Abstract

O presente trabalho teve por objetivo analisar os resultados do Programa Tuia Cheia (PTC), buscando compreender o papel das políticas públicas de segurança alimentar e nutricional, suas contribuições, avanços e retrocessos em municípios da região norte do Brasil. Foi realizado estudo de caso do PTC no município de Couto Magalhães, situado na região Norte do Brasil, ao Nordeste do estado do Tocantins, que faz parte do contexto de municípios afetados pela insegurança alimentar e nutricional. Utilizou-se das abordagens qualitativa e quantitativa, com a combinação de análise documental, observação participante, entrevistas informais e grupo focal. Os resultados da pesquisa demonstram que o PTC contribuiu para a diminuição da insegurança alimentar e nutricional no município de Couto Magalhães. O programa atendeu às escolas públicas municipais por meio do Programa Nacional de Alimentação Escolar (PNAE) e demandas da comunidade local. No decorrer da pesquisa se observou a importância do PTC para a produção de alimentos e garantia de segurança alimentar. Portanto, no município em pauta, o PTC está em consonância com a legislação vigente e atende à política pública de segurança alimentar e nutricional. Por fim, é importante considerar o protagonismo dos agricultores para a continuidade do programa, já que tem como público prioritário os assentados da reforma agrária da região. This study aimed to analyze the results of the Tuia Cheia Program (TCP), seeking to understand the role of public policies on food and nutrition security, their contributions, advances and setbacks in municipalities in the northern region of Brazil. A case study of the TCP was carried out in the municipality of Couto Magalhães, located in the northern region of Brazil, in the northeast of the state of Tocantins, which is part of the context of municipalities affected by food and nutritional insecurity. A qualitative and quantitative approach was used, with the combination of document analysis, participant observation, informal interviews and focus group.The results of the research demonstrate that the TCP contributed to the reduction of food and nutritional insecurity in the municipality of Couto Magalhães. The program served municipal public schools through the National School Feeding Program (NSFP) and demands from the local community. During the research, the importance of the TCP for food production and food security guarantee was observed. Therefore, the TCP in the city in question is in line with current legislation and complies with public policy on food and nutritional security. Finally, it is important to consider the role of farmers for the continuity of the program, as it has a well defined target audience: family farmers settled in the region.

Item does not contain fulltext In the AML 8A study patients were treated with remission-induction therapy followed by one consolidation course. Patients in complete remission (CR) were randomised between autologous bone marrow transplantation (ABMT) and a second intensive consolidation course, except for those with a histocompatible sibling donor, who received allogeneic bone marrow transplantation (alloBMT). This analysis was performed to determine whether centres which only performed induction and consolidation therapy, achieved similar results as centres who also performed transplantation. 542/676 (80%) from transplantation centres and 150/194 (77%) from referring centres achieved CR, with an early death rate of 5% and 11%, respectively (P = 0.01). 66% of patients with a donor from transplantation centres received alloBMT in first CR compared with 57% from referring centres (P = 0.2). Transplantation centres randomised 64% of patients without a donor, referring centres 47% (P = 0.04). The full protocol treatment was completed by 275/542 (51%) and 61/150 (41%) patients, respectively (P = 0.04). The overall survival rate at 6 years from diagnosis was 34% and 36%, respectively (P = 0.9). In conclusion, the type of centre did not appear to have an influence on overall survival. The feasibility of the study was acceptable for both types of centres. The referring centres applied more selection for transplantation. Despite a more intensive second-line treatment at transplantation centres, the overall outcome remained similar to that of referring centres.

Abstract

The possible prognostic value of the position of the breakpoint within the M-BCR in patients with Phl+ CML is still being debated. We analyzed the DNA rearrangements and the transcript types of 244 patients and tried to correlate the data obtained with prognostic features, defined according to Sokal's risk index, and with chronic phase and/or survival duration. The exact location of the breakpoint, either 5' or 3' to the Hind III restriction site within the M-BCR was identified. Moreover, the exact M-BCR subregion was also identified. As a whole, 150 pts were rearranged in the 5' part and 94 in the 3' part of the M-BCR. No correlation was observed between the site of rearrangement on the one hand and the Sokal's prognostic index and survival, on the other. Transcript analysis was performed in 130 patients; 59 carried an a2b2 and 69 an a2b3 pattern. Two patients carried both transcripts. Abstract

To evaluate the efficacy and safety of itraconazole oral solution for preventing fungal infections, a randomized, placebo-controlled, double-blind, multicenter trial was conducted: 405 neutropenic patients with hematologic malignancies were randomly assigned to receive either itraconazole, 2.5 mg/kg every 12 hours (201 patients), or placebo (204 patients). Proven and suspected deep fungal infection occurred in 24% of itraconazole recipients and in 33% of placebo recipients, a difference of 9 percentage points (95% confidence interval [CI], 0.6% to 22.5%; P = .035). Fungemia due to Candida species was documented in 0.5% of itraconazole recipients and in 4% of placebo recipients, a difference of 3.5 percentage points (95% CI, 0.5% to 6%; P = .01). Deaths due to candidemia occurred in none of the itraconazole recipients compared with 4 placebo recipients, a difference of 2 percentage points (95% CI, 0.05% to 4%; P = .06). Aspergillus infection was documented in four itraconazole recipients (one death) and one placebo recipient (one death). Side effects causing drug interruption occurred in 18% of itraconazole recipients and 13% of placebo recipients. Itraconazole oral solution was well-tolerated and effectively prevented proven and suspected deep fungal infection as well as systemic infection and death due to Candida species.

Abstract

This is a review of current treatment for hairy cell leukemia (HCL). Data for this analysis were obtained from the Italian HCL Registry, as well as from other published series. We have given space to the impact of interferon and pentostatin on the management of this disease. Other issues are also discussed, such as the relevance of achieving a complete remission with respect to overall and relapse-free survival. We include a final section on recommendations which may prove useful in designing an appropriate therapeutic strategy.
