1. Results of TRIO-15, a multicenter, open-label, phase II study of the efficacy and safety of ganitumab in patients with recurrent platinum-sensitive ovarian cancer.

Abstract

BACKGROUND: IGF signaling has been implicated in the pathogenesis and progression of ovarian carcinoma (OC). Single agent activity and safety of ganitumab (AMG 479), a fully human monoclonal antibody against IGF1R that blocks binding of IGF1 and IGF2, were evaluated in patients with platinum-sensitive recurrent OC. METHODS: Patients with CA125 progression (GCIG criteria) or measurable disease per RECIST following primary platinum-based therapy received 18 mg/kg of ganitumab q3w. The primary endpoint was objective response rate (ORR) assessed per RECIST 1.1 by an independent radiology review committee (IRC) and/or GCIG CA125 criteria. Secondary endpoints included clinical benefit rate (CBR), progression free survival (PFS) and overall survival (OS). RESULTS: 61 pts. were accrued. Objective responses were seen in 5/61 patients (ORR 8.2%, 95% CI, 3.1-18.8) with 1 partial response (PR) by RECIST and 2 complete responses (CR) as well as 2 PR by CA125 criteria. CBR was 80.3% (95% CI, 67.8-89.0%). The median PFS according to RECIST by IRC was 2.1 months (95% CI, 2.0-3.1). The median PFS per RECIST IRC and/or CA125 was 2.0 months (95% CI, 1.8-2.2). The median OS was 21 months (95% CI, 19.5-NA). The most common overall adverse events were fatigue (36.1%) and hypertension (34.4%). Grade 1/2 hyperglycemia occurred in 30.4% of patients. Hypertension (11.5%) and hypersensitivity (8.2%) were the most frequent grade 3 adverse events. CONCLUSIONS: IGF1R inhibition with ganitumab was well-tolerated, however, our results do not support further study of ganitumab as a single agent in unselected OC patients.

9. A Framework for Designing MIMO systems with Decision Feedback Equalization or Tomlinson-Harashima Precoding

Abstract

We consider joint transceiver design for general Multiple-Input Multiple-Output communication systems that implement interference (pre-)subtraction, such as those based on Decision Feedback Equalization (DFE) or Tomlinson-Harashima precoding (THP). We develop a unified framework for joint transceiver design by considering design criteria that are expressed as functions of the Mean Square Error (MSE) of the individual data streams. By deriving two inequalities that involve the logarithms of the individual MSEs, we obtain optimal designs for two classes of communication objectives, namely those that are Schur-convex and Schur-concave functions of these logarithms. For Schur-convex objectives, the optimal design results in data streams with equal MSEs. This design simultaneously minimizes the total MSE and maximizes the mutual information for the DFE-based model. For Schur-concave objectives, the optimal DFE design results in linear equalization and the optimal THP design results in linear precoding. The proposed framework embraces a wide range of design objectives and can be regarded as a counterpart of the existing framework of linear transceiver design., Comment: To appear in ICASSP 2007

Abstract

Objective: COVID-19 toes represent the main dermatological COVID-19 cutaneous manifestation in pediatric patients. Its diagnosis exposes the whole family to social stigma and this aspect was not previously evaluated. Patients and methods: This was a multicenter, case-control, observational study that compared the family impact of COVID-19 toes vs. psoriasis (PsO). We enrolled 46 pediatric patients (23 with psoriasis and 23 with COVID-19 toes, age and gender matched) and their parents/caregivers that had to fill the Dermatitis Family Impact (DFI) questionnaire. Results: DFI index did not differ significantly between both subgroups (p=0.48), and in psoriatic patients did not correlate with both Psoriasis Area Severity Index (PASI) (p=0.59) and itch-VAS (p=0.16). Conclusions: COVID-19 toes, a transitory dermatosis, exerted a similar impact/perturbation on family dynamics than PsO, a well-known stigmatizing, chronic inflammatory dermatosis.

12. Patient safety with orthognathic surgery in an outpatient setting

Abstract

Orthognathic surgery is traditionally performed in inpatient care. The question is whether patient safety is maintained when orthognathic surgery is performed in outpatient care. This retrospective cohort study was conducted to investigate patient safety in selected single-jaw orthognathic surgeries performed in outpatient care compared to inpatient care. Postoperative infection, postoperative bleeding, postoperative pain, plate removal, and re-operation, as well as emergency visits/phone calls and postoperative admission during the first 12 months after surgery were recorded. Predictor variables were sex, age, smoking, general disease, antibiotics, operation type, and operation time. Of the 165 patients included, 58 were treated in inpatient care and 107 in outpatient care. No significant difference was found between the groups regarding postoperative bleeding, pain, plate removal, re-operation, or emergency visits/phone calls. Ninety-four percent of outpatients (n = 101) were able to leave the hospital on the day of surgery as planned. There was an increased risk of postoperative infection in the outpatient care group (odds ratio 2.46, P = 0.049). Selected single-jaw orthognathic surgery can be performed in the outpatient setting, with maintained patient safety. The reason for the increased risk of postoperative infection among patients operated in outpatient care should be investigated in further studies.

16. Synthesis, structure, and properties of semi-fluorinated polymers

Abstract

This thesis describes the synthesis main-chain and side-chain semi-fluorinated polymers by free radical and step growth techniques, and investigates trends in their thermal, solution and structural characteristics. Chapters 1 and 2 introduce liquid crystallinity and fluorinated materials and shows the liquid crystalline and structural behaviour of known fluorinated and semi-fluorinated materials. Chapter 3 describes the use of a free radical chain transfer polymerisation technique to synthesise polymer series with an alternating arrangement of fluorocarbon and hydrocarbon segments of varying length (FnRm (4), (6)). The thermal behaviour of these polymers was found to be quite complex and varied, a number of polymers with both short and longer fluorocarbon segments exhibiting mesophases, and multiple thermal transitions. An alternation of the transition temperatures with odd or even lengths of hydrocarbon segment was also observed. Structurally these materials are also complex, appearing to show an alternating packing arrangement with hydrocarbon packing against fluorocarbon. Chapter 4 details the synthesis of a polyester series with an alternating arrangement of fluorocarbon and hydrocarbon ester segments of varying length (FnRmPE (9)-(12)). The thermal characteristics of the polymers was more simple than the non-ester analogues, and liquid crystalline behaviour was not observed. The production of high molecular weight materials enabled the drawing of fibres, and X-ray diffraction of these showed a high number of reflections in most cases, with no obvious trend, indicating a very complex and varied packing behaviour. Finally, Chapter 5 investigates a series of amphiphilic semi-fluorinated polymers, synthesised by the same polyesterification technique, with varying hydrophilic ethylene oxide content (FOSAn (4), FOSPn (6), FSEGn (22)). The aim of the work was to try and facilitate water solubility with a view for use as a surfactant, but it was found that very high ethylene oxide content was required in order to approach water solubility rendering the use of ethylene oxide as the hydrophilic spacer non-viable. Polymers with shorter spacers were found to exhibit smectic A phases.

38. A systematic review of direct preference measurements in health states treated with plastic surgery

Abstract

One way to compare health care needs and outcomes on common scales is by estimating the strength of preferences or willingness-to-pay (WTP). The aim of this study was to review directly measured preference values and WTP estimates for health states treated by plastic surgery. The included articles had to meet the criteria defined in the SPIDER (Sample, Phenomenon of Interest, Design, Evaluation, Research type). Relevant databases were searched using predetermined strings. Data were extracted in a standardised manner. Included studies were appraised according to the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach for rating the importance of outcomes. In total, 213 abstracts were retrieved. Of these, 179 did not meet the inclusion criteria and were excluded, leaving 34 studies in the review. The risk of bias was considered moderate in four studies and serious in the rest. The overall certainty of evidence for directly measured preference values and WTP estimates for health states treated by plastic surgery is low (Grade ƟƟОО). The lowest preference scores were generally elicited for facial defects/anomalies and the highest for excess skin after massive weight loss. Scientific knowledge about preferences and the resulting health gains might play an essential role in deciding which procedures should be considered for public funding or rather rationed within the system. Better quality studies are required to allow for such applications.
