Serotonin is well known to be involved in the mediation of cognitive processes, but also in the regulation of multiple distinct physiological processes, such as pain. It is also involved in the modulation of nociception of the endogenous analgesic system. Among the monoamine neurotransmitters, serotonin plays an important role in signaling chronic pain, hence the importance of its monitoring in lumbar pain due to degenerative pathology, one of the common cause of this kind of pain. The aim of our study is to evaluate the impact of peloidotherapy on the variation of serum serotonin levels in patients with degenerative low back pain. In our study, conducted in Balneal and Rehabilitation Sanatorium of Techirghiol, we evaluated 130 patients who performed balneal and physical treatments for a period of 2 weeks. The patients were divided into 3 groups: 51 patients were enrolled into the hot mud baths group (HMB), 48 in the cold mud baths (CMB) group and 31 in the control group (CG). Each patient performed physiotherapy treatments which includes electrotherapy, massage and kinetotherapy, patients from HMB group had also hot mud therapy and patients from CMB group had cold mud therapy. Serum serotonin was dosed at admission and at discharge, for each patient, and was determined using ELISA method with kits from Demeditec Diagnostics GmbH for human serotonin. Necrotizing fasciitis (NF) is a severe infection involving the superficial fascial layers, subcutaneous cellular tissue, and possibly skin. It usually has a fulminant evolution, rapidly leading to death in the absence of early diagnosis and aggressive surgical treatment. We herein report a rare case of NF secondary to a traumatized occipital psoriatic plaque in an alcoholic 47-year-old woman and compare this case with the published literature. The NF extended to the entire scalp, right face, and posterior and lateral cervical region. Despite the initially guarded prognosis, the patient’s survival emphasizes the importance of aggressive surgical treatment with wide excision of all necrotic structures without any aesthetic compromise.

Introduction: A great variety of medical issues can occur after the COVID-19 infection including fatigue, muscle weakness, locomotor disability, self-care dysfunction, polyneuropathy, persistent dyspnea on exertion and a hypercoagulable state. Materials and methods: This paper presents the case of a nonsmoker 49-year-old male with right lung lower lobe lobectomy for post tuberculosis bronchiectasis and diabetes mellitus, who developed multiple serious physicals, neurological, hematological and respiratory consequences, related to critical COVID-19 infection and prolonged hospitalization. Results: A favorable evolution of the patient's respiratory sequels and motor impairment on both lower limbs was noticed after a complex individualized rehabilitation program started in the post COVID-19 Rehabilitation Department of Balneal and Rehabilitation Sanatorium, Techirghiol, Romania, consisting in better functional parameters and exercise tolerance, significant improvement in daily activities, remission of exertional dyspnea, social and family reintegration. Conclusions: multidisciplinary approach and complex individualized programs of rehabilitation is required after a critical form of COVID in a patients known with tuberculosis, and other complex pathologies, in order to restore physical function and mobility and optimize respiratory parameters. This systematic review examines groundbreaking advancements in hand rehabilitation, driven by significant progress in medical technology. Adhering to PRISMA guidelines, the review surveyed articles from various databases, emphasizing the utilization of advanced technologies for hand rehabilitation across diverse conditions such as fractures, burns, amputations, and postoperative care. Out of the 1562 studies analyzed, ten articles that met the inclusion criteria were identified. Key technologies like IMES (Implantable Myoelectric Sensor) technology are scrutinized for their potential to transform prosthetic control, providing intuitive functionality and substantial physiological hand function for individuals with limb loss. Similarly, ad-vancements in orthotic devices like the VacoHand Orthosis are showcased for their improve-ments in stabilizing and aiding the healing process in wrist immobilization and rehabilitation. The integration of virtual reality (VR) emerges as a pivotal convergence point, offering sup-plementary therapy to enhance direction-specific improvements in wrist and forearm mobility, hand function, and work-related tasks. Additional advantages include program customization, heightened engagement through interactive approaches, and improved functional outcomes such as hand strength and range of motion. While showcasing transformative potential, the re-view acknowledges the need for further research through larger sample sizes, standardized methodologies and more in-depth investigations to optimize these technologies. Overall, this review underlines the remarkable advancements achieved in hand rehabilitation through technological innovations, offering hope and improved outcomes for individuals grappling with various hand-related challenges.

Background: The main aim of this study was to compare conventional radiograph and computer tomograph (CT)-scan measurements of femoral offset. The second aim was the compare the pre- and postoperative values of femoral offsets as well as the variations created by total hip arthroplasty (THA) replacements using the same protocol of analysis for a patient operated on both hips by the same surgeon using a modified lateral approach. Methods: The CT protocol involves 1mm slices from top of the iliac crest to the isthmus of the femur, 2mm slices from top to the bottom of femoral condyles and top and bottom of the ankle joint. The CT protocol involves 1mm slices from top of the iliac crest to the isth-mus of the femur, 2mm slices from top to the bottom of femoral condyles and top and bot-tom of the ankle joint. The implant came with an operative plan to guide the neck osteotomy site, and with its own bone compacting or bone removing rasp according to the intramedullary cortical or cancellous bony architecture indicated by the 3D-CTplan. Results: In addition to a clinical review and oxford hip scoring, the patient had postoperative radiographs and were reviewed regularly at 6 weeks and 6 months. Conclusions: We concluded that this 3D-CT guided custom design femoral stem produces reliable proximal ‘fit and fill’ and primary stability with restoration of limb length discrepancy with complex proximal femoral deformity without the need of a corrective osteotomy.

Background: The aim of the study was to determine the contribution of 3-dimensional computer tomography (3D-CT) in the diagnosis of the articular fractures compared with X-rays and computer tomography (CT), and to establish if 3D-CT can confirm or modify the preoperative plans. Methods: We have conducted a retrospective study between 2020 - 2022, 46 patients with articular fractures. We have asked 5 independent surgeons to make comparative analyses of the information required for a correct diagnosis and preoperative planning, obtained from X-ray, CT and 3D-CT. Each time, the surgeon was requested to categorize the fracture using specific classification systems and articulate their recommended treatment approach. Results: The study showed us the importance of 3D-CT to achieve a correct diagnosis for the articular fractures by ana-lyzing the discrepancies between X-ray, CT and 3D-CT, with direct influence upon the presurgical plan. In 37% of the cases, fracture lines were not apparent on X-rays but were visible on CT scans. However, in more than 27% of these instances, specific bone injuries, like central articular depression or unique split-comminution patterns, were not evident on standard CT scans but could be identified using 3D-CT scans. Conclusions: To establish the correct diagnosis and the proper therapeutical plan for the articular fractures, it is necessary to make a complete ra-diological evaluation, which consists of X-ray, CT and 3D-CT.
