The structural organization of the Golgi stacks in mammalian cells is intrinsically linked to function, including glycosylation, but the role of morphology is less clear in lower eukaryotes. Here we investigated the link between the structural organization of the Golgi and secretory pathway function using Pichia pastoris as a model system. To unstack the Golgi cisternae, we disrupted 18 genes encoding proteins in the secretory pathway without loss of viability. Using biosensors, confocal microscopy and transmission electron microscopy we identified three strains with irreversible perturbations in the stacking of the Golgi cisternae, all of which had disruption in genes that encode proteins with annotated function as or homology to calcium/calcium permeable ion channels. Despite this, no variation in the secretory pathway for ER size, whole cell glycomics or recombinant protein glycans was observed. Our investigations showed the robust nature of the secretory pathway in P. pastoris and suggest that Ca2+ concentration, homeostasis or signalling may play a significant role for Golgi stacking in this organism and should be investigated in other organisms.

Halitose is een frequent voorkomend probleem, waarvan de oorzaak meestal in de mondholte te vinden is. Omdat de mens zijn eigen ademgeur niet goed kan vaststellen, is het aan te raden om tijdens een tandartsbezoek de ademgeur van patiënten te controleren. De belangrijkste oorzaken van intraorale halitose zijn tongbeslag, gingivitis en parodontitis. Bacteriën in het tongbeslag en de tandplaque produceren vluchtige zwavelverbindingen, die bij intraorale halitose hoofdzakelijk verantwoordelijk zijn voor de ademgeur. Hierdoor richt de behandeling van halitose zich voornamelijk op het verwijderen van het tongbeslag en de supra- en subgingivale plaque.

Het is bekend dat veel tandheelkundige apparatuur lawaai maakt, maar hoe is het gesteld met het gehoor van de tandarts? Er zijn veel aanwijzingen dat tandartsen meer risico lopen op het ontwikkelen van lawaaislechthorendheid dan andere medische professionals. Dit hoofdstuk geeft een beschrijving van de werking van het gehoor, gehoorverlies door lawaai, het meten van gehoorverlies en wetgeving gericht op bescherming van het gehoor. Ook wordt beschreven aan welke geluidsniveaus de tandarts wordt blootgesteld en wat de literatuur vermeldt over lawaaischade aan het gehoor van tandartsen.

Hoewel metalen de belangrijkste veroorzakers van huidgerelateerde contactallergie zijn, lijken diezelfde metalen in de mond van zelfs allergische patiënten tot veel minder problemen te leiden. Anderzijds is er een groep patiënten met onbegrepen klachten, die (vooral door de patiënten) worden toegeschreven aan blootstelling aan tandheelkundige metalen restauraties. Reguliere allergietesten zijn bij deze patiënten geregeld negatief. Wat is eraan de hand bij deze patiënten? Zijn deze klachten psychisch van aard? Is de diagnostiek ontoereikend? Of zijn er andere mechanismen dan allergie in het spel? In dit hoofdstuk wordt ingegaan op deze vragen en wordt een diagnostische werkwijze geschetst die poogt rekening te houden met alle facetten die mogelijk een rol spelen in deze slecht begrepen problematiek. Zodoende kunnen ook deze patiënten zo goed mogelijk worden behandeld, want één ding is zeker, de klachten zijn echt.

Most insertional mutagenesis screens of Drosophila performed to date have not used target chromosomes that have been checked for their suitability for phenotypic screens for viable phenotypes. To address this, we have generated a selection of stocks carrying either isogenized second chromosomes or isogenized third chromosomes, in a genetic background derived from a Canton-S wild-type strain. We have tested these stocks for a range of behavioral and other viable phenotypes. As expected, most lines are statistically indistinguishable from Canton-S in most phenotypes tested. The lines generated are now being used as target chromosomes in mutagenesis screens, and the characterization reported here will facilitate their use in screens of these lines for behavioral and other viable phenotypes.

Het os zygomaticum, of jukbeen, en de arcus zygomaticus, of jukboog, zijn prominente structuren van het middengezicht. De os zygoma fractuur is de meest voorkomende breuk van het maxillofaciale skelet, gevolgd door een fractuur van de arcus. Zygomafracturen kunnen naast functionele problemen, zoals dubbelbeelden, gevoelsstoornissen of een beperkte mondopening leiden tot een asymmetrie van het middengezicht en daarmee tot cosmetische bezwaren. Symptomen samengaand met een zygomafractuur zijn vaak karakteristiek en bestaan uit onder andere een periorbitaal oftewel monocle hematoom, een afvlakking van de wangkoon en een sensibiliteitsstoornis van de nervus infraorbitalis. Zygoma- en solitaire arcusfracturen, kunnen op verschillende manieren worden behandeld: door middel van een gesloten of open repositie. Behandeling is mede afhankelijk van de mate van dislocatie van de fractuur en de leeftijd, of conditie van de patiënt.

Een orale leukoplakie is een ‘overwegend witte verandering van het mondslijmvlies, die klinisch en histopathologisch niet als een andere witte afwijking kan worden gediagnosticeerd’. De incidentie van orale leukoplakie wereldwijd is ongeveer 2%. Hiervan ontaardt zo’n 1%-2% per jaar in een plaveiselcelcarcinoom. Er is een aantal factoren bekend die mogelijk een hoger risico geven op maligne ontaarding. Het betreft hogere leeftijd, vrouwelijk geslacht, niet-roken, niet-homogene orale leukoplakie, een orale leukoplakie ≥ 4 cm, locatie op de laterale tongrand of de mondbodem en een hoge graad van epitheeldysplasie.Een belangrijk onderdeel van de periodieke controle bij de tandarts is de inspectie van de slijmvliezen. Bij een witte slijmvliesafwijking in de mondholte moet de tandarts altijd nagaan of er mogelijk een oorzaak aanwezig is en of deze kan worden weggenomen. Mocht er geen mogelijke oorzaak aanwezig zijn, dan dient de patiënt meteen verwezen te worden naar de MKA-chirurg. De MKA-chirurg zal dan een biopt nemen van de witte slijmvliesafwijking en de laesie definiëren. Aan de hand van de soort afwijking zal het verdere beleid worden bepaald.

Het is al tientallen jaren bekend dat snoepgedrag bijdraagt aan de ontwikkeling van cariës. Het nuttigen van frisdrank en vruchtensappen wordt beschouwd als een van de belangrijkste oorzaken voor het ontstaan van erosieve gebitsslijtage, maar ook de consumptie van andere zure voedingsmiddelen zoals snoep kan demineralisatie van tandglazuur tot gevolg hebben. De afgelopen decennia heeft het aanbod aan snoepgoed een enorme ontwikkeling doorgemaakt, waarbij nieuwe soorten en (extreem) zure varianten werden ontwikkeld. De meeste risicovolle varianten met fruitsmaak bevatten één of meer organische zuren en zijn ontwikkeld om lang in de mond te blijven. Ook is er vloeibaar zuur snoep ontwikkeld, waarvan de smaak binnen enkele minuten verdwijnt. Dit leidt tot frequente consumptie met een hoge cumulatieve contacttijd van zuur met de gebitselementen. Naast risicovol snoep zijn er gelukkig ook tandvriendelijke producten beschikbaar die geen risico vormen voor het ontstaan van caries en/of erosie.

Tandartsen worden steeds vaker geconfronteerd met patienten met een pacemaker of implanteerbare cardiale pacemaker. Deze apparaten zijn gevoelig voor elektromagnetische interferentie, die de werking van de pacemaker kan verstoren. Allerlei tandheelkundige apparatuur kan als bron fungeren van lichte of matige elektromagnetische velden. Terughoudendheid bij patienten met een pacemaker lijkt geboden met het gebruik van magnetostrictieve scalers en ultrasone trilbaden in de nabijheid. Het gebruik van een elektrotoom wordt afgeraden. Andere tandheelkundige elektrische apparatuur lijkt bij moderne pacemakers geen risico op storing te vormen, indien men zich houdt aan het algemene advies om deze alleen te gebruiken op meer dan 15 cm afstand van de pacemaker en de elektroden. Als een tandarts twijfelt of bepaalde apparatuur bij een patient gebruikt mag worden, dient voorafgaand overleg met de behandelend cardioloog plaats te vinden.

In Nederland zijn verschillende mondgels, -sprays en -spoelmiddelen beschikbaar ter verlichting van monddroogte. In een literatuuronderzoek werd gezocht naar wetenschappelijke onderzoeken naar de effectiviteit van deze middelen. Vervolgens werd het gebruik van speekselsubstituten door sjögrenpatiënten onderzocht.Speekselsubstituten zouden bij patiënten verlichting van monddroogteklachten moeten geven. Op grond van de tot nu toe gepubliceerde onderzoeken lijkt de wetenschappelijke onderbouwing voor de werkzaamheid van speekselsubstituten beperkt. Enkele middelen lijken een, relatief beperkt, positief effect te hebben op de monddroogte. Patiënten stoppen vaak met het gebruik van speekselsubstituten, omdat zij het effect als te gering ervaren en de smaak van de producten tekortschiet.Geconcludeerd mag worden dat er behoefte is aan de ontwikkeling van meer effectieve speekselsubstituten, waarbij de verschillende doelgroepen met monddroogte nauw betrokken dienen te worden.

Background: The aim of our study was to determine which factors are associated with switching from oral hypoglycaemic agents to insulin therapy in patients with type 2 diabetes mellitus in general practice. Methods: Longitudinal, observational study in a Dutch general healthcare centre. All pharmacologically treated patients with type 2 diabetes mellitus were included (n=152). Comorbidity, laboratory results and medication use were obtained from the general practitioners' files. Results: A total of 31 (20.4%) patients switched from oral hypoglycaemic agents to insulin therapy; they were significantly younger at the onset of diabetes, 50.5 versus 57.7 years. Fasting blood glucose levels and HbAIC values were significantly higher after the switch compared with patients on oral treatment, 10.0 mmol/l versus 8.4 mmol/l and 8.8% versus 7.9%, respectively. Concerning comorbidity, they suffered more frequently from acute myocardial infarction, lipid disorders,'depression, retinopathy and atrial fibrillation. Cardiovascular disease in general was more often present in patients who switched over to insulin, 77.4% versus 52.9% (OR 3.1; CI 1.2-7.6). Conclusions: Patients who switch over to insulin therapy are younger at diagnosis, suffer from more health problems besides diabetes, especially cardiovascular disease, and have worse metabolic control, compared with users of oral hypoglycaemic agents.

AIMS: To determine the prevalence of heart failure and symptomatic as well as asymptomatic left ventricular systolic dysfunction in the general population.METHODS AND RESULTS: In 5540 participants of the Rotterdam Study (age 68.9+/-8.7 years, 2251 men) aged 55-95 years, the presence of heart failure was determined by assessment of symptoms and signs (shortness of breath. ankle oedema and pulmonary crepitations) and use of heart failure medication. In 2267 subjects (age 65.7+/-7.4 years, 1028 men) fractional shortening was measured. The overall prevalence of heart failure was 3.9% (95% CI 3.0+/-4.7) and did not differ between men and women. The prevalence increased with age, with the exception of the highest age group in men. Fractional shortening was higher in women and did not decrease appreciably with age. The prevalence of left ventricular systolic dysfunction (fractional shortening CONCLUSIONS: The prevalence of heart failure is appreciable and does not differ between men and women. The majority of persons with left ventricular systolic dysfunction can be regarded as having asymptomatic left ventricular systolic dysfunction.

Objective. To determine the prevalence of risk factors for cardiovascular disease among elderly people. Design. Prospective cohort study. Setting. A district of Rotterdam, the Netherlands. Methods. As a part of the Rotterdam Study information about smoking habits, blood pressure, Quetelet index and serum cholesterol of 7,983 responding persons (78%) (3,105 men and 4,878 women) of 55 years and older was obtained by an interview and physical examination during two visits to a research center. Results. Among men and women there were 29.7% and 16.7% smokers and 60.1% and 25.9% ex-smokers, respectively. Among men the proportion of smokers decreased from 31.0% in the age category 55-59 years to 15.9% in the category ≥85 years, among women from 28.0% to 2.7% . Systolic blood pressure increased with age in both sexes, while diastolic blood pressure hardly changed. Hypertension (systolic blood pressure 160 mmHg and/or diastolic blood pressure ≥95 mmHg at a single measurement and/or use of antihypertensive drugs) occurred in 23.3% of men and 28.0% of women. In men, total serum cholesterol decreased gradually with age, whereas in women there was a slight increase up to the category 70-74 years. No evident change in HDL cholesterol with increasing age was observed in men, but in women a decrease was observed until the same level was reached as in men. Thirty-five per cent of men and 49.5% of women had an elevated level of serum cholesterol (≥6,5 mmol/l). The prevalence of obesity (Quetelet index > 25 kg/m2) was about 50% in both men and women but was less in the categories from 75 years onwards. Approximately 80% of men and women had at least one risk factor, while in almost half of them two or more risk factors were found. Conclusion. Risk factors for cardiovascular disease are common among elderly people.
