Enquadramento: a automedicação envolve o uso de medicamentos para tratar distúrbios ou sintomas autodiagnosticados, ou o uso intermitente ou continuado de medicação prescrita por um médico para tratamento crónico ou doenças ou sintomas recorrentes. Objetivos: caraterizar a prática da automedicação nos estudantes do ensino superior da saúde. Metodologia: estudo quantitativo e descritivo. Amostra constituída por 301 estudantes. Recolha de dados com recurso a questionário e análise de dados efetuada através do Statistical Package for the Social Sciences (SPSS). Resultados: verifica-se automedicação em 93,4% dos estudantes do ensino superior saúde. Os sinais e sintomas mais frequentes foram as cefaleias (29,8%), dismenorreia (13,5%) e febre (9,9%). Os medicamentos mais utilizados foram os analgésicos/antipiréticos (34,6%), os anti-inflamatórios (29,2%), vitaminas e sais minerais (7,4%). Conclusão: a automedicação entre os estudantes do ensino superior é elevada, pelo que é fulcral abordar esta problemática ao longo do curso de forma a minimizar os riscos associados a este consumo.

A cross-sectional study was conducted with 227 adults, 162 using antiretroviral therapy (ART), both sexes, in Secondary Immunodeficiency Outpatient Clinic of the Department of Dermatology of the Hospital das Clínicas of the Faculty of Medicine of University of São Paulo. To investigate the prevalence of osteopenia/osteoporosis among HIV-infected people living in São Paulo city, we studied 108 HIV-infected patients (79 men and 29 women). Redução da densidade mineral óssea tem sido descrita como uma complicação clínica entre as pessoas vivendo com HIV/AIDS. Entretanto não há dados descrevendo essa alteração entre os pacientes brasileiros. Nosso objetivo foi investigar a prevalência de baixa densidade mineral óssea entre pessoas vivendo com HIV/AIDS na cidade de São Paulo. Nós estudamos108 pacientes infectados pelo HIV (78 homens e 30 mulheres). Foram utilizados neste estudo dados secundários. Os dados foram originalmente coletados com o objetivo de acompanhamento de rotina dos pacientes na clínica e para este estudo estes dados foram coletados dos prontuários médicos. Todos os pacientes foram submetidos ao exame de densitometria óssea, que é uma técnica radiológica que mensura a densidade mineral óssea. Os pacientes foram classificados como tendo baixa densidade mineral óssea de acordo com a classificação da organização mundial da saúde que define osteopenia quando o T-score a partir de -1,1 e osteoporose quando T-score abaixo de -2,5 quando se tratava de homens e mulheres com idade acima de 50 anos. Quando homens e mulheres tinham idade até 50 anos, utilizamos a classificação da ISCO, neste grupo baixa densidade mineral óssea foi definida quando os pacientes apresentavam um Z-score abaixo de -2. Entretanto para este estudo ambas as classificações foram definidas como baixa densidade mineral óssea. A mediana de idade, tempo de infecção pelo HIV a partir da data do diagnóstico, tempo sob terapia antiretroviral, número de células linfócitos TCD4+ no momento da avaliação e Nadir foram similares entre homens e mulheres. A mediana de idade foi de 43 anos (intervalo interquartílico [li] 43-48 anos) e mediana de tempo de infecção pelo HIV foi de 3,66 anos (intervalo interquartílico [li] 1,72- 10,91 anos). Os pacientes tinham adquirido o HIV principalmente pela via sexual (homens que fazem sexo com home 46% e 50% eram heterossexuais). A mediana de células linfócitos TCD4+ foi de 399céls/mm 3(intervalo interquartílico [li] 275 - 566,5). Vinte e cinco pacientes foram classificados como tendo baixa DMO (23,15%). Não houve associação estatisticamente significante entre sexo, IMC, nadir e baixa DMO. Os fatores de risco associados à baixa DMO foram células linfócitos TCD4+

Micronutrient deficiency is common in patients with HIV/AIDS, usually caused by mal-absorption and/or drug interactions. 25-hydroxyvitamin D is of fundamental importance for the homeostasis of musculoskeletal health. The current study aimed to evaluate the nutritional status of HIV-infected subjects in order to make their nutritional diagnoses, including their vitamin D blood levels, and to estimate their consumption of vitamin D. The study included 98 HIV-1-infected subjects, followed at University of São Paulo Medical School - HC-FMUSP. We performed a nutritional evaluation, along with the determination of patients’ serum 25-hydroxyvitamin D and calcium concentration, biochemical analyses, and an anthropometric assessment. In the medical interview a 24-hour food recall was used (R24) to estimate daily calorie intake, macronutrients, calcium, and vitamin D. A high level of vitamin D deficiency was observed in our patients: 83.4% of them had levels below 30 ng/ml; they also presented an increased risk of cardiovascular disease, along with a high consumption of dietary fat. Factors related to the virus itself and to the use of antiretroviral drugs may have contributed for the low vitamin D levels seen in our HIV-1-infected patients.

Introduction: Highly Active Antiretroviral Therapy (HAART) has improved and extended the lives of thousands of people living with HIV/AIDS around the world. However, this treatment can lead to the development of adverse reactions such as lipoatrophy/lipohypertrophy syndrome (LLS) and its associated risks. Objective: This study was designed to assess the prevalence of self-reported lipodystrophy and nutritional status by anthropometric measurements in patients with HIV/AIDS. Methods: An observational study of 227 adult patients in the Secondary Immunodeficiencies Outpatient Department of Dermatology, Hospital das Clínicas, Faculty of Medicine, University of São Paulo (3002 ADEE-HCFMUSP). The sample was divided into three groups; Group 1 = 92 patients on HAART and with self-reported lipodystrophy, Group 2 = 70 patients on HAART without self-reported lipodystrophy and Group 3 = 65 patients not taking HAART. The nutritional status of individuals in the study sample was determined by body mass index (BMI) and percentage of body fat (% BF). The cardiovascular risk and diseases associated with abdominal obesity were determined by waist/hip ratio (WHR) and waist circumference (WC). Results: The prevalence of self-reported lipoatrophy/lipohypertrophy syndrome was 33% among women and 59% among men. Anthropometry showed depletion of fat mass in the evaluation of the triceps (TSF) in the treatment groups with HAART and was statistically independent of gender; for men p = 0.001, and for women p = 0.007. Similar results were found in the measurement of skin folds of the upper and lower body (p = 0.001 and p = 0.003 respectively). In assessing the nutritional status of groups by BMI and % BF, excess weight and body fat were more prevalent among women compared to men (p = 0.726). The WHR and WC revealed risks for cardiovascular and other diseases associated with abdominal obesity for women on HAART and with self-reported LLS (p = 0.005) and (p = 0.011). Conclusions: Anthropometric measurements were useful in the confirmation of the prevalence of LLS. BMI alone does not appear to be a good parameter for assessing the nutritional status of HIV-infected patients on HAART and with LLS. Other anthropometric measurements are needed to evaluate patients with the lipoatrophy/lipohypertrophy syndrome. Objetivos: A terapia antirretroviral altamente ativa (HAART) tem melhorado e aumentado a vida de milhares de pessoas que vivem com a infecção pelo HIV/AIDS em todo o mundo. No entanto, este tratamento pode levar ao desenvolvimento da síndrome da lipodistrofia (LDS). Este estudo foi desenvolvido para avaliar a prevalência de auto-relato de LDS, perfil nutricional e medidas antropométricas de pacientes com HIV/AIDS. Métodos: Estudo observacional de 227 pacientes adultos, divididos em: Grupo 1: 92 pacientes em HAART e com LDS; Grupo 2: 70 pacientes em tratamento com HAART e sem LDS e Grupo 3: 65 pacientes que não tomam HAART. O estado nutricional foi avaliado pelo índice de massa corporal (IMC) e o percentual de gordura corporal (%GC) por meio de medidas antropométricas. Resultados: A prevalência de auto-relato de LDS foi de 44% entre as mulheres e 39% entre os homens. DC do tríceps (PCT) apresentou-se mais elevada no grupo HAART e LDS (homens p < 0,001; mulheres p < 0,007) em comparação com aqueles sem HAART, respectivamente. IMC revelou excesso de peso para a maioria dos indivíduos. Conclusões: As medidas antropométricas foram úteis para confirmar a prevalência de auto-relato da síndrome da lipodistrofia. A avaliação das dobras dos braços e pernas revelou-se um bom método para avaliação antropométrica de lipoatrofia de membro, independentemente do sexo. Estes resultados permitiram o estabelecimento de estratégias para o diagnóstico precoce da LDS na prática clínica, em pessoas vivendo com HIV / AIDS.

TB is currently considered to be the most important infectious disease among HIV-1-infected subjects in developing countries, such as Brazil. A retrospective analysis of TB cases was performed, occurring from January 1995 to December 2010 in our cohort of 599 HIV positive patients. The primary outcome was the occurrence of active TB. Forty-one TB cases were diagnosed over this period of 16 years, among 599 HIV positive patients in an open cohort setting in the city of Sao Paulo, Brazil. All-time lowest mean CD4 T cell count at the time of TB diagnosis was 146 and 186 cells/mm³, respectively. The mean HIV viral load was 5.19 log10 copies/mL, and 59% of the patients were on HAART. TB incidence was 1.47 per 100 person-years, for a total follow-up time of 2775 person-years. The probability of surviving up to 10 years after diagnosis was 75% for TB patients as opposed to 96% for patients with other, non-TB opportunistic diseases (p = 0.03). TB can be considered a public health problem among people living with HIV in Brazil despite of the widespread use of antiretrovirals for the treatment of HIV infection/AIDS. Atualmente, a tuberculose (TB) é considerada a doença infecciosa mais importante entre os pacientes infectados pelo HIV-1 nos países em desenvolvimento, como o Brasil. Análise retrospectiva dos casos de tuberculose ocorridos a partir de janeiro 1995 até dezembro de 2010 foi realizada em nossa coorte de 599 pacientes HIV positivos. O desfecho primário foi a ocorrência de TB ativa, e 41 casos da doença foram diagnosticados durante este período de 16 anos. As contagens médias do nadir de células T CD4 e ao momento do diagnóstico de TB foram de 146 e 217 células/mm³, respectivamente. A carga viral média de HIV foi de 5,19 log10 cópias/mL, e 59% dos pacientes estavam em tratamento com ART. A incidência de TB foi de 1,47 casos por 100 pessoas-ano, para um tempo total de seguimento da coorte de 2775 pessoas-ano. A probabilidade de sobreviver até 10 anos após o diagnóstico foi de 75% para pacientes com TB, em oposição a 96% para pacientes com outras doenças oportunistas não-TB (p = 0,03). A tuberculose pode ser considerada problema de saúde pública entre as pessoas que vivem com HIV no Brasil, apesar da ampla utilização de anti-retrovirais para o tratamento da infecção pelo HIV / AIDS.

The polymorphism of IL28B was described as important in the pathogenesis of infections caused by some viruses. The aim of this research was to evaluate whether IL28B gene polymorphisms (SNP rs8099917 and SNP rs12979860) were associated with HAM/TSP. The study included 144 subjects were classified according to their neurological status in two groups: Group I (60 asymptomatic HTLV-1 carriers) and Group II (84 HAM/TSP patients). Blood samples were collected, and PBMC separated by Ficoll density gradient centrifugation. DNA was extracted using a commercial kit, and the DNA was stored at -80°C for later analysis. After this, the proviral load was quantified, and the rs8099917 and rs12979860 SNPs in the region of IL28B-gene were analyzed by StepOnePlus Real-time PCR System. A multivariate model analysis, including gender, age, and HTLV-1 DNA proviral load, showed that IL28B polymorphism SNP rs12979860 was not independently associated with HAM/TSP outcome. In contrast, the SNP rs8099917 allele GG was independently associated with HAM/TSP outcome (OR=6.25; IC95%=1.22– 32.00). Persons with SNP rs8099917 genotype GG may present a distinct immune response against HTLV-1 infection. So, it seems reasonable to suggest that a search for IL28B polymorphisms should be performed for all HTLV-1-infected subjects in order to monitor their risk for disease development; however, since this is the first description of this finding in the literature, we should first replicate this study with more HTLV-1-infected persons to strengthen the evidence already provided by our results.
