OBJECTIVE To evaluate the use of administrative data for identification of labor induction and to estimate the variation in cesarean delivery rates among low-risk women who underwent labor induction. METHODS A cross-sectional study was performed examining live births in California hospitals during 2016 and 2017 using birth certificate data linked with maternal patient discharge records. Initially, eight hospitals performed medical record reviews by using reVITALize definitions on 46,916 deliveries to assess the validity of induction identification by birth certificate or discharge diagnosis records or both. Hospital-level variation in cesarean delivery rates was then assessed among all California hospitals for women with low-obstetric-risk first births before and after further risk adjustment and after the exclusion of potential medical and obstetric indications for induction. Variation in physician-level cesarean delivery rates after induction at four large hospitals also was examined. The relationships between cesarean delivery rates among women with induced labors compared with noninduced labors and with the hospital rate of induction also were explored. RESULTS Identifying induction by a combination of discharge diagnosis codes and birth certificate data had the highest accuracy (92.9%, 95% CI 92.7-93.2). Among 917,225 births at 238 birthing hospitals, there were 99,441 nulliparous women with term, singleton, vertex pregnancies who were induced. The median cesarean delivery rate after labor induction for nulliparous women with term, singleton, vertex pregnancies was 32.2%, with a range of 18.5-84.6%. This wide variation was not reduced after risk adjustment or after exclusion of all women with induction indications. A similar wide variation was noted within geographic regions, neonatal intensive care levels, and among individual physicians in the same facility. This National Partnership for Maternity Safety bundle, as with other bundles, is organized into four domains: Readiness, Recognition and Prevention, Response, and Reporting and Systems Learning. Bundle components may be adapted to individual facilities, but standardization within an institution is advised. Evidence-based resources and recommendations are provided to assist implementation.

Objective This study aims to compare the accuracy of visual, quantitative gravimetric, and colorimetric methods used to determine blood loss during cesarean delivery procedures employing a hemoglobin extraction assay as the reference standard. Study Design In 50 patients having cesarean deliveries blood loss determined by assays of hemoglobin content on surgical sponges and in suction canisters was compared with obstetricians' visual estimates, a quantitative gravimetric method, and the blood loss determined by a novel colorimetric system. Agreement between the reference assay and other measures was evaluated by the Bland–Altman method. Results Compared with the blood loss measured by the reference assay (470 ± 296 mL), the colorimetric system (572 ± 334 mL) was more accurate than either visual estimation (928 ± 261 mL) or gravimetric measurement (822 ± 489 mL). The correlation between the assay method and the colorimetric system was more predictive (standardized coefficient = 0.951, adjusted R2 = 0.902) than either visual estimation (standardized coefficient = 0.700, adjusted R2 = 00.479) or the gravimetric determination (standardized coefficient = 0.564, adjusted R2 = 0.304). Conclusion During cesarean delivery, measuring blood loss using colorimetric image analysis is superior to visual estimation and a gravimetric method. Implementation of colorimetric analysis may enhance the ability of management protocols to improve clinical outcomes.

The aim of the study was to assess if the cost of robotic-assisted total laparoscopic hysterectomy is similar to the cost of standard laparoscopic hysterectomy when performed by surgeons past their initial learning curve. A retrospective chart review of all hysterectomies was performed for benign indications without concomitant major procedures at Orange Coast Memorial Medical Center (OCMMC) and Saddleback Memorial Medical Center between January 1, 2013 and September 30, 2013. Robotic-assisted total laparoscopic hysterectomies (RTLH) and standard laparoscopic hysterectomies (LAVH and TLH) were compared. Data analyzed included only those hysterectomies performed by surgeons past their initial learning curve (minimum of 30 previous robotic cases). The primary outcome was the direct total cost of patient’s hospitalization related to hysterectomy. The secondary outcomes were estimated blood loss, surgery time, and days in hospital post-surgery. A multiple linear regression model was applied to evaluate the difference between RTLH and LAVH/TLH in hospital cost, blood loss, and surgery time, while adjusting for hospital, patient’s age, body mass index (BMI), whether or not the patient had previous abdominal/pelvic surgery, and uterine weight. The χ 2 test was applied to examine the association between hospital stay and surgery type. There were 93 hysterectomies (5 LAVH, 88 RTLH) performed at OCMMC and 90 hysterectomies (6 LAVH, 17 TLH, 67 RTLH) performed at Saddleback Memorial Medical Center. The hospitalization total cost result showed that, after adjusting for hospital, age, BMI, previous abdominal/pelvic surgery, and uterine weight, RTLH was not significantly more expensive than LAVH/TLH (mean diff. = $283.1, 95 % CI = [−569.6, 1135.9]; p = 0.51) at the 2 study hospitals. However, the cost at OCMMC was significantly higher than Saddleback Memorial Medical Center (mean diff. = $2008.7, 95 % CI = [1380.6, 2636.7]; p

The rise in maternal mortality has raised a significant concern for epidemiologists and providers. One of the most preventable and frequent causes of maternal death is hemorrhage-related events. Most providers of obstetrical care see such complications on a frequent basis, and the costs in maternal lives and blood usage have become a universal concern. As a result, a number of important responses by state and national groups have taken place to address these preventable bad outcomes.A number of well designed step-by-step protocols have had success in preventing morbidity and mortality. Expert panels have applied toolkits, including risk screening, medication protocols, transfusion protocols and clinical procedures, to either abate or mitigate the effects of uterine bleeding. The usage of these 'toolkits' has allowed institutions to lower blood utilization and improve maternal outcomes in various care settings. Readiness of institutions to rapidly respond in an organized fashion prevents many of the secondary complications.Postpartum hemorrhage toolkits and their application are excellent examples of system readiness improvement to address a specific obstetrical problem. Widespread adoption should lead to a reduction in maternal mortality and morbidity.

The amount of data generated by health information technology systems is staggering, and using those data to make meaningful care decisions that improve patient outcomes is difficult. The purpose of this article is to describe the Maternal Health Information Initiative, a multidisciplinary group of maternity care stakeholders charged with standardizing maternity care data. Complementary strategies that practicing clinicians can use to support this initiative and improve the usability of maternity care data are provided.

Objectives To estimate the risk of preterm delivery of twin pregnancies based upon sonographic cervical length measurement and gestational age at measurement. Methods Twin pregnancies that delivered between 1999 and 2005 and that underwent sonographic measurement of cervical length between 13 and 34 + 6 weeks' gestation were identified and a retrospective review performed. Women with anomalous pregnancies, multifetal reduction, cerclage placement or medically indicated deliveries before 35 weeks were excluded. Logistic regression analysis was used to estimate the risk of preterm delivery before 35 weeks. Results A total of 561 women underwent 2975 sonographic cervical length measurements during the study period. The rate of preterm delivery before 35 weeks was 19.4%. The risk of delivery before 35 weeks decreased by approximately 5% for each additional mm of cervical length (odds ratio (OR) 0.95 (95% CI, 0.93–0.97); P < 0.001) and by approximately 6% for each additional week at which the cervical length was measured (OR 0.94 (95% CI, 0.92–0.96); P < 0.001). Conclusion The gestational age at which cervical length is measured is an important consideration when estimating the risk of spontaneous preterm birth in twins. The risk of preterm delivery is increased at earlier gestational ages and as cervical length decreases. Copyright © 2012 ISUOG. Published by John Wiley & Sons, Ltd.

To evaluate the frequency of persistent pulmonary hypertension of the newborn (PPHN) following elective cesarean at greater than 34 weeks' gestation in an academically affiliated community hospital.Retrospective cohort study involving chart review of 300 newborns with PPHN between 1999 and 2006. Infants less than 34 weeks' or with congenital anomalies were excluded. Subjects were divided into two groups: (1) intended vaginal delivery and (2) elective cesarean.A total of 125 neonates were included. In all, 46 were delivered vaginally, 53 by cesarean after a trial of labor, and 26 by elective cesarean. No statistically significant differences were noted between groups in birth weight, gestational age, or length of stay. The crude relative risk (RR) of PPHN in cesareans prior to labor (elective cesareans) when compared to intended vaginal deliveries was 2.0 (95% CI 1.3-3.1). The RR of PPHN in elective cesareans when compared to spontaneous labor resulting in vaginal deliveries was 3.4 (95% CI 2.1-5.5). The adjusted RRs for these outcomes comparing the same delivery groups when considering gestational age at birth (less vs. equal to or more than 37 weeks') were 2.2 (95% CI 1.4-3.4) and 3.7 (95% CI 2.3-6.1), and birth weight (less vs. equal to or more than 2500 g) were 1.9 (95% 1.3-3.0) and 3.4 (95% CI 2.1-5.5), respectively. The incidence of PPHN in the elective cesarean group was 6.9 per 1000 deliveries. The number of cesareans to be avoided to prevent one case of PPHN in this cohort was 387 (number needed to harm, 95% CI 206.8-3003.1).Our findings include a high rate of PPHN following elective cesarean delivery, and suggest that physicians should consider this added morbidity when performing elective cesareans.

Purpose The mission of the California Maternal Quality Care Collaborative is to eliminate preventable maternal death and injury and to promote equitable maternity care in California. This article describes California Maternal Quality Care Collaborative's (CMQCC's) statewide multistakeholder quality improvement initiative to improve readiness, recognition, response, and reporting of maternal hemorrhage at birth and details the essential role of nurses in its success. Project design and approach In partnership with the State Department of Maternal, Child, and Adolescent Health, CMQCC identified maternal hemorrhage as a significant quality improvement opportunity. CMQCC organized a multidisciplinary, multistakeholder task force to develop a strategy for addressing obstetric (OB) hemorrhage. Project description The OB Hemorrhage Task Force, co-chaired by nurse and physician team leaders, identified four priorities for action and developed a comprehensive hemorrhage guideline. CMQCC is using a multilevel strategy to disseminate the guideline, including an open access toolkit, a minimal support-mentoring model, a county partnership model, and a 30-hospital learning collaborative. Clinical implications In participating hospitals, nurses have been the primary drivers in developing both general and massive hemorrhage policies and procedures, ensuring the availability of critical supplies, organizing team debriefing after a stage 2 (or greater) hemorrhage, hosting skills stations for measuring blood loss, and running obstetric (OB) hemorrhage drills. Each of these activities requires effort and leadership skill, even in hospitals where clinicians are convinced that these changes are needed. In some hospitals, the burden to convince physicians of the value of these new practices has rested primarily upon nurses. Thus, the statewide initiative in which nurse and physician leaders work together models the value of teamwork and provides a real-time demonstration of the potential for effective interdisciplinary collaboration to make a difference in the quality of care that can be achieved. Nurses provide significant leadership in multidisciplinary, multistakeholder quality projects in California. Ensuring that nurses have the opportunity to participate in formal leadership of these teams and are represented at all workgroup levels is critical to the overall initiative. Nurses brought key understanding of operational issues within and across departments, mobilized engagement across the state through the regional perinatal programs, and developed innovative approaches to solving clinical problems during implementation. Nursing leadership and integrated participation was especially critical in considering the needs of lower-resource settings, and was essential to the toolkit's enthusiastic adoption at the unit/service level in facilities across the state.

To compare latency period, infectious morbidity, neonatal morbidity and neonatal mortality in twin versus singleton pregnancies complicated by preterm premature rupture of membranes (PPROM) remote from term.A retrospective, matched cohort study comparing 41 twin and 82 singleton pregnancies complicated by PPROM between 24-0/7 and 31-6/7 weeks' gestation. The data were obtained by reviewing maternal and neonatal charts.The median latency periods were 3.6 days (interquartile range 1.5-13.9 days) for twins and 6.2 days (interquartile range 2.9-11.8 days) for singletons (p = 0.86). Twins were less likely to be complicated by clinical chorioamnionitis when compared with singletons (4/41 [9.8%] vs. 19/82 [23.2%], relative risk [RR] 0.42, 95% confidence interval [CI] 0.18-0.96). Histological evidence of chorioamnionitis was also lower in twins compared with singletons (14/39 [35.9%] vs. 46/68 [67.7%], RR 0.56, 95% CI, 0.34-0.92). These differences persisted after adjusting for race, insurance status, latency period and route of delivery. Neonatal morbidity and mortality rates were similar between the two groups.There was not a statistically significant difference in the latency periods for twin and singleton pregnancies complicated by PPROM. Clinical chorioamnionitis and histological evidence of infection were significantly less common in twins compared with singletons.

Objective The purpose of this study was to evaluate ethnic differences in low birth weight and gestational age–dependent birth weight. Study design A retrospective cohort study of 47,669 newborn infants was conducted with the use of the perinatal database of the Memorial Health Care System. Logistic regression was used to estimate the odds ratio for low birth weight by ethnic group, which took into account gestational age at delivery. Multiple linear regression models were then developed to predict ethnic differences in birth weight by gestational age. Results When compared with white women, the adjusted odds ratio for low birth weight in of Mexican origin women (1.16; 95% CI, 1.02-1.32) was substantially lower than that of African American women (2.21; 95% CI, 1.87-2.60). An analysis of birth weight by gestational age suggested that African American neonates were intrinsically smaller than Hispanic neonates (difference, 4.22%). Conclusion At least some of the differences in low birth weight that are noted among ethnic minorities may be attributable to ethnic differences in birth weight by gestational age.

The 'Triple Aim' concept of improving the health of the population, improving the experience of the patient, and controlling cost can be used as a guide post for the adoption of the necessary changes to thrive in a new construct of women's health care. Following these guiding principles should lead to improved clinical outcomes at affordable costs with high patient and provider satisfaction. The actual changes will come in the form of various 'transformational forces.' One of the driving forces will be conversion of the current payment structure from a fee-for-service model to value-based payments. In addition, the methods of care must be redesigned into a 'team-based' approach in which providers and patients use best practice protocols that are individualized to specific patient needs. Redesign will continue to drive consolidation of providers into larger groups to cover the cost of the needed infrastructure.

Thriving in the new system not only will require time-honored traits such as leadership and compassion but also will require the obstetrician/gynecologist to become comfortable with technology, care redesign, and quality improvement.

Our purpose was to compare the practice patterns and outcomes of physicians delivering in our institution to identify risk factors and management techniques that could explain the differences in individual cesarean section rates.We retrospectively reviewed detailed computerized delivery records (n = 16,230) collected from May 16, 1988, to July 30, 1995. We excluded physicians who had100 deliveries at our institution during the study period. The physicians were divided into two groups depending on whether their individual cesarean section rates were greater than (control group) or less than 15% (target group). Various cesarean section rates, risk factors for abdominal delivery, labor management techniques, and neonatal outcome parameters were calculated for each group. The cesarean section rates of the two groups were analyzed by year to assess changes.As expected by study design, the overall cesarean section rate was markedly different between the two groups (13.8% vs 23.8%). In addition, the primary, repeat, primigravid, and multiparous cesarean section rates were all lower for the target group. The rates of cesarean section for fetal distress (1.5% vs 3.3%) and cephalopelvic disproportion (5.3% vs 8.5%) were also significantly less in the target group. The rates of breech presentation, third-trimester bleeding, and active herpes cesarean sections were not lower. The control group had more postterm (8.6% vs 14.7%) and4000 gm infants (12.0% vs 13.7%) but similar numbers of low birth weight, multiple gestation, and preterm infants. The target group used more epidural anesthesia, oxytocin induction, and trial vaginal births after cesarean delivery and more successful trial vaginal births after cesarean sections. Over the study period the cesarean section rate in the target group remained unchanged, whereas it steadily declined in the control group.Individual physician's lower cesarean sections are primarily obtained by labor management and attempting vaginal birth after cesarean delivery. These practice patterns did not appear to lead to any increase in perinatal morbidity or mortality. Efforts to lower cesarean section rates of individual practitioners should focus on the areas of fetal distress, cephalopelvic disproportion, and repeat cesarean section.

To describe the impact of previous cervical surgery on preterm birth prior to 34 weeks in twins.A retrospective review of twin pregnancies delivered between January 1998 and December 2005 at two institutions was performed. Women with a prior cold knife cone (CKC), loop electrosurgical excision procedure (LEEP), or ablative procedure were compared to a control group of women who had not undergone a previous treatment for cervical dysplasia. The primary outcome was delivery before 34 weeks of gestation.A total of 876 women met inclusion criteria. Of these, 110 (12.6%) had previous surgical procedures for cervical dysplasia, including CKC (n = 10), LEEP (n = 36), cryotherapy (n = 59) and CO2 laser treatment (n = 5). Delivery prior to 34 weeks was more common in women with a previous CKC compared to women with no prior treatment (40% versus 11.3%; odds ratio [OR], 3.6; 95% confidence interval [CI], 1.7-8.0). Delivery prior to 34 weeks was not more common in women with a previous LEEP (8.3%; OR, 0.8; 95% CI, 0.3-2.3) or ablative procedure (9.4%; OR, 0.9; 95% CI, 0.4-1.9) in comparison to the untreated group. Adjusting for the potential confounders of age, tobacco use, infertility treatments and previous preterm birth did not change the results.Previous CKC is associated with delivery prior to 34 weeks while LEEP and ablative procedures are not. CKC should be carefully considered and avoided when possible in reproductive age women.

OBJECTIVE: We analyzed the delivery statistics from our institution to describe a successful program of cesarean section delivery reduction and to help us understand what factors explained the reduction. STUDY DESIGN: A retrospective analysis of various cesarean section rates and risk factors from a prospectively collected delivery database of all patients delivered between May 15, 1988, and June 30, 1994. During the study period we instituted a program of increasing awareness, confidential provider feedback, more aggressive laboring techniques, and other clinical guidelines. The delivery data were divided into 6-month intervals and analyzed by χ 2 tables. RESULTS: The overall cesarean section rate fell from 31.1% to 15.4%. Similar reductions were noted in the primary (17.9% to 9.8%) and repeat cesarean section rates (13.2% to 5.7%). The primary cesarean section rate fall was accompanied by a drop in abdominal delivery for cephalopelvic disproportion and fetal distress. The repeat cesarean section rate is explained by a significant increase in trial and successful vaginal birth after cesarean delivery. No increase in maternal, fetal, or neonatal morbidity or mortality was observed. CONCLUSIONS: We have demonstrated that the cesarean delivery rate can be safely lowered in a private hospital without mandated clinical changes. Measurements of the fetal cisterna magna were obtained from a prospective sample of patients undergoing diagnostic obstetric ultrasound examinations. These normal measurements were then compared to a retrospective sample of ultrasound scans from fetuses with the diagnosis of trisomy 18 by amniocentesis. The fetal cisterna magna increases in size throughout pregnancy. The sample from fetuses with trisomy 18 was different and had a higher rate of small and large cisterna magnas. After the two samples were mixed, it was determined that a cut-off of 2.5 standard deviations for the fetal cisterna magna would give a sensitivity of 26.3%, specificity of 99.1%, positive predictive value of 50.0% and negative predictive value of 97.6% for trisomy 18. The probability that a fetal cisterna magna of abnormal size will predict trisomy 18 in an early second-trimester ultrasound examination with the 2.5 standard deviation cut-off was estimated at 0.019. A fetal cisterna magna that is of abnormal size appears to be of diagnostic value in identifying the fetus with trisomy 18, but its sensitivity is low.

The CST is one of the first forms of fetal surveillance; it is still the only form that tries to use the principle of induced stress to reveal marginal placental insufficiency. With the expansion of biophysical evaluation, there are many choices in antenatal testing. Clearly the CST should not be the only form of testing to follow high-risk pregnancies. However, its proved efficacy and close correlation with intrapartum monitoring make it an essential tool. With nipple stimulation techniques, it is an easy and quick method for evaluating the fetus. Clinicians who follow high-risk pregnancies need to understand fully the administration and proper interpretation of the CST.

Our objective was to determine the most appropriate interval for assessing amniotic fluid volume with amniotic fluid index.In a retrospective analysis amniotic fluid indexes performed every 3 to 4 days in antepartum testing patients were compared with their follow-up values. Of 10,742 amniotic fluid indexes there were 7393 with follow-up values within 4 days. The results were stratified by current amniotic fluid index, gestational age, and concurrent nonstress test results. The groups were compared with chi 2 analysis.Patients with normal amniotic fluid index (or = 8 cm) had a 0.54% chance of oligohydramnios developing in the next 4 days. Those patients with low normal amniotic fluid indexes (5 to 8 cm) had a 5% chance of oligohydramnios developing within the next 4 days, and patients with low amniotic fluid indexes (or = 5 cm) had a 59% chance of persistent oligohydramnios 4 days after the index examination. Subdividing by gestational age demonstrated that patientsor = 41 weeks' gestation had a 2.6% chance of oligohydramnios developing within 4 days if current amniotic fluid index was between 8 and 15 cm. Results of concurrent fetal heart rate findings did not appear to change the risk for development of oligohydramnios.In patients41 weeks' gestation with normal amniotic fluid indexes, a repeat amniotic fluid index is not necessary for 7 days.

We evaluated serial sonography for the antenatal detection of small-for-gestational-age (SGA) infants in pregnancies with elevated human chorionic gonadotropin (hCG) levels on midtrimester triple-marker screen. A retrospective cohort study was performed at Saddleback Memorial Medical Center where serial ultrasounds from 26 weeks to delivery are generally recommended for patients with hCG levels2.0 Multiple of the Median (MoM). From 1999 to 2007, 659 subjects were identified for analysis. The incidence of intrauterine growth restriction (IUGR) and SGA were 5.2% and 7.3%, respectively. Antenatal ultrasound identified 31.3% of SGA infants. Compared with estimated fetal weight (EFW)10th percentile alone, abdominal circumference (AC)10th percentile improved the detection of SGA from 31.3% (95% confidence interval [CI], 18.7 to 46.3) to 35.4% (95% CI, 22.2 to 50.5). Using either EFW or AC further increased the sensitivity to 45.8% (95% CI, 31.4 to 60.8). The sensitivity for the detection of SGA was 100% when an EFW cutoff of 75% was used. Ultrasound can be used to detect SGA infants in patients with elevated hCG levels on midtrimester serum screening. A sonographic estimated fetal weightor = 75th percentile appears to be a safe cutoff to rule out all fetuses at risk for SGA.

To assess maternal postpartum and neonatal outcomes associated with previous method of delivery.We analyzed prospectively collected maternal and neonatal data from July 2002 to December 2003. Data were collected from dedicated perinatal database and neonatal database from discharge and procedure codes. Groups were: (i) multiparous, prior vaginal delivery (VD), and (ii) multiparous, prior cesarean (CS). This group was subdivided by subsequent pregnancy trial or no trial of labor (No TOL). Results were compared with chi (2)-analysis; significance P0.05.There were 17 406 births. Prior CS patients without trial of labor (TOL) required more blood transfusions, intensive care unit admissions and hospital readmissions than women with a prior VD. Prior CS patients with TOL required more aminoglycosides for postpartum infection. Term neonates of (CS) mothers without a TOL were more likely to have prolonged hospitalization and require ventilatory support.In their subsequent delivery, women with a prior CS delivery are at significant risks for postpartum maternal and neonatal morbidities compared with parous patients who experienced a prior VD.

We evaluated acute neonatal morbidities in the delivery room associated with primary cesarean performed prior to labor and in the first or second stages of labor. A retrospective cohort study was conducted on subjects undergoing term, primary cesareans at the Women's Pavilion, Miller Children's Hospital in Long Beach, California from 2000 to 2007. Acute neonatal morbidities were tabulated as a function of time during labor when cesarean was performed. Composite neonatal morbidity was defined as the presence of at least one of the following: low 5-minute Apgar score, need for intubation, and/or admission to neonatal intensive care unit. One thousand forty-five subjects delivered by cesarean without labor, 3098 in first stage, and 951 in second stage. Five-minute Apgar score < 7 was more common in cesareans performed during second stage than during first stage or without labor [3/1045 (0.3%) versus 28/3098 (0.9%) versus 12/951 (1.3%), P = 0.039]. Composite neonatal morbidity was not significantly different among the stages. Acute neonatal morbidities were not affected by the presence or absence of labor. Potential long-term sequelae require further study.

Antepartum testing has been recommended for patients whose pregnancies are complicated by hypertension. Although this is considered accepted practice, there are little data available to help the clinician know when to start testing. To help answer this question in patients with chronic hypertension and nonproteinuric pregnancy-induced hypertension, we reviewed the results of all antepartum tests between 1976 and 1987 in patients with these diagnoses. The primary mode of surveillance in the majority of our patients was the contraction stress test. We determined when patients first had positive contraction stress test results and when intervention occurred because of an abnormal antepartum test result. There were a total of 917 patients tested with these diagnoses. Fifty-three (5.8%) of these patients had at least one positive contraction stress test result. Twenty-two patients were delivered of infants before 35 weeks' gestation because of abnormal antepartum test results. Those with early intervention (less than 35 weeks' gestation) could not be differentiated from those with later intervention (greater than or equal to 35 weeks' gestation) by maternal age, diastolic blood pressure, or systolic blood pressure at the time of testing. The majority of patients who were delivered of infants before 35 weeks' gestation had a concomitant diagnosis of systemic lupus erythematosus, intrauterine growth retardation, diabetes mellitus, or superimposed preeclampsia. On the basis of when compromise was evident, patients with these diagnoses may require testing to be started as early as the fetus is considered viable. However, in those without these diagnoses, the clinician may delay the beginning of testing until 33 weeks' gestation without significant risk of pregnancy loss before testing.

External and internal pressures are causing rapid changes to the delivery of health care that markedly will influence the practice of obstetrics and gynecology. These changes can be divided into broad categories: (1) Burden of the high cost of current health care on society; (2) demographic changes in women that include aging, obesity, diversity, and chronic medical conditions; and (3) workforce changes that include growing provider shortages, inexperience, and desires for improved lifestyles. The combination of these factors has brought health care to a strategic inflection point where current practice methods will lead to an inability to meet the demand for health care because of increasing volume while simultaneously controlling costs and improving quality. This necessitates providing women's health care in a redesigned fashion for it to flourish in the new world of medicine.

The purpose of this study was to define the indications and outcomes of emergent cesarean deliveries.We analyzed prospectively collected singleton maternal and neonatal data from January 1, 1998, to December 31, 2004 of all such cesarean deliveries that were performed. A case-control analysis was performed by matching emergent cesarean deliveries with the next 2 acute cesarean deliveries.There were 126 emergent cesarean deliveries (1:159 deliveries). There were no significant differences in identifiable obstetric or chronic medical risk factors between the 2 groups. Compared with the control subjects, the subjects with emergent cesarean deliveries had an increased risks of cesarean delivery for non-reassuring fetal heart rate (P.0001; odds ratio, 12.0), cord prolapse (P.0001), and suspected uterine rupture (P.0001); in addition, greater risk of a low 1-minute Apgar score (P.001; odds ratio, 19.5) and low 5-minute Apgar score (P.001; odds ratio, 10.4), acute respiratory distress of infant (P.001; odds ratio, 4.21), and infant intubations (P.0001; odds ratio, 8.1).These data demonstrate that most emergent cesarean deliveries develop during labor in low-risk women and cannot be anticipated by prelabor factors. The outcomes demonstrate that infants are at risk in these clinical situations and suggest that strategies to improve performance in these clinical situations are important.

Objective This study was undertaken to assess whether racial differences in the risk of cesarean delivery result from differing practices of their caregivers or the hospitals at which they deliver. Study design A retrospective cohort study was performed using the Perinatal Database of the Memorial Health Care System. Logistic regression was used to estimate the risk of primary cesarean delivery among patients eligible for labor. The contribution of hospital and physician level cluster correlation was evaluated using fixed and random effects regression models. Results Compared with white patients, black and Hispanic patients were 75% and 22% more likely to undergo primary cesarean delivery. Further adjustment for hospital and physician level cluster correlation resulted in persistently increased risks of primary cesarean delivery in black (54%) and Hispanic patients (12%). Conclusion Hospital site of delivery and individual physician practices do not fully explain racial differences in the risk of primary cesarean delivery.

Objective: Although antepartum fetal well-being testing is an accepted practice in the management of diabetic patients, there are few data suggesting when to start. Our goal was to examine when testing should be started in the pregnant diabetic woman. Study Design: Antepartum test results and patient histories were prospectively collected on all diabetic pregnancies from January 1981 through December 1991. The data were retrospectively analyzed for when fetal compromise became evident. Fetal compromise was defined as stillbirth, first positive contraction stress test, or intervention because of an abnormal antepartum fetal test result. Results: Six hundred fourteen patients were enrolled in the study. There were three stillbirths, 45 (7.4%) patients had at least one positive contraction stress test, and 71 (11.6%) patients were delivered because of an abnormal fetal test result. Those with early compromise (≤34 weeks' gestation) could not be identified solely by diabetic class. The majority of patients (73%) requiring early intervention because of an abnormal test were class R or F diabetic patients with a growth-retarded fetus or were patients who had a concomitant diagnosis of hypertension. Conclusions: Class R or F diabetic patients or diabetic patients with a growth-retarded fetus or a concomitant diagnosis of hypertension may require testing to be started as early as 26 weeks' gestation. Otherwise, testing may be safely delayed until 32 weeks' gestation.

Objective The purpose of this study was to review the utilization statistics for voluntary physician adoption of a comprehensive inpatient electronic record by community obstetrician-gynecologists. Study Design A retrospective analysis of administrative data on the utilization of a modern electronic medical record system in a private hospital setting was performed. The physician statistics for order entry and physician documentation pre- and postimplementation were compared. Results During the study period monthly hospital average was 268 discharges per month and 64.9 orders per discharge. Utilizing the prior stand alone order entry system, 38.7% of orders were directly entered by physicians, and, following implementation, this rose to 72.1% with phone/verbal orders reduced by 44.9%. Monthly transcription rates of history and physical, consultations, operative and discharge summary notes were reduced by 61.5%, 75.8%, 56.6%, and 88.2%, respectively, over the study time period. Conclusion A well designed electronic medical record can be utilized in the voluntary private inpatient setting with significant physician adoption.

Objective The purpose of this study was to examine complications of labor induction compared to spontaneous labor in multiparas. Study Design This was a retrospective cohort study of multiparous women with live, singleton pregnancies at term, who had no contraindications to labor or labor induction. Cesarean delivery was the primary outcome. Results Of the study subjects, 7208 experienced spontaneous labor, 2190 underwent labor induction with oxytocin, and 239 underwent labor induction requiring cervical ripening agents. Oxytocin-induced multiparas were 37% more likely to require cesarean compared to those with spontaneous labor (OR, 1.37; 95% CI, 1.10-1.71) and nearly 3 times more likely to undergo cesarean when cervical ripening agents were used (OR, 2.82; 95% CI, 1.84-4.53). Women requiring cervical ripening were also 10 times more likely to spend more than 12 hours in labor than those with spontaneous labor. Conclusion Multiparas undergoing labor induction are at increased risk for obstetric complications compared to spontaneous labor.

ASSISTED REPRODUCTION VS SPONTANEOUS TWINS MICHAEL HAYDON (F), CECILIA LYONS, PAMELA RUMNEY, MARK GHAMSARY, DAVID LAGREW, University of California, Irvine, Maternal Fetal Medicine, Orange, California, Loma Linda University, School of Public Health, Loma Linda, California, Saddleback Memorial Medical Center, Laguna Hills, California OBJECTIVE: Conflicting data in the literature exists regarding differences in maternal and neonatal outcomes in twin pregnancies achieved through reproductive assistance and those conceived spontaneously. Our aim was to determine if pregnancy outcomes were worse in twins conceived following assisted reproduction techniques. STUDY DESIGN: A retrospective study of twin pregnancies at 3 hospitals from 1/1998-12/2004. There were 114 pregnancies achieved through in vitro fertilization, 28 by Clomid/intrauterine insemination (IUI), and 158 spontaneous twin pregnancies. We reviewed maternal and neonatal records for maternal and neonatal pregnacy related outcomes. Statistical analysis was performed using the Student’s t test, Mann-Whitney test and ANOVA. RESULTS: Maternal demographics were similar between groups aside from there being more Hispanic patients in the spontaneous pregnancy group and the average age was higher in the IVF group. There were no differences with respect to maternal or neonatal outcomes between those conceived with assisted reproduction vs those conceived spontaneously. CONCLUSION: Our findings suggest that twins conceived via IVF or Clomid/IUI have outcomes similar to those conceived spontaneously.

The purpose of this study was to assess maternal and neonatal postpartum outcomes associated with previous method of delivery. STUDY DESIGN: We analyzed prospectively collected singleton maternal and neonatal data from 7/2002-12/2003 in four MemorialCare hospitals. Birth data were collected into a dedicated perinatal database and combined with coded data from a MedAI database of postpartum maternal and neonatal procedures and outcomes. Patients were subdivided in four groups: (1) Nulliparous; (2) Prior vaginal; (3) Prior Cesarean (CS) without trial of labor (TOL); and (4) Prior Cesarean with TOL. Results were compared by chi square analysis with significance at P ! .05. RESULTS: During the study period there were 17,406 births fitting the above criteria. Compared to prior vaginal delivery, patients with prior CS had significantly increased risks of morbidity. The subgroup of prior CS patients without a TOL were more likely to require blood transfusion (P ! .001, OR 3.1), ICU admission (P ! .001, OR 4.5), and readmission within 30 days (P ! .025, OR 1.7) than patients with prior vaginal deliveries. Prior CS with TOL patients did not have these increased risks but were more likely to receive aminoglycosides for postpartum infection (P ! .01, OR 1.81). Term neonates born to mothers with previous CS were more likely to have prolonged hospitalization (>7 days) in both groups: no TOL (P ! .001, OR 6.69) and TOL (P ! .05, OR 2.51). There were no significant differences based on prior delivery route for neonatal mortality within the first 28 days of life, neonatal seizures or encephalopathy. CONCLUSION: Prior cesarean delivery is a significant risk factor for important postpartum morbidities of the mother and neonate. These risks and benefits should be carefully discussed with women considering elective primary cesarean section. SMFM Abstracts S7
