 5.43), Officer in military rank (AOR = 3.12; 95% CI: 1.14, 10.15) and other military ranks (AOR = 3.08; 2.33, 8.52) were factors facilitate the use of condom consistently.Conclusion: In this study, two in every five military personnel use condoms inconsistently. Designing appropriate intervention and behavioral change strategies, as well as increasing the accessibility and availability of condoms around military camps, will undoubtedly have a significant impact on consistent condom utilization.Keywords: consistent condom utilization, military personnel, eastern command, associated factors, Ethiopia

Satisfied people will have good communication with the health professional, which results in good care and health outcome, so this study aimed to assess the association of caring behavior of nurses with the level of patient satisfaction, in public hospitals of the Harari region, Eastern Ethiopia, from March 10 to April 10, 2021.Methods: A cross-sectional study was conducted among 300 nurses providing inpatient services and 345 randomly selected admitted patients in two public hospitals of Harari region, Eastern Ethiopia. Clinical nurse–patient interaction scale was used to assess the level of caring behavior. The level of patient satisfaction was measured using the patient satisfaction scale tool. We used structural equation modeling to determine the relationship of nurses caring behaviour and the level of patient satisfaction. The association was reported using coefficient (β) along with a 95% confidence interval. The statistical significance of the association was declared at P < 0.05.Results: According to this study, only 51.67% of the nurses had good caring behavior and 62.90% of the patients were satisfied with the nursing care they received. The nurses’ caring behavior score was increased by 41% (β=0.41; 95% CI: 0.20, 0.63) and (β=0.65; 95% CI: 0.48, 0.81) as nurses’ job satisfaction and work environment scores increased by one scale, respectively. Similarly, the score of patient satisfaction was increased by 2% (β=0.02; 95% CI: 0.01, 0.12) as the score of nurse caring behaviour was increased by one scale.Conclusion: Patient satisfaction increased as the nurses care behavior score increased, so nurses must continue to identify ways to evaluate and increase patient happiness, as well as illustrate the impact nurses have on patient satisfaction with hospital experience as a result of their treatment.Keywords: caring, nursing, caring behavior, patient satisfaction

Therefore, this study has aimed to determine the magnitude of the anemia and its associated factors among pregnant women admitted to labor wards of public hospitals in eastern Ethiopia.Methods and Materials: A health facility-based cross-sectional study was conducted among 405 pregnant women admitted to labor wards in four public hospitals of Harar town and Dire Dawa City Administration. A systematic random sampling technique was used to select the study participants. Data were collected through an interview using a structured questionnaire. The hemoglobin level was measured for each study participant. Binary and multivariable logistic regression models were fitted and statistical significance was declared at P< 0.05.Results: The magnitude of anemia was 33.1% (95% CI: 28.4%, 37.8%). Being from rural areas (AOR: 3.8; 95% CI: 1.81, 7.94), no antenatal care (ANC) follow-up (AOR: 3.4; 95% CI: 1.34, 8.79), a habit of drinking milk with tea after meals (AOR: 2.8; 95% CI: 1.48, 5.61), taking a meal only 1– 2 times per day (AOR: 3.9; 95% CI: 1.69, 8.97), experiencing no blood loss in the current pregnancy (AOR: 0.25; 95% CI: 0.06, 0.96) and a habit of eating leafy vegetables (AOR: 0.12; 95% CI: 0.06, 0.24) were significantly associated with anemia.Conclusion: About one in three pregnant women were anemic and anemia was a moderate public health problem in the study settings. Interventions targeting reducing or preventing anemia should focus on pregnant women in rural areas, promoting the benefits of ANC follow-up, reducing the habit of drinking milk with tea after meals, enhancing the consumption of leafy vegetables and increasing meal frequency per day to minimize or prevent anemia and its consequences during pregnancy.Keywords: anemia, pregnant women, eastern Ethiopia

This study evaluates the in vitro vasodilatory effect of the extract of M. stenopetala leaves and the possible mechanisms in precontracted isolated thoracic aorta of guinea pigs. A guinea pig was sacrificed by gentle cervical dislocation, and the thoracic aortic ring was removed, cut spirally, and mounted in an organ bath containing Krebs–Henseleit physiological solution maintained at 37°C, and then the solution was aerated with carbogen (95% O2 and 5% CO2). The vasodilatory activity of cumulative doses of M. stenopetala extracts and fractions was evaluated on intact and denuded endothelium of isolated whole, spirally cut thoracic aortic strips of guinea pigs precontracted with potassium chloride (80 mM), epinephrine (1 µM), methylene blue (10 µM), and glibenclamide (10 µM) using polygraph. All extracts showed a relaxant effect in precontracted isolated whole, spirally cut thoracic aortic strips of guinea pigs in a dose-dependent manner, whereas the greater percentage of relaxant effect was shown with the addition of crude extracts in 80 mM of potassium chloride (99.10% and 95.56% for ethanol and aqueous crude extracts, respectively), and 1 µM of epinephrine (82.85% and 90.16% for ethanol and aqueous crude extracts, respectively) in precontracted isolated whole, spirally cut thoracic aortic strips of guinea pigs. Hence, the possible mechanism of relaxation might be mediated through the blockade of receptor-operated calcium influx and L-type voltage-dependent calcium channels. The aqueous extract showed more significant in vitro vasodilatory effect than its fractions and 70% ethanol extract. Abstract: Nation-wide Sentinel-2 mosaics were used with National Forest Inventory (NFI) data for modelling and subsequent mapping of spruce, pine and deciduous forest in Norway in 16 m $\times$ 16 m resolution. The accuracies of the best model ranged between 74% for spruce and 87% for deciduous forest. An overall accuracy of 90% was found on stand level using independent data from more than 42.000 stands. Errors mostly resulting from a forest mask reduced the model accuracies by approximately 10%. Nonetheless, efficiencies of national forest area estimates increased by 20% to 50% for model-assisted (MA) estimates and up to 90% for poststratification (PS). Greater minimum number of observations, however, constrained the use of PS. For estimates of sub-populations (municipalities), efficiencies improved by up to a factor of 8. However, even for municipalities with a decent number of NFI plots, direct NFI estimates were sometimes more precise than MA estimates. PS estimates were always more precise than direct and MA estimates but were applicable in fewer municipalities.
