espanolLa hemorragia subaracnoidea aneurismatica es un sangrado secundario a rotura aneurismatica en el cual la sangre se colecciona en el espacio subaracnoideo. La hemorragia subaracnoidea es una emergencia y necesita atencion medica inmediata, lo que hace fundamental su sospecha diagnostica. Esta debera ser realizada en funcion de la clinica, para lo cual es fundamental el reconocimiento de los signos de alarma de una cefalea, ya que en ese caso habra que realizar pruebas complementarias, siendo la TAC de eleccion. El tratamiento sera principalmente endovascular y requerira un tratamiento precoz para mejorar el pronostico en el paciente. En atencion primaria es fundamental discernir entre las cefaleas urgentes y las que no lo son. EnglishAneurysmal subarachnoid hemorrhage is a bleeding secondary to aneurysmal rupture, collecting blood in the subarachnoid space. Subarachnoid hemorrhage is an emergency and needs immediate medical attention, which makes its diagnostic suspicion fundamental. The diagnostic suspicion will have to be carried out according to the clinic, for which the recognition of the headache warning signs is essential, since in the case of a suspicion of a headache with warning signs, complementary tests will have to be carried out, being of choice the CT scan. The treatment will be mainly endovascular and will require early treatment to improve the prognosis in the patient. In Primary Care level, a good orientation of headache is essential to distinguish between those that are urgent and those that are not.

espanolEl insulinoma es un tumor formado por celulas especializadas beta de los islotes de Langerhans pancreaticos, que segregan constantemente insulina, lo que causa hipoglucemia. El diagnostico del insulinoma se realiza mediante la clinica (sintomas neuroglucopenicos y adrenergicos), la analitica (glucemia, peptido C e insulina) y la prueba de imagen complementaria (normalmente tomografia axial computerizada) que objetiva el insulinoma. El tratamiento consiste en la intervencion quirurgica mediante enucleacion del insulinoma, pancreatectomia parcial o total o cirugia con la tecnica de Whipple. Aunque es cierto que el insulinoma es una patologia rara, en atencion primaria hay que sospechar su presencia en pacientes con clinica sugestiva y solicitar analitica para completar estudio. EnglishInsulinoma is a tumor formed by specialized beta cells of the islets that are constantly secreting insulin, which causes hypoglycemia. The diagnosis of insulinoma is made through the clinic (neuroglycopenic and adrenergic symptoms), the analytical (glycemia, C-peptide and insulin) and the complementary image test (usually computerized axial tomography) that targets the insulinoma. The treatment consists of surgical intervention by enucleation of the insulinoma, partial or total pancreatectomy or surgery with the Whipple technique. At the Primary Care level, although it is true that insulinoma is a rare pathology, it is necessary to suspect it in patients with suggestive symptoms and order lab tests (or blood tests) to complete the study.

espanolLos sindromes linfoproliferativos engloban una gran variedad de subtipos cuyo diagnostico y abordaje difiere en funcion del subtipo. El diagnostico de los sindromes linfoproliferativos se basa en criterios que abarcan tanto clinica como analitica. Algunas de las pruebas solo pueden realizarse desde el Servicio de Hematologia, por lo que la sospecha a partir de la clinica es fundamental en atencion primaria para completar estudio en el hospital. El tratamiento varia en funcion del subtipo de sindrome linfoproliferativo: desde tratamiento conservador en determinados subtipos asintomaticos hasta quimioterapia. En atencion primaria es fundamental sospechar dichas patologias, por lo que es importante el conocimiento de la clinica a partir de una buena anamnesis y exploracion fisica. EnglishThe lymphoproliferative syndromes include a wide variety of subtypes whose diagnosis and management vary depending on the subtype. The diagnosis of lymphoproliferative syndromes is based on criteria that include both clinical and analytical. Some of the tests can only be performed from the Hematology Service, so the suspicion from the clinic is essential in Primary Care to complete a hospital-level study. The treatment will vary depending on the subtype of lymphoproliferative syndrome and may range from conservative treatment in certain asymptomatic subtypes to chemotherapy. At Primary Care level it is basic to suspect such pathologies, being important the knowledge of the clinic with a good anamnesis and physical examination.

espanolLas distrofias musculares congenitas son enfermedades con una escasa prevalencia e incidencia por lo que son consideradas enfermedades raras. Son enfermedades diagnosticadas en su mayoria durante la infancia, con un pronostico variable en funcion del tipo de distrofia. No tienen tratamiento, ya que el origen de la patologia es genetico. Por tanto, este es sintomatico. Como enfermedad rara, es importante el conocimiento de la misma a la hora del manejo a nivel de Atencion primaria. EnglishCongenital muscular dystrophies are diseases with a low prevalence and incidence so they are considered rare diseases. They are diseases diagnosed mostly during childhood with a variable prognosis depending on the type of dystrophy. They do not have treatment since the origin of the pathology is genetic and the treatment is symptomatic. As a rare disease, it is important to be aware of it when handling primary care.

espanolLa endoftalmitis es una infeccion ocular grave generalmente producida por bacterias y hongos en el contexto de una cirugia ocular previa, aunque existen otras causas. Se trata de una enfermedad inflamatoria infecciosa grave que puede originar la perdida de vision e incluso la perdida ocular. El tratamiento consiste en antibioticos intravitreos y sistemicos, ajustados segun el antibiograma. El diagnostico de la enfermedad es clinico, si bien cuando se trata una urgencia puede ser vista desde atencion primaria, motivo por el cual es importante reconocerla, dada la gravedad de la misma, y derivarla como tal para un tratamiento posterior por parte del oftalmologo. EnglishEndophthalmitis is a serious eye infection usually produced by bacteria and fungi in the context of previous eye surgery although there are other causes. It is a serious infectious inflammatory disease that can lead to loss of vision and even eye loss. The treatment consists of intravitreal and systemic antibiotics, adjusting them according to antibiogram. The diagnosis of the disease is clinical although it is an emergency, it can be seen in Primary Care which is why it is important to recognize it, given the severity of it, and derive it as such for a subsequent treatment by the ophthalmologist.
