Metal fabrication workshops (MFWs) are common businesses in Ugandan cities, and especially those producing metallic security gates, window and door frames (burglar-proof), and balcony and staircase rails. The objective of this study was to comparatively assess the pollution levels and potential health risks of manganese (Mn), chromium (Cr), cadmium (Cd), lead (Pd) and nickel (Ni) in pooled surface soil samples from four 5-, 7-, 8-, and 10-year-old MFWs (n = 28) and a control site (n = 8) in Mbarara City, Uganda. The concentration of the potentially toxic elements (PTEs) was determined using inductively coupled plasma–optical emission spectrometry. Contamination, ecological, and human health risk assessment indices and models were used to identify any risks that the PTEs could pose to the pristine environment and humans. Our results showed that PTE pollution of soils is occuring in the MFWs than at the control site. The mean concentrations of the PTEs (mg kg−1) in the samples were: Mn (2012.75 ± 0.23–3377.14 ± 0.31), Cr (237.55 ± 0.29–424.93 ± 0.31), Cd (0.73 ± 0.13–1.29 ± 0.02), Pb (107.80 ± 0.23–262.01 ± 0.19), and Ni (74.85 ± 0.25–211.37 ± 0.14). These results indicate that the PTEs could plausibly derive from the fabrication activities in these workshops, which is supported by the high values of contamination factors, index of geoaccumulation, and the overall increase in pollution load indices with the number of years of operation of the MFWs. Human health risk assessment showed that there are non-carcinogenic health risks that could be experienced by children who ingest PTEs in the soils from the 7-, 8- and 10-year-old MFWs. The incremental life cancer risk assessment suggested that there are potential cancerous health effects of Cd and Ni that could be experienced in children (who ingest soils from all the four MFWs) and adults (ingesting soils from the 8- and 10-year-old MFWs). This study underscores the need to implement regulatory guidelines on the operation and location of MFWs in Uganda. Further research should be undertaken to investigate the emission of the PTEs during welding operations in the MFWs.

Abstract Introduction Unhealthy alcohol use significantly contributes to viral non‐suppression among persons with HIV (PWH). It is unknown whether brief behavioural interventions to reduce alcohol use can improve viral suppression among PWH with unhealthy alcohol use in sub‐Saharan Africa (SSA). Methods As part of the SEARCH study (NCT04810650), we conducted an individually randomized trial in Kenya and Uganda of a brief, skills‐based alcohol intervention among PWH with self‐reported unhealthy alcohol use (Alcohol Use Disorders Identification Test–Consumption [AUDIT‐C], prior 3 months, ≥3/female; ≥4/male) and at risk of viral non‐suppression, defined as either recent HIV viral non‐suppression (≥400 copies/ml), missed visits, out of care or new diagnosis. The intervention included baseline and 3‐month in‐person counselling sessions with interim booster phone calls every 3 weeks. The primary outcome was HIV viral suppression (200 ng/ml (RR 0.97, 95% CI: 0.92–1.02). Conclusions In a randomized trial of 401 PWH with unhealthy alcohol use and risk for viral non‐suppression, a brief alcohol intervention reduced unhealthy alcohol use but did not affect viral suppression at 24 weeks. Brief alcohol interventions have the potential to improve the health of PWH in SSA by reducing alcohol use, a significant driver of HIV‐associated co‐morbidities.

Two new heteroleptic cobalt(III)dialkyldithiophosphate complex cations with a 1,2-bis(diphenylphosphino)ethane (dppe) ancillary ligand having formula [Co{S2P(OR)2}2(dppe)](B(C6H5)4) (R = –C2H5 (Co–Et); –CH(CH3)2 (Co–Pr)) have been synthesized and characterized spectroscopically as well as via single crystal X-ray diffraction analysis. The single crystal X-ray diffraction analyses reveal that the immediate geometries around Co(III) in both complexes are distorted octahedral, which are satisfied by four sulfur atoms of two dialkyldithiophosphate ligands and two phosphorus centers of the dppe ligand. These complex cations are neutralized by tetraphenyl borate B(C6H5)4 anions. The solid-state framework of both complexes are stabilized by C–H⋯π and C–H⋯C interactions. The nature of these interactions have been addressed with the help of Hirshfeld surface analysis, and percent contributions of pertinent interactions in the crystal structure of these complexes have been addressed using fingerprint plots. Both complexes have been used as heterogeneous electrocatalysts for the oxygen evolution reaction (OER). Results suggest that Co–Et is a better electrocatalyst for the OER, displaying an onset potential of 1.68 V and Tafel slope of 114 mV dec−1. Furthermore, in the homogeneous electrocatalysis of the hydrogen evolution reaction (HER) using trifluoroacetic acid, the overpotential for Co–Et and Co–Pr have been 1.05 V and 0.92 V, respectively, with turnover frequencies of 1518 s−1 and 287.5 s−1, respectively. Overall, heterogeneous OER and homogenous HER results suggest that Co–Et can be used as an efficient molecular electrocatalyst for oxygen/hydrogen evolution reactions. [ABSTRACT FROM AUTHOR]

In this work, a new d10-configuration based Cd(II) coordination polymer (CP) with the formula [Cd2(CH3COO)2(bimb)2]·2ClO4 (1) have been synthesized using 4,4′-bis(imidazolyl)biphenyl (bimb) ligand and characterized. Single-crystal X-ray analysis reveals that the CP forms a 2D layer connected to a 3D supramolecular framework with weak packing interactions between benzyl and imidazolyl rings. CP 1 has been used as a photoluminescent sensor and a photocatalyst for the photodegradation of antibiotics, in particular nitrofurantoin (NFT). Sensing experiments indicate that 1 can selectively detect nitrofurantoin with a limit of detection of 0.025 μM, whereas when employed as a photocatalyst, CP 1 can photodecompose NFT up to 91.83% in a time span of 20 min. The decline in the photoluminescence intensity of CP 1 in the presence of NFT and the photocatalytic mechanism are addressed using theoretical calculations. [ABSTRACT FROM AUTHOR]

Background: The phenomenon of Knowledge Translation (KT) is a key intervention towards bridging the ‘know–do’ gap. We conducted a KT initiative in Isingiro district to positively change attitude and improve on the uptake of Insecticide Treated Mosquito Nets (ITNs) as a malaria prevention strategy. Methods: This was a community based interactive initiative that was carried out within the seventeen administrative units of Isingiro district using varied dissemination activities, namely: health talks; drama activities, and the sharing of ITNs success stories. Results: We reached out to 34 dissemination groups, comprising communal gathering, religious crusades, open markets, secondary schools, and district administration. In addition, we spot-visited 46 households to ascertain the physical presence of ITNs, and their appropriate use. The major intervention was improved knowledge base of malaria causation and prevention strategies. The indicators for improved knowledge were hinged on the five-interventions, namely: (a) communal sensitization on malaria to provide, (b) monitoring and support of selected households, (c) emphasis of ITN use as a malaria prevention strategy, (d) promotion of care for ITNs, and (e) promotion of ITN use. In all, the major output was improved knowledge base of malaria causation and prevention strategies by providing accurate information to redress the myths and misconceptions related to malaria and ITNs use. Conclusion: This undertaking describes a consolidated community intervention to promote ITN utilization. It is plausible that this intervention positively enhances and promotes uptake and utilization of ITNs.

Abstract Background The burden of malaria in Uganda remains unacceptably high, especially among children and pregnant women. To prevent malaria related complications, household possession and use of Insecticide Treated mosquito Nets (ITNs) has become a common practice in the country. Despite the availability of ITNs, malaria remains a foremost public health concern in Uganda. We sought to explore knowledge, attitude, and behaviour towards the use of ITNs as a nightly malaria prevention strategy among pregnant women and children under five years of age in Isingiro district, Southwestern Uganda. Materials and Methods This was a community based, descriptive cross-sectional study, in which households with children under 5 years, and/or pregnant women were enrolled. We used a structured questionnaire to collect data on participants’ understanding of the causes, signs and symptoms of malaria; use of ITNs to prevent malaria; attitudes and behaviours towards the use of ITNs. We also conducted key informant interviews (KIIs) to get in-depth understanding of responses from the participants. We analysed quantitative data using STATA version 12.Qualitative findings from the KIIs were transcribed and translated, and manually analysed using thematic content analysis. Results Of the 369 households enrolled, 98.4% (N = 363) households had children under five. Most participants (41.2%, N = 152) were in the 21–30 age category (mean age; 32.2 years). 98.1% (N = 362) of the respondents considered ITNs a key malaria prevention strategy. The ITN possession rate was 84.0% (N = 310), of these, 66.1% (N = 205) consistently used them. 39% of the respondents did not have a positive attitude towards ITNs. Conclusions Although 84.0% of the respondents possessed ITNs, many were not consistently using them. To this, there is need to engage all stakeholders (including cultural leaders, community health workers, religious leaders and the government) in the malaria prevention campaigns using ITNs through: a) government’s concerted effort to ensure universal access of right fit ITNs, b) end-user directed health education to emphasize positive attributes of ITN use, c) telling the ITN success stories to improve on the usage.

Background. H. influenzae remains an organism of a major public health challenge worldwide despite the availability of the Hib vaccine, particularly among children under 5 years. Information on the current carriage status and antibiotic susceptibility is key on proper health-care provision. Therefore, we conducted a study to determine H. influenzae carriage rate and antibiotic susceptibility testing of the isolates among the children. Methods. This was a cross-sectional study conducted between January and May 2018, among clinically healthy children under five years attending Maternal and Child Health (MCH) Clinic in Mbarara Regional Referral Hospital (MRRH). We carried out standard microbiology methods to culture, isolate, and identify H. influenzae, and then, we tested for their susceptibility to commonly used antibiotics following the CLSI standards. Results. Of the 248 participants included in the study, 116 (46.77%) were females and 132 (53.23%) males and 78 (31.45%) were below the age of 3 months. Fifty one of the study participants had H. influenzae in their nasopharynx, which represents 20.56% carriage (95% CI 15.49 to 25.63). There was a general high susceptibility of the isolates to the antimicrobial agents commonly used. There was 100% susceptibility to ciprofloxacin and imipenem antibiotic agents, though 6 (11.76%) and 4 (7.84%) of the isolates showed resistance to chloramphenicol and ampicillin, respectively. Conclusion. The high burden presented by H. influenzae and the resultant impact on child health require much attention to prevention of infections associated with the organism. A well-funded molecular study focusing on typing the isolates would determine the impact of the vaccine, given the carriage rates are still high.
