- Abstract
Background: Lombardy was severely hit by the COVID-19 pandemic since February 2020 and the Health System underwent rapid reorganization. Outpatient clinics were stopped for non-urgent patients: it became a priority to manage hundreds of fragile neurological patients who suddenly had less reference points. In Italy, before the pandemic, Televisits were neither recognized nor priced. Methods: At the Fondazione IRCCS Istituto Neurologico C. Besta, we reorganized outpatient clinics to deliver Neuro-telemedicine services, including Televisits and Teleneurorehabilitation, since March 2020. A dedicated Working Group prepared the procedure, tested the system, and designed satisfaction questionnaires for adults and children. Results: After a pilot phase, we prepared a procedure for Telemedicine outpatient clinics which was approved by hospital directions. It included prescription, booking, consenting, privacy and data protection, secure connection with patients (Teams Microsoft 365), electronic report preparation and delivery, reporting, and accountability of the services. During the March–September 2020 period, we delivered 3167 Telemedicine services, including 1618 Televisits, to 1694 patients (972 adults, 722 children) with a wide range of chronic neurological disorders. We successfully administered different clinical assessment and scales. Satisfaction among patients and caregivers was very high. Conclusions: During the dramatic emergency, we were able to take care of more than 1600 patients by organizing Neuro-telehealth in a few weeks, lessening the impact of the pandemic on fragile patients with chronic neurological disorders; this strategy is now stably embedded in our care pathways. In Italy, Telehealth is at present recognized and priced and is becoming a stable pillar of the health system.

Few data are available on the ICU management and on the possible respiratory complications of invasively ventilated pregnant patients affected by COVID-19 pneumonia, especially in the early phase of pregnancy. Tension pneumothorax has been previously described as a rare cause of respiratory failure after delivery, but its occurrence in the postpartum of COVID-19 patient has not been reported yet. We hereby describe the ICU management of a 23rd gestational week pregnant woman who underwent invasive mechanical ventilation, prone positioning, and cesarean delivery during her ICU stay for COVID-19 related pneumonia. Moreover, we focused on the occurrence and management of recurrent tension pneumothorax after the cesarean delivery.A 23rd gestational week pregnant woman was admitted to the ICU for a COVID-19 bilateral pneumonia and underwent invasive mechanical ventilation and prone positioning. Cesarean delivery was planned during the ICU stay, while the patient was receiving invasive mechanical ventilation. After delivery, the patient experienced a recurrent pneumothorax that required the positioning of multiple chest drains.In pregnant critically ill COVID-19 patients, mechanical ventilation management is particularly challenging, especially in the postpartum period. Prone positioning is feasible and can improve oxygenation and respiratory system compliance, while tension pneumothorax must be suspected if the respiratory function suddenly deteriorates after delivery.

Aggregated alpha-synuclein (α-syn) is a principal constituent of Lewy bodies (LBs) and glial cytoplasmic inclusions (GCIs) observed respectively inside neurons in Parkinson’s disease (PD) and oligodendrocytes in multiple system atrophy (MSA). Yet, the cellular origin, the pathophysiological role, and the mechanism of formation of these inclusions bodies (IBs) remain to be elucidated. It has recently been proposed that α-syn IBs eventually cause the demise of the host cell by virtue of the cumulative sequestration of partner proteins and organelles. In particular, the hypothesis of a local cross-seeding of other fibrillization-prone proteins like tau or TDP-43 has also been put forward. We submitted sarkosyl-insoluble extracts of post-mortem brain tissue from PD, MSA and control subjects to a comparative proteomic analysis to address these points. Our studies indicate that: i) α-syn is by far the most enriched protein in PD and MSA extracts compared to controls; ii) PD and MSA extracts share a striking overlap of their sarkosyl-insoluble proteomes, consisting of a vast majority of mitochondrial and neuronal synaptic proteins, and (iii) other fibrillization-prone protein candidates possibly cross-seeded by α-syn are neither found in PD nor MSA extracts. Thus, our results (i) support the idea that pre-assembled building blocks originating in neurons serve to the formation of GCIs in MSA, (ii) show no sign of amyloid cross-seeding in either synucleinopathy, and (iii) point to the sequestration of mitochondria and of neuronal synaptic components in both LBs and GCIs.

Background Cardiometabolic disorders may worsen Covid-19 outcomes. We investigated features and Covid-19 outcomes for patients with or without diabetes, and with or without cardiometabolic multimorbidity. Methods We collected and compared data retrospectively from patients hospitalized for Covid-19 with and without diabetes, and with and without cardiometabolic multimorbidity (defined as ≥ two of three risk factors of diabetes, hypertension or dyslipidaemia). Multivariate logistic regression was used to assess the risk of the primary composite outcome (any of mechanical ventilation, admission to an intensive care unit [ICU] or death) in patients with diabetes and in those with cardiometabolic multimorbidity, adjusting for confounders. Results Of 354 patients enrolled, those with diabetes (n = 81), compared with those without diabetes (n = 273), had characteristics associated with the primary composite outcome that included older age, higher prevalence of hypertension and chronic obstructive pulmonary disease (COPD), higher levels of inflammatory markers and a lower PaO2/FIO2 ratio. The risk of the primary composite outcome in the 277 patients who completed the study as of May 15th, 2020, was higher in those with diabetes (Adjusted Odds Ratio (adjOR) 2.04, 95%CI 1.12–3.73, p = 0.020), hypertension (adjOR 2.31, 95%CI: 1.37–3.92, p = 0.002) and COPD (adjOR 2.67, 95%CI 1.23–5.80, p = 0.013). Patients with cardiometabolic multimorbidity were at higher risk compared to patients with no cardiometabolic conditions (adjOR 3.19 95%CI 1.61–6.34, p = 0.001). The risk for patients with a single cardiometabolic risk factor did not differ with that for patients with no cardiometabolic risk factors (adjOR 1.66, 0.90–3.06, adjp = 0.10). Conclusions Patients with diabetes hospitalized for Covid-19 present with high-risk features. They are at increased risk of adverse outcomes, likely because diabetes clusters with other cardiometabolic conditions.

- Abstract
Background and Purpose - As a reliable scoring system to detect the risk of symptomatic intracerebral hemorrhage after thrombectomy for ischemic stroke is not yet available, we developed a nomogram for predicting symptomatic intracerebral hemorrhage in patients with large vessel occlusion in the anterior circulation who received bridging of thrombectomy with intravenous thrombolysis (training set), and to validate the model by using a cohort of patients treated with direct thrombectomy (test set). Methods - We conducted a cohort study on prospectively collected data from 3714 patients enrolled in the IER (Italian Registry of Endovascular Stroke Treatment in Acute Stroke). Symptomatic intracerebral hemorrhage was defined as any type of intracerebral hemorrhage with increase of ≥4 National Institutes of Health Stroke Scale score points from baseline ≤24 hours or death. Based on multivariate logistic models, the nomogram was generated. We assessed the discriminative performance by using the area under the receiver operating characteristic curve. Results - National Institutes of Health Stroke Scale score, onset-to-end procedure time, age, unsuccessful recanalization, and Careggi collateral score composed the IER-SICH nomogram. After removing Careggi collateral score from the first model, a second model including Alberta Stroke Program Early CT Score was developed. The area under the receiver operating characteristic curve of the IER-SICH nomogram was 0.778 in the training set (n=492) and 0.709 in the test set (n=399). The area under the receiver operating characteristic curve of the second model was 0.733 in the training set (n=988) and 0.685 in the test set (n=779). Conclusions - The IER-SICH nomogram is the first model developed and validated for predicting symptomatic intracerebral hemorrhage after thrombectomy. It may provide indications on early identification of patients for more or less postprocedural intensive management.

- Abstract
With the unexpected appointment of Giovanni Battista Montini as archbishop of Milan, the meeting began between one of the main Church personalities and a great industrial metropolis in profound transformation for the "economic boom". The archbishop immediately warned that, despite the town's entrenched Catholic tradition, Milan was hit by a great "religious question": the removal of the masses from the faith. For Montini it was necessary to renew the pastoral care to restore a religious sense both to "distant" strangers to the life of the parishes and to the faithful "neighbors" but turned in a bourgeois Christianity. The City Mission of 1957 in this sense represented a relevant event. Milan was considered a "mission land" to the warmed Catholics and the working masses of the suburbs, to immigrants from southern Italy and the "low" peasants.

Background and aim: PAC-SYM is widely adopted to asses constipation severity. However, it has been validated in a small sample, few items have been included based on expert opinion and not on empirical grounds, and its factor structure has never been replicated. We aimed at evaluating the psychometric properties of PAC-SYM in patients with chronic constipation. Methods: We enrolled 2,203 outpatients with chronic constipation in two waves. We used wave I sample to test the psychometric properties of the PAC-SYM and wave II sample to cross-validate its factor structure, to assess criterion validity, responsiveness to clinical change, and its minimal clinically important difference. Results: Only a minority of patients reported any rectal tearing (38 %). Deletion of such item leads to a 11-item version (M:PAC-SYM). The remaining items in the rectal domain were moderately correlated with the stool domain. Exploratory factor analysis and confirmatory factor analysis revealed a bifactor structure with two subscales (stool and abdominal symptoms) and a general severity factor. The M:PAC-SYM demonstrated excellent reliability, moderate correlation with SF-12 and treatment satisfaction (r = 0.28–0.45), discrimination across Rome III criteria for functional constipation and abdominal pain, and responsiveness to clinical change (β = −0.49; ω 2 = 0.25). M:PAC-SYM minimal clinically important difference was 0.24. Conclusion: Our analysis shows that the rectal domain may not represent a relevant cluster of symptoms for patients with chronic constipation. We developed a modified version of the PAC-SYM which might better represent symptom severity of most patients seeking care in gastroenterology referral centers

- Abstract
Background: The management of bloating is unclear and its relationship with patients' well-being and treatment satisfaction independent of other abdominal symptoms is uncharacterized. We evaluated the association of bloating with patient-reported outcomes. Methods: Thirty-nine centers for functional gastrointestinal disorders joined the laxative inadequate relief survey. We enrolled 2203 consecutive outpatients with functional constipation (FC) or constipation-predominant irritable bowel syndrome (IBS-C) in two cross-sectional waves. Both wave 1 and 2 included the SF-12, the patient assessment of constipation-symptoms (PAC-SYM), and the treatment satisfaction questionnaire for medication (TSQM-2). Wave 2 only included a global rating of change (GRC) scale to assess patients' assessment of efficacy concerning treatment switches occurred in the 3 months prior to the interview. Bloating in the abdomen was defined on the basis of PAC-SYM item 3. Key Results: The average age was 50.1 years (SD, 16.7) and 82.1% of patients were women. The prevalence of bloating was 91.6% (n = 1970). Bloating was associated with SF-12 Physical Composite Score (p < 0.01), SF-12 Mental Composite Score (p < 0.01), GRC (p < 0.01), Satisfaction with treatment effectiveness (p < 0.01), convenience of administration (p < 0.01), and side effects (p < 0.01) after adjustment for possible confounders. Conclusions & Inferences: Our data suggest that patients regard bloating as a key element in assessing clinical changes and treatments' efficacy as this symptom exerts a strong influence on patient-reported outcomes independent of possible confounders and other symptoms of constipation. Our data provide the rationale to investigate the efficacy and tolerability of new treatments specifically addressing this important, yet disregarded, patients' complain.

Background and aim: Diverticular disease represents the first most important gastrointestinal disease in Western countries. The vast ma- jority of patients experience symptomatic uncomplicated diverticular disease (SUDD), whose most frequent abdominal symptoms are pain and bloating. No specific standard treatment for SUDD is accepted, guidelines of the Am College Gastroent suggest a high–fibre diet in SUDD pts. A possi- ble role of gut microflora in determining symptoms related to SUDD is suggested, thus these pts may benefit from probiotics treatment. Aim: to assess the efficacy of Lactobacillus paracasei F19 in SUDD pts Material and methods: Multicentric randomized controlled study on 50SUDD pts, aged 47-78 years (18 M, 32 F) diagnosed by double contrast enema and biochemistry to exclude acute inflammation. SUDD was defined as: presence of abdominal symptoms (pain < 24h and bloating) for at least 6 months before enrollment. All pts had a validated detailed questionnaire for abdominal symptoms and functional dyspep- sia (FD) including a visual analogic scale (VAS). Exclusion criteria: complicated diverticular disease, previous colon surgery, antibiotics or laxatives 30 days before enrollment, use of NSAIDs. In all pts a daily intake of 30 g dietary fibre was recommended. Pts were randomly assigned to receive: 1 sachet b.i.d of L. paracasei F19 for 14 days/month for 6 months (Group A, n=16), only fibre diet (Group B, n=16) or 1 sachet once daily of L. paracasei F19 for 14 days/month for 6 months (Group C, n=18). Endpoint of the study was to evaluate the decrease of abdominal pain and bloating after 6 months of treatment. Results expressed as mean ± SD. Results: At baseline, 40 pts had both abdominal pain and bloating, whereas 10 pts had only bloating. 43 (86%) patients completed the study: 1 patient (Group A) was withdrawn for diarrhea and 6 patients (2 from Group A, 1 from Group B, 3 from Group C) were lost at follow-up. A significant decrease of bloating VAS was observed in group A and C whereas of pain VAS in group C. Conclusions: Six-months treatment with L. paracasei F19 significantly decreases the score of abdominal bloating in patients with SUDD. The higher dosage of probiotics is able to also reduce the score of abdominal pain. Thus, treatment with L. paracasei F19 is more efficacious than dietary fibres in controlling abdominal symptoms in SUDD patients.

Background and aim: Irritable bowel syndrome (IBS) is characterized by disturbed intestinal motility and sensations. Modulation of intestinal microflora has the potential to target both the disturbances but, to date, few studies have evaluated the efficacy and safety of probiotic administration in IBS. The aim of this study was to evaluate the effect of a symbiotic preparation (Bifidobacterium longum W11 5 × 10 9 viable bacteria + fructooligosaccharide –FOS- 2.5 gr per sachet) in IBS patients with constipation. Material and methods: A total of 30 IBS patients with constipation (22 women, age 18-46 years) according to the Roma III criteria were en- rolled. Patients were randomized into 3 groups receiving 2 or 6 sachets of the symbiotic plus 5 g of partially hydrolyzed guar gum (PHGG), or 5 g of PHGG5 alone, respectively. A daily visual analog score-based questionnaire, investigating the severity of abdominal pain, bloating and the feeling of incomplete evacuation, was administered during 2 run-in weeks and 4 treatment weeks, respectively; also, the number of weekly evacuation and the laxative consumption were considered. At baseline and at the end of the treatment the Bristol scale, the colonic transit time and the SF-36 were also evaluated. Results: At the end of the study, none of the patients reported ad- verse effects. Regardless of the treatment there was no significant improvement of abdominal pain or feeling of incomplete evacuation. A significant reduction of bloating severity was observed in subjects receiving 2 sachets of the symbiotic (4 ± 2vs1.6 ± 1 mm, p=0.01) compared to those receiving 6 or fibers alone (3 ± 2vs2 ± 1and2 ± 1 vs 1.5 ± 1 mm, respectively p=NS). The treatment regimens were similarly and significantly effective in improving frequency of defecation, colonic transit time and the stool consistency, as measured by the Bristol scale, whereas no significant improvement of both mental and physical component of the SF-36 questionnaire was observed. Conclusions: Our data demonstrate the utility of symbiotics over fibres in improving the severity of abdominal bloating in patients with IBS with constipation

Il virus della Diarrea Virale del bovino BVD/MD, è un RNA virus del genere Pestivirus, famiglia Flaviviridae. Il BVD/MD, come la maggior parte dei virus a RNA, ha un elevato tasso di evoluzione che dipende dall'infedeltà della RNA polimerasi-RNA dipendente; come conseguenza del suo elevato tasso di mutazione, il BVDV produce varianti a diversa patogenicità ed antigenicità, che complicano la diagnosi e il controllo della malattia. In questo lavoro usiamo una tecnica di screening genetico, l'SSCP (single-strand conformation polymorphism), ai fini di evidenziare la presenza di varianti virali con mutazioni nel gene che codifica la proteina immunodominante E2.

A multi-centre study has been conducted, during 2005, by means of a questionnaire posted on the Italian Society of Emergency Medicine (SIMEU) web page. Our intention was to carry out an organisational and functional analysis of Italian Emergency Departments (ED) in order to pick out some macro-indicators of the activities performed. Participation was good, in that 69 ED (3,285,440 admissions to emergency services) responded to the questionnaire.The study was based on 18 questions: 3 regarding the personnel of the ED, 2 regarding organisational and functional aspects, 5 on the activity of the ED, 7 on triage and 1 on the assessment of the quality perceived by the users of the ED.The replies revealed that 91.30% of the ED were equipped with data-processing software, which, in 96.83% of cases, tracked the entire itinerary of the patient. About 48,000 patients/year used the ED: 76.72% were discharged and 18.31% were hospitalised. Observation Units were active in 81.16% of the ED examined. Triage programmes were in place in 92.75% of ED: in 75.81% of these, triage was performed throughout the entire itinerary of the patient; in 16.13% it was performed only symptom-based, and in 8.06% only on-call. Of the patients arriving at the ED, 24.19% were assigned a non-urgent triage code, 60.01% a urgent code, 14.30% a emergent code and 1.49% a life-threatening code. Waiting times were: 52.39 min for non-urgent patients, 40.26 min for urgent, 12.08 for emergent, and 1.19 for life-threatening patients.
