Almost 200 journalism outlets closed, decreased their service, ended print editions, or merged with other newsrooms between January 2019 and Februrary 2021, accelerated by the COVID-19 crisis. Regional news outlets were among the hardest hit, intensifying calls for effective government policy and industry interventions to bolster the sector. Although there is a well-established tradition of local and regional journalistic role performance scholarship, few studies in an Australian context have taken a multiplatform approach, investigating the perceptions of TV, radio, online news, and print journalists. To help address this gap, this paper draws on survey data of 307 regional Australian journalists and 31 in-depth interviews. Our analysis reveals differences in the way regional reporters understand and narrate their roles, highlighting important distinctions between media platforms that need to be considered in contemporary debates about the sustainability of Australian regional journalism.

Background: In 2014, germline signal transducer and activator of transcription (STAT) 3 gain-of-function (GOF) mutations were first described to cause a novel multisystem disease of early-onset lymphoproliferation and autoimmunity. Objective: This pivotal cohort study defines the scope, natural history, treatment, and overall survival of a large global cohort of patients with pathogenic STAT3 GOF variants. Methods: We identified 191 patients from 33 countries with 72 unique mutations. Inclusion criteria included symptoms of immune dysregulation and a biochemically confirmed germline heterozygous GOF variant in STAT3. Results: Overall survival was 88%, median age at onset of symptoms was 2.3 years, and median age at diagnosis was 12 years. Immune dysregulatory features were present in all patients: lymphoproliferation was the most common manifestation (73%); increased frequencies of double-negative (CD4−CD8−) T cells were found in 83% of patients tested. Autoimmune cytopenias were the second most common clinical manifestation (67%), followed by growth delay, enteropathy, skin disease, pulmonary disease, endocrinopathy, arthritis, autoimmune hepatitis, neurologic disease, vasculopathy, renal disease, and malignancy. Infections were reported in 72% of the cohort. A cellular and humoral immunodeficiency was observed in 37% and 51% of patients, respectively. Clinical symptoms dramatically improved in patients treated with JAK inhibitors, while a variety of other immunomodulatory treatment modalities were less efficacious. Thus far, 23 patients have undergone bone marrow transplantation, with a 62% survival rate. Conclusion:: STAT3 GOF patients present with a wide array of immune-mediated disease including lymphoproliferation, autoimmune cytopenias, and multisystem autoimmunity. Patient care tends to be siloed, without a clear treatment strategy. Thus, early identification and prompt treatment implementation are lifesaving for STAT3 GOF syndrome.

OBJECTIVES: The prevalence of hormonal contraceptive (HC) use and the associated symptomology of use or non-use are under-studied in athletic populations, and in particular, in strength and collision sports. This study aimed to investigate the prevalence of hormonal contraceptive use and reported side effects of the menstrual cycle and hormonal contraceptive use in powerlifters and rugby players. \ud \ud METHODS: Competitive female powerlifters and rugby players (aged ≥18 y), representing a strength and a collision sport respectively, completed an anonymous online questionnaire for the purpose of assessing self-reported prevalence of HC use, and symptoms of the menstrual cycle and HC use. Athletes were categorized by sport (powerlifters, n=149; rugby players, n=135) in order to conduct a stratified analysis. For open-ended questions, a content analysis was conducted to categorize responses, and frequency analyses were performed. \ud \ud RESULTS: Current HC use was reported by 51.1% of athletes, with similar prevalence for the two sports (powerlifting, 48.3% vs. rugby, 54.1%, P=0.34). Side effects of the menstrual cycle were reported in 83.5% of non-HC users, with the most common being unspecified cramping (42.4%), headache/migraine (24.5%), and fatigue (24.5%). Side effects were reported in 40.0% of HC users, with the most common being mood changes (17.9%), stomach pain (8.3%) and headaches/migraines (6.9%). \ud \ud CONCLUSION: A large proportion of HC users and non-users in this study experience negative side effects of HC use and the menstrual cycle, respectively. The symptoms experienced by both groups are wide-ranging, with a high degree of variation between individuals. The negative side-effects experienced by HC users and non-users may have an influence on athletic performance, and this requires future investigation.

A recent research study showed that blood flow restriction (BFR) therapy was safe and well tolerated but failed to demonstrate efficacy as a modality that provides greater gains in quadriceps strength when added to a standard home program in patients awaiting anterior cruciate ligament (ACL) reconstruction. Despite employing a validated method of measurement, the results were highly variable, indicating the need for measurements with sufficient accuracy to detect the small, but potentially meaningful, gains in quadriceps strength that's been attributed to BFR. The results inform future investigations of BFR prior to ACL surgery by demonstrating the need for accurate methods of measurements when the anticipated effects are small.

BACKGROUND: Combined physical and psychological programmes (CPPP) are recommended for people with disabling low back pain (LBP). Cognitive Functional Therapy (CFT) is a physiotherapist-led low intensity CPPP with positive effects in previous studies. The clinical and cost effectiveness of CFT has not previously been evaluated in a randomised controlled trial (RCT) in the United Kingdom (UK) National Health Service (NHS). Before a definitive RCT can be completed it is necessary to determine if completing such a study is possible. PURPOSE: To determine the feasibility of completing a definitive RCT, that will evaluate the clinical and cost-effectiveness of CFT in comparison to usual physiotherapy care for people with persistent LBP in the UK NHS. METHODS: A pragmatic two-arm parallel feasibility RCT comparing CFT with usual physiotherapy care for people with persistent LBP will be completed. Sixty participants will be randomly allocated to receive CFT or usual physiotherapy care. The primary outcome will be feasibility of completing a definitive RCT. Participant reported outcome measures will be recorded at baseline, three, six and twelve-month follow-up, including disability, pain intensity, quality of life and psychosocial function. Data will be analysed descriptively. A qualitative process evaluation will explore the acceptability of the research processes and interventions. DISCUSSION: The rationale and methodological design of a mixed methods feasibility RCT is presented. This study aims to inform the planning, design and completion of a future definitive RCT in the UK NHS. The results will be disseminated through peer reviewed open access journal publication.

Abacavir hypersensitivity syndrome (ABC HSS) is strongly associated with carriage of human leukocyte antigen (HLA)-B*57:01, which has a 100% negative predictive value for the development of ABC HSS. However, 45% of individuals who carry HLA-B*57:01 can tolerate ABC. We investigated immune and non-immune related genes in ABC HSS (n = 95) and ABC tolerant (n = 43) HLA-B*57:01 + patients to determine other factors required for the development of ABC HSS. Assignment of phenotype showed that ABC HSS subjects were significantly less likely than tolerants to carry only ERAP1 hypoactive trimming allotypes (p = 0.02). An altered self-peptide repertoire model by which abacavir activates T cells is in keeping with observation that endoplasmic reticulum aminopeptidase 1 (ERAP1) allotypes that favour efficient peptide trimming are more common in ABC HSS patients compared to patients who tolerate ABC. Independently, non-specific immune activation via soluble cluster of differentiation antigen 14 (sCD14) may also influence susceptibility to ABC HSS.

Proposals to use technology to cool sea surface temperatures have received attention for the potential application of weakening a tropical cyclone ahead of landfall. First, application of an ocean-mixing aware maximum potential intensity theory finds that artificial ocean cooling could drastically weaken tropical cyclones over high sea surface temperature, deep mixed layer environments, especially for fast translation speeds. In contrast, realistic mesoscale numerical simulations reveal that massive regions of artificial cooling could weaken a TC two days before landfall by 15% but only under the most ideal atmospheric and oceanic conditions. Thus, the fundamental theory provides an unreachable upper-bound that is only attainable via unlimited resources. Capital would likely be better spent on endeavors such as improved forecasting and near-shore infrastructure.

Optimal physical performance is a product of specific and tailored training. There are well-established sex differences in anatomical, physiological, and performance factors between biological males and females, which may have implications for physical preparation. A potential knowledge gap exists in relation to sex-specific differences in physical preparation because practitioners largely rely upon empirical evidence collected in male subjects for reference when devising interventions for female athletes. Therefore, this study explored the attitudes, beliefs and practices of strength and conditioning coaches ( n = 8; M/F, 6/2) in elite level (international) women's rugby union using semi-structured interviews (mean ± standard deviation duration 59 ± 15 minutes). The interviews explored differences in coaching elite female rugby players compared to males, with specific focus on training methodologies and understanding of pertinent aspects of female physiology. Reflexive thematic analysis was utilised to generate a rich qualitative dataset. Analysis resulted in the identification of higher-order themes: developmental stage of women's rugby, physical preparation, and education. Additional subthemes were created to facilitate organisation and presentation of data. The majority of coaches consider sex-specificity when devising physical preparation interventions as a function of training experience, rather than physiological between-sex differences, yet there were conflicting, and often erroneous understanding of female-specific considerations. To the authors knowledge, this is the first study to investigate attitudes, beliefs, and practices in elite level strength and conditioning coaches regarding sex-specific differences, and as such, illustrates the current understanding and opinions of practitioners in international level women's rugby union.

Although data from large implementation trials suggest that sexually transmissible infection (STI) risk increases among gay and bisexual men who initiate HIV pre-exposure prophylaxis (PrEP), there are few data on the trends in population-level STI incidence in the years following widespread PrEP implementation. We aimed to describe trends in bacterial STI incidence among gay and bisexual men using PrEP across Australia in the context of broad PrEP availability through Australia's subsidised medicines scheme.We analysed linked clinical data from HIV-negative gay and bisexual men aged 16 years or older who had been prescribed PrEP across a sentinel surveillance clinical network, including 37 clinics in Australia, between Jan 1, 2016, and Dec 31, 2019. Patients were included if they had STI testing at least twice during the observation period. Repeat testing methods were used to calculate chlamydia, gonorrhoea, syphilis, and any STI incidence rates during individuals' periods of PrEP use. Incidence rate ratios (IRRs) for estimated change in incidence per half calendar year (6-month) period were calculated using negative binomial regression. Secondary analyses compared STI incidence rates across individuals initiating PrEP in each year from 2016 to 2019, as well as by length of time using PrEP (per each additional 6 months of PrEP use).22 730 men were included in the analyses. During the observation period, 11 351 chlamydia infections were diagnosed in 6630 (30·1%) of 22 034 men over 25 991·2 person-years of PrEP use (incidence rate 43·7 cases [95% CI 42·9-44·5] per 100 person-years). Chlamydia incidence decreased from 48·7 cases per 100 person-years in July-December, 2016, to 42·0 cases per 100 person-years in July-December, 2019 (IRR for estimated change per 6-month period 0·98 [95% CI 0·97-0·99]; p=0·0031). 9391 gonorrhoea infections were diagnosed in 5885 (26·9%) of 21 845 men over 24 858·7 person-years of PrEP use (incidence rate 37·8 cases [95% CI 37·0-38·5] per 100 person-years). Gonorrhoea incidence decreased from 45·5 cases per 100 person-years in July-December, 2016, to 37·2 cases per 100 person-years in July-December, 2019 (IRR 0·97 [95% CI 0·96-0·98]; p0·0001). Declines in chlamydia and gonorrhoea incidence were most prominent in the first 18 months of observation and incidence was stable thereafter. 2062 syphilis infections were diagnosed in 1488 (7·7%) of 19 262 men over 21 978·9 person-years of PrEP use (incidence rate 9·4 cases [95% CI 9·0-9·8] per 100 person-years). Syphilis incidence increased from 6·2 cases per 100 person-years in July-December, 2016, to 9·8 cases per 100 person-years in July-December, 2019 (IRR 1·08 [95% CI 1·05-1·10]; p0·0001).Chlamydia and gonorrhoea incidence among gay and bisexual men using PrEP were highest in the early months of PrEP implementation in Australia and stabilised at slightly lower rates thereafter following wider PrEP uptake. Lower prospective STI risk among people initiating PrEP in later years contributed to the observed trends in STI incidence. Nolan, D, Lynch, AE, and Egan, B. Self-reported prevalence, magnitude, and methods of rapid weight loss in male and female competitive powerlifters. J Strength Cond Res 36(2): 405-410, 2022-Rapid weight loss (RWL) is common practice in weight category sports, but no empirical data exist documenting the weight-making practices of competitive strength athletes. This study investigated the self-reported prevalence, magnitude, and methods of RWL used by male and female powerlifters when preparing for competition. Competitive powerlifters (n = 321; M/F, 194/127) completed an anonymous online questionnaire previously validated for assessment of methods of RWL. Respondents were categorized by their federation's respective antidoping policy, weigh-in procedure, and degree of assistive equipment allowed, in addition to their use or not of RWL. Subgroup analyses were performed on the largest category of respondents (n = 200, M/F, 117/83; ≤2-hour weigh-in, drug-tested, "raw") based on sex, weight category, and competitive status. Prevalence of RWL was 85.8%, with an average RWL of 3.0 ± 1.9% body mass and an RWL score of 25.1 ± 7.4. Neither sex nor weight category influenced the RWL score, but in male athletes, a lower RWL score (22.7 ± 6.3) was reported in athletes in the lowest tertile of the Wilks score (p = 0.015). Frequencies of "always use" were reported as 54.0% for fluid restriction and 49.0% for water loading. Coaches (37.5%) and online resources (35.0%) were "very influential" on RWL practices in these athletes, while doctors (85.5%) and dieticians (63.0%) were reported to be "not influential." The prevalence of RWL is high in competitive powerlifting, and the methods used are akin to other weight category sports, but the reported RWL scores are lower than reported in combat sports with longer recovery periods after weigh-in.

Background It is reported that up to 29–52% of patients with cardiac sarcoidosis (CS) may have isolated cardiac sarcoidosis (ICS). The wide variation in prevalence may be related to the diagnostic methods for assessing extracardiac involvement. Whole-body 18F-fluorodeoxyglucose positron emission tomography-computed tomography (18F-FDG PET-CT) imaging is a useful and increasingly used technique for screening for extracardiac involvement in cases of suspected ICS. This study aims to determine the rate of isolated cardiac involvement with clinically manifest CS using cardiac 18F-FDG PET-CT. Methods We performed a retrospective analysis of data in the West Australian Cardiac Sarcoid (WACaS) Database. After cardiologist review and workup, all cases of proven or probable CS, based on either current Heart Rhythm Society criteria for the diagnosis of CS or local expert consensus were included. Only patients who underwent whole body 18F-FDG PET-CT were included in the final analysis. Results Fifty-two (52) cases of CS were identified. Data on symptoms, imaging findings, treatment and outcomes were collected. Of the 42 patients who underwent diagnostic 18F-FDG PET-CT, 32 demonstrated changes consistent with CS. Of the 32, 69% were male, mean age 50 years at diagnosis. Only 3/32 (9.4%) patients had ICS. Pulmonary involvement occurred in 91% with varied involvement in other organs. The mean number of extracardiac sites at diagnosis was 2.2. Conclusions This study demonstrates the utility of 18F-FDG PET-CT in diagnosing extracardiac organ involvement in cases of CS. With the use of this modality, ICS may be rarer than previously reported.

Background The Back-Pain Attitudes (Back-PAQ) questionnaire measures back beliefs across 6 domains. Our previous study showed that manual handling advisors (MHAs) have more negative beliefs than physiotherapists (PTs), and those who think straight back lifting is safer than a rounder back have more negative beliefs. However, exactly which domains of the Back-PAQ are most negative is unknown. Objectives Gain deeper understanding of how MHAs and PTs construct their back beliefs, and relate this safe lifting posture beliefs. Design Data was collected via an electronic survey. Method Participants' back beliefs were collected via the Back-PAQ. They were also asked to select the safest lifting posture from four options: two with a straight back; two with a rounder back. Back beliefs were analysed in the 6 domains that construct the Back-PAQ. Relationships were investigated using multiple linear and regression models. Results 400 PTs and MHAs completed the survey. MHAs scored higher (more negative beliefs) than PTs across all 6 domains, and those who perceive straight back lifting as safest scored higher across five of the 6 domains. The belief to keep active with back pain was common among all groups, but MHAs and those who prefer straight back lifting believe the back is vulnerable and more in need of protection. Conclusion While all believe staying active is beneficial for back pain, residual negative beliefs regarding the vulnerability of the spine persist. Education campaigns may need to emphasise a ‘trust your back’ message rather than a ‘protect your back’ message while encouraging activity.

Fish-derived proteins, particularly fish protein hydrolysates (FPH), offer potential as high-quality sources of dietary protein, whilst enhancing economic and environmental sustainability. This study investigated the impact of a blue whiting-derived protein hydrolysate (BWPH) on aminoacidaemia in vivo and skeletal muscle anabolism in vitro compared with whey protein isolate (WPI) and an isonitrogenous, non-essential amino acid (NEAA) control (0.33 g·kg-1·body mass-1) in an ex vivo, in vitro experimental design. Blood was obtained from seven healthy older adults (two males, five females, age: 72 ± 5 years, body mass index: 24.9 ± 1.6 kg·m2) in three separate trials in a randomised, counterbalanced, double-blind design. C2C12 myotubes were treated with ex vivo human serum-conditioned media (20%) for 4 h. Anabolic signalling (phosphorylation of mTOR, p70S6K, and 4E-BP1) and puromycin incorporation were determined by immunoblotting. Although BWPH and WPI both induced postprandial essential aminoacidaemia in older adults above the NEAA control, peak and area under the curve (AUC) leucine and essential amino acids were more pronounced following WPI ingestion. Insulin was elevated above baseline in WPI and BWPH only, a finding reinforced by higher peak and AUC values compared with NEAA. Muscle protein synthesis, as measured by puromycin incorporation, was greater after incubation with WPI-fed serum compared with fasted serum (P = 0.042), and delta change was greater in WPI (P = 0.028) and BWPH (P = 0.030) compared with NEAA. Myotube hypertrophy was greater in WPI and BWPH compared with NEAA (both P = 0.045), but was similar between bioactive conditions (P = 0.853). Taken together, these preliminary findings demonstrate the anabolic potential of BWPH in vivo and ex vivo, thus providing justification for larger studies in older adults using gold-standard measures of acute and chronic MPS in vivo.

The concept of uncritical incidents allows us to consider how the norms and practices of journalism are implicated in processes of societal racism, consistent with previous work that has focused on the routine application of news values and a reliance on predominantly white individuals and institutions as news sources. While drawing on textual evidence and emphasizing journalism as part of the problem contributing to both excessive incarceration and mistreatment, the Royal Commission into Aboriginal Deaths in Custody (RCIADIC) adopted a sociological, rather than merely moralistic, approach to understanding news production. In the wake of the RCIADIC, Indigenous deaths in custody remain a totemic issue, though one that is a little closer to resolution. The release of RCIADIC confronted Australian journalism with direct criticism of its norms and practices and could have served as an opportunity for a critical reflection.

Caesar’s skill in rhetoric means that in the Bellum Gallicum each word was chosen with careful consideration of its impact on the audience. His references to fortuna are no exception; the word was used persuasively throughout the work, such as to characterise the Gauls or exonerate Caesar from potential criticism. While it therefore serves literary purposes, closer examination of the term provides insight into Caesar’s strategic thinking, as he endeavoured to present himself as the ideal commander. This chapter examines how the term is utilised, where it is placed in each episode, and how it is balanced alongside other concepts that relate to risk or luck. The appearance of fortuna is also contrasted with its absence, and how its meaning varies depending on the characters involved. An examination of the narrative patterns and objectives that surround the use of fortuna is a valuable insight into Caesar’s presentation of command, and reveals a consistent set of principles regarding his concept of generalship.

With the advent of next-generation sequencing (NGS), monogenic forms of common variable immunodeficiency (CVID) have been increasingly described. Our study aimed to identify disease-causing variants in a Western Australian CVID cohort using a novel targeted NGS panel. Targeted amplicon NGS was performed on 22 unrelated subjects who met the formal European Society for Immunodeficiencies-Pan-American Group for Immunodeficiency diagnostic criteria for CVID and had at least one of the following additional criteria: disease onset at age18 years, autoimmunity, low memory B lymphocytes, family history, and/or history of lymphoproliferation. Candidate variants were assessed by in silico predictions of deleteriousness, comparison to the literature, and classified according to the American College of Medical Genetics and Genomics-Association for Molecular Pathology criteria. All detected genetic variants were verified independently by an external laboratory, and additional functional studies were performed if required. Pathogenic or likely pathogenic variants were detected in 6 of 22 (27%) patients. Monoallelic variants of uncertain significance were also identified in a further 4 of 22 patients (18%). Pathogenic variants, likely pathogenic variants, or variants of uncertain significance were found in TNFRSF13B, TNFRSF13C, ICOS, AICDA, IL21R, NFKB2, and CD40LG, including novel variants and variants with unexpected inheritance pattern. Targeted amplicon NGS is an effective tool to identify monogenic disease-causing variants in CVID, and is comparable or superior to other NGS methods. Moreover, targeted amplicon NGS identified patients who may benefit from targeted therapeutic strategies and had important implications for family members.
