Abstract

Central giant cell reparative granuloma (CGCRG) of the jaw is a neoformation localized in the mandible or in the maxillary bone and characterized by fibrous tissue, osteoclast-like giant cells and reactive bone formation. The CGCRG is a less frequent benign tumor but sometimes it is characterized by an aggressive behavior with a very rapid growth. It affects the young adults mainly and the children occasionally. Nowadays no medication therapy is approved for CGCRG in pediatric cases. We present a case of an aggressive form of a mandibular CGCRG in a 5-year child with Arnold-Chiari syndrome. This case is unique for the use of bisphosphonates (BPs) as neoadjuvant therapy in pediatrics. The therapy were administrated with the purpose of arresting the rapid growth of the tumor in order to avoid a demolitive surgery to the young patient. The child was without symptoms and presented an unusual swelling in the left mandible developed in a few weeks. The lesion was diagnosed by a CT scan and it was confirmed by a biopsy performed for histopathological assessment a few days later. The drug therapy consisted of seven cycles iv of Zoledronate associated to Calcium Gluconate. The child was closely observed through clinical and serological evaluations during the following months. About five months after the last cycle of BPs the child underwent CT scan and a conservative surgical treatment, consisted in a deep curettage, was programmed. Seven months after surgery the aesthetic profile of the patient improved and CT scan reported a significant calcic neoapposition in the area of the previous bone lesion. After more than one year from surgery, no relapse was observed. This case report demonstrates that BPs can be used safely in pediatric patients with CGCRG. Especially BPs could have a role as neoadjuvant therapy: If administrated before surgical treatment BPs avoid the necessity of resective surgery and reduce the risk of recurrence in pediatric CGCRG after conservative curettage. Abstract

Bisphosphonates are a category of drugs that are commonly used in dentistry and orthopedics to treatment of several bone disorders. The two most potent and widely used nitrogen-containing BPs are zoledronate and alendronate, which inhibit the intracellular mevalonate pathway. In recent years it has been observed Biphosphonate-associated osteonecrosis of the jaws which is a real complication of intravenous biphosphonates therapy in patients with cancer or osteoporosis. In our previous studies, performed on rat model, we observed mandibular bone characteristics after long term of low dose zoledronate treatment without tooth extraction or trauma; results showed several areas of bone with empty osteocyte lacunae, absence of matrix and presence of unorganized fibrillar structures but no spontaneous bone exposure has been observed. In the present study we have treated 20 rats with intraperitoneal injections of zoledronate at the lower dose for three times a week. After 30 days of treatment we applied trauma on mandibular bone by application of screw. Specimens were analysed by histological staining, immunofluorescence techniques and scanning electron microscopy. Our results show that the bone area of screw application is characterized of empty osteocyte lacunae, empty Volkmann’and Havers’canals and some inflammatory cells. Although the presence of small necrotic areas no bone exposure has been observed after low dose zoledronate treatment and trauma application. These results suggest us that the bisphosphonates-associated osteonecrosis of jaw is strictly correlated to drug’s dose. Abstract

The objective of our study is to explore the feasibility and utility of Diffusion Tensor Imaging (DTI) in the non invasive evaluation of any lesion [1] in Head and Neck structure. We would reach a verified and certified step of Virtual biopsy. Ten patients with different histological lesions were recruited for this study. Morphological and diffusion weighted images were acquired with a 3T scanner during last 3 months. Probabilistic tensor-based tractography reconstruction of any lesions was performed and mean fractional anisotropy (FA) values for everyone were extracted [2]. The results showed that the Diffusion Tensor Imaging (DTI) was able to identify the lesion geometrical morphology and diffusion microstructural changes. The patient with a benign lesion reported a significant improvement in ADC mean values. We have evidence that pushes us to continue with this method. We are at the beginning of this work. Abstract

Bisphosphonates (BP) are stable analogues of pyrophosphate with a P-C-P structure and 2 side chains attached to the carbon atom. Intravenous bisphosphonates are primarily used and effective in the management of cancer-related conditions in the context of solid tumors, such as breast cancer, prostate cancer, and lung cancer. Moreover bisphosphonates are subministred to patients with metabolic bone disease such as osteoporosis and Paget disease. Bisphosphonate-associated osteonecrosis of the jaws (BONJ) is a really complication of intravenous bisphosphonates therapy in patients with cancer. It is common knowledge that the jaws have a greater blood supply than other bones and a fasterbone turnover rate, related both to their daily activity and presence of teeth which mandates daily bone remodeling around the periodontal ligament; moreover bisphosphonates toxicity to epithelial cells has been well documented. On this basis, the aim of this experimental study is to evaluate the pathological changes of the mandibular bone and oral mucosa in rat treated with bisphosphonates. In details we have analyzed, by immunoistochemical and scanning electron microscopic methods, biopsy of mandibular bone and of gingival mucosa in rat treated with bisphosphonates after 7, 15, 30, 45, 60 days of assumption of drugs and after 7 and 30 days from the end of the treatment. Our results show great area demineralization bone mixed to normal bone, moreover in the demineralization bone it’s possible to observe numerous micro lacunae. In the correspondence samples of gingival epithelium we observe changes of histological structure and the disappearance of protein adhesion system cells to cells and cells to matrix. On this basis it is intriguing to speculate that the adverse effects of BP on oral epithelium may play a critical role in the initiation of BONJ an “outside-in” hypothesis., Italian Journal of Anatomy and Embryology, Vol 119, No 1 (Supplement) 2014

Abstract

Lymphangioma is a benign, hamartomatous tumor of the lymphatic channels. It is usually found in the head and neck region. Most lymphangiomas are present at birth and are usually diagnosed in infancy or early childhood. When lymphangioma occurs in the mouth, the anterior two thirds of the tongue is the most commonly affected region. A risk of secondary growth is classically described after surgical reduction. In this paper, we present 9 cases of tongue lymphangioma. We provide a literature review of tongue lymphangioma.
