Abstract

Previous studies have suggested that single-walled carbon nanotubes (SWCNTs) may pose a pulmonary hazard. We investigated the pulmonary toxicity of single-walled carbon nanohorns (SWCNHs), a relatively new carbon-based nanomaterial that is structurally similar to SWCNTs. Mice were exposed to 30 µg of surfactant-suspended SWCNHs or an equal volume of vehicle control by pharyngeal aspiration and sacrificed 24 hours or 7 days post-exposure. Total and differential cell counts and cytokine analysis of bronchoalveolar lavage fluid demonstrated a mild inflammatory response which was mitigated by day 7 post-exposure. Whole lung microarray analysis demonstrated that SWCNH-exposure did not lead to robust changes in gene expression. Finally, histological analysis showed no evidence of granuloma formation or fibrosis following SWCNH aspiration. These combined results suggest that SWCNH is a relatively innocuous nanomaterial when delivered to mice in vivo using aspiration as a delivery mechanism.

Abstract

Inhalation is the most important route of exposure for phosgene. A latency period occurs before phosgene affects the target organ, the lungs. The odor threshold is between 0.5 and 1.5 ppm, and the odor has been described as similar to newly mown hay. Toxic effects have been reported at concentrations below the threshold. On initial exposure, phosgene can undergo hydrolysis and form hydrogen chloride, which can be slightly irritating to the upper respiratory tract and eyes; the amount formed is limited by the low water solubility of phosgene. Once inhaled to the lower respiratory tract, phosgene undergoes an acylation reaction with amino, hydroxyl, and sulfhydryl groups, causing destruction of protein, lipids, and disruption of cellular functions. In response to this destruction, after a latency period of 1–24 h, a breakdown in the blood–air barrier occurs and protein-rich fluid accumulates in the lungs. Most commonly, death occurs within 48 h after exposure from progressive pulmonary edema and anoxia. At very high concentrations, death can occur from acute heart failure before the start of the pulmonary edema. Data regarding humans are limited to occupational exposures or accounts from the use of phosgene in World War I. Animal studies with phosgene show a steep exposure–response curve for pulmonary edema and mortality. Animal studies also indicate little species variability because all species exposed developed similar clinical signs (dyspnea, pulmonary edema, labored breathing) and histopathological lesions in the lungs. Although there are no chronic animal data, subchronic studies indicate little accumulation of phosgene or increased severity of lesions with continuous exposure.

Abstract

The Provisional Advisory Level (PAL) protocol was applied to estimate inhalation exposure limits for phosgene (CG). Three levels (PAL 1, PAL 2, and PAL 3), distinguished by severity of toxic effects, are developed for 24-hour, 30-day, 90-day, and 2-year durations of potential drinking water and inhalation exposures for the general public. For background on the PAL program and a description of the methodology used in deriving PALs, the reader is referred to accompanying papers in this Supplement. Data on humans are limited to occupational exposures or accounts from the use of phosgene as a chemical warfare agent in World War I. Animal studies with phosgene show a steep dose-response curve for pulmonary edema and mortality, with little species variability in effects. Although immediately upon exposure lacrimation and upper respiratory irritation can occur, the main effect in the target organ, a progressive pulmonary edema, occurs after a latency period of 1-24 hours. PAL estimates were approved by the Expert Consultation Panel for Provisional Advisory Levels in May 2007. Exposure limits for oral exposure to CG are not developed due to insufficient data. PAL estimates for inhalation exposure to CG are presented: The 24-hour PAL values for severity levels 1, 2, and 3 are 0.0017, 0.0033 and 0.022 ppm, respectively. The 30- and 90-day PAL values are 0.0006 and 0.0012 ppm for the PAL 1 and 2 values, respectively. These inhalation values were also accepted as the 2-year PAL 1 and 2 values because severity of lesions in the key study did not increase when exposures were extended from 4 weeks to 12 weeks. Data were not available for deriving 30-day, 90-day, and 2-year PAL 3 values.

Publisher Summary Phosgene is a colorless gas at ambient temperature and pressure, and inhalation is the most important route of exposure for phosgene. Phosgene is manufactured from a reaction of carbon monoxide and chlorine gas in the presence of activated charcoal. Inhalation is the most important route of exposure for phosgene. Because of its mild upper respiratory, eye, and skin irritancy, and a mildly pleasant odor, an exposed victim may not actively seek an avenue of escape before lower respiratory damage has occurred. Small amounts of phosgene can be irritating to the eyes and throat as phosgene undergoes hydrolysis to create hydrochloric acid that acts as an irritant. Far more dangerous, however, is the pulmonary edema that can develop after a latent period of 1–24 h post-exposure. Treatments that have been proposed to prevent pulmonary edema in exposed-asymptomatic persons include steroids, ibuprofen, N-acetyl cysteine, and positive pressure airway ventilation. However, there is no known antidote for phosgene poisoning, and although animal studies suggest that these treatments may be effective, no clinical data are available to verify efficacy in humans.
