Language production has often been described as impaired in psychiatric diseases such as in psychosis. Nevertheless, little is known about the characteristics of linguistic difficulties and their relation with other cognitive domains in patients with a first episode of psychosis (FEP), either affective or non-affective. To deepen our comprehension of linguistic profile in FEP, 133 patients with FEP (95 non-affective, FEP-NA; 38 affective, FEP-A) and 133 healthy controls (HC) were assessed with a narrative discourse task. Speech samples were systematically analyzed with a well-established multilevel procedure investigating both micro- (lexicon, morphology, syntax) and macro-linguistic (discourse coherence, pragmatics) levels of linguistic processing. Executive functioning and IQ were also evaluated. Both linguistic and neuropsychological measures were secondarily implemented with a machine learning approach in order to explore their predictive accuracy in classifying participants as FEP or HC. Compared to HC, FEP patients showed language production difficulty at both micro- and macro-linguistic levels. As for the former, FEP produced shorter and simpler sentences and fewer words per minute, along with a reduced number of lexical fillers, compared to HC. At the macro-linguistic level, FEP performance was impaired in local coherence, which was paired with a higher percentage of utterances with semantic errors. Linguistic measures were not correlated with any neuropsychological variables. No significant differences emerged between FEP-NA and FEP-A (p≥0.02, after Bonferroni correction). Machine learning analysis showed an accuracy of group prediction of 76.36% using language features only, with semantic variables being the most impactful. Such a percentage was enhanced when paired with clinical and neuropsychological variables. - Abstract
Clinical guidelines, while representing an objective reference to perform appropriate treatment choices, contain grey zones, where recommendations are not supported by solid evidence. In a conference held in Bergamo in October 2018, an attempt was made to highlight some of the main gray zones in Cardiology and, through a comparison between experts, to draw shared conclusions that can illuminate our clinical practice. This manuscript contains the statements of the symposium concerning the controversies regarding dual antiplatelet therapy (DAPT). The manuscript represents the organization of the meeting, with an initial review of current guidelines on this topic, followed by an expert presentation of pros (white) and cons (black) related to the identified "gaps of evidence". For every issue is then reported the response derived from the votes of the experts and the public, the discussion and, finally, the highlights, which are intended as practical "take home messages" to be used in everyday clinical practice. The first topic concerns the utility of scores to shorten the duration of DAPT in patients at high bleeding risk. The second issue examines the appropriateness of the level of evidence to prolong DAPT beyond 1 year in patients at high ischemic risk. The last "gap in evidence" concerns the possibility of adopting the single antiplatelet therapy plus an anticoagulant vs the triple therapy in patients with atrial fibrillation and acute coronary syndrome. The work has also been implemented with evidences deriving from important randomized studies published after the date of the Conference.

- Abstract
Aims Midwall fractional shortening (MWFS) is a measure of left ventricular (LV) systolic function that is more reliable in case of concentric LV geometry compared to LV ejection fraction (LVEF). We hypothesized that MWFS might predict heart failure (HF) and death in a high-risk asymptomatic population, beyond other echocardiographic parameters. Methods Among 4047 subjects aged >_55/<_80 years followed by 10 general practitioners in northern Italy, the DAVID-Berg and results study prospectively enrolled 623 asymptomatic outpatients at increased risk for HF. Baseline evaluation included clinical visit, electrocardiogram, N-terminal pro-brain natriuretic peptide (NT-proBNP), and echocardiogram. Mean age of the population was 69 ± 7 years, 56% were men, 88% had hypertension, mean LVEF was 61 ± 9%, and mean MWFS 16.2 ± 3.3. During a median follow-up of 5.7 years, 95 subjects experienced HF/death events. At Cox analysis, lower MWFS was the only echocardiographic parameter, among structural/functional ones, associated with higher risk of HF/death [hazard ratio (HR) 0.89, 95% confidence interval (CI) 0.84–0.95, Padjusted < 0.001]. The risk of HF/death related to clinical data and NT-proBNP (baseline model) was reclassified by echocardiography only when MWFS was included into the model (baseline C-statistics 0.761; adding conventional structural/functional echocardiographic data 0.776, P = 0.09; adding MWFS 0.791, P = 0.007). Compared to subjects with normal LVEF and MWFS, only subjects with combined systolic dysfunction (11% of the population) were at higher risk (P = 0.001 for both abnormal; P > 0.24 for either LVEF or MWFS abnormal). Conclusion DAVID-Berg data suggest to include MWFS assessment in clinical practice, a simple and reliable echocardiographic parameter able to improve risk stratification in subjects at high risk for HF.

Clinical guidelines, while representing an objective reference to perform appropriate treatment choices, contain grey zones, where recommendations are not supported by solid evidence. In a conference held in Bergamo in October 2018, an attempt was made to highlight some of the main gray zones in Cardiology and, through a comparison between experts, to draw shared conclusions that can illuminate our clinical practice. This manuscript contains the statements of the symposium concerning the controversies regarding dual antiplatelet therapy (DAPT). The manuscript represents the organization of the meeting, with an initial review of current guidelines on this topic, followed by an expert presentation of pros (white) and cons (black) related to the identified "gaps of evidence". For every issue is then reported the response derived from the votes of the experts and the public, the discussion and, finally, the highlights, which are intended as practical "take home messages" to be used in everyday clinical practice. The first topic concerns the utility of scores to shorten the duration of DAPT in patients at high bleeding risk. The second issue examines the appropriateness of the level of evidence to prolong DAPT beyond 1 year in patients at high ischemic risk. The last "gap in evidence" concerns the possibility of adopting the single antiplatelet therapy plus an anticoagulant vs the triple therapy in patients with atrial fibrillation and acute coronary syndrome. The work has also been implemented with evidences deriving from important randomized studies published after the date of the Conference.

- Abstract
The involvement of gastrointestinal system in SARS-CoV2 related disease, COVID-19, is increasingly recognized. COVID-19 associated pancreatic injury has been suggested, but its correlation with pancreatic disease is still unclear. In this case report, we describe the detection of SARS-CoV2 RNA in a pancreatic pseudocyst fluid sample collected from a patient with SARS-CoV2 associated pneumonia and a pancreatic pseudocyst developed as a complication of an acute edematous pancreatitis. The detection of SARS-CoV2 within the pancreatic collection arise the question of whether this virus has a tropism for pancreatic tissue and whether it plays a role in pancreatic diseases occurrence. Despite significant therapeutic advances, patients with chronic heart failure and reduced ejection fraction (HFrEF) remain at high risk for heart failure progression and death. The PARADIGM-HF study, the largest outcome trial in HFrEF, has shown improved cardiovascular outcomes with sacubitril/valsartan (Entresto®, Novartis), previously known as LCZ696, compared with angiotensin-converting enzyme (ACE) inhibitor therapy, possibly leading us to a new era for heart failure treatment. Sacubitril/valsartan represents a first-in-class drug acting through inhibition of angiotensin receptor and neprilysin, thus modulating the renin-angiotensin-aldosterone system and vasoactive substances such as natriuretic peptides. This approach can be considered a "paradigm shift" from neurohumoral inhibition to neurohumoral modulation. Based on the PARADIGM-HF results, the European Society of Cardiology and the American Heart Association/American College of Cardiology guidelines proposed a substitution of ACE-inhibitor/angiotensin receptor blocker therapy rather than an "add-on" strategy in HFrEF. Sacubitril/valsartan can be considered a milestone in cardiovascular therapy, like aspirin, statins, beta-blockers. Of course there are many questions that arise spontaneously from this trial, three recognized experts can help us to answer them.

16. RELATION BETWEEN CALCIUM METABOLISM AND DEVELOPMENT OF AORTIC VALVE STENOSIS

- Abstract
The role of calcium metabolism in the development of degenerative Aortic Valve Stenosis (AVS) is not completely understood. Valvular cell modifications and passive calcification can be both involved in the diseases. Vitamin D (Vit D) and Parathyroid Hormone (PTH) are important factor in calcium regulation and data on their modification in AVS are scanty. Their potential role in AVS prevention is not yet identified. Osteocalcin can be considered a marker of calcification and its potential usefulness in identification of patient at risk for AVS was not studied. In order to correlate calcium metabolism to AVS development we have studied PTH, Vit D and Osteocalcin in a population of patients with AVS. We recruited 100 patients admitted in our Cardiac Surgery Unit for AVS confirmed by electrocardiogram, echocardiogram and hemodynamic evaluation. Subjects with rheumatic or congenital valvulopathy and infectious endocarditis were excluded from the study. Calcium, phosphorus, calcium–phosphorus ratio (Ca/P ratio), PTH, Vit D, renal function and osteocalcin were measured in each patient. In our population, 93.5% of patients had low levels of Vit D (<30 ng / ml), 42.4% had elevated levels of PTH (>38.4 pg / ml) and 28% had low levels of osteocalcin (<14 ng / ml). 87% of the population had normal calcium levels (8.4–10.2 mg / dl) and 93.3% had normal levels of phosphoremia (2.7–4.5 mg / dl). Defining as normal a Ca/P ratio between 1.5 and 2.5, 60.7% of our population had high levels of Ca/P. In conclusion, our findings suggest that patients suffering from AVS have abnormal levels of factors involved in the regulation of calcium metabolism. Some factors can be target for preventive therapies.

- Abstract
In this study the effects on the thermal field of a medium scale GSHP system located in Alessandria (Italy) and installed in highly heterogeneous alluvials are considered. The system was monitored over a year and data showed a progressive loss of efficiency. Simulation results obtained from a 3D numerical model show that the analyzed GSHP system has an inadequate design even for a short/medium period operation. A strong probe interference phenomenon was observed, due to the particular layout of the probe field and to the high energy request for building conditioning. The efficiency loss is also amplified by the presence of alluvial deposits with poor thermal properties. Adopting a homogeneous distribution of the thermal properties of the subsurface results in an underestimation of the thermal alteration of 25% while the increase of probe distance by 55% produces a reduction of thermal alteration of 45%. In this case study the homogeneous subsurface assumption leads to excessive simplification of the observed strong heterogeneity and it underrates thermal impact on the soil, especially in layers with poor thermal properties.

- Abstract
Severe eutrophication of freshwater lakes, with the subsequent risk of algal blooms, has a critical effect on the safety of drinking water supplies in China and is one of the main environmental emergencies in the country. This paper focuses on Chao Lake, a large, shallow eutrophic lake used as a source of drinking water. The study considers the possibilities of improving the lake monitoring system and developing a SCADA system to manage the emergencies relating to water quality in order to meet the need of ensuring safe drinking water to the population of Chaohu City. The paper is presented in sub-sections that reflect the multitasking nature of the study, which focused on: (a) upgrading the monitoring system at lake and water treatment plant levels and also applying remote sensing, to develop a SCADA (Supervisory Control And Data Acquisition) system using neural networks to support prompt and effective management of emergency situations; (b) upgrading water collection and treatment technologies

Per predisporre efficaci sistemi di allerta al verificarsi,in zone montane, di eventi meteorologici estremi, è indispensabile conoscere: previsioni meteorologiche estremamente accurate e le caratteristiche idrogeologiche e geomorfologiche della zona. A partire da queste informazioni è possibile addestrare un insieme di reti neurali che permettano di convertire le previsioni meteorologiche in probabilità di accadimento di frane e/o alluvioni e, qualora se ne verifichi la necessità, predisporre le procedure di protezione civile in modo automatico e con anticipo sufficiente a salvare persone, luoghi e attività.

- Abstract
The interconnection of distributed generators in low voltage (LV) networks, together with storage systems, could cause a substantial change in grid operational practice: currently a rising idea proposes to treat generators, storage systems and even loads as distributed energy resources (DER), able to supply services to the grid and accept on-line requests. To emphasise the advantages of Distributed Generation (DG) it is necessary to face the problem with a system approach. In this work both type of services, namely those provided by each generator and those deriving from the co-ordination of different resources, have been investigated. The option represented by a complex master-slave configuration has been discarded in order to avoid its fast communication and control requirements. Instead a prototype based on a centralised resource management solution and a somewhat plug-and-play functionality for each generator has been developed. For this purpose, different control systems have been implemented in order to act during fast transient dynamics due to random load or configuration changes, or to manage the slow modifications of the system conditions due to daily, weekly and seasonal load variations. The regulators of every distributed resource are requested to give response to fast and unpredictable transient dynamics, i.e. to participate to voltage and frequency regulations, harmonic compensations and phase balancing, while a central dispatcher has been conceived as to periodically update the regulators set-points in order to exploit the capabilities of each distributed resource either for present and foreseen grid operating conditions. For voltage and frequency regulations, a specific droop control algorithm has been designed so to be implemented on board of generators connected to the LV grid by an inverter device. - Abstract
Studying heat transfer in 3D water or ice basins involves the solution of Navier Stokes or Stokes systems of PDE, coupled with a scalar equation for thermal energy transport. The method used in this paper is based on a Finite Volume technique where physical quantities and boundary conditions are approximated by means of high order formulae and the time advance is dealt with by a fractional step technique. This paper is mainly concerned with applications, but, with respect to a preceding version of the method, an evaluation of performance of techniques dealing with the turbulent viscosity in water basins is presented and a study of the temperature field in a glacier is included. The method is applied to a thermal discharge in a water basin of lower temperature and to a portion of the Priestley Glacier (Antarctica). The results are very accurate and coherent with the physical theory and with measured data.

- Abstract
A semi-implicit numerical method for the three-dimensional incompressible Navier-Stokes equations is presented. The method describes the velocity and the non-hydrostatic pressure field and the free surface evolution in time. The governing equations are discretized by means of the finite volume method on a structured non-uniform grid; for this reason both local and global mass conservation are guaranteed. Convective and diffusive fluxes on the control volume faces, as well as boundary conditions, are approximated by high-order formulae. Copyright © 2006 John Wiley & Sons, Ltd.

- Abstract
The problem of describing ice dynamics has been faced by many researchers; in this paper a fully three-dimensional model for ice dynamics is presented and tested. Using an approach followed by other researchers, ice is considered a non-linear incompressible viscous fluid so that a fluid-dynamic approach can be used. The model is based on the full three-dimensional Stokes equations for the description of pressure and velocity fields, on the Saint-Venant equation for the description of the free-surface time evolution and on a constitutive law derived from Glen's law for the description of ice viscosity. The model computes the complete pressure field by considering both the hydrostatic and hydrodynamic pressure components; it is time-evolutive and uses high-order numerical approximation for equations and boundary conditions. Moreover it can deal with both constant and variable viscosity. Three theoretical tests and two applications to Priestley Glacier, Antarctica, are presented in order to evaluate the performance of the model and to investigate important phenomena of ice dynamics such as the influence of viscosity on pressure and velocity fields, basal sliding and flow over perturbed bedrocks. All these applications demonstrate the importance of treating the complete pressure and stress fields.

In the present work the thermal flow in a section of Lys Glacier, in Italian Western Alps. is investigated. This work represents the application of the model-presented in De Biase and Deponti [1]. The velocity field is calculated assuming ice to be a perfectly plastic material. With this input data, the energy conservation equation is solved by the Finite Difference Method. In order to accurately model the complex geometry of the glacier section a regular but non-uniform grid has been used. General Finite Difference Formulae of second order accuracy have been used. The model has been applied to a section of Lys Glacier taking into account all the particular atmospheric conditions of the site and making two different hypotheses for the geothermal flux value at the base

- Abstract
The problem of mass and thermal flow in Alpine Glaciers has been approached in the literature by adopting simplifying assumptions such as the Shallow Ice approximation. Most of the proposed models are 2D or quasi-3D; no full 3D models have been proposed to our knowledge; the problem, however, is fully 3D due to the complex geometry of the domain and to the free surface which evolves in time because of atmospheric as well as of internal motion reasons. In the presented work a fully 3D model is proposed. The Finite Volume (FV) method is used for the space discretization in order to guarantee both local and global conservation of mass. The time advancing scheme is based on the Fractional Step theory and is capable of dealing with the free surface evolution. The proposed method is similar to the method proposed by Casulli but a higher approximation order is used for convective and diffusive terms and for the discretization of boundary conditions.
