AbstractBackground This is the first study to examine the clinical utility of measuring plasma secretoneurin (SN) levels in patients with heart failure with reduced ejection fraction (HFrEF), as a predictor of unplanned hospitalization, and all-cause mortality independently, and as a composite endpoint at one-year follow-up.Methods The study group includes 124 caucasian patients in New York Heart Association (NYHA) classes II to IV. Plasma SN concentrations were statistically analyzed in relation to sex, age, BMI, etiology of HFrEF, pharmacotherapy, clinical, laboratory and echocardiographic parameters. Samples were collected within 24 h of admission to the hospital.Key results In the 12-month follow-up, high SN levels were noted for all three endpoints.Conclusions SN positively correlates with HF severity measured by NYHA classes and proves to be a useful prognostic parameter in predicting unplanned hospitalizations and all-cause mortality among patients with HFrEF. Patients with high SN levels may benefit from systematic follow-up and may be candidates for more aggressive treatment.

act Pericarditis is a disease entity associated with the primary or secondary inflammation of its layers, which may be accompanied by the accumulation of fluid in the pericardial sac, the consequences of which depend mainly on the speed of its accumulation. The causes of pericardial diseases can be divided into infectious or non-infectious diseases, but in about 30% of cases the etiology of the disease cannot be determined. Pericarditis causes 5% of all emergency department visits related to non-ischemic chest pain. In the study we presented a case of a 21-year-old Caucasian male with acute exudative pericarditis, which required drainage of the pericardial sac despite the use of pharmacological treatment. The patient underwent a wide range of diagnostic tests and specialist consultations, but ultimately the etiological factor of acute exudative pericarditis could not have been identified. The therapy of acute exudative pericarditis, especially in the case of undetermined etiology, requires an individualized approach from the attending physician. Based on the observation of inflammatory parameters, imaging tests and clinical picture, the physician is to decide whether pericardiocentesis or modification of pharmacological treatment is necessary. Searching for the etiological factor is essential if it has an impact on the successive methods of diagnosis and treatment, especially in patients with recurrent idiopathic pericarditis or in the absence of a response to the therapy.

During the pandemic of Coronavirus disease, many procedures and restrictions were implemented to stop the spread of the virus, though none of which is as efficient as vaccination. COVID-19 vaccines are vital for returning to time before the pandemic. Moreover, analyzing the protective effects and complications of vaccines it is crucial to minimize its adverse effects. Our review analyzed 28 cases described in medical journals, of patients who developed subacute thyroiditis (SAT) after COVID-19 vaccines. Most commonly, SAT follows upper respiratory tract infections. Until 2016 only 5 cases of SAT after vaccinations had been reported. After COVID-19 vaccines, SAT developed mostly among women. The most cases were observed after the first dose, after seven days on average. In all presented patients, symptoms were similar, including mostly hyperthyroidism signs and pain. TSH level among 82% was low or undetectable. Treatment in most cases included anti-inflammatory and analgesics medications, propranololi hydrochloridum, and glucocorticoids, leading to full recovery. SAT occurs in healthy individuals with negative infection and thyroid disease medical history as well as in patients associated with thyroid diseases. The mechanism, which may lead to subacute thyroiditis, may be complex and involve Angiotensin converting enzyme 2 (ACE2) receptors, cross-reactions between thyroid peroxidase and spike protein, and the autoimmune/inflammatory syndrome induced by adjuvants (ASIA). Cross-reaction between thyroid peroxidase and spike protein, nucleoproteins, and membrane protein of SARS-CoV-2 was proved on animal models. SARS-CoV2 infection mechanism requires the presence of ACE2 receptors and transmembrane serine protease (TMPRSS2), which were both found in thyroid follicular cells. Spike protein, produced after vaccination, is modified (HexaPro), and it is more stable and binds higher. Some authors suggest that in effect it may lead to pyroptosis and thyroid cells destruction. Another mechanism of post-vaccine SAT is ASIA, which manifests with various symptoms, and although it is undermined by many health professionals some authors diagnosed ASIA in patients with SAT. SAT is a self-limiting disease and non-life threatening. However, very important is the diagnosis, especially in individuals with risk factors of cardiovascular disease to prevent side effects of hyperthyroidism like cardiac arrhythmias, tachycardia, coronary syndromes and side effects from other organs.

Excessive consumption of alcohol is not only a social problem, but it also significantly increases the morbidity and mortality rates of many societies. A correlation has been demonstrated between alcohol consumption and increased mortality from cancer, accidents and injuries, liver cirrhosis and other causes. Alcohol abuse increases the incidence of hemorrhagic stroke and the risk of ischemic stroke, induces serious arrhythmias, adversely affects blood pressure and damages the heart muscle. The dose and way of drinking alcohol play a crucial role in assessing whether this drink allows people to maintain health or whether it is a great health and social threat. The beneficial effects of low and moderate doses of alcohol on the occurrence of cardiovascular diseases have been shown in many population studies and meta-analyses in which the effect of U-shaped or J-shaped curves relating alcohol intake to cardiovascular mortality was observed, especially in ischemic heart disease. However, due to the fact that alcohol consumption is associated with many health hazards, it is not recommended to consume it as a preventive action of cardiovascular diseases. Moreover, recent studies suggest that association of low-to-moderate alcohol consumption with the reduction in cardiovascular risk is a result of lifestyle changes and that any reduction in alcohol consumption is in fact beneficial in terms of general health.

Background Defensins play an important role in the processes associated with innate immunity. Their growth is observed in sites of intense inflammation, but also in less severe inflammation. Evaluation of alpha-defensin levels in patients with chronic heart failure (CHF) and its possible association with a long-term prognosis may be an interesting supplement to knowledge about the state of the immune system in heart failure. The aim of the study was to compare the plasma concentration of alpha-defensin between the patients with chronic heart failure with impaired ventricular ejection fraction (LVEF)

A healthy diet plays a key role in the prevention and management of cardiovascular diseases. Atherosclerosis and its complications are nowadays one of the leading causes of death in the western population. Among numerous factors known for slowing down the progression of the disease is the consumption of vegetables, fruits and chocolate rich in multiring compounds called polyphenols. Chocolate is a source of catechins, epicatechins and procyanidins. Recently, several studies have demonstrated the beneficial effects of dark chocolate in management of hypertension by hypotensive effect associated with vasorelaxation mediated most likely by the stimulation of nitric oxide production in the vascular wall. Cocoa and its derivatives have also strong antioxidative properties resulting in enhancement of the total antioxidant potential and inhibition of lipid peroxidation. It was also demonstrated that cocoa polyphenols have a favourable antithrombotic effect connected with the inhibition of platelet activation. Chocolate may diminish insulin resistance and inhibit inflammatory process. Cocoa improves also psychical well being. This pleiotropic activity of cocoa results in slowing down the progression and limiting the complications of atherosclerosis in regular chocolate consumers.

Venous thromboembolism (VTE) is a disease that includes both deep vein thrombosis (DVT) and pulmonary embolism (PE), is an important cause of morbidity and mortality worldwide. VTE is the third most common cardiovascular illness after acute coronary syndrome and stroke. Unfractionated heparin, low-molecular-weight heparin, and warfarin have been the foundation of venous thromboembolism (VTE) prevention and treatment but are being replaced step-by-step by recently approved non-vitamin K antagonist oral anticoagulants (NOACs). Betrixaban is a direct FXa inhibitor with clear pharmacological characteristics: minimal renal clearance, minimal hepatic metabolism, and long half-life. FDA and Portola Pharmaceuticals on June 23 announced the approval of betrixaban to reduce the risk of venous thromboembolism (VTE) in adults hospitalized for an acute medical illness who have restricted mobility or other risk factors for thromboembolic complications. FDA stated that it approved the drug on the basis of data from the Acute Medically Ill VTE Prevention With Extended Duration Betrixaban (APEX) trial. The APEX trial compared thromboembolic event and death rates in acutely ill patients treated with betrixaban capsules for 35–42 days or subcutaneously administered enoxaparin for 6–14 days. All study participants had been hospitalized for heart failure, respiratory failure, infection without septic shock, rheumatic disorders, or ischemic stroke. Among hospitalized medically ill patients, extended-duration betrixaban demonstrates a favorable net clinical outcome and is associated with an ≈ 30% reduction in fatal or irreversible ischemic or bleeding events compared with standard-duration enoxaparin followed by placebo.

The World Health Organization (WHO) recognized diabetes as one of the four most important and priority health issues out of non-communicable diseases. According to a report by the WHO with the year 2016 the prevalence of diabetes for 3 decades and continues to grow, this problem applies to the entire world. In 2014. the number of diabetes patients brought the 422 million, by comparison, in 1980. It was 108 million. A badly aligned metabolically diabetes contributes to the development of numerous complications of micro-and macro-angiopathic, which are related to adverse prognosis and increase the risk of cardiovascular disease. Striving for the best possible alignment of the carbohydrate economy reduces both the mortality and cardiovascular. However, some patients with diabetes intensive glucose control is not effective and increases the incidence of severe hypoglycemia, which in turn some patients increases cardiovascular mortality. The aim of the work is the appearance of hypoglycemia as a factor that increases the risk of death in cardiovascular diseases. The work also emphasises the importance of cardiovascular diseases in diabetes, which are the most common complication of diabetes and the most common cause of death in this group of patients.

For several decades the vitamin K antagonist oral anticoagulants were the only outpatient therapy that existed to reduce the risk of stroke and thromboembolism until non-vitamin K oral anticoagulants (NOACs). Direct thrombin inhibitor: dabigatran and factor Xa inhibitors: apixaban, rivaroxaban, and edoxaban directly inhibit the coagulation cascade. DOACs have many advantages over warfarin. In general, the new agents have similar or lower bleeding risk than vitamin K antagonists, especially risk of intracranial bleeding. However, the biggest drawback of DOACs has been the lack of specific antidotes to reverse the anticoagulant effect in emergency situations. New specific antidotes (idarucizumab, andexanet alfa, and ciraparantag) show promising data, and may soon become available for clinical use. In this article, we review the pharmacology of these agents, outcomes of their use, and the more recent advances for the development of specific antidotes.

Wciąż trwają poszukiwania markerów biochemicznych, które pomogłyby lepiej opisać tę grupę pacjentów i posłużyły jako czynniki rokownicze w przewidywaniu zgonu u chorych z NS. Cel: Celem pracy była ocena wartości rokowniczej osoczowego stężenia białka C-reaktywnego oznaczonego metodą o wysokiej czułości (hs-CRP) u chorych z przewlekłą niewydolnością serca, jako ewentualnego predyktora zgonu w ciągu 24–miesięcznej obserwacji chorych ze skurczową przewlekłą niewydolnością serca (PNS). Materiał i metody: Badaną grupę stanowiło 165 pacjentów hospitalizowanych z PNS w II – IV klasie wg NYHA. U wszystkich pacjentów z grupy badanej przy przyjęciu wykonano rutynowe badania laboratoryjne, a także oznaczono osoczowe stężenie białka C-reaktywnego metodą turbidymetryczną o wysokiej czułości (hs-CRP), osoczowe stężenie ferrytyny oraz osoczowe stężenie N-końcowego propeptydu natriuretycznego typu B (NT-proBNP). Grupę kontrolną stanowiło 31 zdrowych ochotników. Jako punkt końcowy badania ustalono zgon z dowolnej przyczyny, a okres obserwacji wynosił 24 miesiące. Wyniki: W trakcie dwuletniego okresu obserwacji zmarło 28,5% pacjentów. Charakteryzowali się oni istotnie wyższą klasą NYHA, niższą frakcją wyrzutową oraz wyższymi medianami osoczowych stężeń hs-CRP, troponiny T i NT-proBNP. Porównując grupę badaną z grupą kontrolną wykazano, że osoczowy poziom hs-CRP różnił się istotnie statystycznie pomiędzy obiema grupami. W analizie jednoczynnikowej przeprowadzonej według modelu hazardu proporcjonalnego Cox’a wykazano istotny statystycznie wpływ na wystąpienie punktu końcowego następujących parametrów: klasy NYHA (p = 0,007), EF (p = 0,018), hs-CRP (p = 0,002), NT-proBNP (p < 0,001) oraz BMI (p = 0,0496). W analizie wieloczynnikowej wykazano, że hs-CRP (p = 0,028, HR=1,032), NT-proBNP(p = 0,007; HR = 1), klasa NYHA (p = 0,032; HR = 0,453) oraz BMI (p = 0,048, HR=1,051) są niezależnymi, istotnymi statystycznie czynnikami wpływającymi na wystąpienie zgonu w obserwowanej grupie chorych. Proponowany punkt odcięcia dla hs-CRP wyliczony na podstawie krzywej ROC wyniósł 11,99 mg/L, AUC=0,617 (0,521-0,712), p=0,0168, natomiast dla NT-proBNP wyniósł 1615 pg/ml, AUC=0,659 (0,569-0,75), p= 0,0005. Nie zaobserwowano istotnej statystycznie różnicy w mocy diagnostycznej testów pomiędzy hs-CRP i NT-proBNP. Dla poszczególnych punktów odcięcia dokonano analizy przeżycia Kaplana-Meiera i wykazano, że proponowane punkty odcięcia zarówno dla hs-CRP (p < 0,001), jak i NT-proBNP (p = 0,006) dzielą chorych na dwie grupy z istotnie statystycznie różnicą w długości życia. Podwyższone stężenia hs-CRP i NT-proBNP wiązały się z dużo większym ryzykiem wystąpienia zgonu, niż w przypadku tylko podwyższonego stężenia NT-proBNP. Wnioski: Osoczowe stężenie hs-CRP można uważać za wartościowy marker rokowniczy zgonu u chorych hospitalizowanych z rozpoznaniem skurczowej przewlekłej niewydolności serca w II-IV klasie wg NYHA. Abstract Introduction: Heart failure (NS) is a cardiological disease which is characterised by a constantly increasing morbidity and mortality rates. There are still searches for bio-chemical markers which better describe this group of patients and which may be used as prognostic factors in forecasting repeated hospitalization and death for patients with NS. Aim: The goal of this paper was to assess the usefulness of marking the highly specific C-reactive protein (hs-CRP) as a potential predicator for death within a 24-month follow-up in patients with systolic chronic heart failure (PNS). Methods: The study group was comprised of 165 patients with chronic NYHA II-IV class heart failure who were hospitalized. All patients from the examined group underwent, on admission, routine lab tests as well as they were tested for the plasmatic concentration of protein C-reactive with a turbidimetric method with high sensitivity (hs-CRP), plasmatic concentration of ferritin and plasmatic concentration of N-final pro-peptide natriuretic type B (NT-proBNP). 31 healthy individuals served as negative controls. The follow-up period took 24 months and the endpoint included the death for any reason during the follow-up. Results: 28.5% of patients died during a 2-year follow-up. Those patients were characterized by significantly statistically higher NYHA class, lower ejection fraction and higher plasma concentration medians of hs-CRP, troponin T and NT-proBNP. The comparison of the examined group and the control group has shown that the plasmatic hs-CRP level was statistically different in both groups. In univariate analysis, a statistically significant impact on the occurrence of the endpoint was observed: NYHA class (p = 0,007), EF (p = 0,018), hs-CRP (p = 0,002), NT-proBNP (p < 0,001) oraz BMI (p = 0,0496). Multivariate analysis has shown that hs-CRP (p = 0,028), NT-proBNP (p = 0,007), NYHA class (p = 0,032) and BMI (p = 0,048) are independent significant predictors of death. In the ROC analysis the suggested cut-off point for hs-CRP amounted to 11,99 mg/L, AUC=0,617 (0,521-0,712), p=0,0168, whereas for NT-proBNP it was 1615 pg/ml, AUC=0,659 (0,569-0,75), p=0,0005. Stosowanie statyn może się jednak wiązać z nietolerancją tej grupy leków i występowaniem działań niepożądanych, których częstość zwiększa się w przypadku terapii skojarzonej z innymi lekami hipolipemizującymi. Ponadto u kilku procent chorych pomimo zastosowania skojarzonej terapii lekami hipolipemizującymi, nie udaje się osiągnąć docelowych wartości stężenia cholesterolu LDL (z ang. low-density lipoprotein – LDL, LDL-C). Nowa grupa leków - inhibitory proproteinowej konwertazy subtylizyny/keksyny 9 (PCSK9), które regulują stężenie receptora dla LDL-C, wydają się być nowym, skutecznym narzędziem terapeutycznym, stosowanym celem osiągnięcia docelowych wartości cholesterolu LDL. Celem pracy było przedstawienie metabolizmu cholesterolu w organizmie człowieka, znaczenia skutecznej terapii zaburzeń gospodarki lipidowej, aktualnego stanu wiedzy na temat białka PCSK9 i jego inhibitorów. Opisano wyniki najważniejszych badań klinicznych z inhibitorami PCSK-9. Słowa kluczowe: cholesterol, proproteinowa konwertaza białkowa subtylizyna/keksyna typu 9 (PCSK9), inhibitory PCSK9. Abstract Statins are currently the most frequenly used group of lipid-lowering drugs. This group of drugs can effectively reduce the risk of adverse cardiovascular diseases in both primary and secondary prevention, however, their impementation may lead to intolerance and other side effects, the frequency of which increases in the case of a combined therapy with other lipid-lowering drugs. Additionally, in spite of the therapy a litte percenatge of the patients still cannot reach the target concentartion of LDL cholesterol. Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors (a new class of drugs that regulate the concentration of LDL cholesterol receptor) seem to be a new and effective therapeutic tool used to reach the target level of LDL cholesterol. The aim of the study was to present the cholesterol metabolism in human body, the significance of an effective therapy for lipid disorders as well as the current knowledge of the PCSK9 protein and its inhibitors. W ponad 95% przypadków przyczyną ChNS jest zmniejszenie przepływu wieńcowego spowodowanego miażdzycą tętnic wieńcowych, dlatego celem opisu tego zespołu chorobowego często zamienne stosuje się termin choroba wieńcowa [z ang. coronary artery disease (CAD) lub coronary heart disease (CHD)]. Manifestacja kliniczna IHD obejmuje swoim spektrum cztery zespoły chorobowe: zawał mięśnia sercowego; dusznicę bolesną, przewlekłą IHD z niewydolnością serca i nagłą śmierć sercową. W wysoko rozwiniętych krajach Europy IHD jest główną przyczyną zgonów i niepełnosprawności. Śmiertelność z powodu choroby wieńcowej w ciągu ostatnich czterech dekad znacząco się zmniejszyła, lecz mimo tego ciągle jest ona odpowiedzialna za około jedną trzecią zgonów u osób powyżej 35 roku życia. Celem pracy jest przedstawienie ewolucji w kardiologii inwazyjnej, która doprowadziła do znaczącego zmiejszenia śmiertelności i wydłużenia życia chorych z IHD. Abstract Ischaemic heart disease (IHD) is a condition which results from reduced blood supply to the heart muscle. In more than 90% of cases, the cause of myocardial ischemia is reduction in coronary blood flow due to atherosclerotic coronary arterial obstruction. Thus, IHD is often termed coronary artery disease (CAD) or coronary heart disease (CHD). The clinical manifestations of IHD can be divided into four syndromes: myocardial infarction (MI), angina pectoris, chronic IHD with heart failure and sudden cardiac death. Coronary heart disease (CHD) is a major cause of death and disability in developed countries. Although CHD mortality rates worldwide have declined over the past four decades, but still CHD remains responsible for about one-third or more of all deaths in individuals over age 35. The aim of this work is to explain of evolution in invasive cardiology, which lead to decrease in mortality and prolonged life in patients with IHD. PNS dzieli się na skurczową oraz na niewydolność serca (NS) z zachowaną frakcją wyrzutową. Choroba ta występuje u około 1-2% dorosłych w krajach rozwiniętych, natomiast u osób >70 roku życia odsetek ten stanowi >10%. Niewydolność serca stanowi jedną z wiodących przyczyn śmierci i niepełnosprawności na świecie, ponadto charakteryzuje się stale rosnącą zapadalnością. W przebiegu tej choroby w mięśniu sercowym dochodzi do niekorzystnych zmian, które mogą zostać zatrzymane lub nawet cofnięte poprzez wczesne, odpowiednie i zintensyfikowane leczenie. Chorzy na PNS stanowią niejednorodną grupę, a wpływ na stężenie peptydów natriuretycznych mają różne czynniki kliniczne, takie jak wiek, płeć, funkcja nerek, funkcja tarczycy, indeks masy ciała oraz niedokrwistości. Ponadto ocena stężenia peptydów natriuretycznych nie charakteryzuje wszystkich mechanizmów, które stanowią podłoże niewydolności serca. Zarówno chorzy ze skurczową PNS jak i chorzy z zachowaną frakcją wyrzutową mają podobne objawy, ale różne przyczyny patofizjologiczne NS oraz odmienną odpowiedź na leczenie. Na skutek tych wszystkich ograniczeń peptydów natriuretycznych w dalszym ciągu poszukuje się nowych markerów, które w lepszy i dokładniejszy sposób będą oceniały rokowanie chorych na PNS w praktyce klinicznej. Celem pracy jest przedstawienie roli wybranych nowych markerów w stratyfikacji ryzyka chorych z przewlekłą skurczową niewydolnością serca. Abstract Chronic systolic heart failure (CHF) is a disease with complex symptomatology. Chronic CHF can divided into systolic and heart failure (NS) with preserved ejection fraction. Approximately 1-2% of adults suffer from the disease in the developed countries, whereas it affects >10% of patients > 70 years of age. Heart failure is one of the leading causes of death and disability in the world, furthermore it is characterized by an ever-increasing incidence. In the course of the disease adverse changes take place in the heart muscle, however, these changes can be stopped or even reversed by timely, relevant and intensified treatment. Patients with CHF are a heterogeneous group and the influence on the concentration of natriuretic peptides have various clinical factors such as age, sex, renal function, thyroid function, body mass index and anemia. Moreover, the evaluation of natriuretic peptides does not characterize all the mechanisms that underlie heart failure. Both patients with systolic CHF and patients with preserved ejection fraction have similar symptoms but different pathophysiological causes of NS and a different response to treatment. As a result of all these limitations related to natriuretic peptides there are still searches for new markers that will assess the prognosis of CHF patients in clinical practice better and more accurately. The aim of the study is to present the role of new (selected) markers in risk stratification of patients with chronic systolic heart failure. Streszczenie Rozpiętość rozkładu objętości erytrocytów (red cell distribution width - RDW) jest wskaźnikiem zmienności wielkości krwinek czerwonych (anizocytoza). Krwinki czerwone zazwyczaj mają standardową wielkość, ale zaburzenia prowadzące do nieskutecznego procesu erytropoezy lub sytuacje zwiększonego niszczenia krwinek czerwonych doprowadzają do większej różnorodności w wielkości krwinki czerwonej, co przekłada się na wzrost wskaźnika RDW. RDW jest parametrem rutynowo oznaczanym w praktyce klinicznej jako część morfologii krwi, jest obecnie stosowany jako pomoc w diagnostyce różnicowej anemii. W celu poprawy stratyfikacji ryzyka chorób sercowo-naczyniowych identyfikacja nowych markerów prognostycznych może zapewnić poznanie dokładnej patofizjologii tych chorób oraz dać nowe możliwości terapeutyczne. Aktualnie szereg różnych parametrów i markerów biologicznych jest wykorzystywana do tworzenia modeli w celu przewidywania przeżycia pacjenta z daną chorobą sercowo-naczyniową. Najnowsze dane sugerują również, że RDW może być istotnym prognostycznie markerem dla śmiertelności długoterminowej w szeregu chorób układu sercowo-naczyniowego, w tym choroby wieńcowej, zawału mięśnia sercowego, niewydolności serca, udaru mózgu, nadciśnienia płucnego i choroby tętnic obwodowych. Abstract Red cell distribution width (RDW) is an index of the variation in red cells size (anisocytosis). Usually, red blood cells have a standard size, but disorders leading to ineffective erythropoiesis or situations of increased red blood cell destruction cause greater heterogeneity in red blood cell size, with a higher RDW value. Red cell distribution width is routinely measured in clinical practice as part of the automated complete blood count. Red cell distribution width is currently used as a diagnostic aid in the differential diagnosis of anemia because it reflects variability in the size of circulating erythrocytes. Higher values reflect greater heterogeneity in erythrocyte cell sizes. To improving risk stratification, identification of new prognostic markers may provide insight into underlying pathophysiology or suggest avenues for therapeutic development. To date, a wide variety of clinical variables and biological markers have been used to create predictive models for survival in patients with cardiovascular disease. Recent data also suggest that red cell distribution width also may be a prognostic marker for long-term mortality. This has been particularly well studied in patients with cardiovascular disease, including coronary disease, myocardial infarction, heart failure, strokes, pulmonary hypertension and peripheral artery disease.
