Abstract Background Tuberculosis (TB) lymphadenitis is the most common form of extra-pulmonary TB, and the treatment duration is six months. This non-inferiority based randomized clinical trial in South India evaluated the efficacy and safety of a four-month ofloxacin containing regimen in tuberculosis lymphadenitis (TBL) patients. Methods New, adult, HIV-negative, microbiologically and or histopathologically confirmed superficial lymph node TB patients were randomized to either four-month oflaxacin containing test regimen [ofloxacin (O), isoniazid (H), rifampicin (R), pyrazinamide (Z) -2RHZO daily/ 2RHO thrice-weekly] or a six-month thrice-weekly control regimen (2HRZ, ethambutol/4RH). The treatment was directly observed. Clinical progress was monitored monthly during and up to 12 months post-treatment, and thereafter every three months up to 24 months. The primary outcome was determined by response at the end of treatment and TB recurrence during the 24 months post-treatment. Results Of the 302 patients randomized, 298 (98.7%) were eligible for modified intention-to-treat (ITT) analysis and 294 (97%) for per-protocol (PP) analysis. The TB recurrence-free favourable response in the PP analysis was 94.0% (95% CI: 90.1–97.8) and 94.5% (95% CI: 90.8–98.2) in the test and control regimen respectively, while in the ITT analysis, it was 92.7% and 93.2%. The TB recurrence-free favourable response in the test regimen was non-inferior to the control regimen 0.5% (95% CI: -4.8-5.9) in the PP analysis based on the 6% non-inferiority margin. Treatment was modified for drug toxicity in two patients in the test regimen, while one patient had a paradoxical reaction. Conclusion The 4-month ofloxacin containing regimen was found to be non-inferior and as safe as the 6-month thrice-weekly control regimen.

Short fibers of Agave Americana (AA) was extracted from its plant leaf, was chemically treated with Ac2O, HCOOH, H2O2, KMnO4 and NaOH, and then characterized by Attenuated total reflectance-Fourier transform infrared spectroscopy (ATR-FTIR), thermo-gravimetric/differential thermo-gravimetric (TGA/DTG), and field emission-scanning electron microscopy (FE-SEM). PVA stabilized copper nanoparticles from chemical reduction method was characterized using field emission-scanning electron microscopy (FE-SEM), energy-dispersive X-ray spectroscopy (EDAX), powder X-ray diffraction (PXRD), Dynamic light scattering analysis (DLS), UV–visible absorption spectroscopy, Fourier Transform infrared (FT-IR) spectroscopy and Thermogravimetric analysis/differential thermogravimetry (TGA/DTG). Bio-composites (AA + Polyester Resin (PE) and hybrid nano bio-composites (AA + Polyester Resin (PE) + Cu) were prepared from the untreated and treated AA fibers and further characterized. The synergistic effect of chemical treatment on morphological (SEM), thermal (TGA/DTG), mechanical properties (flexural, tensile, impact and compressive strength) followed by % water absorption were examined. The average surface roughness values (Ra) of chemical treated fiber was identified to be in decreasing manner along with compression strength of biocomposite in the order of untreated (10.74 μm, 44.01 MPa) > NaOH (8.55 μm, 45.07 MPa) > HCOOH (3.49 μm, 24.10 MPa) Ac2O (3.24 μm, 22.10 MPa) > H2O2 (2.51um, 17.9 MPa) > KMnO4 (1.52 μm, 15.1 MPa) treated fibers. Bare soil mapping is inevitable for the monitoring of soil resources as it acts as a major indicator of urban development. Although remote sensing based indices are largely used for the delineation of bare soil, their effectiveness has always been a challenge due to overlapping spectral characteristics of the bare soil and built up areas. Recently, built up indices have been employed to increase the accuracy of bare soil mapping, which can induce discrepancies due to lesser sensitivity in smaller patches of bare soil areas in high density urban regions. In this context, Modified Normalized Difference Bare Soil Index (MNDBSI) is developed using a logical combination of index with additional constraint for the precise delineation of bare soil area using Sentinel 2 images. Qualitative and quantitative analyses adopted to evaluate the performance of the MNDBSI revealed an overall agreement value of above 95% and the improvement percentage of MNDBSI over the other indices compared in terms of Spectral Discrimination Index and Transformed Divergence varied from 8% to 166% and 15% to 97%, respectively across all study areas. The comparative analyses of results indicate that MNDBSI is an effective alternative to the existing indices for precise bare soil mapping which can be used as a promising indicator of urban expansion.

Abstract Background The management of pain presents a significant challenge in healthcare, particularly in cases where conventional therapies prove inadequate. In response to this need, this study aims to devise two innovative UV spectrophotometric techniques rooted in the principles of green analytical chemistry for the analysis of Aceclofenac (ACE), Paracetamol (PAR), and Tramadol (TRM) in both bulk and tablet forms. Results Utilizing advanced mathematical methodologies such as the double divisor ratio spectra method and area under the curve, the concentrations of these drugs were accurately determined. Validation of the developed methods adhered to the guidelines outlined by the International Council for Harmonisation in the Q2 (R1), revealing linear calibration curves for ACE (8–12 µg/mL), PAR (22.75–35.75 µg/mL), and TRM (2.62–4.12 µg/mL). Furthermore, statistical analyses employing Student’s t test and F test were conducted to ensure the robustness of the proposed method. The evaluation of environmental impact through green metric tools confirmed the eco-friendliness of the proposed methodologies. Conclusion The assessment performed utilizing green metric tools has substantiated the environmental sustainability of the proposed approach. Thus, this methodology offers accurate and reliable outcomes for the determination of three drugs, as indicated by the complete overlap observed in the zero-order spectra.

Introduction: Guggulsterone, derived from Commiphora mukul, is a potent hypolipidemic medication with low bioavailability and water insolubility. To address these challenges, the study aimed to formulate phytosomes and evaluate the efficacy of guggulsterone phytosomes (GPs) in reducing hyperlipidemia in rats on a high-fat diet. Methods: GPs were formulated by incorporating soya lecithin with a suitable solvent to enhance their efficacy against hyperlipidemia induced by a high-fat diet in rats. The optimized GPs were characterized, and in vitro drug release pattern was examined. The hypolipidemic effect of GPs (25 mg/kg body weight) was evaluated in Sprague Dawley rats over 28 days. Results: The GPs demonstrated favorable entrapment effectiveness with a particle size of 145.4 nm and a zeta potential of -17.8 mV. In terms of drug release, the GPs exhibited better stability and bioavailability, with a release of 92.07 ± 1.67% within 24 hours, compared to pure guggulsterone, which only released 28.07 ± 0.81%. Abstract Objectives Polo-like kinase 1 (Plk1) is an important mitotic protein. In particular, this protein is highly overexpressed in many types of tumors and has been identified as a potential biomarker for the treatment and diagnosis of tumors. Plk1 is composed of two domains, an N-terminal kinase domain and a C-terminal polo-box domain (PBD). Presently, inhibitors with improved selectivity and specificity for Plk1 are unavailable. Therefore, we aimed to develop an inhibitor targeting the C-terminal PBD present only in Plk1. Methods & results In this study, three derivatives targeting PBD for Plk1 were designed by protein–protein interactions, which showed high levels of selectivity and specificity for Plk1 PBD, and were evaluated to inhibit tumor cell proliferation through an apoptotic process during tumor cell division. The investigation of the in vitro and in vivo antitumor effects of these inhibitors demonstrated that one of the new small molecules, 1, is a promising anticancer agent. Conclusion Our findings can provide new insights for the design of novel Plk1 peptide inhibitors in the future.

Abstract Marine seaweeds are rich source of polysaccharides present in their cell wall and are cultivated and consumed in China, Japan, Korea, and South Asian countries. Brown seaweeds (Phaeophyta) are rich source of polysaccharides such as Laminarin and Fucoidan. In present study, both the laminarin and fucoidan were isolated was yielded higher in PP (Padina pavonica) (4.36%) and STM (Stoechospermum marginatum) (2.32%), respectively. The carbohydrate content in laminarin and fucoidan was 86.91% and 87.36%, whereas the sulphate content in fucoidan was 20.68%. Glucose and mannose were the major monosaccharide units in laminarin (PP), however, fucose, galactose, and xylose in fucoidan (STM). FT-IR down peaks represent the carbohydrate of laminarin and fucoidan except, for 1219 cm−1, and 843 cm−1, illustrating the sulphate groups of fucoidan. The molecular weight of laminarin was 3–5 kDa, and the same for fucoidan was 2–6 kDa, respectively. Both the Fucoidan and Laminarin showed null cytotoxicity on Vero cells. Contrastingly, the fucoidan possess cytotoxic activity on human liver cancer cells (HepG2) (IC50—24.4 ± 1.5 µg/mL). Simultaneously, laminarin also shown cytotoxicity on human colon cancer cells (HT-29) (IC50—57 ± 1.2 µg/mL). The AO/EB (Acriding Orange/Ethidium Bromide) assay significantly resulted in apoptosis and necrosis upon laminarin and fucoidan treatments, respectively. The DNA fragmentation results support necrotic cancer cell death. Therefore, laminarin and fucoidan from PP and STM were potential bioactive compounds for anticancer therapy.

Coronavirus Disease 2019 (COVID-19) is a viral disease caused by Severe Acute Respiratory Syndrome Coronavirus-2 (SARSCoV-2) that triggered a global pandemic back in 2019 and was primarily found to affect the respiratory system which continues its devastating effect across nations. Obesity is already a pre-existing modern-day complication, the root causes for which are largely attributed to the contemporary lifestyle. Many severe cases of COVID-19 in obese patients were reported globally which sparked many theories relating the two as some of their clinical manifestations are closely related. This review aims at providing the possible links between the two providing an idea based on which future research could be carried out since both these diseases tend to affect multiple organs, thus producing profound detrimental effects.

Introduction: Radiofrequency electromagnetic radiation (RF-EMR) from mobile phones was reported to cause neurological damage. Hispolon pyrazole (HP) and hispolon monomethyl ether pyrazole (HMEP) were tested for their RF-EMR protection in rats. Methods: Juvenile Wistar albino rats were exposed to the mobile phone generating 2400 MHz radiation with a maximum power output of 2 W/kg (Specific absorption rate 1.6 W/kg) for 90 days at a rate of 2 hours/day, treated with HP and HMEP at 20 and 40 mg/kg body weight. The elevated plus maze (EMT) test was used for anxiety and exploration evaluation, the forced swim test (FST) for depression, the Morris water maze test and Y-maze test for learning and memory. The oxidative stress markers like glutathione, superoxide dismutase (SOD), catalase (CAT), and malonaldehyde (MDA), and the neurotransmitters such as gamma-aminobutyric acid, glutamate, dopamine, and acetylcholinesterase along with histopathology in the cortex, striatum, and hippocampus were evaluated to establish the mechanism of the neuronal alterations of HP and HMEP against RF-EMR-induced damage. Results: In the current investigation, HP at a higher dose of 40 mg/kg and HMEP at both doses significantly reduced the oxidative stress generated by RF-EMR from mobile phones and altered neurobehavioral, neurotransmitter, and histological alterations. Conclusion: Based on the findings, HP and HMEP at a dose of 40 mg/kg are protective agents against long-term, continuous mobile phone use and can be regarded viable therapeutic agents.

Mycoplasma pneumoniae is recognized as the leading cause of community-acquired lower respiratory tract infection in children, accounting for a significant proportion of pediatric mortality. Macrolides are the first-line treatment for M. pneumoniae infections. However, the extensive use of macrolides in clinical practice resulted in the emergence of macrolide-resistant M. pneumoniae (MRMP), which has a negative impact on treatment outcomes. Hence, in the present study, MRMP was determined in hospitalized children with community-acquired pneumonia (CAP). Real-time PCR detected M. pneumoniae in 41 of 348 clinical samples. Sanger sequencing revealed that none of the isolates were associated with the A2063G or A2064G base mutation, which confers macrolide resistance, in domain V of the 23S rRNA gene. Although MRMP was not observed in children with CAP in our study, healthcare practitioners should be vigilant about the potential risk of MRMP infections.

Background: Stress and distress are common among Mental Health Professionals (MHP). MHPs avoid seeking professional help to address their stress, leading to increased vulnerability to stress and mental health conditions. Inadequately addressed stress directly impacts their service delivery and well-being. However, a serious dearth of literature from India has examined the extent of work-life balance (WLB) stress and psychological distress (PD) among MHP. Aim: This cross-sectional study aims to assess WLB and examine the relationship between perceived stress (PS), PD, burnout, and WLB among MHP in a tertiary care neuropsychiatry center. Methods: Using stratified random sampling, 101 MHP were evaluated using Pareek's work-life balance scale, Stamm's professional quality of life (ProQoL) scale, WHO's self-reporting questionnaire, and Cohen's perceived stress scale. Data were analyzed using appropriate parametric or non-parametric tests. Results: The overall mean score on WLB was 80.45 (SD = 10.21), the PS scale was 24.86 (SD = 6.87), the burnout score was 22.68 (SD = 6), the ST score was 21.18 (SD = 6.35), and PD score was 5.07 (SD = 5.23). The level of WLB score was average to below average among 64% of participants. There is a positive correlation between WLB and compassion satisfaction (CS) and a negative correlation between ST and PS. The study found that the nature of the job, CS, and PD shall predict WLB. Conclusion: Stress, burnout, ProQoL, and WLB of MHPs vary significantly by job, income, and staying with family, indicating that MHPs are equally vulnerable to stress and burnout and imbalanced WLB, which require appropriate interventions.

Background: COVID-19 has affected the whole world. Apart from the impact of the COVID-19 outbreak on physical health, there has been a significant impact on the mental health of the individuals. To address these mental health concerns, our institute had started a 24/7 national telephonic helpline to enable distressed individuals to reach out and seek help or assistance. Aims: In this article, the authors described the details of 126 tier-2 call backs and the types of psychiatric disorders that were catered to, between March 29, 2020 and May 31, 2020. Methodology: Descriptive analysis was done. Results: The concerns were characterized into five groups: exacerbation of preexisting mental and physical illness (30.9%); known case of mental or physical illness (19%); COVID related mental health concerns (22.2%); administrative or logistic issues (7.1%); new-onset mental health-related concerns or illnesses (15.9%). Majority of the calls (44.4%) were addressed by the second-tier mental health professionals (MHPs) (psychiatrists). Thirty-one percent were referred to the local medical or psychiatric services; 9.5% to the 3rd tier MHPs; 9% were connected to the treating psychiatrist or neurologist in the hospital; and 1.6% were referred to emergency services in the hospital. Conclusions: Using teleservices to address such a huge mental health crisis can be a way ahead to tackle this parallel menace brought about by COVID-19. However, the effectiveness of these helplines needs to be assessed and monitored at frequent intervals.

The aim of this study was to investigate the influence of excipients on retaining the particle size of methotrexate (MTX) loaded chitosan nanocarriers (CsNP) during lyophilization, which relates to the ability to enlarge the particle size and target specific areas. The nanocarriers were prepared using the ionic gelation technique with tripolyphosphate as a crosslinker. Three lyophilized formulations were used: nanosuspension without Lyoprotectant (NF), with mannitol (NFM), and with sucrose (NFS). The lyophilized powder intended for injection (PI) was examined to assess changes in particle size, product integrity, and comparative biodistribution studies to evaluate targeting ability. After lyophilization, NFS was excluded from in-vivo studies due to the product melt-back phenomenon. The particle size of the NF lyophile significantly increased from 176 nm to 261 nm. In contrast, NFM restricted the nanocarrier size to 194 nm and exhibited excellent cake properties. FTIR, XRD, and SEM analysis revealed the transformation of mannitol into a stable β, δ polymorphic form. Biodistribution studies showed that the nanocarriers significantly increased MTX accumulation in tumor tissue (NF = 2.04 ± 0.27; NFM = 2.73 ± 0.19) compared to the marketed PI (1.45 ± 0.25 μg), but this effect was highly dependent on the particle size. Incorporating mannitol yielded positive results in restricting particle size and favoring successful tumor targeting. This study demonstrates the potential of chitosan nanocarriers as promising candidates for targeted tumor drug delivery and cancer treatment.

Abstract Diatoms are photoautotrophic microalgae classified under class Bacillariophyceae, engulfed by hard silicate frustules, which give mechanical support and protection from bacterial infections. They exude polysaccharides extracellularly that help them with their gliding motion (locomotion). However, the bioactivity of such compounds was least explored from freshwater diatoms. In the present study, a single species of pennate diatom identified as Nitzschia palea was isolated and molecularly characterized by 18S rRNA smaller subunit gene (partial) sequencing and submitted to GenBank NCBI and accession number retrieved as ON360983. Based on logarithmic growth curve analysis, the exponential phase was obtained from 3rd to 4th day of diatom culture. The exopolysaccharide was extracted by the hot-water extraction method, and characterized by FT-IR. The total yield of exopolysaccharide from Nitzschia palea was estimated as 1.56 mg in 100 mL of culture after 7 days of incubation. The estimated carbohydrate content was 51.35 µg/100 µL. The monosaccharide constituents were determined by acid hydrolysis of exopolysaccharide, silylation (derivatization), followed by GC–MS analysis and tabulated. The extracted exopolysaccharide was evaluated for its anti-cancer potential against the Human Adenocarcinoma lung cancer cell line (A549) and the estimated IC50 value was 62.64 µg/mL. Acridine orange staining assay and DNA fragmentation assay also confirmed the apoptotic activity of exopolysaccharide derived from the diatom Nitzschia palea.

Abstract A virus enters a living organism and recruits host metabolism to reproduce its own genome and proteins. The viral infections are intricate and cannot be completely removed through existing antiviral drugs. For example, the herpes, influenza, hepatitis and human immunodeficiency viruses are a few dreadful ones amongst them. Significant studies are needed to understand the viral entry and their growth in host cells to design effective antivirals. This review emphasizes the range of therapeutical antiviral drugs, inhibitors along with vaccines to fight against viral pathogens, especially for combating COVID-19. Moreover, we have provided the basic and in depth information about viral targets, drugs availability, their mechanisms of action, method of prevention of viral diseases and highlighted the significances of anticoagulants, convalescent plasma for COVID-19 treatment, scientific details of airborne transmission, characteristics of antiviral drug delivery using nanoparticles/carriers, nanoemulsions, nanogels, metal based nanoparticles, alike the future nanosystems through nanobubbles, nanofibers, nanodiamonds, nanotraps, nanorobots and eventually, the therapeutic applications of micro- and nanoparticulates, current status for clinical development against COVID-19 together with environmental implications of antivirals, gene therapy etc., which may be useful for repurposing and designing of novel antiviral drugs against various dreadful diseases, especially the SARS-CoV-2 and other associated variants.

The COVID-19 pandemic made all organizations and enterprises work on cloud platforms from home, which greatly facilitates cyberattacks. Employees who work remotely and use cloud-based platforms are chosen as targets for cyberattacks. For that reason, cyber security is a more concerning issue and is now incorporated into almost every smart gadget and has become a prerequisite in every software product and service. There are various mitigations for external cyber security attacks, but hardly any for insider security threats, as they are difficult to detect and mitigate. Thus, insider cyber security threat detection has become a serious concern in recent years. Hence, this paper proposes an unsupervised deep learning approach that employs an artificial neural network (ANN)-based autoencoder to detect anomalies in an insider cyber security attack scenario. The proposed approach analyzes the behavior of the patterns of users and machines for anomalies and sends an alert based on a set security threshold. The threshold value set for security detection is calculated based on reconstruction errors that are obtained through testing the normal data. When the proposed model reconstructs the user behavior without generating sufficient reconstruction errors, i.e., no more than the threshold, the user is flagged as normal; otherwise, it is flagged as a security intruder. The proposed approach performed well, with an accuracy of 94.3% for security threat detection, a false positive rate of 11.1%, and a precision of 89.1%. From the obtained experimental results, it was found that the proposed method for insider security threat detection outperforms the existing methods in terms of performance reliability, due to implementation of ANN-based autoencoder which uses a larger number of features in the process of security threat detection.

Acute myeloid leukemia (AML) with the high-risk variant inv(3)/t(3;3) or t(3;3)(q21;26.2) is rarely seen in the pediatric and young adult population. It is associated with poor outcomes with ineffective therapeutic options. Here, we present a case of an 18-year-old female with treatment refractory inv(3) AML in whom remission was unable to be obtained. Better treatment options are needed given the increased resistance to traditional therapy this subtype portrays. Here, we review the literature on pediatric and young adult inv(3) AML along with newer therapeutic options.

Introduction: In most developing countries, antimicrobial resistance is a public threat, and insufficient knowledge of antibiotics among the public adds to it. This study aimed to determine the efficacy of the clinical pharmacist's pamphlets and video-based educational tools to address the public's knowledge, attitude, and practice gaps on antibiotic use. Methods: This was a pre-and post-intervention cohort study of the adult population in South India who can read and understand Tamil or English. The study participants were designated into two groups-usual care group (pamphlet-based) and interventional group (video-based), with a pre-intervention assessment using a self-administered questionnaire, followed by an educational intervention by a clinical pharmacist, and finally, a post-intervention assessment after three months. Result: Of the162 respondents, the majority were female (58%), in the age group of 26–35 (30%), with intermediate education (43%) from a middle-income family. The mean score calculated for each domain among the two groups: knowledge score (Pamphlet based-Pre: 2.26 ± 1.13; post: 3.23 ± 1.02), (Video based-Pre: 2.22 ± 1.45; post: 3.95 ± 0.89), Attitude score (Pamphlet based-Pre: 2.53 ± 1.96; post: 3.23 ± 0.9), (Video based-Pre: 2.39 ± 1.81; post: 4.21 ± 1.35), Practice score (Pamphlet based-Pre: 2.19 ± 1.02; post: 4.46 ± 1.81). Pediatric central nervous system (CNS) tumors are the most common solid tumor in children, with the majority being glial in origin. These tumors are classified by the World Health Organization (WHO) as either being low grade (WHO grade 1 and 2) or high grade (WHO grade 3 and 4). Our knowledge of the molecular landscape of pediatric brain tumors has advanced over the last decade, which has led to newer categorizations along with an expansion of therapeutic targets and options. In this review, we will give an overview of common CNS tumors seen in children along with a focus on treatment options and future considerations.

Diffuse midline glioma (DMG), H3 K27-altered are highly aggressive, incurable central nervous system (CNS) tumors. The current standard palliative treatment is radiotherapy, with most children succumbing to the disease in less than one year from the time of diagnosis. Over the past decade, there have been significant advancements in our understanding of these heterogeneous tumors at the molecular level. As a result, most of the newer clinical trials offered utilize more targeted approaches with information derived from the tumor biopsy. In this systematic review, we used individual participant data from seven recent clinical trials published over the past five years that met our inclusion and exclusion criteria to analyze factors that influence overall survival (OS). We found that the most prominent genetic alterations H3.3 (H3F3A) and TP53 were associated with worse OS and that ACVR had a protective effect. In addition, re-irradiation was the only statistically significant treatment modality that showed any survival benefit. Our findings highlight some important characteristics of DMG, H3 K27-altered and their effects on OS along with the importance of continuing to review clinical trial data to improve our therapies for these fatal tumors.

The current investigation was conducted to maximise the production of the natural anticancer drug from the microbe isolated from the marine soil sample of the Coromandel Coast of the Bay of Bengal region of India. Yellow to red colour pigmented microbes separated by crowd plate method. Bacteria are producing strong colour product subjected to future study. The isolated strains were detected based on biochemical, morphological, and genetic characteristics. Pigment formation was found to be influenced strongly by conditions of the environment. The water-insoluble pigment extracted by acidified methanol and showed maximum absorbance at 535nm. A statistical screening procedure was adopted to select the optimum condition to produce the pigment. The carbon, nitrogen, medium pH, growth condition temperature and revolution of agitation were screened using the response surface methodology statistical model. Dengue is an arboviral infection endemic in tropical countries. Neurological sequelae to dengue infection are not uncommon, and psychiatric manifestations are increasingly reported. This narrative review aims to present the varied manifestations, postulated mechanisms, and the available treatment options for psychiatric morbidity associated with dengue. The evidence available from eight observational studies is summarized in this review. Depression and anxiety are noted to be prevalent during both the acute and convalescent stages of the infection. The presence of encephalopathy and other neurological conditions is not a prerequisite for developing psychiatric disorders. However, treatment options to manage such psychiatric manifestations were not specified in the observational studies. Anecdotal evidence from case reports is outlined. Special attention is paid to the role of epigenetic modifications following dengue infections and the role of histone deacetylase inhibitors in the management. DNA methylation inhibitors such as valproic acid play a significant role in reversing stress-, viral-, or drug-induced epigenetic modifications.
