2. Forecasting intracranial hypertension using multi-scale waveform metrics

Abstract

Objective: Acute intracranial hypertension is an important risk factor of secondary brain damage after traumatic brain injury. Hypertensive episodes are often diagnosed reactively, leading to late detection and lost time for intervention planning. A pro-active approach that predicts critical events several hours ahead of time could assist in directing attention to patients at risk. Approach: We developed a prediction framework that forecasts onsets of acute intracranial hypertension in the next 8 hours. It jointly uses cerebral auto-regulation indices, spectral energies and morphological pulse metrics to describe the neurological state of the patient. One-minute base windows were compressed by computing signal metrics, and then stored in a multi-scale history, from which physiological features were derived. Main results: Our model predicted events up to 8 hours in advance with alarm recall rates of 90% at a precision of 30.3% in the MIMIC-III waveform database, improving upon two baselines from the literature. We found that features derived from high-frequency waveforms substantially improved the prediction performance over simple statistical summaries of low-frequency time series, and each of the three feature classes contributed to the performance gain. The inclusion of long-term history up to 8 hours was especially important. Significance: Our results highlight the importance of information contained in high-frequency waveforms in the neurological intensive care unit. They could motivate future studies on pre-hypertensive patterns and the design of new alarm algorithms for critical events in the injured brain., Comment: 11 pages, 5 figures

Abstract

This paper presents a novel approach for multi-lingual sentiment classification in short texts. This is a challenging task as the amount of training data in languages other than English is very limited. Previously proposed multi-lingual approaches typically require to establish a correspondence to English for which powerful classifiers are already available. In contrast, our method does not require such supervision. We leverage large amounts of weakly-supervised data in various languages to train a multi-layer convolutional network and demonstrate the importance of using pre-training of such networks. We thoroughly evaluate our approach on various multi-lingual datasets, including the recent SemEval-2016 sentiment prediction benchmark (Task 4), where we achieved state-of-the-art performance. We also compare the performance of our model trained individually for each language to a variant trained for all languages at once. We show that the latter model reaches slightly worse - but still acceptable - performance when compared to the single language model, while benefiting from better generalization properties across languages., Comment: appearing at WWW 2017 - 26th International World Wide Web Conference

Abstract

Transesophageal echocardiography (TEE) is widely used in cardiac surgery and interventional cardiology and is often an indispensable tool, giving supportive anatomical understanding and smooth guidance in both settings. Despite it being considered safe, fatal complications can commonly occur after a TEE examination in cardiac surgery operating rooms and catheterization laboratories. Currently, there is a lack of awareness of the scale of the problem, as there are only small amounts of data available, mainly derived from the surgical literature. This review summarizes the main predisposing factors for TEE-associated complications (classified as patient and procedure-related) and the main preventive strategies. We aim to apply preventive strategies more broadly, especially to patients at high risk of developing TEE-related serious adverse events. [ABSTRACT FROM AUTHOR]

Abstract

Context.--Molecular testing is essential for the diagnostic workup of patients with advanced non-small cell lung cancers. Cytology specimens from minimally invasive procedures, such as endobronchial ultrasound-guided transbronchial needle aspiration, are often the only available samples for these patients. The implementation of molecular diagnostic testing, and in particular next-generation sequencing-based testing, on these cytologic specimens is currently an evolving field for lung cytopathology. The application of these molecular analyses on tyrosine kinase inhibitor-resistant non-small cell lung cancers raises unique technical, biologic, and clinical challenges. Objective.--To provide an overview of the implementation of next-generation sequencing analysis on endobronchial ultrasound-guided transbronchial needle aspiration samples to detect the molecular aberrations underneath the phenomenon of acquired resistance in patients with non-small cell lung cancers progressing while on the EGFR/ALK tyrosine kinase inhibitor treatment. Data Sources.--Peer-reviewed original articles, review articles, and published guidelines and expert opinion reports were reviewed, together with our single-center experience. Abstract

International audience; Est-il possible de « sémiotiser » la danse ? En quoi consisterait cette opération de la pensée ? Quelle serait la contribution que la sémiotique peut apporter aux savoirs chorégraphiques ? Cet ouvrage vise à répondre à ces questions à partir du cas d’une danse sociale, le tango argentin.Le caractère sémiotique de la danse, à savoir sa capacité de produire et de stabiliser des configurations de sens spécifiques, s’est avéré problématique dans la tradition structurale aussi bien du point de vue théorique que méthodologique. Pourtant, en dépit de la qualité éphémère du geste dansant, la variété et les attestations de ses manifestations sémiotiques ne font nul doute. Le tango argentin est dans ce sens emblématique, car il se déploie sur plusieurs niveaux d’appréhension sémiotique: le corps dansant, des pratiques sociales, des institutions, des imaginaires culturels et identitaires.Dans cette perspective, l’ouvrage analyse le tango argentin à partir de la constitution de l’acte de danse en ceci qu’il est indissociable de la pratique du bal. Ainsi, il propose une approche sémiogénétique du tango, qui tient compte de ses multiples niveaux de manifestation et de leur dynamique. Du point de vue méthodologique, l’imbrication des traditions post-structurales et morphogénétiques permet de concevoir un modèle plus général du geste dansant qui puisse s’appliquer à d’autres danses à vocation « pratique ». C’est ainsi qu’il faut comprendre la suite gestes, formes, sens: une trajectoire qui décrit la sémiose de ses phases de germination à sa stabilisation et sa transmission culturelle.L’ouvrage est accompagné d’une annexe iconographique du fonds « Carlos Vega », ethnomusicologue argentin et pionnier d’une approche analytique du tango argentin basée sur la notion de « forme chorégraphique ». Les images issues de ce fonds sont inédites dans l’espace francophone européen.

Abstract

Dans cette contribution, nous souhaitons mettre en relief le rôle joué par la matérialité dans la constitution des formes sémiotiques. La notion de matérialité dépasse les aspects relatifs à la constitution de la face signifiante des signes pris isolément, et oriente l’attention vers l’agentivité des matériaux à l’œuvre dans tout type d’interaction symbolique. Ainsi, la réflexion sur les formes sémiotiques inclut d’autres processus et «objets» sémiotiques, allant du vivant jusqu’aux sémioses anthropiques. Dans ce cadre, nous nous attarderons sur la relation entre d’un côté, la notion d’«hyperobjet», élaborée par le philosophe Timothy Morton dans l’enceinte du matérialisme et du réalisme spéculatif à visée écologique, et de l’autre, celle de «constructionde niche», attestée dans les recherches en anthropologie évolutionniste et en biosémiotique.

Abstract

Traducción de Lorena Ventura Ramos; International audience; Las relaciones epistemológicas entre los sistemas complejos y dinámicos y la comprensión de la complejidad semiótica/semiolingüística y antropológica, propiamente humana, se han convertido en una necesidad incontestable. La cuestión de la transferencia terminológica y la transposición de recursos descriptivos de las ciencias físicas y químicas al estudio de los comportamientos sociales y simbólicos suscitó dudas sobre la legitimidad de la transposición misma, pero también sobre la eficacia de los enfoques tomados de las ciencias naturales para describir las interacciones socio-simbólicas y las formas semióticas complejas. Recientemente, el punto de inflexión ecológico en las ciencias de la cultura se ha distanciado de esta problemática y ha resaltado la importancia de una perspectiva pluralista y unificadora en relación con los modos de descripción y objetivación de los fenómenos estudiados. De hecho, un enfoque ecológico sobre un fenómeno cultural dado pretende la integración de una pluralidad de descripciones heterogéneas provenientes de diferentes estrategias disciplinares; al hacerlo, tal enfoque aspira a recuperar notanto la sistematicidad interna del fenómeno como sus líneas de desarrollo y cambio dentro de sistemas de organización cada vez más ricos, esferas de existencia y evolución que están siempre abiertas al cambio, la variación y el reequilibrio. Se podría añadir incluso que los enfoques ecológicos en las ciencias de la cultura han tematizado explícitamente la idea de una complejidad de la vida humana y sus formas, enfatizando principalmente la idea de una interacción constante y no lineal con el medio ambiente y los entornos habitados. Se desprende de aquí una consecuencia importante concerniente a este estudio sobre el comportamiento simbólico: el cuestionamiento epistemológico sobre los rolespor asumir en las estrategias epistemológicas y descriptivas por muchos conceptos, tales como forma, socialidad, incorporación, fluctuación y medialidad.
