Abstract

The diffraction endstation of the NanoMAX beamline is designed to provide high-flux coherent X-ray nano-beams for experiments requiring many degrees of freedom for sample and detector. The endstation is equipped with high-efficiency Kirkpatrick–Baez mirror focusing optics and a two-circle goniometer supporting a positioning and scanning device, designed to carry a compact sample environment. A robot is used as a detector arm. The endstation, in continued development, has been in user operation since summer 2017.

Abstract

Abstract Chronic obstructive pulmonary disease (COPD) treatment guidelines do not currently include recommendations for escalation directly from monotherapy to triple therapy. This 12-week, double-blind, double-dummy study randomized 800 symptomatic moderate-to-very-severe COPD patients receiving tiotropium (TIO) for ≥3 months to once-daily fluticasone furoate/umeclidinium/vilanterol (FF/UMEC/VI) 100/62.5/25 mcg via ELLIPTA (n = 400) or TIO 18 mcg via HandiHaler (n = 400) plus matched placebo. Study endpoints included change from baseline in trough forced expiratory volume in 1 s (FEV1) at Days 85 (primary), 28 and 84 (secondary), health status (St George’s Respiratory Questionnaire [SGRQ] and COPD Assessment Test [CAT]) and safety. 5. Language of Instruction: Concerning Its Choice and Social Prestige in Burkina Faso

Abstract

This article explores and discusses one of the main findings of the author's recent dissertation, namely that parents' and pupils' choice of language of instruction in formal schooling depends on its social prestige. The author first reviews the latest research on language in education in sub-Saharan Africa, and asks why indigenous languages are so rarely used in formal schooling in this region, despite political demands for their greater use and ample scientific research showing their positive effects. Burkina Faso exemplifies this seemingly contradictory situation. Indigenous languages and French are complementary in formal and non-formal schooling as well as in areas of informal education; however, a closer look at the areas of application of each language reveals that indigenous languages have lower prestige than French, as well as lower expected and required outcomes. This is one possible explanation for the low usage rates of indigenous languages in formal schooling and reveals the extent to which the choice of language of instruction depends on its social prestige.

Abstract

International audience; BackgroundAcute exacerbations of chronic obstructive pulmonary disease (AECOPD) are associated with changes in the sputum microbiome, including an increased prevalence of pathogenic bacteria. Vaccination against the most frequent bacteria identified in AECOPD might reduce exacerbation frequency. We assessed the efficacy, safety, and immunogenicity of a candidate vaccine containing surface proteins from non-typeable Haemophilus influenzae (NTHi) and Moraxella catarrhalis (Mcat) in patients with COPD.MethodsThis multicentre, randomised, observer-blinded, placebo-controlled, proof-of-concept, phase 2b trial recruited patients with stable COPD, moderate-to-very severe airflow limitation (Global Initiative for Chronic Obstructive Lung Disease [GOLD] stage 2, 3, or 4), at 67 clinical sites in Belgium, Canada, France, Germany, Italy, Spain, UK, and USA. Eligible patients were aged 40–80 years and had a history of at least one moderate or severe exacerbation in the previous year. Patients were allocated (1:1) using a minimisation algorithm to receive two intramuscular injections of NTHi–Mcat vaccine or placebo 60 days apart, in addition to standard care. The allocation algorithm considered age category, number of previous exacerbations, COPD severity at study entry, and country as minimisation factors, to guarantee treatment balance within each factor. Vaccine recipients and those responsible for evaluating study endpoints were masked to group allocation. In the analysis of efficacy, the primary outcome was the rate of any moderate or severe AECOPD occurring within a 1-year period, starting 1 month after the second dose in patients who received two vaccine doses (modified total vaccinated cohort). Safety was assessed in the total vaccinated cohort. The trial is registered with ClinicalTrials.gov, number NCT03281876, and is complete.FindingsBetween Nov 27, 2017, and Nov 30, 2018, 606 adults were enrolled and included in the total vaccinated cohort (304 in the NTHi–Mcat vaccine group, 302 in the placebo group); 571 received two doses and were included in the primary efficacy analysis (279 in the NTHi–Mcat vaccine group, 292 in the placebo group). 23 participants dropped-out in the NTHi–Mcat vaccine group and 39 in the placebo group; this included 4 patients in the NTHi–Mcat vaccine group and 15 in the placebo group who withdrew from the study because of an adverse event. The primary analysis included 340 exacerbations (in follow-up time 102 123 days) in the NTHi–Mcat vaccine group and 333 (in 104 443 days) in the placebo group, with a yearly rate of moderate or severe AECOPD of 1·22 in the NTHi–Mcat vaccine group and 1·17 in the placebo group, with vaccine efficacy in reducing the yearly rate of moderate or severe AECOPD estimated to be zero (vaccine efficacy point estimate 2·26% [87% CI –18·27 to 11·58]; p=0·82). Solicited local adverse events were more frequent in the NTHi–Mcat vaccine group (216 [72%] of 301 patients) than with placebo (34 [11%] of 299 patients), and the frequency of solicited general adverse events was similar between groups (239 [79%] of 301 vs 235 [79%] of 299 patients). There was one death in the NTHi–Mcat vaccine group (acute respiratory failure, not related to vaccination) and ten in the placebo group (seven due in part to COPD or respiratory failure). There were 158 serious adverse events (89 [29%] of 304 patients) in the NTHi–Mcat vaccine group, not related to vaccination, and 214 (99 [33%] of 302 patients) in the placebo group.InterpretationNTHi–Mcat vaccine administered to patients with COPD did not show efficacy in reducing the yearly rate of moderate or severe exacerbations. No safety concerns were identified.

Abstract

This work is an overview of the multi-modal hard X-ray nanoprobe beamline at the MAX IV Laboratory. The beamline provides an intense and coherent beam, focused to 40–200 nm in the energy range 5–28 eV., NanoMAX is the first hard X-ray nanoprobe beamline at the MAX IV laboratory. It utilizes the unique properties of the world’s first operational multi-bend achromat storage ring to provide an intense and coherent focused beam for experiments with several methods. In this paper we present the beamline optics design in detail, show the performance figures, and give an overview of the surrounding infrastructure and the operational diffraction endstation.

Abstract

Background: ANDHI was done to assess the efficacy of benralizumab, including onset of effect and impact on health-related quality of life (HRQOL), exacerbation rate, lung function, and nasal polyposis symptoms. Methods: This phase 3b, randomised, double-blind, parallel-group, placebo-controlled ANDHI study was completed in adults (aged 18–75 years) with severe eosinophilic asthma with at least 2 exacerbations in the previous year, despite high-dose inhaled corticosteroid plus additional controllers, screening blood eosinophil counts of at least 150 cells per μL, and an Asthma Control Questionnaire 6 (ACQ-6) score of 1·5 or more. Patients who met eligibility criteria were randomly assigned (2:1; stratified by previous exacerbation count [two, or three or more], maintenance oral corticosteroid use, and region), using an integrated web-based response system, to receive benralizumab at 30 mg every 8 weeks (first three doses given 4 weeks apart) or matched placebo for 24 weeks. Primary efficacy measure was annualised asthma exacerbation rate, with rate ratio (RR) calculated over the approximate 24-week follow-up. Secondary efficacy measures included change from baseline to end of treatment (week 24) in St George's Respiratory Questionnaire (SGRQ) total score (key secondary endpoint), FEV1, peak expiratory flow (PEF), ACQ-6, Predominant Symptom and Impairment Assessment (PSIA), Clinician Global Impression of Change (CGI-C), Patient Global Impression of Change (PGI-C), and Sino-Nasal Outcome Test-22 (SNOT-22). All efficacy analyses, except for SNOT-22, were summarised and analysed using the full analysis set on an intention-to-treat population (all randomly assigned patients receiving investigational product, regardless of protocol adherence or continued participation in the study). SNOT-22 was summarised for the subgroup of patients with physician-diagnosed nasal polyposis with informed consent. This study is registered with ClinicalTrials.gov, NCT03170271. Findings: Between July 7, 2017, and Sept 25, 2019, 656 patients received benralizumab (n=427) or placebo (n=229). Baseline characteristics were consistent with severe eosinophilic asthma. Benralizumab significantly reduced exacerbation risk by 49% compared with placebo (RR estimate 0·51, 95% CI 0·39–0·65; p5%) were nasopharyngitis (30 [7%]), headache (37 [9%]), sinusitis (28 [7%]), bronchitis (22 [5%]), and pyrexia (26 [6%]). Fewer serious adverse events were reported for benralizumab (23 [5%]) versus placebo (25 [11%]), and the only common serious adverse event (experienced by >1% of patients) was worsening of asthma, which was reported for nine (2%) patients in the benralizumab group and nine (4%) patients in the placebo group. Interpretation: Our results extend the efficacy profile of benralizumab for patients with severe eosinophilic asthma, showing early clinical benefits in patient-reported outcomes, HRQOL, lung function, and nasal polyposis symptoms. Abstract

Die Bedeutung von mehrsprachiger Bildung für Lernprozesse, für eine inklusive Bildungsumgebung sowie im erweiterten Sinn auch für Individuen und Gesellschaft ist Thema des Sammelbandes. Die Autor:innen plädieren für einen stärkeren Einbezug sprachlicher Vielfalt in Bildungseinrichtungen. Der Schwerpunkt des Sammelbands liegt auf den Potenzialen mehrsprachiger Bildungspraktiken zur Entwicklung von fachlichen und persönlichen Kompetenzen sowie zur Bewältigung des Alltags in einer mehrsprachigen Gesellschaft. Die Autor:innen präsentieren konkrete Vorschläge zur Forschung, Umsetzung und Evaluation mehrsprachiger Bildungspraktiken. Eine zentrale Rolle in der Forschungsdebatte spielt die kritische Reflexion des Translanguaging-Konzepts von Ofelia García. Darüber hinaus werden die Diagnostik von Mehrsprachigkeit, die Rolle von Herkunftssprachen in der Bildungspraxis sowie Diskrepanzen zwischen den Überzeugungen von Grundschullehrkräften und der tatsächlichen Umsetzung einer mehrsprachigen Bildungspraxis thematisiert. Der Sammelband bietet vielfältige Perspektiven auf die Potenziale, Herausforderungen und Chancen von mehrsprachiger Bildungspraxis. Er richtet sich an Forschende, Praktiker:innen sowie Studierende, die an einer evidenzbasierten und inklusionsorientierten Bildung interessiert sind und sich für die Förderung mehrsprachiger Kompetenzen einsetzen möchten.

Abstract

BACKGROUND: The efficacy and safety of benralizumab, an interleukin-5 receptor alpha-directed cytolytic monoclonal antibody, for the prevention of exacerbations in patients with moderate to very severe chronic obstructive pulmonary disease (COPD) are not known.METHODS: In the GALATHEA and TERRANOVA trials, we enrolled patients with COPD (at a ratio of approximately 2:1 on the basis of eosinophil count [≥220 per cubic millimeter vs. RESULTS: In GALATHEA, the estimates of the annualized exacerbation rate were 1.19 per year (95% confidence interval [CI], 1.04 to 1.36) in the 30-mg benralizumab group, 1.03 per year (95% CI, 0.90 to 1.19) in the 100-mg benralizumab group, and 1.24 per year (95% CI, 1.08 to 1.42) in the placebo group; the rate ratio as compared with placebo was 0.96 for 30 mg of benralizumab (P = 0.65) and 0.83 for 100 mg of benralizumab (P = 0.05). In TERRANOVA, the estimates of the annualized exacerbation rate for 10 mg, 30 mg, and 100 mg of benralizumab and for placebo were 0.99 per year (95% CI, 0.87 to 1.13), 1.21 per year (95% CI, 1.08 to 1.37), 1.09 per year (95% CI, 0.96 to 1.23), and 1.17 per year (95% CI, 1.04 to 1.32), respectively; the corresponding rate ratios were 0.85 (P = 0.06), 1.04 (P = 0.66), and 0.93 (P = 0.40). At 56 weeks, none of the annualized COPD exacerbation rate ratios for any dose of benralizumab as compared with placebo reached significance in either trial. Types and frequencies of adverse events were similar with benralizumab and placebo.CONCLUSIONS: Add-on benralizumab was not associated with a lower annualized rate of COPD exacerbations than placebo among patients with moderate to very severe COPD, a history of frequent moderate or severe exacerbations, and blood eosinophil counts of 220 per cubic millimeter or greater (Funded by AstraZeneca [GALATHEA and TERRANOVA] and Kyowa Hakko Kirin [GALATHEA]; GALATHEA and TERRANOVA ClinicalTrials.gov numbers, NCT02138916 and NCT02155660.).

27. Postkoloniale Theorie im DaZ-Modul. Ein Plädoyer

Abstract

Der Beitrag geht von einer am erziehungswissenschaftlichen Diskurs zu sprachlicher Vielfalt orientierten Analyse des Sprachgebrauchs in der Modulbeschreibung des DaZ-Moduls der Westfalischen Wilhelms-Universitat Munster aus. Durch die Analyse kann die Rhetorik der Modulbeschreibung in den Kontext einer defizitorientierten Wahrnehmung von Mehrsprachigkeit im Feld Schule verortet werden. Nach einer Darstellung des Forschungsstandes der Erziehungswissenschaften zum Thema Mehrsprachigkeit in Schule und Unterricht wird deutlich, dass diese implizite Defizitorientierung Stromungen innerhalb der aktuellen erziehungswissenschaftlichen Forschung, die individuelle Mehrsprachigkeit als Ressource wertschatzen, nicht entspricht. Ansatze postkolonialer Theorien konnen helfen, diesen Widerspruch offenzulegen und den Wert einer Unterrichtspraxis zu erkennen, in der sprachliche Vielfalt im Klassenzimmer als Chance fur alle Schulerinnen und Schuler fruchtbar gemacht wird. Zu zeigen, dass und wie postkoloniale Ansatze daruber hinaus eine Auseinandersetzung mit dem Themenkomplex Mehrsprachigkeit und Schule theoretisch fundieren konnen, indem sie dessen Mehrdimensionalitat und Relevanz in einer globalen Welt sichtbar machen, ist das Ziel dieses Aufsatzes. Der Aufsatz untersucht zuerst, welches Verstandnis von Mehrsprachigkeit im Bildungskontext aus der Modulbeschreibung spricht, bietet dann eine alternative Perspektive von Mehrsprachigkeit als Ressource an und zeigt schlieslich die Potenziale einer Auseinandersetzung mit postkolonialen Theorien im Rahmen des Moduls auf, wobei an konkreten Beispielen aufgezeigt wird, wie diese sich gewinnbringend in die Diskussionen im und um das DaZ-Modul mit Lehramtsstudierenden einsetzen lassen.

Abstract

Die Frage nach der Wahl der Unterrichtssprache sowohl in Ländern, deren Multilingualität unter anderem kolonialhistorische Wurzeln hat, als auch in Migrationsgesellschaften wie der deutschen, wird in Erziehungswissenschaft und Bildungspolitik international seit Jahrzehnten viel diskutiert. Dabei fallen u. a. Argumente, die ökonomische Aspekte nationaler Bildungssysteme betreffen, die Marginalisierung von Sprachen einbeziehen oder die Demokratisierung kollektiver Mentalitäten fordern. Deutlich wird: Die Debatte ist hochgradig normativ und politisch aufgeladen. Inwiefern sich der Diskurs in der praktischen Umsetzung im Bildungswesen einerseits und in den subjektiven Wahrnehmungen von Bildungsakteuren andererseits wiederfindet, untersucht die Autorin am Beispiel von Burkina Faso. Dabei liegt der Fokus nicht allein auf dem nationalen formalen Bildungssystem, sondern die Studie bezieht die Bereiche non-formaler und informeller Bildung mit ein. Dadurch entsteht eine multiperspektivische Momentaufnahme, deren Rezeption auch für Gestaltende von Unterricht in anderen multilingualen Settings aufschlussreich sein kann.

Abstract

Abstract Hereditary transthyretin-mediated (hATTR) amyloidosis is an underdiagnosed, progressively debilitating disease caused by mutations in the transthyretin (TTR) gene. V122I, a common pathogenic TTR mutation, is found in 3–4% of individuals of African ancestry in the United States and has been associated with cardiomyopathy and heart failure. To better understand the phenotypic consequences of carrying V122I, we conducted a phenome-wide association study scanning 427 ICD diagnosis codes in UK Biobank participants of African ancestry (n = 6062). Significant associations were tested for replication in the Penn Medicine Biobank (n = 5737) and the Million Veteran Program (n = 82,382). V122I was significantly associated with polyneuropathy in the UK Biobank (odds ratio [OR] = 6.4, 95% confidence interval [CI] 2.6–15.6, p = 4.2 × 10−5), which was replicated in the Penn Medicine Biobank (OR = 1.6, 95% CI 1.2–2.4, p = 6.0 × 10–3) and Million Veteran Program (OR = 1.5, 95% CI 1.2–1.8, p = 1.8 × 10−4). Polyneuropathy prevalence among V122I carriers was 2.1%, 9.0%, and 4.8% in the UK Biobank, Penn Medicine Biobank, and Million Veteran Program, respectively. The cumulative incidence of common hATTR amyloidosis manifestations (carpal tunnel syndrome, polyneuropathy, cardiomyopathy, heart failure) was significantly enriched in V122I carriers compared with non-carriers (HR = 2.8, 95% CI 1.7–4.5, p = 2.6 × 10−5) in the UK Biobank, with 37.4% of V122I carriers having at least one of these manifestations by age 75. Our findings show that V122I carriers are at increased risk of polyneuropathy. These results also emphasize the underdiagnosis of disease in V122I carriers with a significant proportion of subjects showing phenotypic changes consistent with hATTR amyloidosis. Greater understanding of the manifestations associated with V122I is critical for earlier diagnosis and treatment.

Abstract

A joint development program was begun in the spring of 2001 between Creative Pultrusions (Alum Bank, PA) and Tex-Tech Industries (N. Monmouth, ME) to evaluate novel constructions for ballistic defense. Events such as September 11 and the aircraft vulnerability to small arms fire were the motivation for the study. At least 13 samples were generated that tested ballistic resistance to a variety of threats including 9mm, 357 mag, and 44 mag projectiles. A parameter-based model was developed to predict the ballistic performance of novel constructions as a function of material composition. A robotic work station has been constructed for automatically initiating and monitoring microscale chemical reactions. The work station consists of a robot with a general‐purpose arm and interchangeable special‐purpose hands, a carousel of seven reaction vessels, a series of stations for performing quantitative automated thin‐layer chromatography (TLC), an on‐line multichannel absorption spectrophotometer, sensors for verification, syringes and pumps for sample and solvent delivery, a supervisory computer for real‐time data acquisition and process control, and a graphics terminal for data display. The controlling software is menu‐driven and contains a scheduling algorithm that permits multiple reactions to be performed simultaneously. The automated TLC process is performed in four stages (plate dispensing, sample application, plate development, and plate densitometry), and samples are moved sequentially among these stations by the robot. Design, implementation and management constitute the cornerstones of the building process and the current trend, albeit still at an embryonic state in Italy, is represented by the application of a problem-solving approach combining integrated analysis and research between interdisciplinary professionals able to integrate their know-how and experience. The project SEEMPubS (Smart Energy Efficient Middleware for Public Spaces), which ended in August 2013, investigated the theoretical and operational possibility of using a network of sensors to monitor energy consumption and raise awareness of this issue among end-users in academic and working places. The project DIMMER (District Information Modeling and Management for Energy Reduction), that kicked off on October 01st, 2013, represents its evolution, as the research extends from buildings (building scale) to the neighborhood (urban scale), simultaneously expanding the areas of study. Data and information to be processed and analyzed call, in this case, for a reconsideration of the activities to be carried out, through the use of innovative instruments and procedures to ensure correct information flow and optimal data management. For this purpose, we adopted the BIM methodology (Building Information Modeling) for digital representation and modeling of our case studies, facility management and analysis of interoperability between different software applications, investigating how the tools currently available on the market and the interdisciplinary collaboration between professionals from different areas may constitute the standard for tomorrow’s communication.

Abstract

I temi della progettazione, della realizzazione e della gestione costituiscono i capisaldi del processo edilizio e la tendenza attuale, seppure in stato embrionale per quanto riguarda il territorio nazionale, è rappresentata dall’applicazione di un approccio che preveda l’analisi e la ricerca di tipo integrato tra competenze e settori differenti per conoscenze ed esperienza. Nell’ambito del progetto SEEMPubS (Smart Energy Efficient Middleware for Public Spaces), conclusosi ad agosto del 2013, è stata analizzata, dal punto di vista teorico ed operativo, la possibilità di utilizzare un network di sensori per il monitoraggio dei consumi e di incrementare la consapevolezza relativa ai consumi energetici da parte degli utenti degli ambienti di studio e di lavoro. L’evoluzione di tale progetto è rappresentata dall’avvio del progetto DIMMER (District Information Modeling and Management for Energy Reduction) il 1 ottobre 2013, nel quale il livello di analisi viene esteso dall’edificio (scala edilizia) al quartiere/distretto (scala urbana) ampliando contemporaneamente i settori di analisi. La grande mole di dati che deve essere gestita ed analizzata prevede, anche in questo caso, un ripensamento delle attività ordinariamente svolte in merito, adottando strumenti innovativi e procedure diverse per garantire un corretto flusso informativo e una ottimale gestione dei dati. In risposta a ciò è stata adottata la metodologia BIM (Building Information Modeling), per la rappresentazione e la modellazione dei casi studio, la gestione delle facilities e lo studio dell’interoperabilità tra applicativi differenti, cercando di capire come gli strumenti disponibili e la sinergia tra le professionalità coinvolte possano costituire lo standard per la comunicazione del domani.
