OBJECTIVE: Poor responders represent a frustrating condition for couples undergoing IVF and clinicians, and their treatment remains disputed. To assess the efficacy and the most suitable protocol, we conducted a randomized controlled trial comparing three different protocols of ovarian stimulation in poor responder women: clomiphene citrate (CC) plus a high dose of gonadotropins and GnRH antagonist, flexible GnRH antagonist protocol and a short GnRH agonist protocol. PATIENTS AND METHODS: Between July 2014 and December 2015 we enrolled 250 poor responders in a previous IVF cycle at least 3 months before. We divided into three groups: group A, 68 women treated with clomiphene citrate and FSH plus antagonist; Group B, 71 patients treated with FSH plus antagonist; Group C, 75 patients treated with FSH plus GnRH agonist. RESULTS: The GnRH agonist protocol showed a significantly higher pregnancy rate (29.3% vs. 5.9% vs. 14.1% respectively) than the clomiphene and the GnRH antagonist protocol, number of mature oocytes collected, estradiol levels and endometrial thickness. The cost of medications for each baby born was lower for the GnRH agonist protocol than for the others; the implantation rate was significantly lower in the clomiphene group (4.8%) than in the GnRH antagonist group (9.3%) and the GnRH agonist groups (19.2%). No significant differences emerged for total FSH administered, days of stimulation, numbers of oocytes retrieved and embryos transferred. CONCLUSIONS: This study demonstrates that short GnRH agonist protocol should be the first choice in poor responders; instead, clomiphene citrate should be avoided due to its very low success rate and high costs. This paper describes a rapid method for confirming residues of thyreostats in meat-based baby foods by using liquid chromatography - dual polarity electrospray - tandem mass spectrometry (LC-ES(±)-MS/MS). Six thioureylenes, belonging to the group of thiouracil and imidazole, were selected for this work: thiouracil (TU), methylthiouracil (MTU), propylthiouracil (PTU), phenylthiouracil (PhTU), mercaptobenzimidazole (MBI) and tapazole (TAP). The amphoteric nature of these compounds allows their electrospray detection in both positive and negative ionisation. Nevertheless, MS detection is not favoured by their low molecular weights, while their chromatographic retention is also thwarted by their high polarity. A pentafluorophenyl (PFP) core-shell phase column was selected to avoid peak asymmetry or peak splitting, and a dual-polarity ionisation method was optimised to obtain a sensitivity as high as possible. The method was validated according to the Commission Decision 657/2002/EC. A simple and fast procedure based on matrix solid phase dispersion (MSPD) was optimised to extract analytes from baby foods with recoveries exceeding 82%. Limit of decision (CCα) and detection capability (CCβ) were lower than the permissible maximum concentration (10 ng g -1 ). The validated method was then applied to assess the potential occurrence of the six selected thyreostats in nine commercial products. All the samples were found free of contamination.

The autoimmune lymphoproliferative syndrome (ALPS) is a disorder of abnormal lymphocyte survival caused by the dysregulation of the Fas apoptotic pathway. The Fas gene is expressed at the maternal-fetal interface and is involved in the regulation of immune response and implantation. Altered Fas expression may result in altered apoptosis and, ultimately, affect both the immune response and implantation; it is in fact associated with recurrent pregnancy loss, preterm premature rupture of membranes and pre-eclampsia. Currently, there are over 500 cases of ALPS reported worldwide from various racial and ethnic backgrounds. Up to date, the published work contains no specific reports on pregnancy outcome in women affected by ALPS. We present a case of full-term uneventful pregnancy in a patient affected by ALPS. A specific clinical follow-up in a pregnant woman with primary immunologic disease is suggested., (© 2014 The Authors. RON belongs to the c-MET family of receptor tyrosine kinases. As its well-known family member MET, RON and its ligand macrophage-stimulating protein have been implicated in the progression and metastasis of tumors and have been shown to be overexpressed in cancer. We generated and tested a large number of human monoclonal antibodies (mAbs) against human RON. Our screening yielded three high-affinity antibodies that efficiently block ligand-dependent intracellular AKT and MAPK signaling. This effect correlates with the strong reduction of ligand-activated migration of T47D breast cancer cell line. By cross-competition experiments, we showed that the antagonistic antibodies fall into three distinct epitope regions of the RON extracellular Sema domain. Notably, no inhibition of tumor growth was observed in different epithelial tumor xenografts in nude mice with any of the antibodies. These results suggest that distinct properties beside ligand antagonism are required for anti-RON mAbs to exert antitumor effects in vivo.
