Management of intracranial aneurysms is still a therapeutic challenge, especially in cases of complex lesions. Thus, the improvement of the study and intervention planning possibilities correlated with the access to continuous professional training based on simulation and clinical diversity represent optimal conditions for the efficient solution of this pathology. The development of three-dimensional printing technology offers a new opportunity in the modern treatment of intracranial aneurysms. The aim of this study is to present some aspects related to the materials and methods of manufacturing simulation models of individual 3D printed aneurysms and their influence in the optimal management of these lesions.

The role of Nonsteroidal Anti-Inflammatory Drugs (NSAIDs) in neurosurgical practice is a secondary one, however they are still constantly involved in perioperative management of pain or in nonoperative management of acute radiculopathy. Beside the well-known adverse reactions (ADRs), the neurosurgeon practitioner should also take in account the drug hypersensitivity reactions (DHRs) of NSAIDs and be able to deal with it. The aim of this paper was to review the diagnostic and management steps for NSAIDs-induced Hypersensitivity Reactions. The actual stratification of NSAIDs-induced Hypersensitivity Reactions is based on understanding of the heterogeneity of immunological/non-immunological mechanisms of reactions and complexity of clinical manifestations. Practically, this stratification allows the physician to assess suspicion of DHR, based on anamnesis and clinical analysis, and to consider further practical steps to manage and eventually confirm the diagnosis. Drug allergies are considered only the DHRs for which a definite immunological mechanism (either drug-specific antibody or T cell) is demonstrated. In conclusion, clinical analysis and anamnesis of patient with NSAIDs-induced Hypersensitivity Reactions can be realized by any physician and could be enough to diagnose, but it is not sufficient to confirm the diagnosis. In vitro tests and oral provocation challenges may be necessary to be undertaken by an allergy specialist.

Even if meningiomas are the most common radio-induced tumors that occur in the adult population, the epidemiology of these types of tumors after Chernobyl nuclear accident, is still unclear. This paper aims to determine the tumoral behavior of intracranial meningiomas in North-Eastern Romania, affected by the radioactive cloud from Chernobyl nuclear accident , over a period of 25 years, namely between 1990 and 2015. Our research consists of an analytical, observational, cohort-based and retrospective study, conducted in Prof. Dr. N. Oblu Clinical Emergency Hospital of Iasi, Romania, on a group of 1287 patients diagnosed with intracranial meningiomas and operated between 1990 and 2015. In these period there was an increased number of intracranial meningiomas, with first peak between 1993-1996 and the second peak between 2007-2015, corresponding to 7-10 years and 21-30 years, after the Chernobyl accident. Regarding the annual frequency of histopathologic grading, for grade I meningiomas there were no trend or cyclicity of the cases diagnosed each year, but for grade II and III meningiomas there were an ascending trend in the period 1996-2000, that corresponds to the 10-14 years from the Chernobyl accident.

Neurosurgery is a rewarding career choice in which there are many challenges and stress factors that can lower the level of satisfaction and also increase the levels of burnout. The identification and management of common work-related stressors is important for improving the performance of health-care specialists and also for providing high-quality patient care.

Aim: To present the case of a 57-year-old male with progressive bilateral proptosis. Material and method: The patient presented with bilateral proptosis and strength deficiency on the upper limbs. During hospitalization, the progression was unfavorable; proptosis progressed, causing a severe loss of vision in the left eye (from 0.8 Snellen to NLP). Results: The imagistic investigation revealed bilateral infiltration of the orbits, infiltrative lesions to the mediastinum and the abdomen. The patient was referred to neurosurgery for further management. Surgical orbital decompression was performed with biopsy. The histopathological examination revealed non-Hodgkin small cell lymphoma. Conclusions: Assembling the clinical and paraclinical data we have suspected the possible diagnosis of Erdheim-Chester disease, however, positive diagnosis has not been achieved.

Orbitocranial penetrating injuries (OPI) represent a rare type of head injuries encountered in clinical practice. These appear after falls or motor vehicle accidents and are more frequent among children. We present the case of a male patient with OPI, associated with large frontal hemorrhagic contusion, with minimal periorbital trauma findings related to brain injury. Knowing and understanding the clinical anatomy of the orbit and the patterns of injuries in OPI are mandatory, since failure in detecting intracranial complications may lead to the increase of the neurological morbidity, visual loss and death.

Atypical meningioma represent an intermediary group between the benign meningioma (grade I) and anaplastic meningioma (grade III), and are known for high recurrence rate and short life expectancy. After modification of the classification World Health Organization in 2007, subsequent studies have tried to find prognostic factors for recurrence and survival, which are inconstant from author to author. This paper aims to present a short review of the most important prognostic factors in atypical meningioma.

For a long time, because of its location at the skull base level, the sphenoid bone was rather mysterious as it was too difficult for anatomists to reach and to elucidate its true configuration. The configuration of the sphenoid bone led to confusion regarding its sutures with the other skull bones, its shape, its detailed anatomy, and the vascular and nervous structures that cross it. This article takes the reader on a journey through time and space, charting the evolution of anatomists' comprehension of sphenoid bone morphology from antiquity to its conception as a bone structure in the eighteenth century, and ranging from ancient Greece to modern Italy and France. The journey illustrates that many anatomists have attempted to name and to best describe the structural elements of this polymorphous bone.

Schwannomatosis is a rare tumor syndrome characterized by the presence of multiple benign non-vestibular, non-intradermal schwannomas and by the absence of neurofibromatosis type 1 or 2 syndromes. Multiple schwannomas are clinically and genetically distinct from neurofibromatosis, and the main treatment in case of symptomatic lesions is represented by surgical resection. In case of asymptomatic lesions, the indicated treatment is neuroimaging follow-up (MRI). We are presenting the case of a patient with three schwannomas of the cauda equina, as well as the treatment strategy in the case of this rare pathology.

Cutaneous horns are relatively rare benign tumors which occur most frequently on sun exposed skin and develop on various types of underlying skin lesions: benign, premalignant and malignant. The treatment of choice consists in the surgical excision of the lesion to healthy tissue. The histopathological examination is mandatory in order to establish the nature of the lesion, on which the cutaneous horn develops.

Meningioma is a common intracranial neoplasm derived from meningothelial cells, and it is generally associated with a benign clinical course. In spite of this, the malignant behaviour of these tumours as the occurrence of extracranial meningioma metastases in different organs is described in the literature: lung and pleura, spine and other bones, abdominal organs, lymph nodes or even skin. The aim of this review is to analyse the pathways of metastatic spread of the intracranial meningioma tumour cells towards different organs.

The trigeminal neuralgia caused by neurovascular compression is a neurosurgical pathology requiring the preoperative identification as exact as possible of the neurovascular conflict. In this case, neuroimaging is very useful, as it allows not only the determination of the neurovascular conflict of the trigeminal nerve, but also the correct indication of an adequate surgical approach.

This paper aims to give a short presentation of several diseases of the spine mentioned in the Holy Scripture in order to identify the most common disease among the Jews during biblical times and attempts to provide their diagnosis.

Throughout the centuries, anatomists attempting to denominate the new structures they discovered have found inspiration in the civilization of ancient Rome and the clothing worn by its citizens. This aricle presents the origins of seven neuroanatomical terms, fimbria, velum, funiculus, lemniscus, corona, splenium, and cingulum, inspired by the clothing and jewellery of Roman women and the military attire of Roman soldiers. Thus, through their apparel, the Romans influenced the Terminologia Anatomica and "clothed" the structures of the brain and spinal cord, making them immortal. Clin. The orbitofrontal fibrous dysplasia is a rare, non-malignant disease that involves the development and thickening of craniofacial bones, causing facial asymmetry, visual, neurological and dental complications. Decreased visual acuity in orbitofrontal and sphenoidal fibrous dysplasia appears as a result of compressive lesions on the optic nerve. The aim of this article is to present the ophthalmologic complications due to the association of two entities that both involved the optic nerve: orbitofrontal fibrous dysplasia associated with a large sphenoid sinus mucocele.

Having a recent history, the neurological condition called myasthenia gravis has raised dilemmas and questions among doctors since it was first discovered in the 16th century and it has not ceased to be a challenge. Nowadays, neuroscience researchers from around the world have been striving to perfect a modern treatment of this condition. Our paper is an incursion into the past, more precisely into the history of the treatment of this disease, from its origin to date, when immunological therapy has progressed.

Traditionally, the awake cerebral neurosurgical interventions are carried out when the primary motor areas and the areas involved in language are concerned. This procedure is also known as mapping (Intraoperative Brain Mapping). The intraoperative neuropsychological protocol supposes the setting of a list of steps and tasks that the patient must perform during the surgery, taking into account the lesion localization (both the cortical and the subcortical structures), the cognitive functions involved and the individual particularities of each patient. As a result of the high validity of certain tests, we propose a common minimal set of tests, which can allow a minimal standardization and replicability.

Myasthenia gravis is one of the neurological diseases with a relatively recent history, full of mistakes, in which the British and German neurology schools have attempted to find answers when confronted with the unknown. The paper aims at making a historical account of the disease from its discovery in the 16th century, when the first case of myasthenia gravis was medically diagnosed, to the beginning of the 20th century, when the dawn of modern therapy started to show.

Today's neuroanatomical terminology has its origins in the Romans' way of life, in their civil and military house architecture, as well as in the fields of engineering and technology. Despite the fact that they did not know how the nervous system worked and what the role of each neuroanatomic structure was, over time, especially in Renaissance and early modern times, the anatomists sought descriptive names for the nervous structures they have identified by way of similarity with some ancient items. This study aims to briefly review the influence of Roman architecture, engineering, and technology on neuroanatomic nomenclature, the precursor of modern neuroanatomical terminology.

Penetrating orbitocranial injuries caused by intraorbital foreign body are a rare cause of morbidity being most common among young people. The term intraorbital foreign body refers to a foreign body that occurs within the orbit but outside the ocular globe. We report the case of a 12-year-old male child who sustained a right cranial facial trauma due to accidental fall on a piece of wood, which penetrated intraorbitally. Native cranial magnetic resonance imaging (MRI -1.5 T) revealed the presence of an intraorbital foreign body, 6 cm long and 1.5 cm wide, near the orbital apex. In our patient, early surgical extraction of the foreign body had a decisive role on his full recovery. In this case, although a large foreign body penetrated the entire length of the orbit, it did not cause damage to any intraorbital structure. Vision and right ocular globe function had an excellent prognosis.

Traumatic brain injury (TBI) is a leading cause of death and disability for which there is currently no effective drug therapy available. Because drugs targeting a single TBI pathological pathway have failed to show clinical efficacy to date, pleiotropic agents with effects on multiple mechanisms of secondary brain damage could represent an effective option to improve brain recovery and clinical outcome in TBI patients. In this multicenter retrospective study, we investigated severity-related efficacy and safety of the add-on therapy with two concentrations (20 ml/day or 30 ml/day) of Cerebrolysin (EVER Neuro Pharma, Austria) in TBI patients. Adjunctive treatment with Cerrebrolysin started within 48 hours after TBI and clinical outcomes were ranked according to the Glasgow Outcome Scale and the Modified Rankin Disability Score at 10 and 30 days post-TBI. Analyses of efficacy were performed separately for subgroups of patients with mild, moderate or severe TBI according to Glasgow Coma Scale scores at admission. Compared to standard medical care alone (control group), both doses of Cerebrolysin were associated with improved clinical outcome scores at 10 days post-TBI in mild patients and at 10 and 30 days in moderate and severe cases. A dose-dependent effect of Cerebrolysin on TBI recovery was supported by the dose-related differences and the significant correlations with treatment duration observed for outcome measures. The safety and tolerability of Cerebrolysin in TBI patients was very good. In conclusion, the results of this large retrospective study revealed that early Cerebrolysin treatment is safe and is associated to improved TBI outcome.

Severe head trauma remains an important public health issue and the intracranial pressure monitoring is useful in indicating the patients prognosis, variation especially elevated intracranial pressure were associated with a poor prognosis. We illustrate some cases where we insert the monitoring system (intraparenchymal or intraventricular) and the neurological evolution.

AIM: to compare the AgNOR's mean number with the histological type and grade of cerebral astrocytic tumors. MATERIALS AND METHOD: 16 primary cerebral astrocytic tumors (4 diffuse astrocytomas, 4 anaplastic astrocytomas and 8 glioblastomas) stereotactic biopsied in the Department of Neurosurgery, Clinical Hospital „Prof. Dr. N. Oblu” Iaşi, and histopathologically conventional diagnosed in Department of Neuropathology of the same hospital, were retrospectively identified. Tumor specimens were submitted to a combined staining technique: one – step silver nitrate method for AgNOR protein sites (modified after Ploton et al, 1986) counterstained with periodic acid-Schiff staining for basement membrane of vascular components. The mean AgNOR values (mAgNOR) for tumoral and vascular nuclei were determined. RESULTS: The average values of mean AgNORs/nucleus (mAgNOR/nucleus) presented a linear increase with increasing grade of malignancy from 1.96 for diffuse astrocytoma (GII), 2.34 for anaplastic astrocytoma (GIII), to 3.18 for glioblastoma multiforme (GIV). mAgNOR/tumoral nucleus also showed a linear correlation with the histological tumor grade: 2.27 for diffuse astrocytomas, 2.78 for anaplastic astrocytomas, and 3.35 for glioblastomas multiforme. A distinct difference between the mean values of AgNORs/vascular nucleus was expressed: 1.52 for diffuse astrocytoma (GII), 1.90 for anaplastic astrocytoma (GIII), and 3.18 for glioblastoma multiforme (GIV). There were some overlaps between GIII and GIV astrocytic tumors regarding the mAgNOR/tumoral nucleus: maximum value in anaplastic astrocytomas (GIII) was 2.91 and minimum value in small cells glioblastomas (GIV) was 2.47. Differentiation could be achieved with mAgNORs/vascular nucleus as no extreme value overlapped: maximum value in anaplastic astrocytomas (GIII) was 1.99 and minimum value in small cells glioblastomas (GIV) was 2.54. CONCLUSION: The malignancy grade of an astrocytic tumor can accurately be establish both on histological features of the conventional stained sample and on the average number, the shape and the distribution of AgNORs within tumoral and vascular nuclei, as AgNORs determinations supplement the histological information in small biopsies.
