Hypopharyngeal liposarcomas are extremely rare. Due to the lack of experience, pathologists and surgeons find it difficult to make a clear diagnosis and provide accurate, timely treatment. A 43-year-old man with a complaint of foreign body sensation in the throat for 6 months and swallowing difficulty for 2 months was admitted to our department. Contrast-enhanced computed tomography of the chest and larynx revealed a lesion with smooth surface in the esophagus, connected by a pedicle to the hypopharyngeal lesion. The same result was found by gastroscopy. Lateral pharyngotomy was performed for tumor removal, and after 3 weeks, the patient showed good movement of bilateral vocal cords, without dysphagia, choking cough, or hoarseness. This is the first case report of hypopharyngeal liposarcoma invading the esophagus. A thorough preoperative evaluation may be required for the proper diagnosis, prevention, and treatment.

Ideally, orthognathic surgery is indicated to treat skeletal Class III malocclusion with severe open bite in adults. In this borderline case, however, camouflage treatment without any skeletal anchorage was chosen. This report describes the orthodontic treatment of a 23-year-old man who presented with multiple orthodontic problems including severe open bite of the anterior and posterior teeth up to the first molars, bilateral posterior crossbite, bilateral Class III molar relationship, severe crowding, increased vertical dimension, and dental and facial midline deviation. The treatment included the extraction of four wisdom teeth, uprighting and distalisation of the mandibular arch with molar intrusion using curved NiTi wires with intermaxillary elastics. After 36 months of treatment, satisfactory improvements in the vertical overlap, horizontal overlap and sagittal malocclusion were achieved. The design of the mechanical system used in this case confirmed stable results at the 4-year follow-up.

Effects of parameters including temperature, time and coal ratio on the reduction behaviour of preheated iron-bearing converter sludge pellets in a simulated rotary kiln are studied through orthogonal tests. ANOVA analyses show that reduction time and temperature have remarkable influence on the metallisation degree, occupying 55.02 and 30.08% of the total contribution, while temperature is the most significant factor affecting the compressive strength, with 90.98% contribution. The metallisation degree increases with the increasing time from 1.5 to 2.5 h, and first increases and then decreases with the increasing temperature from 1000 to 1100°C. The compressive strength increases with the increasing temperature. Under the optimal condition of temperature 1050°C, time 2.5 h, coal ratio 1.3, the metallisation degree is 72.92%, and the compressive strength is 1310 N/p, which satisfies the requirement of iron burden for blast furnace.

Background The current published prognosis models for brain metastases (BMs) from cancer have not addressed the issue of either newly diagnosed non–small-cell lung cancer (NSCLC) with BMs or the lung cancer genotype. We sought to build an adjusted prognosis analysis (APA) model, a new prognosis model specifically for NSCLC patients with BMs at the initial diagnosis using adjusted prognosis analysis (APA). Patients and Methods The model was derived using data from 1158 consecutive patients, with 837 in the derivation cohort and 321 in the validation cohort. The patients had initially received a diagnosis of BMs from NSCLC at Sun Yat-Sen University Cancer Center from 1994 to 2015. The prognostic factors analyzed included patient characteristics, disease characteristics, and treatments. The APA model was built according to the numerical score derived from the hazard ratio of each independent prognostic variable. The predictive accuracy of the APA model was determined using a concordance index and was compared with current prognosis models. The results were validated using bootstrap resampling and a validation cohort. Results We established 2 prognostic models (APA 1 and 2) for the whole group of patients and for those with known epidermal growth factor receptor (EGFR) genotype, respectively. Six factors were independently associated with survival time: Karnofsky performance status, age, smoking history (replaced by EGFR mutation in APA 2), local treatment of intracranial metastases, EGFR-tyrosine kinase inhibitor treatment, and chemotherapy. Patients in the derivation cohort were stratified into low- (score, 0-2), moderate- (score, 3-5), and high-risk (score 6-7) groups according to the median survival time (16.6, 10.3, and 5.2 months, respectively; P Conclusion Compared with recursive partition analysis and graded prognostic assessment, APA seems to be more suitable for initially diagnosed NSCLC with BMs.

Two new guaiane sesquiterpenes (1⿿2), one new acorane sesquiterpene (3), along with two known sesquiterpenes (4⿿5) were isolated from high quality agarwood ⿿Qi-Nan⿿ originating from Aquilaria sinensis. Their structures were established based on spectroscopic methods including IR, HRESIMS, 1D, and 2D NMR. Compound 1 was a guaiane sesquiterpene lactone, and compound 2 was a guaiane sesquiterpene with a 5/6/7 ring carbon skeleton. Compounds 3 and 4 were acorane-type sesquiterpenes. The two acorane sesquiterpenes (3⿿4) possess lasting fragrant smell at room temperature. These isolated guaiane and acorane sesquiterpenes showed acetylcholinesterase inhibitory activities in different degrees ranged from 19% to 44% at the concentration of 50 μg/mL.

Identification of the anaplastic lymphoma kinase (ALK) gene has refined the classification of non‐small cell lung cancer (NSCLC) and promoted research on molecularly targeted drugs such as crizotinib, an ALK inhibitor with good efficacy, in ALK‐rearranged NSCLC. At present, few studies have reported the efficacy of crizotinib in patients with ALK‐rearranged NSCLC with brain metastases. In a patient with NSCLC harboring ALK‐rearrangement who had brain metastases and poor performance status (PS), we obtained a durable response with crizotinib administered following multi‐line chemotherapy regimens.

Five new 5,11-epoxyguaiane sesquiterpenes ( 1 – 5 ), along with a known one ( 6 ) were isolated from high quality agarwood ‘Qi-Nan’ originating from Aquilaria sinensis . Their structures were established based on spectroscopic methods including IR, HR-ESI-MS, 1D, and 2D NMR. Compounds 1 – 6 were all 5,11-epoxyguaiane sesquiterpenes, sharing the same tetrahydrofuran ring by etherification of C-5 and C-11, and furthermore, functionalized at C-14 in the guaiane skeleton. All the six 5,11-epoxyguaianes ( 1 – 6 ) possess lasting fragrant smell at room temperature. This type of 5,11-epoxyguaiane sesquiterpenes were found to exist in agarwood ‘Qi-Nan’ only originating from A. sinensis rather than other species of the genus Aquilaria . These isolated guaiane sesquiterpenes showed acetylcholinesterase inhibitory activities in different degrees ranged from 15% to 63% at the concentration of 50 μg/mL.

In order to improve the attenuation frequency range of traditional static two-stage H-type hydraulic muffler, a two-stage follow-up hydraulic muffler driven by gear-screw is designed based on PLC. The Muffler can change its structure parameters according to the rotational speed of the hydraulic pump, and attenuate the two pulsation frequencies of the hydraulic pump. Considering the structural parameters and gear transmission of the muffler, the static and dynamic characteristics of the muffler are analyzed systematically, and the stability of the muffler is verified by the case simulation test. The results show that the two-stage follow-up hydraulic muffler based on PLC can attenuate the two pressure pulsation frequencies at the same time, and the operation is stable and the response speed is fast. The two-stage follow-up hydraulic muffler has practical significance and value to provides a way for engineering hydraulic system noise reduction.

The pulsating flow output of plunger pump will produce pressure pulsation after flowing through the pipeline. Using hydraulic muffler to attenuate the pressure pulsation of the pipeline is an economical, reliable and easy to implement method. Intelligent follow-up hydraulic muffler, which adapts to pressure pulsation changing at any time by changing its own parameters, has become a research hotspot. Intelligent follow-up hydraulic muffler can accurately and effectively attenuate the pressure pulsation of the system and maximize the attenuation performance of the hydraulic muffler. The structure and performance of the existing intelligent follow-up hydraulic muffler are mainly introduced. The general methods, principles, advantages and disadvantages of intelligent follow-up hydraulic muffler design are summarized. On this basis, the development trend of intelligent hydraulic muffler is predicted and prospected.

To investigate the clinical significance of peripheral blood NK cell number change in patients with mantle cell lymphoma.Eight-four patients with mantle cell lymphoma treated in our hospital from November 2003 to November 2011 were studied, the venous blood was collected from all patients and detected with flow cytometry. The age, sex, pathologic type, B-symptoms, clinical stage, absolute NK count(ANKC), hemoglobin(Hb), lactate dehydrogenase(LDH) and β2-microglobulin(β2-MG) levels, bone marrow involuement(BMI) and the international prognostic index of mantle cell lymphoma(MIPI) were recorded. All patients were followed up.There were no significant differences in ANKC among different age, sex, B symptom, Ann Arbor stages, Hb, LDH and β2-MG levels, BMI and MIPI of patients with MCL(P0.05). The sensitivity and specificity of ANKC were 76.6% and 73.8%, respectively. The optimal throshold of ANKC was 0.10×10The ANKC in peripheral blood has an important value for judging the prognosis of patients with MCL and can be used as an important index to judge the disease status of patients with MCL.

A boosted convolutional neural network (BCNN) system is proposed to enhance the pedestrian detection performance in this work. Being inspired by the classic boosting idea, we develop a weighted loss function that emphasizes challenging samples in training a convolutional neural network (CNN). Two types of samples are considered challenging: 1) samples with detection scores falling in the decision boundary, and 2) temporally associated samples with inconsistent scores. A weighting scheme is designed for each of them. Finally, we train a boosted fusion layer to benefit from the integration of these two weighting schemes. We use the Fast-RCNN as the baseline, and test the corresponding BCNN on the Caltech pedestrian dataset in the experiment, and show a significant performance gain of the BCNN over its baseline.
