5. Understanding female-perpetrated child sexual abuse in organisational contexts

Organisational child sexual abuse has received unprecedented attention over recent years with numerous local, national and international inquiries taking place. At the same time interest in female sex offenders has increased, however, despite this focus in both areas there is an almost total lack of research examining the phenomenon of female-perpetrated child sexual abuse in organisational contexts. This thesis combines these fields of inquiry and addresses this gap. Situational crime prevention theory framed the mixed methods approach examining 136 cases of sexual abuse perpetrated by women against children they worked with in organisational contexts. The sample originates from the UK, USA and Canada between 2000 and 2016. Freedom of Information request data from the Ministry of Justice and professional regulators was used to examine the current context of this abuse. Qualitative and quantitative content analysis of court reports, professional regulator decisions and media articles then examined 92 variables addressing: perpetrator and victim characteristics; offence processes and modus operandi and situational and environmental factors. The responses of organisations and criminal justice and child protection systems were also investigated, as well as the short- and long-term impacts upon victims. The findings show most women offend alone against a single, post-pubescent male victim, often with particular vulnerabilities. Abuse occurs predominantly outside the organisational environment and the use of electronic communication is common. The findings indicate these women were not pre-disposed offenders but rather their behaviour was influenced by socio-cultural, situational and contextual factors. This highlights the significant influence organisations can have in preventing this abuse and wider implications for policy and practice are also discussed. This thesis makes an original contribution to knowledge by being the first systematic investigation specifically examining female-perpetrated child sexual abuse in a range of organisational contexts.

Abstract

Most outcome studies for children and young people who have displayed harmful sexual behaviour have focused on sexual recidivism as their primary outcome measure. Relatively little is known about broader life outcomes for children displaying such behaviours, nor about the processes involved with longer-term developmental success or failure. This paper examines long-term life course outcomes for 69 adults in the UK who presented with abusive sexual behaviour as children. Between 10 and 20 years after their childhood sexual behaviour problems, few in the sample had sexually reoffended, but general life course outcomes were much less positive. A range of individual, relational and social/environmental factors appeared to be associated with successful and unsuccessful outcomes. Successful outcomes were associated with stable partner relationships, wider supportive relationships, and educational opportunity and achievement. The findings highlight the importance of broad-based, developmental interventions in assisting those with childhood sexual behaviour problems to live successfully. The findings outlined in this paper support a move away from a traditional focus on clinical programmes of "sex offence specific work" with young people towards more developmental approaches that focus actively on wider individual, relational, and socio-environmental factors associated with desistance and resilient life outcomes. This includes, but is not restricted to, interventions that are future-oriented and that promote the skills necessary for the development of supportive personal and intimate partner relationships. 11. Residential Greenness, Lifestyle, and Vitamin D: A Longitudinal Cohort of South Asian Origin and Caucasian Ethnicity Women Living in the South of the UK.

Abstract

The global population is at risk of vitamin D deficiency due to low exposure to sunlight and low intake of the vitamin through diet. The aim of this study was to investigate in women the association between vitamin D status and parathyroid hormone (PTH), ultraviolet radiation, lifestyle, ethnicity, social conditions, and residential greenness. A 1-year longitudinal study assessed vitamin D status in 309 women living at latitude 51°14′ N. Blood samples were taken four times throughout the year for analysis of 25(OH)D and serum PTH concentration. After each seasonal visit, the individuals completed 4-day diet diaries and used two dosimeter badges for 1 week to estimate weekly UVR exposure. A questionnaire was applied to provide information about lifestyle and their ethnicity. Residential greenness was measured by Normalized Difference Vegetation Index (NDVI), within a 1000 m radius around each participant's home address. Women living in greener spaces were more likely to have improved vitamin D status (RR: 1.51; 95%CI: 1.13–2.02), as well as those who were more exposed to UVR (RR: 2.05; 95%CI: 1.44–2.92). Our results provide an insight into the connection between residential greenness, lifestyle, and vitamin D status comparing two ethnicities in a country with a temperate climate and with a high degree of urbanization. 15. Vitamin D, light exposure, sleep and musculoskeletal health in South Asian and Caucasian women : biological and social influences

Abstract

There is an urgent need to better understand the problem of vitamin D deficiency, and its health effects, in population groups of different ethnicity. The principal aim of this project was to examine vitamin D status, sunlight exposure, and health outcomes in UK dwelling South Asian and Caucasian women. A cohort of 80 postmenopausal and 32 premenopausal South Asian and Caucasian women were assessed for vitamin D status (serum 25-hydroxyvitamin D; 25(OH)D), musculoskeletal health, light exposure and sleep-wake cycles. In postmenopausal women, South Asians had a significantly lower vitamin D concentration than Caucasians (p=0. 002), with 83% of Asians vs. 24% of Caucasians below 50nmol/l for 25(OH)D. Despite adaptations in tibial bone structure of the South Asians to improve bone strength, their bones were weaker by 38% compared with Caucasians (p < 0.001). Stand-to-walk time (Asian mean (+-SD) time 8. 1 s +- 1. 8 vs. Caucasian mean (+-SD) time 6. 9 s +- 1. 4); p=0. 002) and grip strength (Asian strength=70% of Caucasian strength, p < 0.001) were worse in the South Asians. For both premenopausal and postmenopausal women, Caucasians showed a significantly higher actigraphic sleep efficiency (p < 0.001) and lower sleep fragmentation (p=0. 002) than Asians. There was a higher outdoor light exposure (over 1000 lux) in premenopausal and postmenopausal Caucasians than in same-age Asians (p=0. 052). Qualitative analysis of interview data suggested that religious and cultural influences on family, work and community life may partly explain the reduced sunlight exposure in South Asian women, which contributes to vitamin D deficiency. The implications of this work are that older South Asian women are in need of intervention to improve vitamin D status. There is also some evidence for poorer musculoskeletal health, lower light exposure and poorer sleep in this group. The qualitative research included in the current study offers future intervention options to improve the health of UK dwelling South Asian women.

23. Vitamin D and bone health status of Australian women

Abstract

Introduction: Vitamin D deficiency is highly prevalent globally, even in countries that are considered to have a ‘sunny’ climate, such as Australia. Twenty three percent of the Australian population are considered vitamin D deficient (25(OH)D Methods: This cross-sectional study was conducted at the University of Wollongong (34.42° S) and primarily explored the serum 25(OH)D women in winter and spring in 2020 and 2021 (n=100, >18 years, pre- or post- menopausal). Ethnicity and skin type were self-defined. Serum 25(OH)D was measured through liquid chromatography mass spectrometry, bone density through dual energy x-ray absorptiometry and sun exposure through a polysulphone film badge. Results: The mean age of the participants was 41.41 ± 15.46 years. N=98 women had a valid 25(OH)D measurement. Of these, n=1.0 (1.0%) were deficient (50nmol/L) and n=42.0 (42.9%) were considered optimal (>75nmol/L). Mean 25(OH)D was significantly different across skin type groups, (F(4,93)=2.6, p=0.04) whilst average sun exposure significantly predicted 25(OH)D(F(1, 91) = 5.276,p=0.02). There was a significant but small positive correlation between 25(OH)D and total BMD, r(96) = 0.27, p=0.007. Analysis is ongoing to explore vitamin D dietary intake and prediction of vitamin D deficiency. Conclusion: Vitamin D status in this sample was found to be higher than in previous Australian studies, which is favourable given the essential role vitamin D plays in calcium homeostasis and bone health. However, further research into vitamin D dietary intake is crucial in Australia, given the increased risk of melanoma as a result of excessive sun exposure as well as the lack of naturally occurring vitamin D containing foods or fortified products in the food supply. &nbsp

Abstract

IntroducciónLa obesidad infantil supone un riesgo evidente para la salud y causa diversas comorbilidades en la vida adulta. La prevalencia de sobrepeso y obesidad infantil es alarmante, pues en 2019 afectaba a casi el 40% de la población escolar española (estudio ALADINO). Las causas principales son la alimentación inadecuada y la falta de ejercicio físico, ambas modificables y motivadas por la oferta de ultraprocesados y la digitalización del ocio.El papel de la enfermería, capaz de modificar el estilo de vida mediante la educación, es clave en la prevención de esta enfermedad. Se decide el juego como fundamento de las jornadas educativas puesto que es considerado el método ideal de aprendizaje en la etapa infantil.Objetivo Elaborar un programa de educación para la salud a través del juego orientado a la promoción de hábitos saludables en el niño sano. Metodología Para el programa de educación para la salud propuesto se realiza una búsqueda bibliográfica en diferentes bases de datos y páginas web relacionadas con el tema abordado. Las jornadas diseñadas se ejecutarán en colegios de educación primaria y tendrán una duración de 6 horas. Conclusiones Es fundamental la educación sanitaria en edades tempranas, pues los hábitos adquiridos en la infancia determinan el estilo de vida en la edad adulta. Los enfermeros serán los encargados de promover hábitos saludables mediante el juego a los más pequeños, por medio del diseño de unas jornadas educativas. Abstract

Introduction: Vitamin D deficiency remains a global health issue, particularly in minority ethnic groups. There is a lack of research looking into the vitamin D status of the African-Caribbean population worldwide Objectives: This review investigates the vitamin D status (as measured by 25(OH)D or dietary intake) of the African-Caribbean population globally. Methods: A systematic review was conducted by searching key databases (PUBMED, Web of Science, Scopus) from inception until October 2019. Search terms included ‘Vitamin D status’ and ‘African-Caribbean’. A random effects and fixed effects meta-analysis was performed by combining means and standard error of the mean. Results: The search yielded 19 papers that included n=5,270 African-Caribbean participants from six countries. A meta-analysis model found this population to have sufficient (>50 nmol/L) 25(OH)D levels at 71.4nmol/L, 95% CI (62.8,80) and poor dietary intake of vitamin D at only 3.0µg/day, 95% CI (1.67,4.31). For those living at low latitudes ‘insufficient’ (defined by study authors) 25(OH)D levels were found only in participants with type 2 diabetes and those undergoing haemodialysis. Suboptimal dietary vitamin D intake was reported in all studies at high latitudes (UK recommended nutrient intake: 10µg/day). Studies at lower latitudes, with lower recommended dietary intakes (Caribbean recommended dietary intake: 2.5µg/day) found ’sufficient’ intake in two out of three studies. Conclusion: In African-Caribbean populations, 25(OH)D deficiency and low dietary vitamin D intake is more prevalent at higher latitudes. 49. Dietary Protein and Bone Health Across the Life-Course : an updated systematic review and meta-analysis over 40 years

Abstract

Purpose: This systematic review and meta-analysis analysed the relationship between dietary protein and bone health across the life-course. Methods: The PubMed database was searched for all relevant human studies from the 1st 4 January 1976 to 22nd January 2016, including all bone outcomes except calcium metabolism. Results: The searches identified 127 papers for inclusion, including 74 correlational studies, 23 fracture or osteoporosis risk studies and 30 supplementation trials. Protein intake accounted for 0 - 4% of areal BMC and areal BMD variance in adults and 0-14% of areal BMC variance in children and adolescents. However, when confounder adjusted (5 studies) adult lumbar spine and femoral neck BMD associations were not statistically significant. There was no association between protein intake and relative risk (RR) of osteoporotic fractures for total (RR(random) = 0.94; 0.72 to 1.23, I2=32%), animal (RR (random) = 0.98; 0.76 to 1.27, I2 = 46%) or vegetable protein (RR (fixed)= 0.97 (0.89 to 1.09, I2 = 15%). In total protein supplementation studies, pooled effect sizes were not statistically significant for LSBMD (total n=255, MD(fixed)=0.04 g/cm2 (0.00 to 0.08, P=0.07), I2=0%) or FNBMD (total n=435, MD(random)=0.01 g/cm2 (-0.03 to 0.05, P=0.59), I2=68%). Conclusions: There appears to be little benefit of increasing protein intake for bone health in healthy adults but there is also clearly no indication of any detrimental effect, at least within the protein intakes of the populations studied (around 0.8-1.3 g/Kg/day).
