Abstract

Aims To determine if urine C-peptide/creatinine ratio is a useful tool for monitoring b-cell function in new-onset Type1 diabetes.Methods Data were obtained from a prospective immunomodulation study in people with Type 1 diabetes ≤ 3 monthsfrom diagnosis, with a standard mixed-meal tolerance test and measurement of urine C-peptide/creatinine ratio carriedout at 0, 3, 6, 9 and 12 months. The change in the insulin-dose-adjusted HbA1clevel was also correlated with the changein serum/urine C-peptide level during the 12-month follow-up period.Results A significant reduction in urine C-peptide/creatinine ratio, measured after a mixed-meal, was reached at9 months (-45.4%), whilst the reduction in stimulated serum C-peptide level reached significance after 3 months(-54.7%) in placebo-treated participants. Neither change in stimulated serum C-peptide nor change in urine C-peptidelevel correlated with each other, and nor did change in insulin-dose-adjusted HbA1clevel in the first 6 months, but allmeasures correlated significantly in the second half of the 12-month follow-up period.Conclusion Mixed-meal-stimulated urine C-peptide/creatinine ratio was similar to, although less sensitive than,stimulated serum C-peptide level in monitoring b-cell function during the first year after diagnosis. Because the former issignificantly less invasive, it warrants inclusion in further studies in Type 1 diabetes and may represent an attractivealternative outcome measure in cohort studies and in children.

Abstract

In autoimmune thyroid disease, thyroid epithelial cells (TEC) express major histocompatibility complex (MHC) class II molecules, potentially enabling them to present thyroid self-antigens to CD4-positive T cells. However, despite this, TEC may fail to present endogenous antigen as a result of limited processing or MHC class II loading capacity, or inadequate MHC class II levels. We addressed these issues using the cloned rat TEC line, Fischer rat thyroid cell line (FRTL5), which was transfected using an adenoviral expression vector that expressed ovalbumin (OVA) as an integral membrane protein. OVA-expressing FRTL5 cells very efficiently activated a panel of OVA-specific, class II-restricted T-cell hybridomas. This response was dependent on induction of MHC class II molecules by interferon-gamma (IFN-gamma) and was blocked by anti-MHC class II antibodies. Poor responses were seen to exogenously added OVA or OVA peptides. These results provide the most direct evidence to date that TEC can form MHC class II-peptide complexes derived from self-antigen in sufficient quantities to activate T cells.

Abstract

Background: Hypothyroidism affects 2-5% of the general population. Patients with uncorrected disease suffer significant morbidity and have an increased risk of cardiovascular disease and neurocognitive impairment. Levothyroxine, the treatment of choice, is inexpensive, easy to administer and in most cases restores well-being while normalizing thyroid function. However, 30-50% of individuals on levothyroxine are either over-treated or under-treated and others remain dissatisfied with treatment despite achieving thyroid hormone concentrations within the laboratory reference interval.Methods: This review is based on a systematic search of the literature for controlled trials, systematic reviews, guideline papers and cohort studies addressing best practice in thyroid hormone replacement.Results: Recent decades have seen improvements in patient management strategies driven by a better appreciation of levothyroxine pharmacokinetics. However, aspects of therapy such as the optimal timing of medication, strategies to overcome treatment non-adherence and target thyroid stimulating hormone concentrations in pregnancy and in patients with differentiated thyroid cancer remain challenging. Furthermore, there is now a substantial body of literature on common genetic variations in the deiodinases and thyroid hormone transporters and their role in the local regulation of thyroid hormone delivery. The benefits of combination therapy with liothyronine and levothyroxine are uncertain, and while it is theoretically probable that subsets of genetically predisposed individuals will benefit from combination therapy the existing evidence is as yet limited.Conclusion: Despite the availability of thyroid hormone replacement for more than a century, there are still substantial challenges in practice and opportunities to improve treatment outcomes.
