3. Efficacy Post-Event: Mass Killing Integrated Response Training

- Abstract
Regarding the killing or attempted killing of people in confined and populated areas, whether defined as an active threat, active shooter, or active killer, the results for public safety professionals responding to these incidents are the same. The Federal Bureau of Investigation statistics indicate that many mass killing events involve the use of firearms(s) with no pattern or method to the killer's selection of victims. The increasing frequency of these events, coinciding with the high number of casualties, is forcing first responders to adopt improved operational responses. Developing procedures through the collaborative efforts between first responders (e.g., fire, emergency medical services, and law enforcement) now addresses a common, unified response goal known as "stop the killing, stop the dying." Stop the killing refers to eliminating the threat, whereas stop the dying refers to treating life-threatening injuries on casualties, both of which are performed expeditiously and at greater risk to all responders. However, where this unified response goal has been applied in agencies and departments nationwide, data is lacking on responder efficacy post-training during a real-world mass killing event. By utilizing qualitative research methodologies, I identified localities where mass killing incidents occurred postdelivery of mass killing response training. Through interviews, surveys, case studies, and after-action reviews, I measured actions, including knowledge, skills, and abilities acquired from the courses.

The training of care staff in care homes for older people is important to ensure the maintenance of evidence‐based and effective care. Moreover, training can assist in changing attitudes and promoting improved quality care for residents.To this end the Allied Health Professional (AHP) Care Homes Training Team provides training to care staff throughout the greater Glasgow area. This study sought to elicit service user views and opinions about the team's current provision to assist in informing prospective developments and enhancing future performance. Many of the responses discussed training in general in care homes and so should be of interest to all AHPs and others with an interest in care homes. Ten semi‐structured interviews were undertaken with home management staff, which were thematically analysed. The response rate was 100%. 648 care staff had received training from the team in the ten care homes. However, only 36% of these staff remained employed by the homes at the time of the study. Therefore, only the remaining staff received postal semi‐structured questionnaires. The response rate was 14% for these staff. A range of service users' opinions and views were elicited.Topics covered included: induction training, career progression through training, training records, team training and support provision, changes implemented as a result of training, barriers to implementing training, structure/times/content and delivery of training sessions, training gaps, information and training for managers and appropriate topics for training. Feedback was extremely positive but the study raised wider implications for older people's care in care homes, which merit further research and debate.

Background Operative blood loss is among the most important factors determining the prognosis of patients undergoing hepatic resection. The best method for preventing bleeding is preliminary selective vascular occlusion of lobar, sectoral, or segmental portal triads, although not always technically feasible. Method Transportal occlusion of the portal triad with a balloon catheter was used in 35 hepatectomies for various tumors. Results In 27 out of 35 resections, there was absence or minimal bleeding from afferent vessels (portal vein, hepatic artery). In the remaining eight cases, there was significant bleeding from the hepatic artery. In these cases, transportal occlusion of portal triad was combined with a temporary interruption of the hepatic artery after the dissection of the hepatoduodenal ligament. The average intraoperative blood loss was 350–1,500 ml. Conclusion The use of a balloon catheter occlusion of the portal triad during liver resection is often technically feasible. It facilitates temporary occlusion of hardly accessible portal veins in the hepatic hilus without their prior exposure and minimizes bleeding. J. Surg. Oncol. 2005;89:39–42. © 2004 Wiley-Liss, Inc.

19. Hmi1p and UvrD: Two Superfamily I helicases and their respective roles in S. cerevisiae mtDNA maintenance and E. coli methyl-directed mismatch repair

Helicases are vital to all aspects of genomic maintenance. Double-stranded DNA, DNA/RNA hybrids, and self-annealed RNA require the action of this class of proteins to separate into single-stranded components that can be manipulated for cellular function and survival. Hmi1p, a Superfamily I helicase shown to localize to the mitochondria, is required to maintain mtDNA in Saccharomyces cerevisiae. Through biochemical studies, we are able to show that Hmi1p is a nucleic acid stimulated NTPase that unwinds DNA in a 3'-5' direction. Furthermore, we show that site-directed mutagenesis of the conserved lysine residue of the Walker A box effectively obliterates ATP hydrolysis and, therefore, helicase activity. Suprisingly, genetic analyses indicate that this ATP hydrolysis mutant is able to partially complement the petite phenotype associated with deltahmi1 mutants. A possible structural role as part of the mtDNA replication machinery and future experimentation is discussed. The Escherichia coli Superfamily I helicase, UvrD, is involved in replication, nucleotide excision repair, and methyl-directed mismatch repair (MMR). The role of UvrD in MMR is well-defined but the mechanism of unwinding stimulation by the protein MutL is poorly understood. We have constructed and tested a substrate designed to track relative distances of MutL and UvrD along a dsDNA region through the use of single-molecule FRET (smFRET) and total internal reflection microscopy (TIRM). We show that UvrD and MutL bind the substrate separately and, when combined, enhance the binding of both proteins to the DNA. An in-depth discussion of necessary controls and experiments is expanded upon to unravel this mechanism.
