BackgroundTo address needs for emotional well-being resources for Californians during the COVID-19 pandemic, the Together for Wellness/Juntos por Nuestro Bienestar (T4W/Juntos) website was developed in collaboration with multiple community partners across California, funded by the California Department of Health Care Services Behavioral Health Division federal emergency response. ObjectiveThis qualitative study was designed to explore and describe the perspectives of participants affiliated with California organizations on the T4W/Juntos website, understand their needs for web-based emotional health resources, and inform iterative website development. MethodsAfter providing informed consent and reviewing the website, telephone interviews were conducted with 29 participants (n=21, 72% in English and n=8, 28% in Spanish) recruited by partnering community agencies (October 2021-February 2022). A 6-phase thematic analysis was conducted, enhanced using grounded theory techniques. The investigators wrote reflexive memos and performed line-by-line coding of 12 transcripts. Comparative analyses led to the identification of 15 overarching codes. The ATLAS.ti Web software (ATLAS.ti Scientific Software Development GmbH) was used to mark all 29 transcripts using these codes. After examining the data grouped by codes, comparative analyses led to the identification of main themes, each with a central organizing concept. ResultsFour main themes were identified: (1) having to change my coping due to the pandemic, (2) confronting a context of shifting perceptions of mental health stigma among diverse groups, (3) “Feels like home”—experiencing a sense of inclusivity and belonging in T4W/Juntos, and (4) “It’s a one-stop-shop”—judging T4W/Juntos to be a desirable and useful website. Overall, the T4W/Juntos website communicated support and community to this sample during the pandemic. Participants shared suggestions for website improvement, including adding a back button and a drop-down menu to improve functionality as well as resources tailored to the needs of groups such as older adults; adolescents; the lesbian, gay, bisexual, transgender, and queer community; police officers; and veterans. ConclusionsThe qualitative findings from telephone interviews with this sample of community members and service providers in California suggest that, during the COVID-19 pandemic, the T4W/Juntos website was well received as a useful, accessible tool, with some concerns noted such as language sometimes being too “professional” or “clinical.” The look, feel, and content of the website were described as welcoming due to pictures, animations, and videos that showcased resources in a personal, colorful, and inviting way. Furthermore, the content was perceived as lacking the stigma typically attached to mental health, reflecting the commitment of the T4W/Juntos team. Unique features and diverse resources, including multiple languages, made the T4W/Juntos website a valuable resource, potentially informing dissemination. Future efforts to develop mental health websites should consider engaging a diverse sample of potential users to understand how to tailor messages to specific communities and help reduce stigma.

Abstract Non-technical summary This article examines the challenges and opportunities to integrate diverse sources of evidence in assessments produced by international platforms working at the science–policy interface. Diversity (or pluralism) of sources of literature, both in terms of their geographic origin and disciplinary focus, is essential for assessments to inform decision-making across social–ecological contexts. Using the recently completed ‘Methodological Assessment of the Diverse Values and Valuation of Nature’ of the Intergovernmental Science–Policy Platform on Biodiversity and Ecosystem Services as a case, we find that significant effort has been dedicated to reviewing diverse literature. We discuss three strategies to expand pluralism in future assessments. Technical summary Representing plural views in science–policy platforms is essential to avoid reproducing geographic and epistemic biases that permeate contemporary scientific knowledge production and synthesis. The Intergovernmental Science–Policy Platform on Biodiversity and Ecosystem Services (IPBES) has strived to produce assessments that incorporate information from diverse regions and knowledge systems. We explore the geographic and epistemic pluralism of the literature included in the ‘Methodological Assessment of the Diverse Values and Valuation of Nature’ (VA), and the challenges and opportunities to achieve such knowledge pluralism. We applied a bibliometric analysis to the sources of evidence cited in the VA, and reflected on the assessment development process, in which we were directly involved. Our results highlight the success of different strategies developed by VA experts to engage with diverse sources of literature. Still, most evidence was English-language academic literature produced in Western Europe, Canada, and the United States, echoing the prominence of this literature in scientific publication in environmental disciplines. Reflecting on our experiences, we discuss strategies that could further enhance the geographic and epistemic pluralism in the information reviewed for future environmental assessments produced by IPBES and other international science–policy platforms. Social media summary Epistemic and geographic pluralism was partially achieved in IPBES Values Assessment, and can be further enhanced in future assessments.

Como resultado de la pandemia por SARS-CoV2, fue aún más necesaria la incorporación de nuevas tecnologías para la impartición de clases en modalidad presencial y a distancia. La tecnología representa una manera de ofrecer educación de vanguardia respondiendo a necesidades del contexto actual. El presente trabajo analiza la adquisición de aprendizajes por parte de estudiantes de diferentes licenciaturas de la Universidad Iberoamericana Torreón, México en relación con la educación presencial y a distancia después de la crisis sanitaria. La investigación tuvo un enfoque cuantitativo con alcance correlacional. El muestreo se planteó probabilístico por conglomerados y estuvo conformado por estudiantes de quinto semestre en delante de 15 licenciaturas de la universidad. Los resultados estadísticos muestran que después de la pandemia, en la modalidad presencial el estudiantado percibe una mejor planeación por parte de los docentes, lo que tiene como consecuencia un mejor aprovechamiento del tiempo y la posibilidad de realizar actividades diversas para fortalecer la comprensión de las temáticas; utilizando como apoyo herramientas digitales y técnicas de trabajo grupal. Por el contrario, las clases a distancia son descritas como poco útiles e interesantes y con una carga académica excesiva. Aunque la experiencia de aprendizaje presencial sigue siendo mejor ponderada, se considera importante implementar estrategias que incrementen la calidad académica en las clases a distancia, utilizando como apoyo las nuevas tecnologías y un diseño instruccional, así como programas de capacitación a docentes y alumnos.

Non-traditional settings, such as schools, early childhood education programs, and healthcare clinics, can play a critical role in preventing obesity by providing free and nutritious food outside of food pantries to households that are experiencing low or very low food security, particularly during emergency situations. This evaluation study describes a low-income population that attended free food and produce distribution events during the COVID-19 pandemic. In 2021, a cross-sectional survey was administered to this group (n = 1498) to assess their food security status, fruit and vegetable (F+V) consumption, and prevalence of chronic disease condition(s). Program and event assessments were carried out using descriptive, bivariate, and multivariable logistic regression analyses. More than a quarter (26%) of the participants attended the free food events for the first time. Approximately 70% reported some level of low food security, 57% to 64% did not meet F+V intake recommendations, and 37% had a diet-related chronic disease condition. In the adjusted models, households with very low food security had lower odds of meeting F+V recommendations (aOR, 0.41, 95% CI, 0.28, 0.58 and aOR, 0.27, 95% CI, 0.19, 0.39, respectively) and higher odds of having a chronic condition (aOR, 3.49, 95% CI, 2.34, 5.20) than those with high or marginal food security. Given these experiences, future research should examine how safety net food assistance programs can incorporate alternative service/distribution models to improve the nutritional value of the foods they offer.

BackgroundThe COVID-19 pandemic increased disparities for communities burdened by structural barriers such as reduced affordable housing, with mental health consequences. Limited data are available on digital resources for public mental health prevention during the COVID-19 pandemic. ObjectiveThe study aim was to evaluate engagement in and impact of free digital resources on the Together for Wellness/Juntos por Nuestro Bienestar (T4W/Juntos) website during COVID-19 in California. MethodsA pilot evaluation of T4W/Juntos was performed, with partner agencies inviting providers, clients, and partners to visit the website and complete surveys at baseline (September 20, 2021, to April 4, 2022) and at 4-6–week follow-up (October 22, 2021, to May 17, 2022). Website use was assessed by three engagement items (ease of use, satisfaction, relevance), comfort in use, and use of six resource categories. Primary outcomes at follow-up were depression and anxiety (scores≥3 on Patient Health Questionnaire-2 item [PHQ2] and Generalized Anxiety Disorder-2 item [GAD2] scales). Secondary outcomes were post-pre differences in PHQ2 and GAD2 scores, and use of behavioral health hotlines and services the month before follow-up. ResultsOf 366 eligible participants, 315 (86.1%) completed baseline and 193 (61.3%) completed follow-up surveys. Of baseline participants, 72.6% identified as female, and 21.3% identified as lesbian, gay, bisexual, transgender, queer/questioning, and others (LGBTQ+). In terms of ethnicity, 44.0% identified as Hispanic, 17.8% as African American, 26.9% as non-Hispanic white, and 11.4% as other ethnicity. Overall, 32.7% had moderate anxiety or depression (GAD2/PHQ2≥3) at baseline. Predictors of baseline website engagement included being Hispanic versus other race/ethnicity (β=.27, 95% CI .10-.44; P=.002) and number of COVID-19–related behavior changes (β=.09, 95% CI .05-.13; P

The adsorptive performance of mesoporous silica-based materials towards inorganic (metal ions) and organic (dyes) water pollutants was investigated. Mesoporous silica materials with different particle size, surface area and pore volume were prepared and tailored with different functional groups. These materials were then characterised by solid-state techniques, namely vibrational spectroscopy, elemental analysis, scanning electron microscopy and nitrogen adsorption–desorption isotherms, allowing the successful preparation and structural modifications of the materials to be confirmed. The influence of the physicochemical properties of the adsorbents towards the removal of metal ions (Ni2+, Cu2+ and Fe3+) and organic dyes (methylene blue and methyl green) from aqueous solutions was also investigated. The results reveal that the exceptionally high surface area and suitable ζ-potential of the nanosized mesoporous silica nanoparticles (MSNPs) seem to favour the adsorptive capacity of the material for both types of water pollutants. Kinetic studies were performed for the adsorption of organic dyes by MSNPs and large-pore mesoporous silica (LPMS), suggesting that the process follows a pseudo-second-order model. The recyclability along consecutive adsorption cycles and the stability of the adsorbents after use were also investigated, showing that the material can be reused. Current results show the potentialities of novel silica-based material as a suitable adsorbent to remove pollutants from aquatic matrices with an applicability to reduce water pollution.

Abstract Clinical decision-making tends to be based on what clinicians have been taught during undergraduate and graduate programs. The aim of the present study was to assess the clinical approach and to identify the factors that influence the decision-making for dental pulp exposure among graduate students and coordinating professors in endodontics programs offered at Brazilian universities. The study used a mail-out survey developed in the Qualtrics platform, based on seven clinical reports in which dental pulp exposure was evidenced. Descriptive statistics showing vital pulp therapy (VPT) and root canal treatment (RCT) were calculated for each clinical report. Data on the participants’ (n = 113) profile and variables related to clinical and radiographic characteristics of the cases were evaluated as to their potential to affect decision-making and analyzed by logistic regression (p < 0.05). VPT was likely to be indicated in cases of patients with immature teeth (OR = 0.017; 95%CI = 0.004 -0.073). RCT indications were related to the presence of symptoms (OR = 5.326; 95%CI = 1.429–19.852) and old age (OR = 21.057; 95%CI=6.809–65.120). In pulp exposure secondary to trauma, time of pulp exposure was significantly associated with RCT indication (OR=3.267; 95%CI=1.332–8.012). The present study demonstrated that patient age, root development, and symptom features were the main factors affecting participants’ decision-making.

Objetivo. Evaluar la asociación entre la presencia del triángulo hioideo negativo y la respiración oral. Métodos. Se analizó un total de 100 telerradiografías de perfil, 50 de pacientes con respiración nasal y 50 con respiración oral, 70% de pacientes de sexo femenino y 30% masculino. El 59% correspondió a pacientes de entre 20 a 30 años de edad, el 21% de entre 10-20 años, el 15% de entre 30-40 años y el 5% de entre 40-50 años. Los criterios de inclusión fueron fichas clínicas de pacientes que posean telerradiografías de perfil con registro del tipo de respiración (oral o nasal). Los criterios de exclusión fueron fichas clínicas de pacientes con telerradiografía en formato digital, con errores de posicionamiento o dañadas o de sujetos con síndromes genéticos, sin tratamiento de ortodoncia/ ortopedia previos. Se realizó trazado cefalométrico manual (técnica de Rocabado). Se aplicó análisis estadístico descriptivo y prueba estadística Chi2 de Pearson a través de tablas de contingencia. Resultados. Se detectó que existe una asociación estadísticamente significativa (p < 0,001) entre la presencia de un triángulo hioideo negativo y la respiración oral, siendo los pacientes de sexo femenino (p < 0,001) y de 10 a 20 años de edad los que la presentaron con mayor frecuencia (81%). Conclusiones. En la población de estudio analizada existe una asociación estadísticamente significativa entre la presencia de triángulo hioideo negativo y la respiración oral, lo que parece estar relacionado con el sexo y la edad.

A ictiose é uma doença dermatológica cujo principal sinal clínico é a descamação generalizada da pele. Trata-se de uma doença genética rara na maior parte das raças, porém relativamente comum no Golden Retriever. Possui caráter autossômico recessivo na grande maioria dos casos e provoca sinais clínicos em animais de ambos os sexos, usualmente quando ainda filhotes. O presente trabalho teve como objetivo reunir as principais informações já publicadas a respeito da ictiose, com ênfase na informação genética, testes comerciais disponíveis, e sua implicação na criação de cães da raça Golden Retriever, e de raças nas quais a doença é mais rara. Espera-se que a compilação dessas informações sirva de auxílio para a atuação profissional na área da dermatologia veterinária e no melhoramento genético para esta doença nas raças mais afetadas.

Addiction is an illness prevalent in the worldwide population that entails multiple health risks. Because of the nature of addictive disorders, users of drugs seldom look for treatment and when they do, availability can be difficult to access. Permanence in treatment and its outcomes vary from case to case. Most models work from a multidisciplinary approach that tackles several dimensions of addictive disorders. However, the different etiological factors claim for a personalized treatment to enhance opportunities for better results. Problems in relationships with others play an important role in the etiology and the recovery process of addiction. This paper focuses on the social-environmental causes of addiction based on an affective neuroscience approach that attempts to integrate the interplay between social instincts, pleasure, and the SEEKING system in addiction. To advance toward better treatment strategies, it is pertinent to understand the limitations of the current multidisciplinary models. Acknowledging the social nature of the human brain may help to identify the quality of different types of traumatic early life experiences in drug users and how to address them in what may become a neuropsychoanalytic treatment of addiction.

Neuropsychoanalysis has been established as a field based on the dialog between psychoanalysis and the neurosciences. Freud was a neurologist for 20 years and used the neuroscientific knowledge of his time as the foundation of his metapsychology. Psychoanalysis has predominantly relied on its own method to develop techniques for the different psychoanalytic treatments. It rarely uses contributions from fields outside psychoanalysis that could enrich its understanding of the mind. Neuropsychoanalysis has informed and revised several topics in psychoanalysis, for example consciousness and the unconscious, dreams, and affect amongst many others. Clear clinical applications of neuropsychoanalysis can be appreciated in the work with neurological patients. However, a constant question from clinicians is whether neuropsychoanalytic findings can contribute to psychoanalytic treatments with non-neurological patients. This paper explores clinical applications of neuropsychoanalysis mainly based on affective neuroscience to propose an analysis of emotions that may contribute to the gradual development of a neuropsychoanalytically informed psychotherapy. The task of integrating neuroscientific knowledge into psychoanalytic technique is still considered a challenge of accentuated complexity, but it is at the same time a necessary and promising endeavor that aims at improving the quality of the treatments available for human suffering and psychopathology.

An effective and sustainable oxidative desulfurization process for treating a multicomponent model fuel was successfully developed using as a heterogeneous catalyst a composite material containing as an active center the europium Lindqvist [Eu(W5O18)2]9− (abbreviated as EuW10) encapsulated into the nanoporous ZIF-8 (zeolitic imidazolate framework) support. The EuW10@ZIF-8 composite was obtained through an impregnation procedure, and its successful preparation was confirmed by various characterization techniques (FT-IR, XRD, SEM/EDS, ICP-OES). The catalytic activity of the composite and the isolated EuW10 was evaluated in the desulfurization of a multicomponent model fuel containing dibenzothiophene derivatives (DBT, 4-MDBT and 4,6-DMDBT) with a total sulfur concentration of 1500 ppm. Oxidative desulfurization was performed using an ionic liquid as extraction solvent and aqueous hydrogen peroxide as oxidant. The catalytic results showed a remarkable desulfurization performance, with 99.5 and 94.7% sulfur removal in the first 180 min, for the homogeneous active center EuW10 and the heterogeneous EuW10@ZIF-8 catalysts, respectively. Furthermore, the stability of the nanocomposite catalyst was investigated by reusing and recycling processes. A superior retention of catalyst activity in consecutive desulfurization cycles was observed in the recycling studies when compared with the reusing experiments. Nevertheless, the nanostructure of ZIF-8 incorporating the active POM (polyoxometalate) was shown to be highly suitable for guaranteeing the absence of POM leaching, although structural modification was found for ZIF-8 after catalytic use that did not influenced catalytic performance.

Summary: A growing number of studies show that innate immune cells can undergo functional reprogramming, facilitating a faster and enhanced response to heterologous secondary stimuli. This concept has been termed “trained immunity.” We outline here a protocol to recapitulate this in vitro using adherent monocytes from consecutive isolation of peripheral blood mononuclear cells. The induction of trained immunity and the associated functional reprogramming of monocytes is described in detail using β-glucan (from Candida albicans) and Bacillus Calmette-Guérin as examples.For complete details on the use and execution of this protocol, please refer to Repnik et al. (2003) and Bekkering et al. (2016).

Endosymbiosis is a type of symbiosis where one species of microscopic scale inhabits the cell of another species of a larger scale, such that the exchange of metabolic byproducts produces mutual benefit. These benefits can occur at different biological levels. For example, endosymbiosis promotes efficiency of the cell metabolism, cell replication, and the generation of a macroscopic layer that protects the organism from its predators. Therefore, modeling endosymbiosis requires a complex-systems and multilevel approach. We propose a model of endosymbiosis based on reaction networks, where species of the reaction network represent either ecological species, resources, or conditions for the ecological interactions to happen, and the endosymbiotic interaction mechanisms are represented by different sequences of reactions (processes) in the reaction network. As an example, we develop a toy model of the coral endosymbiotic interaction. The model considers two reaction networks, representing biochemical traffic and cellular proliferation levels, respectively. In addition, the model incorporates top-down and bottom-up regulation mechanisms that stabilizes the endosymbiotic interaction.

El surgimiento exponencial de la pandemia por el virus SARS-COV-2 en México, es un tema de suma importancia que ha conllevado a la contención de su propagación, empleando diversas disciplinas médicas. Sin embargo, estrategias como la alfabetización en salud, que han sido ampliamente probadas y que son de bajo costo, requieren ser revisadas para mejorar su implementación. Por lo anterior, el propósito principal del presente manuscrito es analizar la importancia de la alfabetización en salud en la contención de la pandemia en México, con un estudio conceptual de los principales factores que participan en el desarrollo de esta enfermedad, como: el tipo de virus, los mecanismos fisiopatológicos, la interacción de factores sociodemográficos y culturales que forman parte del escenario epidemiológico. Las principales premisas integradas que han llevado a reconocer la necesidad de verificar la fuente de información, referente a los conocimientos médicos que continuamente se están difundiendo sobre la pandemia, para evitar la propagación de información falsa. Así como la importancia de aplicar las acciones preventivas sobre el uso del cubre bocas y el correcto lavado de manos, no a base de medidas obligatorias por sanciones, sino mediante la enseñanza razonada, que permita en las personas poder decidir mediante el auto consentimiento. No obstante, es necesario continuar con la enseñanza preventiva, involucrando diversos líderes poblacionales (políticos, religiosos, profesores, comunicólogos etc.) que puedan replicar con el ejemplo y facilitar la enseñanza de estas estrategias.

The high consumption of sugar-added food products by children has stimulated research on formulations using ingredients such as mango bagasse (MB), which deliver health-associated components. This research aimed to characterize, perform a sensory evaluation, and assess the probiotic effect in vitro of MB-added gum confections (1:4 and 1:5 water:MB). The 1:4 formulation displayed the highest acceptance by 51 children from a primary school in Ciudad Juárez (Mexico). This formulation contained a high fiber content (10.50%) and phenolic compounds (mainly mangiferin and (+)-catechin), and it allowed L. plantarum, L. reuteri, L. helveticus, and L. rhamnosus GG growth. The results suggest that MB-confections are fiber- and polyphenol-rich products with a sensory profile accepted by children and prebiotic effects.

The role of mechanical signals in early development are not fully understood. Here, the authors use the simplicity of early killifish embryos to show that tension arising from extra-embryonic epithelial expansion directs the spreading of mesenchymal-like embryonic cells during early morphogenesis.

Addiction poses a complex challenge in spite of all the progress made toward understanding and treating it. A multidisciplinary approach is needed and this paper attempts to integrate relevant neurobiological, behavioral, and subjective data under a common denominator described as a latent type of depression. It is called latent because it remains a silent syndrome due to two main reasons. The first one relates to the natural use of defenses against a predominant effect of chronic subjective pain, which arises from an ambivalent type of separation distress that compromises opioid regulation (PANIC system). Furthermore, it provokes a neurochemical cascade that impacts several neuromodulatory systems. The second reason is that such chronic subjective pain usually exhausts the natural defensive system, frequently leading the person to look for other resources such as the neurochemical manipulation of psychic pain. Thus, both the use of defenses and of psychotoxic drugs make the underlying depression hard to assess, even for the very person suffering from it. The causes, course and treatment of this type of affective configuration are discussed in this paper as an attempt to explain some of the difficulties so far encountered and to contribute to potential alternative lines of treatment.

NEUROSCIENCE AS THE BASIC SCIENCE OF PSYCHOANALYSISNeuroscience was the basic science behind Freud’s psychoanalytic theory and technique. He worked as a neurologist for 20 years before being aware that a new approach to understand complex diseases, namely the hysterias, was needed. Solms coined the term neuropsychoanalysis to affirm that neuroscience still belongs in psychoanalysis. The neuropsychoanalytic field has continued Freud’s original ideas as stated in 1895. Developments in psychoanalysis that have been created or revised by the neuropsychoanalysis movement include pain/relatedness/opioids, drive, structural model, dreams, cathexis, and dynamic unconscious. Neuroscience has contributed to the development of new psychoanalytic theory, such as Bazan’s (2011) description of anxiety driven by unconscious intentions or phantoms. Results of adopting the dual aspect monism approach of idiographic psychoanalytic clinical observation combined with nomothetic investigation of related human phenomena include clarification and revision of theory, restoration of the scientific base of psychoanalysis, and improvement of clinical treatments. By imbricating psychoanalytic thinking with neuroscience, psychoanalysts are also positioned to make contributions to neuroscience research. Freud’s original Project for a Scientific Psychology/Psychology for Neurologists can be carried forward in a way that moves psychoanalysis into the 21st century as a core contemporary science (Kandel 1999). Neuroscience as the basic science of psychoanalysis both improves the field, and enhances its scientific and cultural status.

En este trabajo se muestran los resultados de una investigación de campo, descriptiva y transversal, realizada con estudiantes que elaboraban sus trabajos de grado o tesis, en cinco postgrados de la Universidad Simón Bolívar (Venezuela). Los alumnos opinaron sobre la importancia de las variables que caracterizan el rol del tutor y su desempeño. Se construyeron varios índices: desempeño, importancia, discrepancia, criticidad y la jerarquía de variables de éxito y riesgo en los postgrados. Las diez características más importantes del rol del tutor, en opinión de los estudiantes, fueron: experiencia investigativa, seguridad en sus habilidades, responsabilidad, apertura al abordaje de puntos sometidos a discusión, experiencia para supervisar investigaciones, estabilidad emocional, aporte de ideas y sugerencias constructivas, información actualizada, destrezas en el manejo de información e información sobre líneas de investigación factibles. El estudio permitió diagnosticar el rol del tutor y detectó los elementos más importantes a ser considerados para diseñar, implementar y evaluar programas de asesoría y supervisión de investigaciones, que puedan contribuir a elevar el rendimiento en las tesis y trabajos de grado y el mejoramiento de la función tutorial. Se recomienda ampliar este estudio a otros postgrados para detectar las variables de éxito y riesgo de dichos programas.This study shows the results of a field research, descriptive and transversal, done with graduate students writing their thesis from five graduate programs at the Universidad Simón Bolívar. The students gave their opinions on the relevance of certain variables associated with the tutor’s role and his or her performance. Six indicators were used: performance, importance, discrepancy, level of criticism, and the hierarchy of both the variable of success and the one of risk. According to the students, the ten most important tutor’s role characteristics were: research experience, confidence, responsibility, openness to discuss the material, tutoring experience, emotional stability, constructive contributions and suggestions, abilities managing information, and knowledge of new lines of research. The study was useful to evaluate the tutor’s role and to detect the most important aspects to be considered when designing, implementing and evaluating effective programs for guiding graduate research. It is recommended to extend this investigation to other graduate programs in order to identify their own variables of success and risk.

Este estudio de caso tiene como objetivo general, escribir las características laborales asociadas al Síndrome de Burnout en un Equipo de Cuidados Paliativos Oncológicos del Servicio de Salud Metropolitano Norte, en el año 2013” Las variables evaluadas son Síndrome de Burnout, carga mental y factores de riesgo psicosociales, las que fueron evaluadas mediante pautas estandarizadas: Maslach Burnout Inventory (MBI), NASA-TLX e ISTAS 21, respectivamente, además de un cuestionario de datos personales. La población de estudio está conformada en su mayoría por mujeres. Seis son médicos, cuatro son enfermeras y dos son técnicos en enfermería. Los principales hallazgos son que: todos los participantes tienen Síndrome de Burnout o se encuentran en riesgo de padecerlo, las principales fuentes de carga mental son las exigencias mentales y temporales, y los principales factores de riesgo psicosociales se relacionan con las exigencias psicológicas y la doble presencia. De los resultados obtenidos se concluye la importancia de intervenir no sólo con pacientes oncológicos, sino también con los equipos de salud, ya que este puesto de trabajo se ve expuesto a altas exigencias que pueden perjudicar la calidad de vida laboral y personal

BackgroundThe COVID-19 pandemic increased disparities for communities burdened by structural barriers such as reduced affordable housing, with mental health consequences. Limited data are available on digital resources for public mental health prevention during the COVID-19 pandemic.ObjectiveThe study aim was to evaluate engagement in and impact of free digital resources on the Together for Wellness/Juntos por Nuestro Bienestar (T4W/Juntos) website during COVID-19 in California.MethodsA pilot evaluation of T4W/Juntos was performed, with partner agencies inviting providers, clients, and partners to visit the website and complete surveys at baseline (September 20, 2021, to April 4, 2022) and at 4-6-week follow-up (October 22, 2021, to May 17, 2022). Website use was assessed by three engagement items (ease of use, satisfaction, relevance), comfort in use, and use of six resource categories. Primary outcomes at follow-up were depression and anxiety (scores≥3 on Patient Health Questionnaire-2 item [PHQ2] and Generalized Anxiety Disorder-2 item [GAD2] scales). Secondary outcomes were post-pre differences in PHQ2 and GAD2 scores, and use of behavioral health hotlines and services the month before follow-up.ResultsOf 366 eligible participants, 315 (86.1%) completed baseline and 193 (61.3%) completed follow-up surveys. Of baseline participants, 72.6% identified as female, and 21.3% identified as lesbian, gay, bisexual, transgender, queer/questioning, and others (LGBTQ+). In terms of ethnicity, 44.0% identified as Hispanic, 17.8% as African American, 26.9% as non-Hispanic white, and 11.4% as other ethnicity. Overall, 32.7% had moderate anxiety or depression (GAD2/PHQ2≥3) at baseline. Direct-acting antivirals (DAA) are effective in patients with hepatitis C virus (HCV) infection, but there is little information about real-world effectiveness in people living with human immunodeficiency virus (PLH). The aim of this study was to determinate the effectiveness of DAA to achieve sustained virologic response at week 12 post-treatment (SVR12) in PLH with HCV coinfection and in people with HCV-monoinfection. We conducted a prospective cohort. The full analysis set (FAS) included all subjects enrolled in the study; the modified analysis set (MAS) excluded cases with missing data to evaluate SVR12. A total of 278 people were included, 130 (46.7%) with HCV/HIV-coinfection and 148 (53.2%) with HCV-monoinfection. In the HCV/HIV-coinfection group, 82 (63%) received GLE/PIB for 8 weeks, 45 (34.6%) received SOF/VEL for 12 weeks, and 3 (2.3%) were treated with SOF/VEL + RBV for 12 weeks. In the HCV-monoinfection group, 62 (41.8%) received GLE/PIB for 8 weeks, 28 (18.9%) received SOF/VEL for 12 weeks, and 58 (39.1%) participants were treated with SOF/VEL + RBV for 12 weeks. In the FAS analysis, SVR12 was 81.6% in the HCV/HIV-coinfection group and 86.4% in the HCV-monoinfection group (p = 0.128). In the MAS analysis, both groups achieved 100% of SVR12. In this cohort, the effectiveness of DAA to achieve SVR12 was similar between HCV/HIV-coinfection and HCV-monoinfection cases, regardless of advanced liver disease with no differences between treatment regimens.

Leptin is associated with cardiometabolic complications of obesity, such as metabolic syndrome and atherosclerosis. In obese men, the presence of metabolic syndrome is associated with higher circulating leptin and interleukin (IL)-6 concentrations and increased monocyte cytokine production capacity. Here, we investigated the effects of leptin on monocyte function and systemic inflammatory markers in obese individuals. We specifically explored whether leptin can induce long-term changes in innate immune function by inducing innate immune memory (also called trained immunity). We exposed human primary monocytes for 24 h to relevant leptin concentrations in vitro and measured cytokine production. In addition, after removing leptin, we incubated monocytes for 5 d in culture medium, and we restimulated them on day 6 to assess cytokine production capacity, phagocytosis, and foam cell formation. Direct stimulation with leptin did not induce cytokine production, but exposure to 50 ng/mL leptin augmented lipopolysaccharide- and R848-induced tumor necrosis factor α (TNF-α) production after 1 wk. In a separate in vivo study in a cohort of 302 obese subjects (body mass index [BMI] >27 kg/m2, 55 to 81 yr), we measured circulating leptin, inflammatory markers, and cytokine production upon ex vivo stimulation of isolated peripheral blood mononuclear cells. Circulating leptin concentrations positively correlated with circulating IL-1β and IL-6, which was more pronounced in men than in women. Four single nucleotide polymorphisms in the leptin gene influenced circulating IL-6 concentrations in men, suggesting a direct effect of leptin on IL-6. In conclusion, in vitro, leptin does not directly stimulate monocytes to produce cytokines, yet induces long-term monocyte hyperresponsiveness, i.e. trained immunity. In obese subjects, leptin is associated with circulating IL-6 in a sex-dependent manner.
