Abstract

Abstract Background Opportunistic invasive fungal infections are increasingly frequent in intensive care patients. Their clinical spectrum goes beyond the patients with malignancies, and for example invasive pulmonary aspergillosis has recently been described in critically ill patients without such condition. Liver failure has been suspected to be a risk factor for aspergillosis. Case presentation We describe three cases of adult respiratory distress syndrome with sepsis, shock and multiple organ failure in patients with severe liver failure among whom two had positive Aspergillus antigenemia and one had a positive Aspergillus serology. In all cases bronchoalveolar lavage fluid was positive for Aspergillus fumigatus. Outcome was fatal in all cases despite treatment with voriconazole and agressive symptomatic treatment. Conclusion Invasive aspergillosis should be among rapidly raised hypothesis in cirrhotic patients developing acute respiratory symptoms and alveolar opacities.

Abstract

Penicillium marneffei infection should be suspected in any HIV patient who develops fever, skin lesions, lung disease and weight loss, and has a history of travel to southeast Asia. Penicillium marneffei is a fungus that is widespread in southeast Asia, and there have been almost 50 reported cases in southeast Asia in HIV patients as well as in healthy individuals. There have been several additional cases in HIV patients who have traveled to southeast Asia. The symptoms include fever, anemia, skin lesions, fluid in the lungs and an enlarged liver. The fungus probably enters the body through the respiratory tract, but can infiltrate other organs. It can be isolated from blood, bone marrow, skin scrapings and liver biopsies. It is usually treated with amphotericin B, ketoconazole or itraconazole.

Abstract

International audience; BACKGROUND: The performance of serum biomarkers for the early detection of invasive aspergillosis expectedly depends on the timing of test results relative to the empirical administration of antifungal therapy during neutropenia, although a dynamic evaluation framework is lacking. METHODS: We developed a multi-state model describing simultaneously the likelihood of empirical antifungal therapy and the risk of invasive aspergillosis during neutropenia. We evaluated whether the first positive test result with a biomarker is an independent predictor of invasive aspergillosis when both diagnostic information used to treat and risk factors of developing invasive aspergillosis are taken into account over time. We applied the multi-state model to a homogeneous cohort of 185 high-risk patients with acute myeloid leukemia. Patients were prospectively screened for galactomannan antigenemia twice a week for immediate treatment decision; 2,214 serum samples were collected on the same days and blindly assessed for (1->3)- β-D-glucan antigenemia and a quantitative PCR assay targeting a mitochondrial locus. RESULTS: The usual evaluation framework of biomarker performance was unable to distinguish clinical benefits of β-glucan or PCR assays. The multi-state model evidenced that the risk of invasive aspergillosis is a complex time function of neutropenia duration and risk management. The quantitative PCR assay accelerated the early detection of invasive aspergillosis (P = .010), independently of other diagnostic information used to treat, while β-glucan assay did not (P = .53). CONCLUSIONS: The performance of serum biomarkers for the early detection of invasive aspergillosis is better apprehended by the evaluation of time-varying predictors in a multi-state model. Our results provide strong rationale for prospective studies testing a preemptive antifungal therapy, guided by clinical, radiological, and bi-weekly blood screening with galactomannan antigenemia and a standardized quantitative PCR assay.

44. Detection of AspergillusGalactomannan Antigenemia To Determine Biological and Clinical Implications of Beta-Lactam Treatments

Abstract

ABSTRACTDetection of Aspergillusgalactomannan (GM) in serum with the Platelia Aspergillusenzyme immunoassay (EIA) is useful for diagnosing invasive aspergillosis. From May 2003 to November 2004, 65 patients who did not develop aspergillosis had at least two positive sera while receiving a beta-lactam treatment (GM index [GMI], =0.5). Of the 69 treatment episodes scored, 41 consisted of a beta-lactam other than piperacillin-tazobactam (n= 29), namely, amoxicillin-clavulanate (n= 25), amoxicillin (n= 10), ampicillin (n= 3), or phenoxymethylpenicillin (n= 2). In all cases, antigenemia became negative 24 h to 120 h upon stopping the antibiotic. Monitoring of 35 patients, including 26 with hematological malignancies, revealed three antigenemia kinetic patterns: each was observed with any drug regimen and consisted of a persistent GMI of >2.0 (65.7%), >0.5, and =1.5 (25.7%) or a variable GMI (14.3%) from the onset of antibiotic therapy. All available drug batches given to 26 patients cross-reacted with the EIA. Galactomannan titration in batches failed to predict the GM titers in the five patients studied at cumulative doses of ampicillin or amoxicillin-clavulanate, regardless of the time lapse between serum sampling and infusion period. Our results show that beta-lactams other than piperacillin-tazobactam may lead to false presumption of aspergillosis. The resulting kinetic patterns of GM antigenemia are variable, and sampling serum prior to the next beta-lactam dose may not decrease GMI below the threshold. Consequently, testing of suspected antibiotic batches remains the only indicator of possible false EIA positivity.

Abstract

ABSTRACTAn original, reliable, and reproducible method for the purification of Enterocytozoon bieneusispores from human stools is described. We recently reported the production of a species-specific monoclonal antibody (MAb) 6E52D9 immunoglobulin G2a (IgG2a) raised against the exospore of E. bieneusispore walls. The MAb was used as a ligand to develop an immunoaffinity matrix. The mouse IgG2a MAb was bound directly to a Streamline rProtein A adsorbent, used for expanded-bed adsorption of immunoglobulins, for optimal spatial orientation of the antibody and maximum binding efficiency of the antigen. The complex was then cross-linked covalently using dimethyl pimelimidate dihydrochloride. After incubation of the immunoaffinity matrix with filtered stool samples containing numerousE. bieneusispores and before elution with 6 M guanidine HCl, the expansion of the adsorbent bed eliminated all the fecal contaminants. The presence of spores in the elution fractions was determined by an indirect immunofluorescence antibody test (IFAT).E. bieneusispores were found in the elution fraction in all four experiments and were still highly antigenic as indicated by IFAT. Smears examined by light microscopy contained very clean spores with no fecal debris or background bacterial and fungal contaminants. However, spore recovery rates were relatively low: an average of 107spores were purified per run. This technique for isolating E. bieneusispores directly from human stool samples with a high degree of purity opens up new approaches for studying this parasite.

50. Efficacy of Ivermectin in the Therapy of Larva Currens

Abstract

Larva currens is the most typical cutaneous manifestation of human strongyloidiasis.1 The recommended therapy for strongyloidiasis is oral tiabendazole or albendazole, but the former is poorly tolerated while the latter is poorly effective.2 Oral ivermectin, the current drug of choice for onchocerciasis,3 has recently been reported as very effective and well tolerated in patients with gastrointestinal helminthiases, such as strongyloidiasis.4,5 These findings prompted us to test this drug in three cases of larva currens. REPORT OF CASES. CASE 1. A 38-year-old man weighing 66 kg was first seen in August 1990. He was born in Brazil and had lived there up until 1987. He complained of recurrent episodes of watery diarrhea, abdominal discomfort, and nausea; larva currens appeared in 1985. The blood count showed mild hypereosinophilia (0.7×109/L) and a stool test was positive for Strongyloides stercoralis larvae. We treated him with 2 tablets of
