11. Optical Flow Estimation Improves Automated Seizure Detection in Neonatal EEG

Abstract

PurposeExisting automated seizure detection algorithms report sensitivities between 43% and 77% and specificities between 56% and 90%. The algorithms suffer from false alarms when applied to neonatal EEG because of the high degree of nurse handling and rhythmic patting used to soothe neonates. Computer vision technology that quantifies movement in real time could distinguish artifactual motion and improve automated neonatal seizure detection algorithms.MethodsThe authors used video EEG recordings from 43 neonates undergoing monitoring for seizures as part of the NEOLEV2 clinical trial. The Persyst neonatal automated seizure detection algorithm ran in real time during study EEG acquisitions. Computer vision algorithms were applied to extract detailed accounts of artifactual movement of the neonate or people near the neonate though dense optical flow estimation.ResultsUsing the methods mentioned above, 197 periods of patting activity were identified and quantified, of which 45 generated false-positive automated seizure detection events. A binary patting detection algorithm was trained with a subset of 470 event videos. This supervised detection algorithm was applied to a testing subset of 187 event videos with 8 false-positive events, which resulted in a 24% reduction in false-positive automated seizure detections and a 50% reduction in false-positive events caused by neonatal care patting, while maintaining 11 of 12 true-positive seizure detection events.ConclusionsThis work presents a novel approach to improving automated seizure detection algorithms used during neonatal video EEG monitoring. This artifact detection mechanism can improve the ability of a seizure detector algorithm to distinguish between artifact and true seizure activity.

Abstract

Immunotherapy using T cells genetically engineered to express a chimeric antigen receptor (CAR) is rapidly emerging as a promising new treatment for haematological and non-haematological malignancies. CAR-T-cell therapy can induce rapid and durable clinical responses, but is associated with unique acute toxicities, which can be severe or even fatal. Cytokine-release syndrome (CRS), the most commonly observed toxicity, can range in severity from low-grade constitutional symptoms to a high-grade syndrome associated with life-threatening multiorgan dysfunction; rarely, severe CRS can evolve into fulminant haemophagocytic lymphohistiocytosis (HLH). Neurotoxicity, termed CAR-T-cell-related encephalopathy syndrome (CRES), is the second most-common adverse event, and can occur concurrently with or after CRS. Intensive monitoring and prompt management of toxicities is essential to minimize the morbidity and mortality associated with this potentially curative therapeutic approach; however, algorithms for accurate and consistent grading and management of the toxicities are lacking. To address this unmet need, we formed a CAR-T-cell-therapy-associated TOXicity (CARTOX) Working Group, comprising investigators from multiple institutions and medical disciplines who have experience in treating patients with various CAR-T-cell therapy products. Herein, we describe the multidisciplinary approach adopted at our institutions, and provide recommendations for monitoring, grading, and managing the acute toxicities that can occur in patients treated with CAR-T-cell therapy.

20. Implementing Institutional Change through the Professional Development School Concept.

Abstract

The recruitment, selection, and preparation of teachers, counselors, and administrators are among the areas within the educational establishment which are subject to change. The Holmes Group, a consortium of nearly 100 American research universities, which addressed its attention to improving teacher preparation programs as a step toward improving schooling, advocates establishment of professional development schools (PDSs) and outlines six principles which are fundamental to the design of PDSs. These schools, which are analogous to teaching hospitals in the medical profession, are considered vehicles that provide necessary linkages between colleges of education and public schools. Although PDSs are new and unique institutions, there are precedents for them in educational history, particularly within the laboratory school movement. PDSs are intended to be places of demonstration, inquiry, and self-renewal that are collaboratively operated by school and college personnel and reflect real societal conditions. Establishing a PDS is a complex and challenging endeavor for both schools and colleges. Certain activities and planning steps are recommended for successful implementation of a PDS by a school-university partnership. These steps include: analyzing the situation, choosing a task force, securing commitment from the college, and making recommendations. Because establishing a PDS involves a serious commitment on the part of a university and its college of education, the implications and responsibilities attendant to this commitment should be considered carefully. 21. Restructuring a Teacher Preparation Program Using the Professional Development School Concept.

Abstract

Improved professional education for teachers, counselors, and school administrators is needed in order to successfully reform schooling. Fundamental change in professional education requires educational partnerships between schools and universities. The professional development school (PDS) represents a pivotal element in the strategy of utilizing partnerships to effect restructuring of schools and teacher education programs. The place of these schools in the professional preparation of educators is considered analogous to the place of teaching hospitals in medical education. Although there are similarities between PDSs and laboratory schools, there are also differences that contribute to the PDSs' uniqueness. Establishing a PDS is a complex process, which can be expected to pass through four phases of development: exploration, orientation, implementation, and operation. This paper provides guidelines and identifies issues related to each phase. Two documents are included in the appendices: "Oakland University [Michigan] and Pontiac Schools Professional Development Schools: Criteria for Partners" and "Application for Oakland University-Pontiac Professional Development School." Abstract

This handbook documents efforts to promote the professional development school (PDS) concept at Western Michigan University (WMU) and examines how a university might initiate the restructuring process for teacher education and collabaorate with a local school district through the establishment of a PDS. It discusses the underlying principles of PDSs, their place in school restructuring, and three major PDS components (teacher education, inquiry and research, and professional development). The handbook outlines steps to be taken in the planning process by university administrators and faculty and explores the four stages of PDS development (exploration, orientation, implementation, and operation). It includes information on: finances, evaluation, and PDSs in Michigan. Three mini-bibliographies and a resource list conclude the document. Nine appendixes include: a chronology of the Michigan Partnership for a New Education, PDS position statement, workplan for WMU, partnership school criteria, and a PDS workplan for WMU, partnership school criteria, and a PDS glossary of terms. Abstract

A study investigated the consistence of 222 children's (ages 3-16) reports of sexual and physical abuse. Older children were more consistent, children were more consistent when reporting sexual abuse, and girls were more consistent in sexual abuse reports. Abstract

This study assessed 3- to 17-year-olds' memory and suggestibility in the context of ongoing child maltreatment investigations. Results from 189 subjects indicated that general psychopathology, short-term memory, and intellectual ability predicted facets of children's memory performance. Older compared to young children evinced fewer memory errors and greater suggestibility resistance. Age also related to the amount of detail in children's abuse disclosures. Neither dissociation nor stress arousal predicted children's memory. (Author/KB)

Mock jurors (N=243) rendered judgment about a case involving childhood sexual abuse allegations made with or without allegations of satanic ritual abuse. Although jurors, especially nonreligious jurors, were less likely to believe the satanic ritual allegations than other case details, they were as likely to vote guilty and believe the victim in satanic as in nonsatanic cases.
