1. Immunohistochemical expression in idiopathic inflammatory myopathies at a single center in Vietnam

Abstract

Background The identification of idiopathic inflammatory myopathies (IIMs) requires a comprehensive analysis involving clinical manifestations and histological findings. This study aims to provide insights into the histopathological and immunohistochemical aspects of IIMs. Methods This retrospective case series involved 56 patients diagnosed with IIMs at the Department of Pathology, University of Medicine and Pharmacy at Ho Chi Minh City, from 2019 to 2023. The histology and immunohistochemical expression of HLA-ABC, HLA-DR, C5b-9, Mx1/2/3, and p62 were detected. Results We examined six categories of inflammatory myopathy, including immunemediated necrotizing myopathy (58.9%), dermatomyositis (DM; 23.2%), overlap myositis (8.9%), antisynthetase syndrome (5.4%), inclusion body myositis (IBM; 1.8%), and polymyositis (1.8%). The average age of the patients was 49.7 ± 16.1 years, with a female-to-male ratio of 3:1. Inflammatory cell infiltration in the endomysium was present in 62.5% of cases, perifascicular atrophy was found in 17.8%, and fiber necrosis was observed in 42 cases (75.0%). Rimmed vacuoles were present in 100% of cases in the IBM group. Immunohistochemistry showed the following positivity rates: HLA-ABC (89.2%), HLA-DR (19.6%), C5b-9 (57.1%), and Mx1/2/3 (10.7%). Mx1/2/3 expression was high in DM cases. p62 vacuole deposits were noted in the IBM case. The combination of membrane attack complex and major histocompatibility complex I helped detect IIMs in 96% of cases. Conclusions The diagnosis of IIMs and their subtypes should be based on clinical features and histopathological characteristics. Immunohistochemistry plays a crucial role in the diagnosis and differentiation of these subgroups.

Abstract

Introduction: Carbapenemase-producing Enterobacterales (CPE) is one of the causes of community health-threatening, most common in Escherichia coli and Klebsiella pneumoniae. CPE has appeared in many countries, especially developing ones, but it has not been recognized in routine antibiograms. Also, assessing colistin susceptibility is a significant obstacle for both diagnostic and epidemiological because of technical difficulties. There is rare data about CPE and colistin-resistant Enterobacterales. Objectives: Determine the prevalence of producing carbapenemase and colistin-resistance among K. pneumoniae and E. coli resistant to carbapenems. Methods: In a cross-sectional survey, 129 K. pneumoniae and 31 E. coli strains, which demonstrated carbapenem resistance, were collected from inpatients at Cho Ray Hospital over a three-month period. Carbapenemase production was detected using the modified carbapenem inactivation method and EDTA-modified carbapenem inactivation method, and colistin resistance was detected using the broth microdilution method according to CLSI M100S34. Results: Of 160 participants' strains, 80% of carbapenemase-producing K. pneumoniae and E. coli were detected; among these, Metallo beta-lactamase-producing was taken 69,5%. Of 31 colistin-resistant, 30 were K. pneumoniae, and only 1 E. coli was detected that was resistant to colistin. Conclusion: Among participants' strains, carbapenemase-producing prevalence was very high. There were signs of a widespread extensively drug-resistant K. pneumoniae. Most of E. coliwas susceptible to colistin.

Abstract

Background Anaplastic lymphoma kinase (ALK) mutations have been identified as a prominent cause of some familial and sporadic neuroblastoma (NB). ALK expression in NB and its relationship with clinical and histopathological features remains controversial. This study investigated ALK expression and its potential relations with these features in NB. Methods Ninety cases of NB at the Department of Pathology, University of Medicine and Pharmacy at Ho Chi Minh City, Viet Nam from 01/01/2018 to 12/31/2021, were immunohistochemically stained with ALK (D5F3) antibody. The ALK expression and its relations with some clinical and histopathological features were investigated. Results The rate of ALK expression in NB was 91.1%. High ALK expression (over 50% of tumor cells were positive with moderate-strong intensity) accounted for 65.6%, and low ALK expression accounted for 34.4%. All the MYCN-amplified NB patients had ALK immunohistochemistry positivity, most cases had high ALK protein expression. The undifferentiated subtype of NB had a lower ALK-positive rate than the poorly differentiated and differentiated subtype. The percentages of ALK positivity were significantly higher in more differentiated histological types of NB (p = .024). There was no relation between ALK expression and: age group, sex, primary tumor location, tumor stage, MYCN status, clinical risk, Mitotic-Karyorrhectic Index, prognostic group, necrosis, and calcification. Conclusions ALK was highly expressed in NB. ALK expression was not related to several clinical and histopathological features. More studies are needed to elucidate the association between ALK expression and ALK gene status and to investigate disease progression, especially the oncogenesis of ALK-positive NB.

4. Intra-Articular Calcaneus Osteoid Osteoma With an Ankle Varus and a Red-Brown Synovial Fluid in the Affected Joint, a Case Report

Abstract

Background: Only one article described ankle varus as a typical symptom in the late stage of the intra-articular osteoid osteoma of the calcaneus. And the red-brown color of synovial fluid in the affected joint hasn’t been reported. This report shows a patient with intra-articular osteoid osteoma of the calcaneus who had the 2 above symptoms. Case presentation: A 39-year-old man had left ankle pain and the diagnosis was delayed for 20 months. At the late stage, the ankle was gradually varus. In our hospital, the withdrawal of the subtalar joint gave a red-brown synovial fluid. Together with the typical lesion on MRI, the diagnosis of intra-articular osteoid osteoma of the calcaneus was made. An open operation was performed for treatment. In the procedure, the red-brown synovial fluid was exuded. A specimen was harvested for biopsy confirming osteoid osteoma. Conclusions: It is still essential that intra-articular calcaneal osteoid osteoma should be considered in patients with prolonged pain and varus of the ankle. The red-brown synovial may be used as a finding for diagnosis.

Abstract

Background Langerhans cell histiocytosis (LCH) is more common in children than adults and involves many organs. In children, the BRAF V600E mutation is associated with recurrent and high-risk LCH. Methods We collected paraffin blocks of 94 pediatric LCH patients to detect BRAF_V600E mutation by sequencing. The relationship between BRAF V600E status and clinicopathological parameters were also critically analyzed. Results BRAF V600E mutation exon 15 was detected in 45 cases (47.9%). Multiple systems LCH showed a significantly higher BRAF_V600E mutation rate than a single system (p=.001). No statistical significance was evident for other clinical characteristics such as age, sex, location, risk organs involvement, and CD1a expression. Conclusions In Vietnamese LCH children, the proportion of BRAF V600E mutational status was relatively high and related to multiple systems.

6. Characteristics of Immunogenicity against SARS-CoV-2 in a Community-Based Model of Care during the Fourth Wave of COVID-19 Outbreak in Ho Chi Minh City.

Abstract

Purpose: Although some immune protection from close contact with individuals who have coronavirus disease 2019 (COVID-19) has been documented, there is limited data on the seroprevalence of antibodies against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in individuals who were in lockdown with confirmed COVID-19 cases. This study investigated immunogenicity against SARS-CoV-2 in household members and people who lived near home-quarantined patients with COVID-19. Materials and Methods: This cross-sectional study was conducted during the community-based care that took place during lockdowns in District 10, Ho Chi Minh City, Vietnam from July to September 2021. SARS-CoV-2 antibody levels were determined in index cases of COVID-19, household contacts, and a no-contact group from the same area. Results: A total of 770 participants were included (355 index cases, 103 household contacts, and 312 no contacts). All index cases were unvaccinated, but >90% of individuals in the household and no-contact groups had received ≥1 vaccine dose. SARS-CoV-2 neutralizing antibodies (Nabs) were present in >77% of unvaccinated index cases versus 64%/65.4% in the household/no-contact groups (p=0.001). Antibody concentrations in unvaccinated index cases were significantly higher than those in household contacts and no contacts, with no difference between the latter groups. In all cases, antibody levels declined markedly ≥6 weeks after infection, and failed to persist beyond this time in the household and no-contact groups. Conclusion: Community-based care may have helped to create community immunogenicity, but Nabs did not persist, highlighting a need for vaccination for all individuals before, or from 6 weeks after, infection with SARS-CoV-2. Abstract

The extent, to which people listen to and perceive the speech content at different noise levels varies from individual to individual. In past research projects, the speech intelligibility was determined by rating assessment, which suffered from variation of subjects’ physical features. The purpose of this study is to investigate electroencephalography (EEG) by implementing multi-variate Temporal Response Function (mTRF) to examine neural responses to speech stimuli at different sound and noise levels. The result of this study shows that the front-central area of the brain clearly shows the envelope entrainment to speech stimuli.

11. Adipose-Derived Mesenchymal Stem Cell Therapy for Liver Cirrhosis in Mice

Abstract

Mesenchymal stem cells (MSCs) are now an attractive source of cell therapy. We test the hypothesis that autologous adipose-derived mesenchymal stem cells ameliorate liver cirrhosis in mice. In this study, male Swiss mice were treated orally with olive oil or CCl4 for 11 weeks (3 times/week). Mouse adipose-derived stem cells (mADSCs) from adipose tissue of mice injuried by CCl4 for 3 weeks were cultured prior to transfer by tail vein injection. Hepatocyte-enriched markers were characterized using q-RT-PCR and MSC markers, hepatocyte-enriched proteins, and fibrosis were evaluated by flow cytometry and/or immunohistochemistry. Mice were divided into 4 groups (n = 10 each group): (1) normal, (2) cirrhotic, (3) cirrhotic/PBS, (4) cirrhotic/mADSCs (5 × 105 cells/mice). A separate set of mADSCs was labeled with CFDA to investigate cell homing. The results of in vitro evaluation on mADSCs showed that these cells are highly expression variety of hepatic-related genes such as Hgf, Alb, Ck18, Ck19, Cyp1a1, Afp, MUC1, Ldl receptor and strongly expression of Cyp1a1 and Hgf markers. Dose of 5 × 105 cells/animal improved AST/ALT/bilirubin/albumin index after 7 days of injection (p < 0.05); significantly down-regulate gene expression of TGF-beta 1 (14 fold less), procollagen (3 fold less) and nt5e (8 fold less), (p < 0.05). 100% mice improved histology index (HAI modified) and diminished the accumulation of collagen fibers after 21 days compared to 33.3% cirrhotic/PBS. mADSCs “home” to the liver tissue after 21 days.

Background: Stem cell therapy in liver cirrhosis treatment is attracting the attention of the scientific community. Adipose tissue-derived mesenchymal stem cells are a potential source of cells because they have self-renewal, high proliferation, and differentiation into a variety of cell types, including hepatocytes as potential cell sources for cirrhosis treatment. Platelet-rich plasma (PRP) growth factors contribute to regeneration and wound healing. We test the hypothesis that PRP co-administration enhances MSC treatment for mouse cirrhosis. Method: Male Swiss mice were treated orally with olive oil or CCl4 for 11 weeks. PRP was obtained from healthy mice. Mouse adipose-derived stem cells (mADSCs) from adipose tissue of 3 weeks CCl4 mice were cultured for three passages (P3-mADSCs) before the transfer by tail vein injection with or without PRP into 11 weeks CCl4 mice. Mice were divided into six groups (n=10 each group). 1) normal, 2) cirrhotic, 3) cirrhotic /PBS; 4) cirrhotic/PRP (0.2 ml/mice with PRP from healthy mice), 5) cirrhotic/mADSCs (5 x 105 cells/mice), and 6) cirrhotic/mADSC-PRP. Result: mADSCs were highly positive for CD44, CD90, and CD105. Relative to liver cells, P3-mADSCs highly expressed Alb, Ck18, Ck19, Tnf, c-met, Cyp1a1, Afp, Muc1, Ldl receptor. mADSCs were strongly positive for Cyp1a1 (98.21±1.57%) and Hgf (95.55±3.11%); moderately positive for alfa-fetoprotein (45.99±2.08%), Aat (44.43±7.79%), Alb (57.81±8.49%) and differentiated into hepatocyte-like cells under induction medium. After transplantation, CFDA-transplanted cells into CCl4-treated mice were found in the liver at day 21 st. Abstract

Background: The application of mesenchymal stem cell (MSC) therapy in liver fibrosis treatment has been increasingly investigated in recent years. MSCs obtained from a variety of sources (e.g. bone marrow, umbilical cord blood and adipose tissue) have been studied and have achieved remarkable results. In this study, we compared the effects of adipose-derived mesenchymal stem cells (AD-MSC) transplantation with bone marrow-derived mesenchymal stem cell (BM-MSC) transplantation in a mouse model of liver fibrosis, induced by carbon tetrachloride (CCl4). Methods: Eight-week old mice were treated with CCl4 for 11 weeks to induce liver fibrosis then 5x105 cells were transplanted into mice via the tail vein. Results: After 21 days of transplantation, the results showed that the stem cell treated groups ameliorated better than the placebo group. MSC treated groups showed reduced AST and ALT levels, down-regulated expression of extracellular matrix (ECM) genes, and improved liver histopathology. Both sources of MSCs (bone marrow and adipose tissue) were effective in the mouse model of liver fibrosis. Conclusion: Our results also indicated that AD-MSC transplantation in mice accelerated liver regeneration better than BM-MSC transplantation.

Abstract

Non-expanded adipose tissue-derived stem cells (ADSCs) are commonly used in preclinical and clinical articular cartilage injury treatment; however, there are usually insufficient non-expanded ADSCs for transplantation. This research aims to evaluate the safety and efficacy of expanded ADSC transplantation in a mouse model. The stromal vascular fraction from abdominal adipose tissue was subcultured for ten passages to enrich for and expand the number of ADSCs. The safety of expanded ADSCs was assessed by evaluating their “stemness” via ADSC-specific marker expression, the expression of two tumorigenesis-related genes (Oct-3/4 and Nanog) using real-time RT-PCR, and in vivo tumor formation in NOD/SCID mice. ADSC efficacy was determined by assessing their chondrocytic differentiative potential in vitro in cartilage-inducing medium as well as in vivo via injection into a NOD/SCID mice joint failure model. The results showed that expanded ADSCs were negative for Oct-3/4 and Nanog and did not induce tumor formation in mice. Furthermore, ADSCs differentiated into chondrocytes, both in vitro and in vivo, and enhanced the regeneration of articular cartilage in NOD/SCID mice as compared with the control. These results confirm that expanded ADSCs are safe and effective for the treatment of injured articular cartilage and offer a promising therapy for degenerative cartilaginous diseases.

Abstract

Stromal vascular fraction (SVF) combined with platelet-rich plasma (PRP) is commonly used in preclinical and clinical osteoarthritis as well as articular cartilage injury treatment. However, this therapy has not carefully evaluated the safety and the efficacy. This research aims to assess the safety and the efficacy of SVF combined with PRP transplantation. Ten samples of SVFs and PRPs from donors were used in this research. About safety, we evaluate the expression of some genes related to tumor formation such as Oct-4, Nanog, SSEA3, and SSEA4 by RT-PCR, flow cytometry, and tumor formation when injected in NOD/SCID mice. About efficacy, SVF was injected with PRP into murine joint that caused joint failure. The results showed that SVFs are negative with Oct-4, Nanog, SSEA-3, and SSEA-4, as well as they cannot cause tumors in mice. SVFs combined with PRP can improve the joint regeneration in mice. These results proved that SVFs combined with PRP transplantation is a promising therapy for articular cartilage injury treatment.

Abstract

Adipose-derived stem cells (ADSCs) have been isolated, expanded, and applied in the treatment of many diseases. ADSCs have also been used to treat injured articular cartilage. However, there is controversy regarding the treatment efficiency. We considered that ADSC transplantation with activated platelet-rich plasma (PRP) may improve injured articular cartilage compared with that of ADSC transplantation alone. In this study, we determined the role of PRP in ADSC transplantation to improve the treatment efficiency.ADSCs were isolated and expanded from human adipose tissue. PRP was collected and activated from human peripheral blood. The effects of PRP were evaluated in vitro and in ADSC transplantation in vivo. In vitro, the effects of PRP on ADSC proliferation, differentiation into chondrogenic cells, and inhibition of angiogenic factors were investigated at three concentrations of PRP (10%, 15% and 20%). In vivo, ADSCs pretreated with or without PRP were transplanted into murine models of injured articular cartilage.PRP promoted ADSC proliferation and differentiation into chondrogenic cells that strongly expressed collagen II, Sox9 and aggrecan. Moreover, PRP inhibited expression of the angiogenic factor vascular endothelial growth factor. As a result, PRP-pretreated ADSCs improved healing of injured articular cartilage in murine models compared with that of untreated ADSCs.Pretreatment of ADSCs with PRP is a simple method to efficiently apply ADSCs in cartilage regeneration. This study provides an important step toward the use of autologous ADSCs in the treatment of injured articular cartilage.
