There is a need to identify psychological factors that may predispose such patients to pain chronicity. The primary aim of this study was to examine whether pain acceptance potentially mediated the relationship between pain catastrophizing and post-surgical outcomes in a sample of compensated lumbar fusion patients.Patients and methods: Patients insured with the Workers Compensation Fund of Utah and who were at least 2 years post-lumbar fusion surgery completed an outcome survey. These data were obtained from a prior retrospective-cohort study that administered measures of pain catastrophizing, pain acceptance, mental and physical health, and disability.Results: Of the 101 patients who completed the outcome survey, 75.2% were male with a mean age of 42.42 years and predominantly identified as White (97.0%). The majority of the participants had a posterior lumbar interbody fusion surgery. Pain acceptance, including activity engagement and pain willingness, was significantly correlated with better physical health and mental health, and lower disability rates. The authors report on the views of a group of young people who had joined adoptive or foster families some six years previously when they were between five and 11 years old (the Maudsley Follow-Up Study). The report focuses on the experience of joining a new family, family relationships and feelings of belonging. It was striking that it could take some young people a considerable length of time to feel settled; however, by the follow-up stage the great majority felt very much a part of their family. Several described their placement as an opportunity for a 'new start'. A small minority was less settled and expressed continued discomfort with their adoptive status. The direction of current practice and policy developments with reference to these findings is discussed. [ABSTRACT FROM AUTHOR]

Several studies of family placements have indicated poorer outcomes for children who are placed singly in new families. In an effort to understand why this may be so, this paper explores differential outcome among a group of children who were placed singly with adoptive or long-term foster families during middle childhood. A small number of these children were 'only children' at the time of placement but most had siblings living elsewhere. Particular attention is paid to preferential rejection by birth parents in the children's backgrounds. Preferential rejection indicates that one child has been rejected while siblings are accepted. This was found to be associated with poorer progress in placement. The methodological and statistical detail of the analyses are to be found in another paper. We focus here on describing the patterns which emerged and the ways in which the findings may assist practitioners in making and supporting permanent placements. [ABSTRACT FROM AUTHOR]

There is a growing interest in the relationship between mental images and attentional templates as both are considered pictorial representations that involve similar neural mechanisms. Here, we investigated the role of mental imagery in the automatic implementation of attentional templates and their effect on involuntary attention. We developed a novel version of the contingent capture paradigm designed to encourage the generation of a new template on each trial and measure contingent spatial capture by a template-matching visual feature (color). Participants were required to search at four different locations for a specific object indicated at the start of each trial. Immediately prior to the search display, color cues were presented surrounding the potential target locations, one of which matched the target color (e.g., red for strawberry). Across three experiments, our task induced a robust contingent capture effect, reflected by faster responses when the target appeared in the location previously occupied by the target-matching cue. Contrary to our predictions, this effect remained consistent regardless of self-reported individual differences in visual mental imagery (Experiment 1, N = 216) or trial-by-trial variation of voluntary imagery vividness (Experiment 2, N = 121). Moreover, contingent capture was observed even among aphantasic participants, who report no imagery (Experiment 3, N = 91). The magnitude of the effect was not reduced in aphantasics compared to a control sample of non-aphantasics, although the two groups reported substantial differences in their search strategy and exhibited differences in overall speed and accuracy. Aphantasia is a neurocognitive phenomenon affecting voluntary visual imagery, such that it is either entirely absent, or markedly impaired. Using both the social and medical models of disability, this article discusses the extent to which aphantasia can be understood as a disorder or just a form of neutral neurodivergence, given that imagery plays a central role in thinking and memory for most other people. Preliminary school performance data are presented, showing that low imagery does not necessarily complicate life, especially given compensatory strategies and low societal barriers. In addition, we discuss the consequences of labelling aphantasia a disorder with regard to self- and public stigma, and we provide further data regarding a confidence gap, by which aphantasics perceive themselves as performing worse than they objectively do. Visual mental imagery is the ability to create a quasi-perceptual visual picture in the mind's eye. For people with the rare trait of aphantasia, this ability is entirely absent or markedly impaired. Here, we aim to clarify the prevalence of aphantasia in the general population, while overcoming limitations of previous research (e.g., recruitment biases). In Experiment 1, we screened a cohort of undergraduate students (n502) using the Vividness of Visual Imagery Questionnaire (Marks, 1973) and found that 4.2% had aphantasia. To establish the reliability of our estimate, we then screened a new sample of people (n502) at an online crowdsourcing marketplace, again finding that approximately four percent (3.6%) had aphantasia. Overall, our combined prevalence from over a thousand people of 3.9% - which shows no gender bias - provides a useful index for how commonly aphantasia occurs, based on measures and diagnostic thresholds in line with contemporary aphantasia literature., (Copyright © 2021 Elsevier Inc. Although Galton recognized in the 1880s that some individuals lack visual imagery, this phenomenon was mostly neglected over the following century. We recently coined the terms "aphantasia" and "hyperphantasia" to describe visual imagery vividness extremes, unlocking a sustained surge of public interest. Aphantasia is associated with subjective impairment of face recognition and autobiographical memory. Here we report the first systematic, wide-ranging neuropsychological and brain imaging study of people with aphantasia ( n  = 24), hyperphantasia ( n  = 25), and midrange imagery vividness ( n  = 20). Despite equivalent performance on standard memory tests, marked group differences were measured in autobiographical memory and imagination, participants with hyperphantasia outperforming controls who outperformed participants with aphantasia. Face recognition difficulties and autistic spectrum traits were reported more commonly in aphantasia. The Revised NEO Personality Inventory highlighted reduced extraversion in the aphantasia group and increased openness in the hyperphantasia group. Resting state fMRI revealed stronger connectivity between prefrontal cortices and the visual network among hyperphantasic than aphantasic participants. In an active fMRI paradigm, there was greater anterior parietal activation among hyperphantasic and control than aphantasic participants when comparing visualization of famous faces and places with perception. For people with aphantasia, visual imagery is absent or markedly impaired. Here, we investigated the relationship between aphantasia and two other neurodevelopmental conditions also linked to imagery differences: synaesthesia, and autism. In Experiment 1a and 1b, we asked whether aphantasia and synaesthesia can co-occur, an important question given that synaesthesia is linked to strong imagery. Taking grapheme-colour synaesthesia as a test case, we found that synaesthesia can be objectively diagnosed in aphantasics, suggesting visual imagery is not necessary for synaesthesia to occur. However, aphantasia influenced the type of synaesthesia experienced (favouring 'associator' over 'projector' synaesthesia - a distinction tied to the phenomenology of the synaesthetic experience). In Experiment 2, we asked whether aphantasics have traits associated with autism, an important question given that autism - like aphantasia - is linked to weak imagery. This study compared the effectiveness and outcomes of different family finding methods in adoption in England, over-selecting harder to place children. The case files of 149 children with adoption recommendations in ten local authorities were reviewed and a sub-sample of sixty-seven cases were followed in real time, through interviews with professionals and families until six months after adoptive placement. Most matches were of good quality, but 14 per cent were fair and 13 per cent poor, involving serious compromises on matching requirements or adopters' preferences. There were more poor matches when in-house placements were made or children's difficulties were underplayed with new parents and, necessarily, more compromises were made when matching children with significant health or developmental needs. More good quality matches were made when case responsibility was transferred early to the adoption team. Poorer quality matches were related to poorer outcomes six months after adoptive placement. To improve matching, searches for families need to be widened early to avoid delays and to maximise the pool of adopters. Formal processes to track and review the progress of adoptions for children with complex needs (including matching meetings) can help avoid delay and ensure that a group of professionals, rather than an individual professional, makes key decisions.

Objective: To discover the outcomes for children placed late for adoption (between 5 and 11 years old) from public care and to establish which factors predict poorer outcome., Method: Data were collected prospectively (1993-2003) from a representative sample of domestic U.K. adoptive placements (N = 108) at the start of placement, at 1 year, and 6 years later. Most of the children entered care because of abuse and neglect. Outcome was assessed by the disruption rate, by a twofold classification of the character of continuing placements, and by an assessment of psychological well-being., Results: At the adolescent follow-up, 23% of placements had disrupted, 49% were continuing positively, and 28% were continuing but with substantial ongoing difficulties. Four factors contributed independently to a higher risk of disruption: older age at placement (odds ratio = 1.07), having been singled out from siblings and rejected (5.87), time in care (1.04), and a high level of behavioral problems (1.25). Two factors predicted differences in continuing placements., Conclusions: Late adoption can be successful in that half the children made good progress, but the extent of disruptions and difficulties in continuing placements gives rise to concern. Knowledge of predictors will help in devising planning pre- and postplacement support services.

Quality Protects was introduced in 1998 as the government's main initiative to improve children's social services. This review considers the aims, origins and intended effects of the policy. While applauding the attempt to improve services, attention is drawn to four areas on which progress will depend: better performance indicators; the extent and quality of research based evidence; the expansion of professional social work training and user feedback and involvement. The first of the Quality Protects objectives on promoting secure attachment is highlighted and the evidence for the current emphasis on adoption is considered. The article concludes that the extent of progress that is being made to help children and families in difficulty will need to be assessed by systematic research as well as by routine service performance indicators.

Background: Several studies of family placements have indicated poorer outcomes for singly placed children. Two of our own studies have additionally shown that late placements of children who were actively singled out from siblings and alone in the care system were at increased risk of poor progress in the first year., Method: This paper draws on the data available for singly placed children from both studies to explore this phenomenon in more detail. The current analyses are limited by the fact that neither study was designed specifically to explore the effect of singling out or parental rejection, rather it emerged as a characteristic of potential explanatory importance., Results: The findings suggest that a history of being singled out or 'preferentially rejected' by birth parents, particularly in combination with 'false affection' from the child and lower levels of sensitivity in the new parents, was associated with poorer outcomes in the first year. Such children were more likely to show deteriorating behaviour patterns and to have more problems in forming satisfactory relationships with new family members., Conclusions: Older age at placement was associated with poorer outcome, but only significantly so among those children not classified as false in their displays of affection.
