Background: COVID-19 has currently well-described signs and symptoms. However, older adults often present with an atypical condition, which can confuse and delay diagnosis and treatment. Summary: A scoping review was performed according to Joanna Briggs Institute methodology and prepared using the PRISMA-ScR. It included 61 studies. The objectives were to identify the atypical symptoms of COVID-19 in older adults and summarize and synthesize the evidence regarding the clinical outcomes of older adults with atypical presentations of COVID-19. Fifty-eight atypical symptoms were identified. Geriatric syndromes were the most prevalent atypical presentations of COVID-19. The prevalence of older adults with atypical presentation was 29 ± 18.45%. The mortality rate of older adults with atypical presentation was 28.3 ± 10.26 (17–37%). Key Messages: This scoping review showed that geriatric syndromes represent a frequent atypical presentation of COVID-19 in older adults. It is necessary to give visibility to those presentations in older populations with COVID-19 to advance the required interventions.

Abstract Introduction Patient and public involvement (PPI) initiatives involving patients with chronic respiratory disease (CRD) are rare. Therefore, this study aimed to explore the perspectives of patients with CRD, carers and interested citizens regarding the relevance and need for a PPI network and suggestions for its implementation. Methods A qualitative study based on focus groups was conducted. Recruitment occurred through invitations on social media platforms and to patients who have participated in previous asthma studies of the team. Three focus groups were conducted, via video conference, using a semi‐structured guide. Thematic analysis was performed by two independent researchers and discussed with the extended team. Results Fifteen patients with CRD, one carer and one interested citizen (13 females, median 36 (range: 18–72) years) participated. All participants acknowledged the importance of implementing a collaborative network and demonstrated interest in being integrated. Participants acknowledged the importance of their involvement in several phases of the research cycle. The main aim identified for this network was to facilitate communication between patients and researchers. Participants regarded the integration of patients, carers, researchers and healthcare professionals from different scientific areas as relevant. The use of digital platforms to attract members and support the work, together with group dynamics and regular meetings, were some of the most relevant practical considerations for implementing the network. The identified facilitators for their engagement were sharing experiences, researchers' and healthcare professionals' support and feedback and schedule flexibility. The identified barriers included the amount of time dedicated, low health/digital literacy and the potential detachment of nondiagnosed patients or those with low symptom impact in daily life. Conclusion Patients, carers and citizens acknowledged the relevance of implementing a collaborative network and demonstrated interest in active participation in every stage of the health research cycle. A deeper knowledge of the barriers and facilitators identified in this study could support implementing these initiatives in Portugal. Patient or Public Contribution This study was designed by a research team that included one patient with asthma and one carer. They were specifically involved in building the study protocol and the interview guide. They also gave feedback regarding the electronic consent form and the short sociodemographic questionnaire created, namely by removing noncontributing words or phrases and rewording expressions. The lay summary was written by another patient with asthma. All participants of this study were invited to implement and integrate the ConectAR network—a collaborative network of research in respiratory health. Public Summary In Portugal, chronic respiratory patients do not have an active role as ‘coinvestigators’. This study aimed to acknowledge if patients and citizens considered a patient and public involvement network useful, whose main purpose would be to facilitate communication between patients and researchers. A study based on online group interviews was carried out with patients with chronic respiratory diseases and interested citizens, both recruited on social media platforms. Participants considered that bringing together patients, carers, researchers and healthcare professionals is valuable because sharing different experiences and perspectives may help patients to improve their daily lives and increase research quality. In conclusion, patients agree that implementing a collaborative network with researchers and healthcare professionals and participating in the health research cycle is quite preponderant. Acknowledging what can help and deter this network may be beneficial to implementing this type of initiative in Portugal.

The decrease in cognitive and physical ability among people with dementia can significantly affect eating performance, resulting in mealtime support needs that could lead to inadequate oral intake, weight loss, malnutrition, and reduced functionality in activities of daily living. This scoping review aimed to identify and summarize available research literature on mealtime interventions for people with dementia, and their impact on older people with dementia living in a residential care setting, care staff, and care context/environment. A scoping review of available research published in English, French, Portuguese, or Spanish, was conducted according to the methodology established by The Joanna Briggs Institute. The search was conducted between November 2022 and February 2023 in the following databases: MEDLINE, Web of Science, Scopus, CINAHL Complete, and SciELO. A total of 275 articles were retrieved, of which 33 studies were selected according to inclusion criteria. The interventions were classified into four general categories: environmental, mealtime assistance, staff training, and multicomponent. Most studies demonstrated effectiveness in increasing oral intake and improving behaviors such as agitation and aggression in people with dementia. The impact of interventions on care staff was linked to greater knowledge and attitudes towards mealtime support needs. There is a lack of reporting on the impact of interventions on the care context/environment. Most interventions examined the effects exclusively on residents, focusing on their oral intake and behavioral patterns, particularly agitation among individuals with dementia. However, it is crucial to conduct studies that evaluate the impact on administrators, to comprehend the viewpoints of various hierarchical levels within an organization regarding challenges associated with mealtime. The findings of this scoping review can support the development of new supportive programs, or strategies to improve mealtime experience with positive impact according to the reality and needs of each person or institution.

Treatment options for hypertension have been evolving over time. However, prevalence rates keep increasing and perpetuate hypertension as a major cardiovascular risk factor. Exercise training is effective in reducing blood pressure, cardiovascular disease risk factors and mortality, besides improving quality of life. However, participation rates for hypertensive patients remain shockingly low and adherence to exercise training tends to decline following exercise programs. These trends emphasize the need to deepen our knowledge of modifiable intrapersonal, interpersonal, and socioeconomic and environmental factors that help explain exercise adherence among people with hypertension. The present review focuses on the determinants of adherence and long-term maintenance of a physically active lifestyle in hypertensive individuals.

People spend most of their time indoors, particularly in their houses where daily activities are carried out, enhancing particulate matter (PM) emissions with consequent adverse health impacts. This study intended to appraise the toxicological and mutagenic responses of particulate matter with a diameter less than 10 μm (PM10) released from cooking and ironing activities under different conditions. The cytotoxicity of the PM10 total organic extracts was tested in A549 cells using the WST-8 and the lactate dehydrogenase (LDH) assays, while the interference in cell cycle dynamics and reactive oxygen species (ROS) production was analysed by flow cytometry. The S. typhimurium TA98 and TA100 Ames tester strains with and without metabolic activation were employed to determine the mutagenic potential of the PM10-bound polycyclic aromatic hydrocarbons (PAHs). PM10 organic extracts decreased the metabolic activity of A549 cells; however, no effects in the LDH release were observed. An increase in ROS levels was registered only for cells treated with PM10 at IC20 from steam ironing, in low ventilation conditions, while cell cycle dynamics was only affected by exposure to PM10 at IC20 from frying horse mackerel and grilling boneless pork strips. No mutagenic effects were observed for all the PM10-bound PAHs samples.

Introduction: In advanced stages, people with dementia (PwD) may develop feeding difficulties. The assessment of these difficulties is the first step towards an adequate intervention. The Edinburgh Feeding Evaluation in Dementia Scale (EdFED) is an instrument for assessing the feeding difficulties of older PwD. Objective: The objective of this observational, cross-sectional, exploratory-descriptive study was to contribute to the adaptation of the EdFED to the Portuguese population and to analyze some of its psychometric properties, namely internal consistency and interobserver reliability. Methods: First, translation, cultural and linguistic adaptation, as well as validation were performed using an expert panel and calculation of the content validity index (CVI). Then the instrument was applied in 51 institutionalized older PwD (mean age 86.06 ± 6.77 years, 86.2% women). Results: The results showed excellent content validity, with a CVI of 0.950. The Portuguese version of the EdFED showed reasonable internal consistency (Cronbach’s α = 0.705) and very satisfactory interobserver reliability (Cohen’s κ = 0.882). Conclusion: The main results suggest that the Portuguese version of the EdFED is a reliable instrument that allows the assessment of eating/feeding difficulties among institutionalized older PwD.

Accurate measurement of adherence is crucial to rigorously evaluate interventions aimed at improving this outcome in patients undergoing in-center hemodialysis. Previous research has shown great variability in non-adherence rates between studies, mainly due to the use of different direct (e.g., clinical biomarkers) and indirect (e.g., questionnaires) measures. Although self-reported adherence in hemodialysis has been widely explored, it is still unclear which is the most accurate questionnaire to assess this outcome; therefore, the question of how to optimize adherence measurement in research and clinical practice has emerged as a key issue that needs to be addressed. This systematic review and meta-analysis aimed to explore the criterion validity of self-report measures of adherence in hemodialysis established through the association between test scores and clinical biomarkers (the criterion measure). The protocol was registered in PROSPERO (2021 CRD42021267550). The last search was performed on March 29th, 2022, on Web of Science (all databases included), Scopus, CINHAL, APA PsycInfo, and MEDLINE/PubMed. Twenty-nine primary studies were included, and thirty-eight associations were analyzed. The Hunter-Schmidt's meta-analysis was computed for the associations with more than two studies (n = 20). The results showed that six associations were large (16%), 11 were medium (29%) and the remaining were of small strength. BackgroundSeveral studies have investigated the acoustic effects of diagnosed anxiety and depression. Anxiety and depression are not characteristics of the typical aging process, but minimal or mild symptoms can appear and evolve with age. However, the knowledge about the association between speech and anxiety or depression is scarce for minimal/mild symptoms, typical of healthy aging. As longevity and aging are still a new phenomenon worldwide, posing also several clinical challenges, it is important to improve our understanding of non-severe mood symptoms' impact on acoustic features across lifetime. The purpose of this study was to determine if variations in acoustic measures of voice are associated with non-severe anxiety or depression symptoms in adult population across lifetime.MethodsTwo different speech tasks (reading vowels in disyllabic words and describing a picture) were produced by 112 individuals aged 35-97. To assess anxiety and depression symptoms, the Hospital Anxiety Depression Scale (HADS) was used. The association between the segmental and suprasegmental acoustic parameters and HADS scores were analyzed using the linear multiple regression technique.ResultsThe number of participants with presence of anxiety or depression symptoms is low (>7: 26.8% and 10.7%, respectively) and non-severe (HADS-A: 5.4 ± 2.9 and HADS-D: 4.2 ± 2.7, respectively). Adults with higher anxiety symptoms did not present significant relationships associated with the acoustic parameters studied. Adults with increased depressive symptoms presented higher vowel duration, longer total pause duration and short total speech duration. Finally, age presented a positive and significant effect only for depressive symptoms, showing that older participants tend to have more depressive symptoms.ConclusionsNon-severe depression symptoms can be related to some acoustic parameters and age. Depression symptoms can be explained by acoustic parameters even among individuals without severe symptom levels.

Abstract The construct of family integrity denotes that older persons' attainment of ego integrity is linked to family relationships. Family integrity is the positive outcome (overall satisfaction with life); disconnection (dissatisfaction) and alienation (estrangement) are the negative outcomes. This study focuses on elderly people who are primary caregivers of relatives with dementia, and examines their experience of events related to that role, which are perceived as being of influence on their sense of family integrity. The sample included 26 participants. Open-ended interviews were conducted. The interview transcripts were submitted to content analysis. The main findings suggest that caregivers who experience family integrity tend to embrace the new life demand associated with the caregiving role, while those tending towards disconnection struggle to maintain their pre-existing goals, and those experiencing alienation tend to feel helpless. Caring for a relative with dementia has an impact on multiple aspects of family life.

Background: Resistant Hypertension (RH) is associated with an increased risk of cardiovascular events and poor prognosis. Exercise training studies in RH patients have shown promising outcomes, nonetheless, none determine the impact of exercise on endothelial damage and repair. Circulating endothelial cells (CECs) are a reliable indicator of vascular damage and dysfunction. Recent studies in hypertension suggest that increased levels of endothelial progenitor cells (EPCs), a marker of endothelial repair, are related to increased CECs in order to compensate endothelial damage. Purpose: This study aimed to determine the effect of 12-week aerobic exercise program on the percentage of EPCs and CECs in RH patients. Methods: Patients with RH were randomized to a 12-week aerobic exercise program (3 xs/week) (n = 13) and a usual care control group (n = 8). Outcome measures included clinical data, ambulatory blood pressure data and circulating levels of EPCs, hematopoietic stem cells (HSC) and CECs, quantified by flow cytometry. (ClinicalTrials.gov: NCT03090529). Results: Baseline characteristics were similar between groups, including the number of antihypertensive drugs (5.0 ± 0.9 vs. 4.8 ± 0.7, p = 0.517). After 12 weeks, no significant changes were found in the levels of HSCs in both groups. The levels of CECs decreased in the exercise group [0.0073 (0.0016)% to 0.0058 (0.0029), p = 0.019]; no changes were observed in the control group. EPC’s decreased only in the exercise group [0.0071 (0.0027)% to 0.0052 (0.0037)%, p = 0.046]. Conclusions: Exercise training reduces endothelial injury/damage (reduced CECs levels) in RH patients, a specific group who is a challenge for clinicians as the available treatment options have reduced success.

Background: The relationship between arterial stiffness, blood pressure (BP) and cardiorespiratory fitness (CRF) has been studied in healthy populations and cardiovascular patients, since all of which proved to be independent predictors of all-cause mortality. We aimed to investigate the association of CRF with arterial stiffness and peripheral and central hemodynamics, in patients with resistant hypertension (RT). Methods: In this cross-sectional study, 30 patients (13 men, 17 women; age, 57.7 ± 8.1 years; weight, 79.2 ± 11.6 kg; body mass index, 29.7 ± 4.0 kg/m2) with resistant hypertension were recruited in the Hospital Infante D. Pedro (Aveiro) and Hospital Pedro Hispano (Matosinhos). Outcome measures included CRF (VO2peak), peripheral and central BP, and carotid-femoral pulse wave velocity (cf-PWV). Correlation analysis was conducted to assess the association between variables. Results: A significant negative correlation was found between VO2peak (33.7 ± 6.2 mLO2/kg/min) and central (141.0±21.3 mmHg; r = −0.395, p = 0.031) and peripheral systolic BP (148.4 ± 21.3 mmHg; r = −0.363, p = 0.049). VO2peak was also correlated with pulse pressure (PP) amplification ratio (1.2 ± 0.1 mmHg; r = 0.361, p = 0.050). The association with central systolic BP (r = −0.403, p = 0.035) and PP amplification ratio (r = 0.408, p = 0.033) remained significant after adjusted for age. VO2peak showed no correlation with PWV (9.4 ± 2.9 m/s; r = −0.075, p = 0.694) and peripheral (88.1 ± 12.2 mmHg; r = −0.138 p = 0.467) and central diastolic BP (88.5 ± 12.7 mmHg; r = −0.133, p = 0.483). Conclusion: This study confirms the inverse relationship between CRF and central systolic BP and PP amplification ratio in RH patients, regardless of age.

Objective: Carotid-femoral Pulse Wave velocity (cfPWV), the gold standard for measuring stiffness, is a marker of organ damage (OLD). Even though cfPWV correlates with casual (BPc), central (CBP) and ambulatory (ABPM) blood pressure (BP), evidence is limited for resistant hypertension (RH). Method: Thirty-three patients (age, 56.1 ± 8.2 years; weight, 78.0 ± 12.4 kg; height, 1.62 ± 0.08 m) with RH participated in a cross-sectional study. Outcomes included clinical data, BPc, ABPM, and carotid-femoral, cfPWV. Correlation analysis was conducted to assess the association between variables; independent t-tests were conducted to compare variables between those participants with cfPWV < and ≥ 10 m/s. Results: Patients (20 women and 13 men) presented a peripheral systolic and diastolic BPc of 144.0 ± 3.8 mmHg and 82.0 ± 1.9 mmHg, respectively. Reflections discussed here originated from workshops developed to provide a space for elders to express themselves and to be heard as well to provide better socio-cultural interactions and improvement in their wellbeing and quality of life. These workshops were conducted at a Center Day for Elderly in São Paulo, Brazil, from September 2011 to February 2013, as part of a PhD project entitled “Geras Vitalis”. Service gaps and lack of gerontological knowledge by care-takers and directors from the institution where the work was undertaken were observed during the workshops. These shortcomings, in our view, can exacerbate inappropriate behavior, contributing to a lower quality of life of the older people, as well as those who deal with them in everyday life. As a result of this study, we suggest training programs, where gerontological interventions are oriented by components such as counseling, psychological and educational listening and, if necessary, the use of “Case Management”.

Este estudo visa comparar os motivos e os fatores de stress entre cuidadores informais primários e secundários de pessoas idosas dependentes no domicílio. Foi constituída uma amostra de 180 cuidadores: 90 cuidadores primários e 90 cuidadores secundários. No geral, as subamostras partilham das mesmas razões para cuidar. Contudo, diferem no que refere aos fatores de stress. Os resultados são relevantes para o planeamento de intervenções mais individualizadas e adequadas aos dois grupos de cuidadores.

The growing incidence of dementia poses several challenges to the social support networks of people with dementia. Managing the symptoms of the disease is highly demanding and affects the well-being of the caregivers. Psycho-educational approaches have the potential to reduce the stress and burnout related to dementia caregiving demands. This paper describes the experience of two psycho-educational programs for family and formal caregivers. Main results are reported and recommendations for further intervention developments are identified.

OBJECTIVOS: Caracterizar a qualidade de vida e bem-estar dos idosos do ponto de vista dos próprios. MÉTODOS: Foi caracterizada uma amostra de 1.665 idosos com 75 anos ou mais, residentes em 13 Distritos de Portugal. Destes, 311 sujeitos foram excluídos por incapacidade de expressarem opiniões. O instrumento utilizado na recolha dos dados dos 1.354 sujeitos (81,3%) da amostra foi o EASYcare (Sistema de Avaliação dos Idosos). A análise estatística baseou-se na Análise em Componentes Principais e Análise de Clusters. RESULTADOS: Os estudos estatísticos realizados revelaram que o EASYcare possui boas qualidades psicométricas (a de Cronbach =0,92) e permitiram encontrar quatro factores: actividades de vida diária, bem-estar, mobilidade e comunicação. A análise de clusters identificou quatro grupos em 81,3% da amostra: autónomos (62,8%), quase autónomos (8,5%), quase dependentes (4,3%) e dependentes (3,2%). CONCLUSÕES: A qualidade de vida, para a maioria dos idosos, pode ser considerada bastante positiva, sendo que uma minoria apresentou problemas de diminuição cognitiva grave ou algum grau de dependência.OBJECTIVES: To characterize elderly people's perception of quality of life and well-being. METHODS: A sample of 1,665 elderly aged 75 years or more living in 13 districts in Portugal were studied. Of them, 311 subjects were excluded because they were not able to convey their opinions. The EASYcare (Elderly Assessment System) was used to collect data from 1,354 (81.3%) of 1,665 subjects. Statistical analysis was based on principal components analysis and cluster analysis. RESULTS: EASYcare's showed to have good psychometric proprieties (Cronbach a =0.92). The principal component analysis identified 4 factors: mobility, communication competencies, well-being and daily living activities. Cluster analysis showed 4 groups of elderly people: autonomous (62.8%), almost autonomous (8.5%), almost dependent (4.3%) and dependent (3.2%). CONCLUSIONS: Quality of life for most of the elderly is quite positive, and only a small group presents problems related to severe cognitive impairment or some degree of dependence.

Objetivos: Avaliar sintomas de ansiedade e depressão em pacientes Portugueses com doença pulmonar obstrutiva crónica (DPOC) e fatores preditores associados. Tipo de estudo: Estudo multicêntrico, transversal. Local: Três centros de saúde e um hospital distrital da região centro de Portugal. População: 302 pacientes com DPOC. Métodos: Os sintomas de ansiedade e depressão foram avaliados através da Escala de Ansiedade e Depressão (HADS). Pontuações ≥8 foram consideradas clinicamente significativas. Resultados: Esta amostra de pacientes tinham em média de 67±12 anos de idade e 41,1% era do género feminino. A DPOC era de grau ligeiro em 106 (35,1%) pacientes, moderado em 106 (35,1%) e grave-a-muito-grave em 90 (29,8%). Sintomas de ansiedade clinicamente significativos estavam presentes em 53,3% dos pacientes e sintomas de depressão em 45,4%. Os sintomas de ansiedade estavam associados a limitação nas atividades (OR 1,39, 95% CI 1,04-1,84), idade (0,97, 95% CI 0,94-0,99) e género feminino (OR 0,37; 95% CI 0,20-0,66). Os sintomas de depressão estavam associados à mobilidade funcional (OR 1,91, 95% CI 1,38-2,66) e a limitação nas atividades (OR 1,12, 95% CI 1,04-1,21). Conclusão: Uma elevada proporção de pacientes com DPOC experiencia sintomas de ansiedade e depressão. Os programas nacionais de reabilitação respiratória deveriam incluir intervenções colaborativas de autogestão que respondam a estas necessidades emocionais.

A OMS (2002) alertou para a importância do apoio formal adequado aos cuidadores familiares, caso contrário tornar-se-ão um novo grupo de pacientes nos sistemas de saúde. Nesta pesquisa estudou-se a percepção do estado de saúde e a satisfação com a vida em dois grupos de cuidadores de idosos: com demência e sem demência. Os resultados apontam para a interferência adversa da tarefa de cuidar no bem-estar dos participantes, sendo importante a necessidade de se utilizarem diferentes programas de intervenção dependendo do tipo de dependência do idoso. ABSTRACT Objective: To report a case of recurrent isolated sleep paralysis (RISP), a benign parasomnia with worrisome and frightening sleep paralysis episodes. Case description: We describe a case of RISP in a sixteen-year-old girl who seeks medical attention for anxiety symptoms. The sleep paralysis and associated auditory and tactile hallucinations began three years before with worsening in the last year, causing fear of sleeping. The episodes were intensely frightening causing negative impact in patient’s sleep, school performance and social function. Medical conditions were excluded, and she started treatment with a selective serotonin reuptake inhibitor with complete resolution of symptoms. Comments: Sleep complaints are often devalued. Therefore, clinicians should actively ask their patients about their sleep during health assessment.
