Abstract

Introduction: Molecular profiling of NSCLC is essential for optimising treatment decisions, but often incomplete. We assessed the efficacy of protocolised molecular profiling in the current standard-of-care (SoC) in a prospective observational study in the Netherlands and measured the effect of providing standardised diagnostic procedures. We also explored the potential of plasma-based molecular profiling in the primary diagnostic setting. Methods: This multi-centre prospective study was designed to explore the performance of current clinical practice during the run-in phase using local SoC tissue profiling procedures. The subsequent phase was designed to investigate the extent to which comprehensive molecular profiling (CMP) can be maximized by protocolising tumour profiling. Successful molecular profiling was defined as completion of at least EGFR and ALK testing. Additionally, PD-L1 tumour proportions scores were explored. Lastly, the additional value of centralised plasma-based testing for EGFR and KRAS mutations using droplet digital PCR was evaluated. Results: Total accrual was 878 patients, 22.0% had squamous cell carcinoma and 78.0% had non-squamous NSCLC. Stage I-III was seen in 54.0%, stage IV in 46.0%. Profiling of EGFR and ALK was performed in 69.9% of 136 patients included in the run-in phase, significantly more than real-world data estimates of 55% (p<0.001). Protocolised molecular profiling increased the rate to 77.0% (p = 0.049). EGFR and ALK profiling rates increased from 77.9% to 82.1% in non-squamous NSCLC and from 43.8% to 57.5% in squamous NSCLC. Plasma-based testing was feasible in 98.4% and identified oncogenic driver mutations in 7.1% of patients for whom tissue profiling was unfeasible. Conclusion: This study shows a high success rate of tissue-based molecular profiling that was significantly improved by a protocolised approach. Tissue-based profiling remains unfeasible for a substantial proportion of patients. Combined analysis of tumour tissue and circulating tumour DNA is a promising approach to allow adequate molecular profiling of more patients. [ABSTRACT FROM AUTHOR]

Item does not contain fulltext, INTRODUCTION: Chemoradiotherapy (CRT) is the standard of care in inoperable non-small-cell lung cancer (NSCLC) patients, favoring concurrent (cCRT) over sequential CRT (seqCRT), with adjuvant immunotherapy in responders. Elderly and frail NSCLC patients have generally been excluded from trials in the past. In elderly patients however, the higher treatment related morbidity of cCRT, may outweigh the possible lower tumor control of seqCRT. For elderly patients with locally advanced NSCLC real-world data is essential to be able to balance treatment toxicity and treatment outcome. The aim of this study is to analyze acute toxicity and 3-month mortality of curative chemoradiation (CRT) in patients with stage III NSCLC and to analyze whether cCRT for elderly stage III NSCLC patients is safe. METHODS: The Dutch Lung Cancer Audit-Radiotherapy (DLCA-R) is a national lung cancer audit that started in 2013 for patients treated with curative intent radiotherapy. All Dutch patients treated for stage III NSCLC between 2015 and 2018 with seqCRT or cCRT for (primary or recurrent) stage III lung cancer are included in this population-based study. Information was collected on patient, tumor- and treatment characteristics and the incidence and severity of acute non-hematological toxicity (CTCAE-4 version 4.03) and mortality within 3 months after the end of radiotherapy. To evaluate the association between prognostic factors and outcome (acute toxicity and mortality within 3 months), an univariable and multivariable analysis was performed. The definition of cCRT was:radiotherapy started within 30 days after the start of chemotherapy. RESULTS: Out of all 20 Dutch departments of radiation oncology, 19 centers participated in the registry. A total of 2942 NSCLC stage III patients were treated with CRT. Abstract

Objectives: Data on national patterns of care for patients with superior sulcus tumors (SST) is currently lacking. We investigated the distribution of surgical care and outcome for patients with SST in the Netherlands. Material and methods: Data was retrieved from the Dutch Lung Cancer Audit for Surgery (DLCA-S) for all patients undergoing resection for clinical stage IIB-IV SST from 2012 to 2019. Because DLCA-S is not linked to survival data, survival for a separate cohort (2015–2017) was obtained from the Netherlands Cancer Registry (NCR). Results: In the study period, 181 patients had SST surgery, representing 1.03% (181/17488) of all lung cancer pulmonary resections. For 2015–2017, the SST resection rate was 14.4% (79/549), and patients with stage IIB/III SST treated with trimodality had a 3-year overall survival of 67.4%. 63.5% of patients were male, and median age was 60 years. Almost 3/4 of tumors were right sided. Surgery was performed in 20 hospitals, with average number of annual resections ranging from ≤ 1 (n = 17) to 9 (n = 1). 39.8% of resections were performed in 1 center and 63.5% in the 3 most active centers. 12.7% of resections were extended (e.g. vertebral resection). 85.1% of resections were complete (R0). Morbidity and 30-day mortality were 51.4% and 3.3% respectively. Despite treating patients with a higher ECOG performance score and more extended resections, the highest volume center had rates of morbidity/mortality, and length of hospital stay that were comparable to those of the medium volume (n = 2) and low-volume centers (n = 1). Conclusion: In the Netherlands, surgery for SST accounts for about 1% of all lung cancer pulmonary resections, the number of SST resections/hospital/year varies widely, with most centers performing an average of ≤ 1/year. Morbidity and mortality are acceptable and survival compares favourably with the literature. Although further centralisation is possible, it is unknown whether this will improve outcomes.

Abstract

Key life-course transitions often instigate, or even require, at least one residential move. Leaving the parental home, coupling or separating are clear examples. Life-course transitions therefore also have clear and inherent spatial dimensions, but the question how life course plays out at small spatial scales is rarely studied in a structured way. Instead, we often have to make-do with crude generalizations such as seemingly stable urban–suburban dichotomies. In this chapter, we challenge this perspective by providing a new framework to analyze contemporary changes in the regional dynamics of life course. Housing plays a key role in this: housing market conditions structure the degree to which individuals are able to make transitions. Contemporary housing transformations, taking place in most Western countries, have an unequal impact on different generations. In many successful cities housing affordability and accessibility are decreasing. This leaves a mark on life-course trajectories, as the trade-offs involved in making certain transitions become sharper – in terms of housing and space. Housing transformations therefore contribute to a changing geography of life course within urban regions. We illustrate our arguments through a case study of life-course dynamics in the Amsterdam metropolitan region in the wake of the 2008 Global Financial Crisis.

Het Nederlandse woonbeleid heeft lang de ambitie gehad om gemengde woonmilieus te creëren. Inmiddels is deze ambitie afgezwakt. Hierdoor kunnen woonvoorkeuren leiden tot grotere buurtverschillen. Meer keuzeruimte op de woningmarkt stelt vermogende huishoudens in staat hun voorkeuren te realiseren. Maar bij gebrek aan sturing ligt ook het gevaar van ongewenste bevolkingsconcentraties op de loer. Om een balans te vinden tussen deze twee extremen, geven we in deze bijdrage een aanzet tot het denken over de regio als garderobe van uiteenlopende woonmilieus.

De regio als garderobe presenteert een nieuwe visie op stedelijke vernieuwing. De basis daarvoor is een omvangrijk onderzoek over een periode van tien jaar. Daarin zijn voor drie miljoen huishoudens alle individuele huishoudtransities onderzocht en in verband gebracht met hun verhuisgedrag in de metropoolregio’s van Amsterdam, Rotterdam/Den Haag, Utrecht en Eindhoven. Hoe passen huishoudens hun woonsituatie aan op belangrijke veranderingen in de levensloop, zoals het krijgen van kinderen, een echtscheiding, of het verliezen van een baan? Naar wat voor buurten verhuizen zij? Individuele keuzes kunnen zorgen voor sterke ruimtelijke verschillen. Dit hoeft echter geen probleem te zijn. In De regio als garderobe pleiten we voor meer individuele keuzevrijheid, maar dan binnen een context van betaalbare en leefbare woonmogelijkheden voor alle typen huishoudens. Per saldo zouden meer mensen hun woonwensen moeten kunnen realiseren. Verandering, verschil en dynamiek moeten niet als probleem bestempeld worden — zoals nu vaak gebeurt — maar moeten worden gekoesterd.

Abstract

BACKGROUND AND PURPOSE: The aim was to investigate whether the use of metformin during concurrent chemoradiotherapy (cCRT) for locally advanced non-small cell lung cancer (NSCLC) improved treatment outcome. MATERIAL AND METHODS: A total of 682 patients were included in this retrospective cohort study (59 metformin users, 623 control patients). All received cCRT in one of three participating radiation oncology departments in the Netherlands between January 2008 and January 2013. Primary endpoint was locoregional recurrence free survival (LRFS), secondary endpoints were overall survival (OS), progression-free survival (PFS) and distant metastasis free survival (DMFS). RESULTS: No significant differences in LRFS or OS were found. Metformin use was associated with an improved DMFS (74% versus 53% at 2years; p=0.01) and PFS (58% versus 37% at 2years and a median PFS of 41months versus 15months; p=0.01). In a multivariate cox-regression analysis, the use of metformin was a statistically significant independent variable for DMFS and PFS (p=0.02 and 0.03). CONCLUSIONS: Metformin use during cCRT is associated with an improved DMFS and PFS for locally advanced NSCLC patients, suggesting that metformin may be a valuable treatment addition in these patients. Evidently, our results merit to be verified in a prospective trial.
