3. Workforce Development for People with Intellectual Disabilities: The Perspective from People with Intellectual Disabilities

Abstract

Health Education England working across Kent, Surrey and Sussex (HEE KSS) have been developing a project over the last few years with the single aim to create a sustainable and secure workforce supply, for people that have intellectual disabilities who require support from and/ or access to services across Kent, Surrey and Sussex. Their report, "Workforce Development for People with Intellectual Disabilities", was published in March 2015. Following on from this, HEE KSS commissioned the Foundation for People with Learning Disabilities to produce a report that would detail the opinions and perspectives of individuals who have a learning disability from across Kent, Surrey and Sussex, asking them what they would like their workforce to know. The project ran from April to August 2016. The Foundation for People with Learning Disabilities is a national charity that promotes the rights, quality of life, and opportunities of people with learning disabilities and their families. The aim of the report is to convey to HEE KSS an in-depth review of opinions and perspectives of individuals who have a learning disability from across Kent, Surrey and Sussex, asking them what they would like their workforce to know. In order to reach those with more complex needs the report will also include the views of a small number of families. Appended to the report are a: (1) Questionnaire; and (2) The five good communication standards by the Royal College of Speech and Language Therapists, 2013.

4. Learning Disabilities: Positive Practice Guide. Improving Access to Psychological Therapies (IAPT)

Abstract

The Improving Access to Psychological Therapies (IAPT) programme aims to provide equitable access to NICE-recommended psychological therapies to people from all sectors of the community. There is evidence that people with mild to moderate learning disabilities can benefit from the therapies offered by IAPT services, especially when reasonable adjustments are made to the way in which these therapies are delivered. This Positive Practice Guide, which is written by experts in learning disabilities, summarises the needs of people with learning disabilities and clearly outlines the reasonable adjustments that are recommended to ensure that people with learning disabilities get the maximum benefit from treatment within an IAPT service.

6. Facilitator's Handbook: Raising the Aspirations and Employment Prospects of Young People with Learning Disabilities

Abstract

Across the UK, fewer than 5.8% of people with a learning disability have a job, and many of those who are employed only work part-time. Yet most people indicate that they want to work. Many miss out on the basic right to aspire to a career, the wider social networks, the better emotional and physical health, and the independence that come from having a job. The Foundation for People with Learning Disabilities believes that young people with learning disabilities have the right to a working life and this must be embedded early on. Expectations that people with learning disabilities will find paid work are low due to years of under representation in the labour market. The Foundation wanted to highlight the fact that people with learning disabilities make hardworking and enthusiastic employees, bringing new skills, talents, and perspectives to their employers. In July 2011 the Foundation met with families, employers and teachers at schools in Kent, Redbridge, and West Berkshire in the UK. The 'When I Grow Up' programme was developed from these consultations. It has worked in a number of ways to raise the aspirations and employment prospects of young people with learning disabilities, through collaboration with young people, families, schools and employers. All elements of this programme have been designed to reflect changes introduced by the Children and Families Act 2014 and will therefore contribute to Preparing for Adulthood Reviews and Education and Health Care Plans. This handbook provides the training for the curriculum and workshops element of the programme.

Abstract

Purpose: To evaluate the evolution of fertility preservation surgery in children and young adults at high risk of infertility from a single centre to a networked 'Hub and Spoke' service. Methods: A case note review of patients referred for ovarian or testicular cryopreservation between Jan 2013 and Dec 2023. Demographic data, procurement numbers, and site of procurement were collected. Specialist feedback was obtained to identify the challenges faced. Results: Over time, the number of referrals increased from 4 to 349 patients per year with the number of Spoke centres rising to 36 ovarian and 16 testicular. In 2013–2014; 100% of procurement was ovarian as compared to 2023; 51% ovarian, 49% testicular. Of the 395 referrals in 2021, 81% (n = 319) went on to have procurement and storage of tissue. Between 2013 and 2016, 96% of cases were performed at the Hub. In 2023, 53/349 (15%) cases were performed at the Hub with the remaining 296 (85%) procured at Spoke sites. Surgical issues such as access to theatre, variation and availability of surgical equipment, thermal injury to ovarian tissue and variation in the size of the testicular specimen were identified. Conclusion: The Hub and Spoke model successfully delivers treatment to patients close to home as safely possible within their local treatment centre. [ABSTRACT FROM AUTHOR]

10. We Can Dream! Ways of Planning for the Future for Young People with Autistic Spectrum Disorders

Abstract

This booklet is for young people and their families, friends and supporters to read and talk about together. It is based on the stories of four young people. Big changes happened for some of them; others are still waiting for things to change. The goal of the booklet is to give students ideas about how they may want to plan or change things when they leave school or college. Topics discussed include: person centered planning (key questions to think about; the planning process and people with autism; adapting your language; making sure planning meetings are not too busy (low arousal); recognizing and valuing each person's special interests); circles of support; and making it happen (self-directed support/individual budgets). A list of resources on person centered planning and self-directed support is provided. [This booklet was written with support from Justin Pearce-Neudorf and Lucy Smith.]

11. What about Us? Promoting Emotional Well-Being and Inclusion by Working with Young People with Learning Difficulties in Schools and Colleges

Abstract

"What about us?" was a participative action research project promoting the emotional well-being of young people with learning difficulties in inclusive secondary schools and colleges. The project began by reviewing the impact of inclusion on the emotional well-being and personal and social development of young people with learning difficulties. The project went on to develop strategies for overcoming barriers to inclusion suggested by young people themselves and explored through action research processes led by the young people. The action research and development phase of the project was conducted in clusters of case study sites in three contrasting geographical locations in England. Young people with learning difficulties were involved as partners in the research through processes that were facilitated by the research team and staff in the participating schools and colleges. The young people who took part in the project derived clear and direct benefits from their involvement. The project's findings and recommendations have implications, in terms of enhanced opportunities for educational and social inclusion, improved quality of life and more effective education for personal and social development, for a wider group of young people, including those with learning difficulties, disabilities and/or special educational needs. Background to the project, participating organizations, project methods, project findings, and conclusions and implications for policy and practice are presented. Four appendices are included: (1) Membership of the "What about us?" Reference Group and Advisory Committee; (2) Criteria for the selection of the "What about us?" case study sites; (3) The "What about us?" code of conduct; and (4) An introduction to person-centred planning. Abstract

BACKGROUND: The Personal Genome Project Canada is a comprehensive public data resource that integrates whole genome sequencing data and health information. We describe genomic variation identified in the initial recruitment cohort of 56 volunteers. METHODS: Volunteers were screened for eligibility and provided informed consent for open data sharing. Using blood DNA, we performed whole genome sequencing and identified all possible classes of DNA variants. A genetic counsellor explained the implication of the results to each participant. RESULTS: Whole genome sequencing of the first 56 participants identified 207662 805 sequence variants and 27494 copy number variations. We analyzed a prioritized disease-associated data set (n = 1606 variants) according to standardized guidelines, and interpreted 19 variants in 14 participants (25%) as having obvious health implications. Six of these variants (e.g., in BRCA1 or mosaic loss of an X chromosome) were pathogenic or likely pathogenic. Seven were risk factors for cancer, cardiovascular or neurobehavioural conditions. Four other variants--associated with cancer, cardiac or neurodegenerative phenotypes--remained of uncertain significance because of discrepancies among databases. We also identified a large structural chromosome aberration and a likely pathogenic mitochondrial variant. There were 172 recessive disease alleles (e.g., 5 individuals carried mutations for cystic fibrosis). Pharmacogenomics analyses revealed another 3.9 potentially relevant genotypes per individual. INTERPRETATION: Our analyses identified a spectrum of genetic variants with potential health impact in 25% of participants. When also considering recessive alleles and variants with potential pharmacologic relevance, all 56 participants had medically relevant findings. 21. Reference, or Advisory, Groups Involving Disabled People: Reflections from Three Contrasting Research Projects

Abstract

Increasingly in recent years, the involvement of disabled people as co-researchers has been regarded as "good practice." This has been informed by growing participatory and emancipatory research paradigms as well as user-focused policy imperatives. The benefits of these shifts apply to the research itself (improved definition, direction, applicability and impact), to non-disabled researchers (personal growth and enhanced understanding of the reflexive research process), to people with disabilities involved as researchers or collaborators (personal growth and enhanced opportunities), and (if externally funded) to the funder whose ways of operating are likely to be challenged profoundly. In this paper, Ann Lewis, Sarah Parsons and Christopher Robertson (based at the University of Birmingham), Anthony Feiler, Beth Tarleton and Debby Watson (based at the University of Bristol) and Richard Byers, Jill Davies, Ann Fergusson and Claire Marvin (based at the University of Cambridge) discuss the work of three independent research teams carrying out concurrent projects. The authors share their experiences of trying to take seriously the participation of disabled people in research. All three projects were informed, to a significant degree, by their respective reference groups of disabled people. The work of these groups in each of the three projects is outlined and then discussed in relation to five common themes: formal contracts with members of reference groups; considerations concerning drawing on an established reference group; planning for reference group involvement; style of reference group involvement; and building on good practice.
