Le flux sanguin coronaire est hautement régulé :il est maintenu constant sur une large gamme de pression de perfusion (autorégulation) et augmente lorsque nécessaire (régulation métabolique). La microcirculation est le principal siège des mécanismes régulant le flux coronaire (participe à plus de 90% des résistances vasculaires coronaires) et l’endothélium y joue un rôle clé :en réponse à des stimuli physico-chimiques, il libère dans un équilibre précis, des vasodilatateurs (oxide nitrique, prostacyclines et « endothelium derived hyperpolarizing factors) et des vasoconstricteurs (prostaglandines, thromboxane, endothéline-1) qui modulent le tonus vasculaire. Le sepsis est connu pour altérer la microcirculation ainsi que la régulation de certains flux régionaux, pouvant mener à des manifestations localisées d'hypoperfusion et un dysfonctionnement d’organes, malgré un débit cardiaque préservé. Notre hypothèse est que le dysfonctionnement microvasculaire lié au sepsis, observé dans d'autres lits vasculaires, affecte également la circulation coronaire et en altère ses mécanismes de régulation du flux. La première partie de ce travail avait pour objectif de développer une méthode fiable, stable dans le temps et reproductible permettant d’évaluer les mécanismes intrinsèques de régulation du flux sanguin coronaire (l’autorégulation, la régulation métabolique et la fonction vasomotrice de l’endothélium). La deuxième partie de ce travail avait pour objectif d’évaluer l’hémodynamique du flux sanguin coronaire dans un modèle porcin de sepsis hyperdynamique. Quatorze porcs ont été randomisés en deux groupes :1) le groupe sepsis par péritonite fécale (n=7) et 2) le groupe contrôle (n=7). Une évaluation de l’hémodynamique générale et de l’hémodynamique coronaire a été réalisée au temps de base, 6 heures et 12 heures après l’induction de la péritonite fécale. Cardiogenic shock (CS) is a complex multifactorial clinical syndrome with extremely high mortality, developing as a continuum, and progressing from the initial insult (underlying cause) to the subsequent occurrence of organ failure and death. There is a large spectrum of CS presentations resulting from the interaction between an acute cardiac insult and a patients underlying cardiac and overall medical condition. Phenotyping patients with CS may have clinical impact on management because classification would support initiation of appropriate therapies. CS management should consider appropriate organization of the health care services, and therapies must be given to the appropriately selected patients, in a timely manner, whilst avoiding iatrogenic harm. Although several consensus-driven algorithms have been proposed, CS management remains challenging and substantial investments in research and development have not yielded proof of efficacy and safety for most of the therapies tested, and outcome in this condition remains poor. Future studies should consider the identification of the new pathophysiological targets, and high-quality translational research should facilitate incorporation of more targeted interventions in clinical research protocols, aimed to improve individual patient outcomes. Designing outcome clinical trials in CS remains particularly challenging in this critical and very costly scenario in cardiology, but information from these trials is imperiously needed to better inform the guidelines and clinical practice. The goal of this review is to summarize the current knowledge concerning the definition, epidemiology, underlying causes, pathophysiology and management of CS based on important lessons from clinical trials and registries, with a focus on improving in-hospital management.

Progress toward determining the true worth of ongoing practices or value of recent innovations can be glacially slow when we insist on following the conventional stepwise scientific pathway. Moreover, a widely accepted but flawed conceptual paradigm often proves difficult to challenge, modify or reject. Yet, most experienced clinicians, educators and clinical scientists privately entertain untested ideas about how care could or should be improved, even if the supporting evidence base is currently thin or non-existent. This symposium encouraged experts to share such intriguing but unproven concepts, each based upon what the speaker considered a logical but unproven rationale. Such free interchange invited dialog that pointed toward new or neglected lines of research needed to improve care of the critically ill.
