Preoperative labeling of gastric cancer is an important means to determine the surgical margin. At present, there are many commonly used labeling methods. However, which is more accurate and has fewer complications remains to be studied. Through animal experiments, this study explored the feasibility, accuracy, and safety of a magnetic detector combined with magnetic beads for the preoperative labeling of gastric cancer.A total of 10 beagle dogs were included in the study. Each dog was randomly labeled with magnetic beads in the gastric body and antrum. After labeling, the magnetic detector was used to explore the gastric serosa surface, and the positioning titanium clip was released at the detected magnetic bead. The main monitoring index was to measure the distance between the labeled magnetic beads and the positioning titanium clamped. The secondary indexes were detection time, magnetic induction intensity, magnetic bead shedding rate, mucosal injury rate, bleeding, and leukocyte and C-reactive protein levels before and 24 hours after the operation.All 10 beagle dogs completed the marking and exploration successfully. The average distance between the magnetic beads and the positioning titanium clip in 20 cases was 5.90±2.36 mm. The average detection time was 1.60±0.69 min, and the average magnetic induction intensity was 3.76±1.11 mT. No magnetic beads were found to fall off, 1 case had a mild mucosal injury, and 2 cases had a small amount of bleeding when releasing the positioning titanium clip. The white blood cells before and 24 hours after the operation were 7.43±0.94(×109/L) versus 7.79±0.67(×109/L) (P=0.34). The C-reactive protein before and 24 hours after the operation were 5.24±0.97 µg/mL versus 5.95±1.02 µg/mL (P=0.13).A magnetic detector combined with magnetic beads for gastric cancer labeling is feasible, accurate, and safe. It is expected to be further applied in the clinic.

The first choice of treatment for rectal neuroendocrine tumors (R-NETs) 10 mm in size is endoscopic resection, there is still controversy concerning the optimal endoscopic treatment for resecting R-NETs. This study evaluated the efficacy and safety of multiple ligation-assisted endoscopic submucosal resection combined with endoscopic ultrasonography (EMR-MLUS) for R-NETs.We retrospectively analyzed the data of 62 patients with R-NETs ≤10 mm in size who underwent EMR-MLUS or ligation-assisted endoscopic submucosal resection combined with endoscopic ultrasonography (EMR-LUS) between May 2019 and April 2022, including tumor characteristics, endoscopic complete resection, pathological complete resection, the procedure time, adverse events, and follow-up were compared between the two groups of patients.Of the 62 patients, 19 underwent EMR-MLUS and 43 underwent EMR-LUS. The endoscopic morphology of lesions was statistically different between group EMR-MLUS and group EMR-LUS (P = 0.015), and most of them were flat and slightly raised lesions in group EMR-MLUS. Although the pathological complete resection rate was slightly higher in the EMR-MLUS group than in the EMR-LUS group (94.74% vs. 90.70%; P = 0.290), the endoscopic complete resection rate was high in both groups. Involvement of the lateral resection margin was found four cases in the ESMR-LUS group; one case of deep resection margin involvement in the EMR-MLUS group. The mean procedure time was longer in the EMR-MLUS group than in the EMR-LUS group (12.79 ± 1.01 min vs. 11.08 ± 1.89 min; P = 0.041). In group EMR-LUS, there were two cases of immediate bleeding; in group EMR-MLUS, one case of perforation, all of them were successfully treated by endoscopy. No recurrence, progression, or metastasis was found in all patients.EMR-MLUS is a safe and effective technique that could be considered when removing small rectal NETs, especially flat and slightly raised lesions.

To evaluate the value and compare the effectiveness of linked color imaging-based endoscopic grading of gastric intestinal metaplasia (LCI-EGGIM) and operative link on gastric intestinal metaplasia (OLGIM) in risk stratification of early gastric cancer (EGC).Eighty-one patients with EGC who underwent endoscopic submucosal dissection were included. The general data and EGC-related risk factors of all participants were recorded. LCI-EGGIM and OLGIM were used for both groups.The number of patients with LCI-EGGIM score ≥ 5 was significantly higher in the EGC group than in the control group (58.02% vs. 12.35%,OLGIM and LCI-EGGIM can be used and have the same value for predicting the risk stratification of EGC in patients with gastric intestinal metaplasia.

Rectal neuroendocrine tumors (R-NETs) usually invade the submucosa, and so complete resection is difficult. The treatment of choice for R-NETs ≤10 mm in size is endoscopic resection, but there is still controversy concerning the best endoscopic method. This study evaluated the efficacy and safety of ligation-assisted endoscopic submucosal resection combined with endoscopic ultrasonography (ESMR-LUS) for treatment of R-NETs.We retrospectively analyzed the data of 101 patients with R-NETs ≤10 mm in size who underwent ESMR-LUS (The endoscopic complete resection rate was slightly higher in the ESMR-LUS group than in the ESMR-L group (100 vs. 96.2%,ESMR-LUS appears to be a safe and effective technique for removal of small rectal NETs confined to the submucosal layer without metastasis. Further studies are warranted to compare the efficacy and safety of different methods.

There is no consensus on whether a gastroscopic biopsy is necessary during the emergency treatment of gastrointestinal (GI) diseases such as gastric ulcer bleeding. In this study, we examined the clinical utility and safety of an emergency gastroscopic biopsy for the assessment of gastric ulcer bleeding.We enrolled 150 patients with a single bleeding gastric ulcer after emergency gastroscopy (EG) from April 2020 to April 2022. The patients were randomly divided into the biopsy and no biopsy groups, and they were followed-up until June 2022 to examine whether recurrent gastric ulcer bleeding had occurred.Re-bleeding occurred in 15 out of 150 (10%) patients. We diagnosed malignancies in 17 (11.3%) patients and validated 14 (9.3%) of them during the initial gastroscopy procedure. Factors that could predict the occurrence of gastric ulcer re-bleeding with biopsy during EG included an absence of ischemic heart disease (odds ratio [OR] = 0.395, confidence interval [CI]: 0.24-0.65,Patients undergoing biopsy during EG did not exhibit an increased risk of re-bleeding compared with those who did not undergo a biopsy. An early biopsy facilitates an early pathologic diagnosis, early clinical intervention, safe discharge of low-risk patients, and improved outcomes in high-risk patients.

Background and aims Effective traction is an important prerequisite for successful endoscopic submucosal dissection (ESD). The combined orthodontic rubber band (ORB) and clip method was effective in colorectal cancer ESD. To date, the method was not reported in gastric ESD. This study aimed to investigate its efficacy and safety for gastric neoplasms ESD. Methods We retrospectively analyzed data of 118 patients with gastric neoplasms treated by ESD from November 2020 to April 2022, 43 by ORB-ESD and 75 by the conventional ESD. The primary outcome measure was the ESD procedure time. Clinical data on efficacy and safety were also collected and analyzed. Propensity score matching (PSM) matched the patients in both groups. Results PSM successfully matched 31 pairs of patients. The ORB-ESD operation time was shorter (median [interquartile range], 35 [30–48] vs. 49 [40–70] min, P 2/min, P P > 0.05). Conclusions Compared to the conventional ESD, ORB-ESD significantly reduced the procedure time and increased the dissection speed, proving beneficial to gastric ESD.

Background and study aims The present study was designed to evaluate the safety, efficacy, and tolerability of antofloxacin-based bismuth quadruple therapy in Chinese patients with Helicobacter pylori infection. Patients and methods Total 290 patients with H. pylori infection were randomly and equally divided into two groups as per different bismuth quadruple therapies for 14 d: colloidal bismuth pectin 200 mg thrice a day, lansoprazole 30 mg twice a day, amoxicillin 1 g twice a day, and antofloxacin 200 mg once a day (ACLA group) or levofloxacin 500 mg once a day (LCLA group). Eradication was assessed with 13C-urea breath test 6 wk after treatment completion; the primary endpoint was the eradication rate by intention-to-treat (ITT) and per-protocol (PP) analyses. The minimum inhibitory concentration was measured with the PDM epsilometer test to assess the susceptibility of H. pylori strains on gastric biopsy specimens to antofloxacin and levofloxacin. Results The eradication rates of H. pylori in the ACLA group were 93.8% and 97.8% for the ITT and PP analysis, respectively; these rates were significantly higher than those in the LCLA group, at 86.2% and 92.6%, respectively (p = 0.031 and 0.041, respectively). The total incidence of adverse events during the eradication therapy did not significantly differ between the ACLA and LCLA groups (31.7% vs. 37.9%%, p = 0.267), and the two groups displayed similar severity of adverse events (p = 0.156) and compliance rate (100% by ACLA vs. 97.8% by LCLA, p = 0.080). The eradication rate with the antofloxacin susceptible strains in the ACLA group was significantly higher than that with the resistant strains (99.2% vs. 66.7%, p = 0.045). Moreover, the eradication rate with the levofloxacin susceptible strains in the LCLA group was significantly higher than that with the resistant strains (95.3% vs. 80.0%, p = 0.013). Conclusion Antofloxacin is safe and effective for H. pylori eradication. Antofloxacin-based bismuth quadruple therapy could be an alternative treatment for H. pylori eradication.

With the continuous integration of deep learning and the technique of molecular biology, target detection models must accurately detect the position of each cell in the image and classify it correctly. We present a model for the multi-scale feature fusion of the existing human cell image dataset based on Gaussian mixedly clustering. First, a novel feature extraction network for extracting preliminary features at picture multi scales was presented, which was based on a residual neural network with Instance Normalization and a Mish activation function. Second, the presented model adopts the idea of feature fusion and introduced a new type of feature fusion network to integrate feature graphs on different scales. Furthermore, a Gaussian hybrid clustering algorithm was proposed to cluster the hyperparameters. Based on the experimental results, the average accuracy of the proposed model in the human cell image dataset exceeds 0.96, which improves by 11.9% compared with the existing target detection methods in the same field. Experiments show that the proposed model had been adapted to datasets with uneven sample distribution, providing new ideas for research on medical images.

Quinolone-containing triple therapy has been considered as the second-line therapy for eradication of Helicobacter pylori (H. pylori). At present, there are no data to show the efficacy and safety of antofloxacin-based rescue therapy for the eradication of H. pylori, and this pilot clinical trial was designed. A total of 196 patients who failed H. pylori eradication using the clarithromycin-based or metronidazole-based triple or bismuth quadruple therapy were randomly allocated to one of the following rescue eradication therapy groups: AEA group (antofloxacin 200 mg once daily, esomeprazole 20 mg + amoxicillin 1000 mg twice daily) for 14 days, or LEA group (levofloxacin 500 mg once daily, esomeprazole 20 mg + amoxicillin 1000 mg twice daily) for 14 days. The minimal inhibitory concentrations were tested by the E-test method. The gyrA mutation was analyzed by sequencing. Follow-up 13/14C-urea breath test was examined at 1 month after discontinuation. A total of 178 eligible patients were included in this study. The eradication rate was significantly higher in AEA group than in LEA group according to both ITT (87.6% vs. 68.5%; P = 0.002) and PP analyses (90.7% vs. 70.1%; P = 0.001). ITT analyses indicated that the eradication rate was significantly higher in AEA group than in LEA group with Asn87 mutation (78.9% vs. 31.3%; P = 0.005) and levofloxacin-resistant strains (76.9% vs. 44.2%; P = 0.003). Two groups exhibited similar adverse event rates (AEA 14.6% vs. LEA 20.2%, P = 0.323). The findings showed that antofloxacin may be a promising candidate in rescue therapy for H. pylori eradication failure in China.

Endoscopic mucosal resection (EMR) is frequently used for the removal of colorectal neoplasms. However, the use of prophylactic antibiotics in patients undergoing EMR is debatable. The aim of this randomized controlled trial was to assess whether antimicrobial prophylaxis is crucial in the perioperative period of EMR, especially for 10- to 20-mm lesions in this setting.Two hundred and sixty-four patients were randomized equally into 2 groups, the antibiotic (cefixime) group and the control group. The occurrence of adverse events was examined at 1 to 3 days after EMR. Plasma levels of inflammatory markers were analyzed at pre-operation, 1 day post-operation and 3 days post-operation. Blood samples collected at 1 day post-operation were used for culture.A total of 264 and 268 polyps were removed by EMR in the antibiotic group and the control group, respectively. There were 5 cases of fever, with 2 in the antibiotic group and 3 in the control group. In the antibiotic group, 12 patients had abdominal pain and 10 suffered bleeding, whereas in the control group, abdominal pain and bleeding were observed in 10 and 11 patients, respectively. There were no significant differences in the proportion of patients with fever or the incidences of postoperative complications between the groups. No significant differences between the groups were reported in plasma levels of white blood cell count, erythrocyte sedimentation rate, C-reactive protein or procalcitonin at pre-operation or post-operation. No patients provided positive blood cultures.The use the prophylactic antibiotics for EMR procedures in the perioperative period is no longer required when the lesions are 10 to 20 mm in size.

Given the inconsistency between the experimental environment and the production environment, the high hardware cost of the big data production environment which is difficult for ordinary universities to bear, and the complicated installation of software related to the big data field which is not easy to reproduce and other practical teaching problems in the current stage of big data experimental teaching, the advantages and disadvantages of the existing solutions are analyzed and a virtualization method is proposed to virtualize hundreds of thousands of virtual The design method of Big Data virtual experiments based on containers and Python language is proposed to meet the practical teaching needs of undergraduate Big Data courses by virtualizing hundreds of virtual servers on several physical servers to construct a private cloud of Big Data experiment servers within the university. By optimizing the design for a small-scale server cluster environment in universities, the redundant modules are streamlined, and only teaching experiment-related modules are retained, geared towards teaching simulation, saving the limited funds of ordinary universities and revitalizing state-owned assets; at the same time, open-source software is used to avoid intellectual property costs in future teaching sessions.

Pancreatic fibrosis is one of the most important pathological features of chronic pancreatitis (CP), and pancreatic stellate cells (PSCs) are considered to be the key cells. Puerarin is the most important flavonoid active component in Chinese herb Radix Puerariae, and it exhibited anti-fibrotic effect in various fibrous diseases recently. However, the impact and molecular mechanism of puerarin on CP and pancreatic fibrosis remain unknown. This study systematically investigated the effect of puerarin on CP and pancreatic fibrosis in vivo and in vitro. H&E staining, Sirius Red staining, qRT-PCR and Western blotting analysis of fibrosis and inflammation related genes of pancreatic tissues showed that puerarin notably ameliorated pancreatic atrophy, inflammation and fibrosis in a model of caerulein-induced murine CP. Western blotting analysis of pancreatic tissues showed the phosphorylation level of MAPK family proteins (JNK1/2, ERK1/2 and p38 MAPK) significantly increased after modeling of cerulein, while puerarin could inhibit their phosphorylation levels to a certain extent. We found that puerarin exerted a marked inhibition on the proliferation, migration and activation of PSCs, determined by CCK-8 assay, transwell migration assay, scratch wound-healing assay and expression levels of α-SMA, Fibronectin, Col1α1 and GFAP. Western blotting result demonstrated that puerarin markedly inhibited the phosphorylation of MAPK family proteins (JNK1/2, ERK1/2 and p38 MAPK) of PSCs in a dose-dependent manner whether or not stimulated by platelet-activating factor. In conclusion, the present study showed that puerarin could be a potential therapeutic candidate in the treatment of CP, and the MAPK pathway might be its important target.

Internet of Things will play a vital role in the public transport systems to achieve the concepts of smart cities, urban brains, etc., by mining continuously generated data from sensors deployed in public transportation. In this sense, smart cities applied artificial intelligence techniques to offload data for social governance. Bicycle sharing is the last mile of urban transport. The number of the bike in the sharing stations, to be rented in future periods, is predicted to get the vehicles ready for deployment. It is an important tool for the implementation of smart cities using artificial intelligence technologies. We propose a DBSCAN-TCN model for predicting the number of rentals at shared bicycle stations. The proposed model first clusters all shared bicycle stations using the DBSCAN clustering algorithm. Based on the results of the clustering, the data on the number of shared bicycle rentals are fed into a TCN neural network. The TCN neural network structure is optimized. The effects of convolution kernel size and Dropout rate on the model performance are discussed. Finally, the proposed DBSCAN-TCN model is compared with the LSTM model, Kalman filtering model, and autoregressive moving average model. Through experimental validation, the proposed DBSCAN-TCN model outperforms the traditional three models in terms of two metrics, root mean squared logarithmic error, and error rate, in terms of prediction performance.

Cumulative evidence suggests that linked color imaging (LCI) can be used to identify gastric intestinal metaplasia (GIM). We aimed to develop endoscopic grading for GIM (EGGIM) with LCI.Two hundred and seventy-seven patients who underwent high-resolution white-light gastroscopy followed by LCI for EGGIM estimation were included. LCI was performed for the entire mucosa, and images of five areas each were recorded from the lesser and greater curvatures of the antrum and corpus, and for the incisura. For each area, scores of 0 (no GIM), 1 (focal GIM, ≤30% of the area), and 2 (extensive GIM,30% of the area) were attributed for 10 points. If GIM was suspected based on endoscopy findings, targeted biopsies were performed; if GIM was not evident, random biopsies were performed according to the Sydney system to estimate the operative link on GIM (OLGIM).GIM was staged as OLGIM 0, I, II, III, and IV in 136, 70, 37, 28, and 6 patients, respectively. For OLGIM III/IV diagnosis, the area under the receiver operating curve was 0.949 (95% CI 0.916-0.972). EGGIM of 4, with sensitivity and specificity of 94.12% (95% CI 80.3%-99.3%) and 86.42% (95% CI 81.5%-90.5%), respectively, was determined the best cut-off value for identifying OLGIM III/IV patients.Our findings demonstrated the ability of EGGIM for diagnosing the extent of intestinal metaplasia and showed that EGGIM is related to OLGIM staging. EGGIM of 4 was the best cut-off value for identifying OLGIM III/IV patients.

Ability to solve realistic problems in chemical big data, the CDIO model inherits and develops the concept of the significant reform of engineering education in Europe and the United States for more than 20 years, and nearly 40 universities in China have become the demonstration university of China's CDIO engineering education model. China CDIO Engineering Education Model has officially accredited Shenyang University of Chemical Technology's Computer Science Technology College in 2020. This article is a review of Shenyang University of Chemical Technology's Computer Science Technology College in the process of China's CDIO Engineering Education Model Certification. Universal experience with CDIO engineering education has shown that CDIO standards not only contribute to enhancing the quality of education but also provide an opportunity to improve the quality of engineering education. A good foundation is laid for the systematic development of education. To this end, we introduced the CDIO engineering education model into the Shenyang University of Chemical Technology's big data analytics and applications, and in the design of pragmatic course teaching. We apply the CDIO concept to the human workforce training program, course system construction for chemistry majors, and faculty of Computer Science Technology majors. A series of investigations and practices have been carried out. To address the problem of offering big data analytics and applications courses for chemistry students, we introduce the CDIO engineering education model into the content, teaching methods, and teacher preparation are all aspects of this project. We have given a "problem-oriented" for both theoretical teaching and practical teaching, considering the learning situation of chemistry students. Based learning with an inspiring interactive theoretical teaching method, "result-oriented" practical teaching mode, and project design-oriented, students learn professional knowledge and engineering thinking through practical engineering projects and improve their engineering awareness and practical skills. Finally, through the combination of theory and practice, chemistry majors can understand the theories and methods of big data analysis and applications, as well as the principles and methods of big data analysis and processing. With the significant population growth in megacities everywhere, traffic congestion is becoming a severe impediment, leading to long travel delays and large economic loss on a global scale. Platooning is a promising intelligent transportation framework that can improve road capacity, on-road safety, and fuel efficiency. Furthermore, enabling inter-vehicle communications within a platoon and among platoons (in a multiplatoon) can potentially enhance platoon control by keeping constant inter-vehicle and inter-platoon distances. However, an efficient resource allocation (RA) approach is required for the timely and successful delivery of inter-vehicle information within multiplatoons. In this paper, subchannel allocation scheme and power control mechanism are proposed for LTE-based inter-vehicle communications in a multiplatooning scenario. We jointly consider the evolved multimedia broadcast multicast services and device-to-device (D2D) multicast communications to enable intra- and inter-platoon communications such that a desired tradeoff between the required cellular resources and the imposed communication delay can be achieved. Simulation results are given to demonstrate that the proposed approaches can reduce the communication delay comparing to D2D-unicast based RA scheme, especially in a multiplatoon scenario with a large number of vehicles.
