Abstract

Objective: The objective was to describe the methodology used to develop new response criteria for adult DM/PM and JDM.Methods: Patient profiles from prospective natural history data and clinical trials were rated by myositis specialists to develop consensus gold-standard ratings of minimal, moderate and major improvement. Experts completed a survey regarding clinically meaningful improvement in the core set measures (CSM) and a conjoint-analysis survey (using 1000Minds software) to derive relative weights of CSM and candidate definitions. Six types of candidate definitions for response criteria were derived using survey results, logistic regression, conjoint analysis, application of conjoint-analysis weights to CSM and published definitions. Sensitivity, specificity and area under the curve were defined for candidate criteria using consensus patient profile data, and selected definitions were validated using clinical trial data.Results: Myositis specialists defined the degree of clinically meaningful improvement in CSM for minimal, moderate and major improvement. The conjoint-analysis survey established the relative weights of CSM, with muscle strength and Physician Global Activity as most important. Many candidate definitions showed excellent sensitivity, specificity and area under the curve in the consensus profiles. Trial validation showed that a number of candidate criteria differentiated between treatment groups. Top candidate criteria definitions were presented at the consensus conference.Conclusion: Consensus methodology, with definitions tested on patient profiles and validated using clinical trials, led to 18 definitions for adult PM/DM and 14 for JDM as excellent candidates for consideration in the final consensus on new response criteria for myositis.

Abstract

Flow processes onboard ships are common in order to transport fluids like oil, gas, and water. These processes are controlled by PID controllers, acting on the regulation valves as actuators. In case of a malfunction or refitting, a PID controller needs to be re-adjusted for the optimal control of the process. To avoid experimenting on operational real systems, models are convenient alternatives. When real-time information is needed, digital twin (DT) concepts become highly valuable. The aim of this paper is to analyze and determine the optimal NARX model architecture in order to achieve a higher-accuracy model of a ship’s flow process. An artificial neural network (ANN) was used to model the process in MATLAB. The experiments were performed using a multi-start approach to prevent overtraining. To prove the thesis, statistical analysis of the experimental results was performed. Models were evaluated for generalization using mean squared error (MSE), best fit, and goodness of fit (GoF) measures on two independent datasets. The results indicate the correlation between the number of input delays and the performance of the model. A permuted k-fold cross-validation analysis was used to determine the optimal number of voltage and flow delays, thus defining the number of model inputs. Permutations of training, test, and validation datasets were applied to examine bias due to the data arrangement during training.

Abstract

Timetable stability depends on the regularity of trains. Any deviation from the planned timetable leads to its instability. Railway network characteristics determine the capacities of the transport service. Depending on the capacity calculation method, time components are added to the minimum headway to ensure timetable stability. The UIC 405 method is simple and can be used on all railways. The disadvantage is that the calculations are based on average data. According to the method, the minimum headway consists of the time of the average headway interval, additional time and the buffer time. The additional time is precisely defined by the number of APB sections, while the buffer time is in the average value. When creating the timetable, the goal is optimal utilisation of the infrastructure. If the headway is too long, the capacity is not used, and if it is too short, timetable instability will ensue. Instead of averaging, this work calculates a buffer time that depends on the ratio of the travel time of the previous and the following trains. In this way, the headway is optimised and the calculation of the UIC 405 method is improved.

Abstract

OBJECTIVE: To identify new genetic associations with juvenile and adult dermatomyositis (DM). METHODS: We performed a genome-wide association study (GWAS) of adult and juvenile DM patients of European ancestry (n = 1,178) and controls (n = 4,724). To assess genetic overlap with other autoimmune disorders, we examined whether 141 single-nucleotide polymorphisms (SNPs) outside the major histocompatibility complex (MHC) locus, and previously associated with autoimmune diseases, predispose to DM. RESULTS: Compared to controls, patients with DM had a strong signal in the MHC region consisting of GWAS-level significance (P < 5 × 10(-8)) at 80 genotyped SNPs. An analysis of 141 non-MHC SNPs previously associated with autoimmune diseases showed that 3 SNPs linked with 3 genes were associated with DM, with a false discovery rate (FDR) of

Abstract

The paper deals with collision prevention problem in maritime transport in the area of the narrow canals with predefined routes. The Dover incident, which is analysed and described in the paper, has shown that the control of the passage of ships through the critical areas must be upgraded with an automatic supervising system, which warns the human operator of incorrect ship motion and help the operator to make the right and timely decision. The general idea is to improve the safety of navigation by introduction of automatic collision prevention based on automated supervisor helping to human operator in Vessel Traffic System (VTS) control centre. The VTS supervisor automatically monitors marine traffic by using data from Automatic Radar Plotting Aid (ARPA) radar and others sensors. Such supervisor detects real time and Course Over Ground (COG) of the vessel entering a particular sector, and then estimates the required time for vessel’s passage into another sector. VTS supervisor compares the real time and estimated time of passage of the specific ship through particular sector as a part of surveillance area. In addition, it compares and monitors the deviation of the course during transition of zones (sectors). If significant difference for both values are occurred VTS supervisor triggers a time alarm or a course alarm respectively. In the paper authors have modelled and simulated collision prevention with performed by the alarm actions of VTS supervisor improved with algorithm module based on hybrid Petri net formalism and Visual Object Net ++ tool.

Abstract

The purpose of this study is to show how coloured Petri nets can be used to select the crossing rules guaranteeing that two ships avoid collisions at sea in accordance with the international regulations on this matter. This paper is exclusively focused on the solution to this small sub-problem within the overall “Collision Avoidance System” that the authors of the present study are currently developing. For easier understanding, the overall system is also briefly presented. How the Petri net in the CPN Tools software is fed with “Real Time Real World” data is presented. These data are generated outside the Petri net from the function block to predict a possible collision and from the current meteorological data. We also demonstrate how the rule selections made by the Petri net are transferred from the CPN Tools software into the “Real Time Real World”. This transferred information is used outside the Petri net in the function block to calculate avoidance routes. The definition of the colour sets that are used and the individual operations applied to these colour sets in the coloured Petri net are presented.

Abstract

Selective inhibitors of cyclo-oxygenase-2 (COX-2) appear to be safer than conventional NSAIDs on the gastrointestinal (GI) tract. Amtolmetin guacyl (AMG), a NSAID that inhibits both COX-1 and COX-2, has an anti-inflammatory effect comparable to that of traditional NSAIDs, with a better GI safety profile. The primary end-point of this study was to evaluate the gastrointestinal safety of amtolmetin guacyl in comparison with celecoxib in patients affected with rheumatoid arthritis. The assessment of efficacy was the secondary end-point. This study was a 24-week, randomized, parallel group, double-blind, double dummy, multicentre trial ; 235 patients were enrolled and 180 patients (85 in the AMG group and 95 in the celecoxib group) completed the study. Each patient received twice daily amtolmetin guacyl 600 mg or celecoxib 200 mg. Assessment of safety was performed by upper GI endoscopy, gastrointestinal symptoms evaluation, electrocardiography, blood and urine laboratory tests, adverse events recording. Assessment of efficacy was performed by using the American College of Rheumatology (ACR-20) responder index. Neither amtolmetin guacyl nor celecoxib determined a worsening of baseline gastro-duodenal endoscopy findings. The percentage of patients with normal findings did not significantly change after treatment with both drugs, being virtually identical with AMG (i.e. 75.29%) and increasing from 75.79% to 77.66% with celecoxib. Moreover an evaluation of the other safety parameters did not reveal any difference between the two treatment groups. Therapeutic efficacy was equivalent in both groups, with no statistical difference between the two drugs at all time intervals. In patients affected with rheumatoid arthritis, AMG and celecoxib proved to be equivalent, showing comparable gastrointestinal safety and therapeutic efficacy of treatment.

Abstract

The problem of maritime traffic in Singapore Strait is traffic density, also the probability of collision, which is increased beside the existing Vessel Traffic System (VTS). The paper discusses the synthesis early warning system, or automatic crossing supervisor for Singapore Strait by using the Timed Petri Nets (TPN). Authors proposes dividing the strait in zones, so called crossings, where routes are crossing and in which the number of ships must be limited. The maximum number of vessels in period of time of the highest traffic density through the crossings are determined. Derived constraints are used for synthesis of crossing supervisor. The authors uses Petri nets to make model Singapore Strait, and use P-invariant method to syntheses crossing supervisor, which limits the number of ships in all critical crossings. Finally, the author verified derived supervisor by using Visual Object Net ++ programme for computer simulation. With the aid of Transas nautical simulator, the traffic in the strait is analysed. Abstract

The ever-increasing maritime traffic becomes a great threat to the safety of navigation particularly in confined waters. It is therefore necessary to develop new solutions in order to enhance the safety at sea. One of the possible solutions involves the control and management of vessel flow by radar supervision and automated guiding systems. This paper discusses the collision avoidance issue in the area of predefined routes and analyses real-life problems in maritime traffic by using appropriate software tools aimed at the enhancement of the safety of navigation. The goal of this study is to suggest a method for synthesizing the navigation supervisor by using Petri nets, which would monitor and control maritime traffic. The objective of installing a supervisor in the coastal centers is to prevent collisions and to improve safety in hazardous sea passages.

Abstract

In order to develop a preliminary core set of disease outcome measures for use in clinical trials of idiopathic inflammatory myopathies (IIM), we evaluated those measures used in previous trials, assessed the validation of published instruments and discussed these at an international consensus conference. The initial proposals were further refined by a multidisciplinary group of adult and paediatric specialists experienced in IIM using the Delphi method. The proposed preliminary core set of disease activity measures consists of five domains: physician and patient/parent global assessments of disease activity; muscle strength; physical function; serum activity of muscle enzymes; and an assessment tool to capture extra-skeletal muscle disease activity. The group recommended further development of a core set of disease damage measures for assessment of persistent changes in anatomy, pathology and function of at least 6 months' duration. The group recommended that patient-reported outcomes should include generic health-related quality of life assessments using the Medical Outcomes Study 36-item Short Form (SF-36) health survey in adult IIM patients and a validated quality of life instrument for paediatric patients. We propose the core set of outcome measures as a minimum group of assessments to include in all IIM therapeutic studies. The use of this core set should assist in standardizing outcome measurement and in optimizing therapeutic trials in myositis.

50. Schistosomal egg granuloma-derived fibroblast-stimulating factor is apparently distinct from interleukin-1

Abstract

We have previously reported that egg granulomas isolated from livers of Schistosoma mansoni-infected euthymic mice in vitro elaborate a factor(s) that stimulates a variety of fibroblast responses including fibroblast proliferation and enhanced synthesis of extracellular matrix proteins. We have postulated that these factors play a role in the pathogenesis of hepatic fibrosis in schistosomiasis mansoni. Serum-free supernatants from egg granuloma cultures also stimulate thymocyte proliferation in an assay that defects interleukin-1 (IL-1). Thymocytes and fibroblasts are stimulated to proliferate by the same fractions of egg granuloma culture supernatant separated by gel filtration, isoelectric focusing, and ion-exchange chromatography. This suggested that granuloma-derived IL-1 is responsible for the observed fibroblast stimulation. Here we report that the ability of granuloma culture supernatants to stimulate the IL-1-sensitive D10.G4.1 cells but not fibroblasts is removed by treatment with immobilized anti-IL-1 antibody. We also observed that dialyzed culture supernatants from egg granulomas obtained from infected congenitally athymic (nude) mice also stimulate fibroblast proliferation. Treatment with anti-IL-1 antibody did not abrogate this response. In contrast to our experience with egg granulomas isolated from euthymic mice, IL-1- and fibroblast-stimulating activity could be separated by gel filtration and isoelectric focusing. We conclude that the fibroblast growth-stimulating activities elaborated by egg granulomas from S. mansoni-infected euthymic and athymic mice may be different but both appear to be distinct from IL-1.
