Introduction: As one of the largest fertilizer industrieswith high competitiveness and high interest from consumers, the Gresik Fertilizer Company has various potential hazards that can cause workplace accidents, one of which is the danger of working at height. This study aims to determine the Occupational Safety and Health (OSH) programs and the application of hazard control of working at height in the Gresik Fertilizer Company. Methods: This study used a descriptive analysis method with a cross sectional study design. The variables studied were the hazard control efforts, the procedures to control the hazard, and the application of the hazard control process of working at height. Data collection techniques derived from secondary data in the form of company’s documents, and the data analysis method used in this study was in the form of qualitative method. Results: the Gresik Fertilizer Company has several OSH programs for the control of hazard of working at height with a success rate of 90%, have control methods of working at height with a success rate of 90%, and have guidelines for working at height with a success rate of 85%. Conclusion: The Gresik fertilizer company has OSH programs to control working at height, has hazard control methods for working at height, and guidelines for working at height. Background and objectives: The efficacy of a weight correction programme can be affected by such predictors as the number of contact hours, gender, age, baseline weight, parental weight status, psycho-emotional status, insulin resistance, and socioeconomic status. The aim of this current study was to evaluate the overall efficacy of the Weight Correction Programme at Children’s Clinical University Hospital, and explore the impact of the probable predictors. We evaluated the efficacy depending on gender, age, parental weight status, signs of depression, baseline body mass index z-score (z-BMI), and baseline waist circumference. Materials and Methods: The data were gathered from medical records. The inclusion criteria were as follows: Entered the programme by 13 June 2017, at least five years old, follow-up data available. All the respondents were divided into two age groups: Results: The study included 181 respondents. They were 5 to 17 years old on the first day of participation in the Weight Correction Programme. Results indicated that 117 (65%) patients managed to reduce z-BMI and 69 (38%) patients achieved clinically significant reduction of z-BMI. Boys had four times higher odds (odds ratio (OR) = 4,22; CI 1.37–13.05; p = 0.012) to reduce their z-BMI by at least 0.20 units than girls. The respondents of the older age group (≥10 years) had a better chance to reduce z-BMI than the younger ones (OR = 11,51; CI 2.04–64.83; p = 0.006). The odds to reduce z-BMI were lower by 7% for every extra cm of waist circumference (OR = 0.93; CI 0.88–0.99; p = 0.014) for reducing z-BMI. The follow-up time was also a positive predictor, and with every month the odds for clinically significant z-BMI reduction increased by 7% (OR = 1.07; CI 1.00–1.15; p = 0.047). The parental weight status, signs of depression, and baseline z-BMI were not significant predictors. Conclusions: More than half of the patients of the respondents managed to reduce their z-BMI. Female gender, younger age, and larger waist circumference were negative predictors.

The article discusses a very important legal institution within all democratic regimes namely the People’s Advocate – this is the name under which the Ombudsman is known in Romania. This institution has received lately an increased attention in the context of debates regarding alternative dispute resolution (ADR) tools, with a focus on its mediator role between state institutions and citizens. In the first part, the article offers a short presentation of the evolution of this institution both at the international and domestic level, the author insisting upon the comparative dimension. The rest of the sections of the article focus on the rapports of the People’s Advocate with the constitutional jurisdiction in Romania, insisting upon the legislative changes that had taken place in the last years. The author assesses these changes upon the way in which People’s Advocate exercises its competences given to him/her through the law.

Background: Despite rigorous focus on extracorporeal CRRT parameters such as access, blood flow, hemoconcentration, and anticoagulation, some patients have unexpected repetitive hemofilter clotting. We instead explored patient or disease-related factors that could be responsible, and present a case of plasma cell dyscrasia in which paraproteins caused hollow fiber failure., Methods: A patient with IgG kappa chain multiple myeloma complicated by sepsis and acute renal failure was started on CVVH with a regional citrate anticoagulation protocol that typically yields filter life of >50 h. Polysulfone hemofilters repetitively clotted every 2-4 h, even after excluding circuit-related problems. Failed filters were examined by light, electron (EM), and immunofluorescence (IF) microscopy., Results: Imaging of the hemofilters revealed several ultrastructural features typical for myeloma-associated alterations in native human tissue. Red cell rouleaux formation occurred within the hollow fibers. There was extensive protein layering on the luminal surface of the fibers with some extension into their walls: these deposits were IF+ for IgG kappa, and had a fibrillary substructure on EM., Conclusion: Extracorporeal hollow fiber phenomena recapitulate many intra-corporeal paraprotein effects such as those described in the kidney with plasma cell dyscrasias. Rapid protein layering suggests fouling of the membrane, decreased solute clearance before total device failure, and raises the theoretical concern that this might also occur during filtration plasmapheresis: we thus suggest serial serum free light chain levels to confirm their removal when using that technique. These findings emphasize the importance of disease rather than circuit-related factors that are under-appreciated causes of premature hemofilter failure.
