20. Extreme Rhabdomyolysis, Acute Renal Failure, and Protracted Ileus in a Case of Legionella Pneumonia

Abstract

A 53-year-old man developed a Legionella pneumophila pneumonia complicated by rhabdomyolysis, acute kidney injury, and protracted ileus. Risk factors were smoking and chronic alcoholism, but the patient had no history of previous abdominal surgery. Hemodialysis was required for a period of 5 weeks with a full renal recovery. Pneumonia required respiratory support but for a limited period of 6 days. The protracted course of the ileus led to explorative laparotomy despite negative computed tomography findings. No cause of mechanical obstruction was found at surgery and common etiologies of intestinal obstruction were excluded. Parenteral nutrition was needed for a total of 4 weeks, before recovery of intestinal motility. This case illustrates the apparent discrepancy between the pulmonary symptoms and the extrapulmonary manifestations that could be seen as a consequence of an exaggerated immune response.

Abstract

We describe the preparation of a novel self-assembling supramolecular nanotube system., We describe a novel self-assembling supramolecular nanotube system formed by a heterocyclic cationic molecule which was originally designed for its potential as an antiparasitic and DNA sequence recognition agent. Our structural characterisation work indicates that the nanotubes form via a hierarchical assembly mechanism that can be triggered and tuned by well-defined concentrations of simple alkali halide salts in water. The nanotubes assembled in NaCl have inner and outer diameters of ca. 22 nm and 26 nm respectively, with lengths that reach into several microns. Our results suggest the tubes consist of DB921 molecules stacked along the direction of the nanotube long axis. The tubes are stabilised by face-to-face π–π stacking and ionic interactions between the charged amidinium groups of the ligand and the negative halide ions. The assembly process of the nanotubes was followed using small-angle X-ray and neutron scattering, transmission electron microscopy and ultraviolet/visible spectroscopy. Our data demonstrate that assembly occurs through the formation of intermediate ribbon-like structures that in turn form helices that tighten and compact to form the final stable filament. This assembly process was tested using different alkali–metal salts, showing a strong preference for chloride or bromide anions and with little dependency on the type of cation. Our data further demonstrates the existence of a critical anion concentration above which the rate of self-assembly is greatly enhanced.

27. Containerless processing at high temperatures using acoustic levitation

Abstract

Advanced techniques are presented which facilitate the development of inert or reducing atmospheres in excess of 2000 K in order to improve processing of containerless capabilities at higher temperatures and to provide more contamination-free environments. Recent testing, in the laboratory and aboard the NASA KC-135 aircraft, of a high-temperature acoustic positioner demonstrated the effectiveness of a specimen motion damping system and of specimen spin control. It is found that stable positioning can be achieved under ambient and heated conditions, including the transient states of heat-up and cool-down. An incorporated high-temperature levitator was found capable of processing specimens of up to 6-mm diameter in a high-purity environment without the contaminating effects of a container at high temperatures and with relative quiescence.

Abstract

Background: Dasatinib (DAS) has shown superiority over imatinib in achieving cytogenetic and molecular responses in chronic phase (CP) CML but with a different toxicity profile, which may impact on its overall benefit. Reported tox- icities from the DASISION trial at 5 years include pleural effusions (PE) (29%), pulmonary hypertension (PHTN) (5%), and low rates of arterial ischemic events. While the literature well describes the incidence of these events, the response to therapy and the impact of subsequent dose modifications on the outcome of the toxicity have generally not been comprehensively characterised. Aim(s): To review the incidence of these side-effects in a snapshot survey of Australian patients (pts) receiving DAS, either as first or subsequent line of therapy for CML-CP, with a focus on risk factors and the response of the toxicity to therapeutic changes. Method(s): Retrospective study of all eligible pts (received DAS for CML-CP and access to sufficient data) at 17 Australian institutions. Each pt's history was tracked in detail to identify any complications potentially attributable to DAS, and the treatment and outcome of these complications in response to cessation or alterations in DAS dose or schedule. Result(s): 221 pts were evaluable, with a median age at DAS commencement of 53 years (range 20 - 86) and median starting dose of 100mg (20-140). 51 (23%) received DAS as 1st line therapy, 133 (60%) 2nd line and 37 (17%) 3rd line. The median follow up from DAS commencement was 27 months (4 -116 months). No side effects were reported in 105 (48%) pts. Of the 116 (52%) pts with side-effects, the most common side effect was PE, observed in 53 (24%) pts with a median age of 65 years (41-86 years) and with median time to onset of 11 months (0.5-59 months). Most (40 pts, 76%) were receiving DAS as 2nd line therapy. Abstract

SMEs are of vital importance for our economy and society. However, they lag behind in their adoption of ICT, particularly interorganisational ICT. Many countries have put policy programmes in place to improve SMEs on this aspect. Previous research provides unclear evidence on the effectiveness of these programs. With this study we aim to provide new insight into the impact of such policy choices on ICT adoption. Through a survey among a relatively large sample of SME participants (n = 516) and a matched control group (n = 124) of non-participating SMEs, we assess a policy programme that ran in the Netherlands from 2002 to 2007. Participants are found to use significantly more interorganisational ICT and also more often find that ICT has enhanced their firm’s performance compared to the control group. However, pre-selection effects cannot be ruled out. Based on our results, we suggest basic, awareness-focused policy programmes, as opposed to complex, government supported implementation processes.

Abstract

To enhance the assessment of reactor pressure vessel life under severe accident, a cooperative research program is conducted between CEA, EDF, and Framatome, parts of which are supported by the European Commission. Within the framework of this program, a model based on Lemaitre and Chaboche with predictive capability in the field of viscoplastic flow, material damage, and failure has been generalized and implemented in CASTEM 2000 finite element code. In order to validate the model in mechanical situations featuring the same basic characteristics as the severe accident scenarios, analytical experiments are conducted on the RUPTHER facility: a thin shell tube is loaded with an internal pressure and submitted to an axial thermal gradient with elevated temperatures. The specific question of the interest of the coupled damage approach in the failure prediction is addressed. Comparisons of the predictions relying on coupled and uncoupled damage-viscoplasticity models show that, at higher temperatures, failure could be predicted through uncoupled damage evaluation provided the main nonlinear effects like large displacements and updated pressure are taken into account, while at lower temperatures coupling between damage and deformation is necessary. Abstract

Ce diagnostic, réalisé en bordure nord de la route de Roumoules au lieu-dit les Capucins, n'a livré aucun vestige archéologique. Six tranchées ont été ouvertes à la pelle mécanique sur les différentes parcelles concernées par l'arrêté de prescription. La réalisation de ce diagnostic a également fournie l'occasion de compléter l'inventaire des structures hydrauliques rupestres conservées sur ce versant de la colline. Les séquences stratigraphiques reconnues dans les différents sondages sont composées d'un niveau de recouvrement de terre végétale, sous lequel s'accumulent des dépôts de pente successifs formés d'amas de galets calcaire pris dans une matrice argilo-limoneuse. Dans la partie basse du terrain, les dépôts de pente atteignent une épaisseur d'au moins 2,60 m sous le niveau de sol actuel. Sur les parties hautes, ces dépôts recouvrent une couche d'argile naturelle très homogène. Les niveaux de colluvions contiennent quelques fragments de céramiques antiques (voire protohistorique ?) à modernes (sgraffito). Ce mobilier épars, drainé par le colluvionnement, provient sans doute d'occupations situées plus haut sur l'adret de la colline Saint-Maxime. La proximité du couvent des Capucins ne semble pas avoir suscité, à la période moderne, d'aménagements spécifiques sur les parcelles adjacentes à la chapelle.

45. Business process management in small business: a case study

Abstract

Small Businesses account for a significant portion of the Australian business sector. With Business Process Management (BPM) gaining prominence in recent decades as a means of improving business performance, it would seem to only be a matter of time before it gains momentum within the Small Business sector. One may even question why it has not already achieved more traction within the sector. This case study involves a BPM initiative to develop process infrastructure in an establishing Small Business. It explores whether mainstream BPM tools, techniques and technologies can be applied in a Small Business setting. The chapter provides a background to the case organisation, outlines the activities undertaken in the BPM initiative and distils key observations drawn from participation in the initiative and consultation with stakeholders. Based on the case study experiences, a number of implications are identified for further consideration by the BPM discipline as it continues to address the question of how it can become more widely adopted amongst Small Businesses.

National audience; La résistance aux anthelminthiques est un phénomène connu depuis de nombreuses années. Dans des élevages de petits ruminants où ce problème avait été identifié vis-à-vis des benzimidazoles et où cette famille d’anthelminthique n’avait pas été utilisée depuis plusieurs années, nous avons voulu voir si cette résistance pouvait être réversible. Des coproscopies avant et après traitement, et avec lots témoins, ont été réalisées. Plusieurs méthodes statistiques ont été utilisées pour le traitement des données. Le test de réduction de l’excrétion des oeufs est le test le plus intéressant en clientèle. Dans ces élevages, il n’y a pas eu réversibilité de la résistance.
