This paper presents an integrated principal component analysis (IPCA) technique for denoising phase-sensitive optical time domain reflectometry ( Φ -OTDR) sensing data for vibration detection. As one of the key distributed optical fiber sensing technologies, it has attracted great attention, mainly due to its high sensitivity, fast response time, dynamic range, and vibration detection abilities. To enhance vibration detection along the sensing fiber, an appropriate denoising method must be carefully selected. Hence, the PCA that can effectively reduce noise on signals while preserving significant details of the denoised signal is identified. It was then applied on the said signal after digital down-conversion where the noise was greatly reduced. Then angle and phase unwrapping was performed and the vibration was clearly detected with a significant enhancement of the signal-to-noise ratio. As proof of concept, the theoretical analysis and an experimental demonstration of a vibration sensing range of 800 m are presented.

The paper presents Principal Component Analysis (PCA) technique in denoising phase sensitive optical time domain reflectometry (Φ-OTDR) sensing data for vibration detection. The PCA is one of the most effective techniques used for noise reduction while preserving significant details of the denoised signals. As a result, we outline the basics of PCA and applied on the said data. It demonstrates that the PCA significantly reduced the noise and vibration along the phase trace clearly detected with a significant enhancement of the Signal to Noise ratio (SNR). A theoretical analysis is presented with an experimental demonstration of the vibration sensing range of 1km and a spatial resolution of 10m.

The radiocapitellar line (RCL) is a routinely referenced radiographic measurement for evaluating injury of the pediatric elbow, such as a Monteggia fracture. It is most commonly described as a line drawn along the radius that should intercept the center of the capitellum in a normal elbow. However, the RCL has not been rigorously defined or validated in young children.The RCL was drawn out on the anteroposterior and lateral radiographs of 116 normal pediatric elbows by 3 examiners. On each radiograph, lines were drawn along the longitudinal center of the radial neck as well as the radial shaft. The distance from each of these lines to the center of the capitellum was defined as the line-capitellar distance (LCD). The LCD was standardized to the patient size and correlated with patient variables.Of the 2052 RCLs drawn, 50% (1029/2052) fell outside the middle third of the capitellum and 8.6% (176/2052) missed the capitellum completely. Lines drawn along the radial neck in the anteroposterior and lateral views missed the capitellum less frequently (1.2% vs. 12.7%, P0.0001) and passed through its central third less (77.8% vs. 44.7%, P0.0001) when compared with lines drawn using the radial shaft. Lines along the radial neck intersected the capitellum significantly more often on the lateral view than on the anteroposterior view (98.8% vs. 96.8%, P=0.06) and were more likely to pass through the center third of the capitellum (77.8% vs. 55.5%, P0.0001). With both techniques of drawing the RCL, the percent of measurements falling within the middle third of the capitellum was higher in patients above 5 years of age (P0.001). The RCL is more reliable in older patients and shows considerable variation in infants.The RCL does not invariably bisect or fall within the middle third of the capitellum in normal pediatric elbows. Therefore, an abnormal RCL is suggestive but not pathognomonic for injury such as Monteggia fracture. An RCL drawn along the radial shaft will miss the capitellum in 15.6% of elbows. The most consistent RCL is drawn along the radial neck on the lateral view. Patient variables such as age and sex should be considered in the ability of the RCL to identify elbow injury.Diagnostic Level 3.

Background: Surveys have demonstrated that wrong-site surgery of the spine is performed by up to 50% of spine surgeons over the course of a career. Inaccurate identification of appropriate vertebral levels is a common reason for wrong-site spine surgery. The present study examined the prevalence of variations in the number of vertebrae in patients with adolescent idiopathic scoliosis. Methods: A retrospective review of radiographs and reports of 364 consecutive patients undergoing operative treatment for adolescent idiopathic scoliosis at a single center was performed. The study included eighty-eight male patients (24%) and 276 female patients (76%) with a mean age of fourteen years (range, ten to twenty years). Radiographs were reviewed to assess the number of thoracic and lumbar vertebrae and the presence of a lumbosacral transitional vertebra. Results: Ten percent of the patients (thirty-eight) had an atypical number of vertebrae in the thoracic and/or lumbar spine. Twenty-one patients (5.8%) had an atypical number of thoracic vertebrae, with fourteen having eleven thoracic vertebrae and seven patients having thirteen. Twenty-four patients (6.6%) had an atypical number of lumbar vertebrae, with four having four lumbar vertebrae and twenty patients having six. A lumbosacral transitional vertebra was present in 6.3% (twenty-three) of the patients. Multilevel vertebral anomalies were present in 1.9% of the patients (seven of 364). A variation in the number of vertebrae had been identified in 0.5% (two) of the reports by the radiologist. Conclusions: Variations in the number of thoracic or lumbar vertebrae were found in 10% of patients with adolescent idiopathic scoliosis but had been identified in only 0.5% of the radiology reports. Clinical Relevance: Assessment of possible variations in the number of vertebrae is important to help avoid surgery at the wrong vertebral level.

A sequence of mathematical models describing the electrothermal ribbon printing process is developed. The models describe the electrical processes, heat generation and heat transfer within the ribbon and the transfer of ink from the ribbon to paper. Account has been taken of the nonlinear electrical properties of the ribbon. A model of the paper surface has been developed in order to model the transfer of the ink from the ribbon to the paper. The partial differential equations describing the electric field, the heat flow and the stresses in the ink have been solved using standard techniques.

This is a case of a child with spastic quadriplegic cerebral palsy who had regrowth of her proximal femurs following bilateral proximal femur resections.

Much attention has been given to the relationship between various training factors and athletic injuries, but no study has examined the impact of sleep deprivation on injury rates in young athletes. Information about sleep practices was gathered as part of a study designed to correlate various training practices with the risk of injury in adolescent athletes.Informed consent for participation in an online survey of training practices and a review of injury records was obtained from 160 student athletes at a combined middle/high school (grades 7 to 12) and from their parents. Online surveys were completed by 112 adolescent athletes (70% completion rate), including 54 male and 58 female athletes with a mean age of 15 years (SD=1.5; range, 12 to 18 y). The students' responses were then correlated with data obtained from a retrospective review of injury records maintained by the school's athletic department.Multivariate analysis showed that hours of sleep per night and the grade in school were the best independent predictors of injury. Athletes who slept on average8 hours per night were 1.7 times (95% confidence interval, 1.0-3.0; P=0.04) more likely to have had an injury compared with athletes who slept for ≥8 hours. For each additional grade in school, the athletes were 1.4 times more likely to have had an injury (95% confidence interval, 1.2-1.6; P0.001).Sleep deprivation and increasing grade in school appear to be associated with injuries in an adolescent athletic population. Encouraging young athletes to get optimal amounts of sleep may help protect them against athletic injuries.Level III.

INTRODUÇÃO: O transplante de intestino delgado, em razão de sua complexidade, apresentou evolução mais lenta que os demais órgãos sólidos. Diversos avanços permitiram sua aplicação clínica. OBJETIVO: Revisão da evolução do transplante de intestino delgado e seu estado atual. MÉTODO: levantamento bibliográfico nas bases de dados MEDLINE e ScIELO. Os termos usados como descritores foram: intestinal failure, intestinal transplant, small bowel transplant, multivisceral transplant. Foram analisados dados sobre evolução histórica, centros, indicações, tipos de enxertos, seleção e captação de órgãos, manejo pós-operatório, complicações e resultados. CONCLUSÃO: Apesar de desenvolvimento mais lento, o transplante intestinal é hoje a terapia para pacientes portadores de falência intestinal irreversível que apresentam complicações da nutrição parenteral. Envolve algumas modalidades: intestino delgado isolado, fígado-intestino, multivisceral e multivisceral modificado. Atualmente a sobrevida é semelhante aos demais órgãos sólidos. A maioria dos pacientes fica livre da nutrição parenteral. BACKGROUND: Small bowel transplantation evolution, because of its complexity, was slower than other solid organs. Several advances have enabled its clinical application. AIM: To review intestinal transplantation evolution and its current status. METHOD: Search in MEDLINE and ScIELO literature. The terms used as descriptors were: intestinal failure, intestinal transplantation, small bowel transplantation, multivisceral transplantation. Were analyzed data on historical evolution, centers experience, indications, types of grafts, selection and organ procurement, postoperative management, complications and results. CONCLUSION: Despite a slower evolution, intestinal transplantation is currently the standard therapy for patients with intestinal failure and life-threatening parenteral nutrition complications. It involves some modalities: small bowel transplantation, liver-intestinal transplantation, multivisceral transplantation and modified multivisceral transplantation. Currently, survival rate is similar to other solid organs. Most of the patients become free of parenteral nutrition.

Background Previous studies have reported that the use of abduction bracing after Pavlik harness failure in patients with developmental dysplasia of the hip (DDH) is often effective in achieving hip reduction. Methods This is a retrospective study of consecutive patients with DDH at a single institution between 2006 and 2011. Patients in this study met all 4 inclusion criteria: (1) DDH, (2) a dislocated or dislocatable hip, (3) unsuccessful treatment with a Pavlik harness, and (4) subsequent treatment with abduction bracing. Exclusion criteria were dislocated hips associated with neurological, teratologic, syndromic, or other nonidiopathic conditions. Results Seven hips met inclusion criteria. Mean age at Pavlik harness initiation was 2.1 months (range, 1 d to 6 mo) and patients spent an average of 1.2 months (range, 0.4 to 2.7 mo) in the harness. After ultrasound demonstrated a persistently dislocated hip, patients were transitioned to an abduction brace and spent an average of 1.3 months (range, 0.1 to 3 mo) in the brace. One hundred percent (7/7) of hips treated with an abduction orthosis failed to reduce and required further treatment; 4 with closed reduction and spica casting, and 3 with open reduction. Patients were followed for an average of 33.6 months (range, 10 to 60 mo), and all patients had stable hips at latest follow-up. Conclusions In this series of 7 patients with developmentally dislocated hips who failed Pavlik harness treatment, subsequent abduction bracing provided no benefit, in contrast to previous studies. All patients who failed Pavlik harness treatment went on to require closed or open reduction of the hip. Abduction bracing may unnecessarily prolong the time to definitive treatment. OBJECTIVE: To compare low and high MELD scores and investigate whether existing renal dysfunction has an effect on transplant outcome. METHODS: Data was prospectively collected among 237 liver transplants (216 patients) between March 2003 and March 2009. Patients with cirrhotic disease submitted to transplantation were divided into three groups: MELD > 30, MELD < 30, and hepatocellular carcinoma. Renal failure was defined as a ± 25% decline in estimated glomerular filtration rate as observed 1 week after the transplant. Median MELD scores were 35, 21, and 13 for groups MELD > 30, MELD < 30, and hepatocellular carcinoma, respectively. RESULTS: Recipients with MELD > 30 had more days in Intensive Care Unit, longer hospital stay, and received more blood product transfusions. Moreover, their renal function improved after liver transplant. All other groups presented with impairment of renal function. Mortality was similar in all groups, but renal function was the most important variable associated with morbidity and length of hospital stay. CONCLUSION: High MELD score recipients had an improvement in the glomerular filtration rate after 1 week of liver transplantation. MÉTODOS: Realizou-se coleta prospectiva de dados em 237 transplantes de fígado (216 pacientes) entre março de 2003 e março de 2009. Pacientes com cirrose submetidos a transplante foram divididos em três grupos: MELD > 30, MELD < 30, e carcinoma hepatocelular. Insuficiência renal foi definida como uma diminuição de ± 25% na taxa de filtração glomerular estimada, observada 1 semana após o transplante. As medianas do MELD foram 35, 21, e 13 para os grupos MELD > 30, MELD < 30, e de carcinoma hepatocelular, respectivamente. RESULTADOS: Receptores com MELD > 30 tiveram mais dias na Unidade de Terapia Intensiva, maior período de internação, e receberam mais transfusões de sangue. Além disso, sua função renal melhorou após o transplante de fígado. Os demais grupos apresentaram diminuição da função renal. A mortalidade foi semelhante em todos os grupos, mas a função renal foi a variável mais importante associada com morbidade e tempo de internação hospitalar. CONCLUSÃO: Em receptores com escores MELD altos houve melhora da taxa de filtração glomerular 1 semana após o transplante de fígado.

Métodos: Oito porcos da raça Large White (quatro doadores e quatro receptores) foram operados. O transplante multivisceral com estômago, duodeno, pâncreas, fígado e intestino foi realizado a semelhança do transplante em seres humanos com algumas diferenças descritas a seguir. Foram realizadas as anastomoses de veia cava supra-hepática do enxerto com a veia cava do receptor justa diafragmática término-terminal, veia cava infrahepática do enxerto com a veia cava inferior (suprarrenal) do receptor término-terminal e patch da aorta do enxerto com a aorta infrarrenal do receptor término-lateral e reconstrução digestiva. Resultados: Foi possível a realização do transplante multivisceral nos quatro animais. A reperfusão do enxerto multivisceral levou a uma grave síndrome de isquemia-reperfusão, apesar do flushdo enxerto. Os animais apresentaram hipotensão com necessidade de drogas vasoativas em altas doses, sendo todos sacrificados com a retirada dessas drogas. Conclusão: Alternativas para minimizar a síndrome de isquemiareperfusão, como o uso de mais de uma droga vasoativa, uso de um terceiro porco apenas para transfusão sanguínea, presença de uma equipe de anestesia na sala de cirurgia e redução do enxerto, serão os próximos passos para possibilitar estudos experimentais. Objective: To present a model for research and training in multivisceral transplantation in pigs. Methods: Eight Large White pigs (four donors and four recipients) were operated. The multivisceral transplant with stomach, duodenum, pancreas, liver and intestine was performed similarly to transplantation in humans with a few differences, described below. Anastomoses were performed as follows: end-to-end from the supra-hepatic vena cava of the graft to the recipient juxta diaphragmatic vena cava; end-to-end from the infra-hepatic vena cava of the graft to the inferior (suprarenal) vena cava of the recipient; and endto-side patch of the aorta of the graft to the infrarenal aorta of the recipient plus digestive reconstruction. Results: The performance of the multivisceral transplantion was possible in all four animals. Reperfusions of the multivisceral graft led to a severe ischemia-reperfusion syndrome, despite flushing of the graft. The animals presented with hypotension and the need for high doses of vasoactive drugs, and all of them were sacrificed after discontinuing these drugs. Conclusion: Some alternatives to minimize the ischemia-reperfusion syndrome, such as the use of another vasoactive drug, use of a third pig merely for blood transfusion, presence of an anesthesia team in the operating room, and reduction of the graft, will be the next steps to enable experimental studies.
