8. Chemokine and chemokine receptor gene expression in human PBMCs in the early renal post-transplant period

Abstract

The aim of this work was to determine whether sequential changes in chemokine and/or chemokine receptor gene expression in the early post-transplant period of human renal allografts can be detected in PBMCs, and whether any such changes are predictive of clinical events. Blood samples from 106 renal transplant recipients and 29 donor nephrectomy patients were taken pre-operatively and then daily for 14 days. Within the two week study period 22 patients had biopsy proven acute rejection. From each blood sample the PBMCs were separated, their RNA extracted and a fixed quantity reverse transcribed to cDNA. Using real-time quantitative PCR (5' nuclease assay by TaqMan® methodology), the gene expression levels for the chemokines CCL3, CCL4, CCL5, CXCL10 and their receptors CCR1, CCR5 and CXCR3 were measured. The possible endogenous control genes tested were GAPDH, MLN51, YWHAZ, EF-1α and UbcH5B. Changes in chemokine and chemokine receptor gene expression by sequential monitoring in PBMCs were detected in the early post-transplant period. Furthermore, different expression patterns between rejector and non-rejector groups for some genes were demonstrated and some of these changes correlated with clinical events. In particular, CCR1 and CXCL10 showed increased expression prior to rejection and returned back to baseline levels with anti-rejection therapy. The search for a suitable endogenous control gene for use in the gene expression model used in this work was unsuccessfully, with significant changes in expression of all five genes tested at some time point post-transplantation. This work has demonstrated that changes in chemokine and chemokine receptor gene expression can be detected in PBMCs in the early post-transplant period, and in particular CXCL10 and CCR1 showed changes that correlate with rejection, and therefore may have potential use in immunomonitoring and as predictive factors of rejection prior to its clinical manifestation.

16. Race Differences in Mental Health Service Access in a Secure Male Juvenile Justice Facility

Abstract

This study examined whether African American and Caucasian male youths had similar rates of referral to mental health services in a juvenile justice secure facility when controlling for differences obtained in the initial screening and assessment process. Data from the Massachusetts Youth Screening Instrument-2 (MAYSI-2), Initial Health Care Screening and Youth Level of Services/Case Management Inventory were analyzed from 937 male youths (12-18 years old) admitted to a secure care facility. There were few differences between African American and Caucasian youths on MAYSI-2 scales and no difference on the YLS total score. However, Caucasian youth reported higher frequencies of mental health treatment history than African American youth. Even when controlling for differences in MAYSI-2 elevations and history of receiving mental health treatment, Caucasian youths were provided access to mental health services at a greater rate than African American youth. This study indicates that race is a potential factor in mental health service access for youth in a secure care setting. (Contains 4 tables.)

Abstract

Urea-plasma species, the most commonly isolated microorganisms in women with chorioamnionitis, are associated with preterm delivery. Chorioamnionitis increases the risk and severity of bronchopulmonary dysplasia and persistent pulmonary hypertension in newborns. It is not known whether the timing of exposure to inflammation in utero is an important contributor to the pathogenesis of bronchopulmonary dysplasia. We hypothesized that chronic inflammation would alter the pulmonary air space and vascular development after 70 days of exposure to infection. Pregnant ewes were given intra-amniotic injection of Ureaplasma parvum serovars 3 or 6 at low (2 x [10.sup.4] cfu) or high doses (2 x [10.sup.7] cfu) or media (controls) at 55 days gestational age. Fetuses were delivered at 125 days (term = 150 days). U. parvum was grown from the lungs of all exposed fetuses, and neutrophils and monocytes were increased in the air spaces. Lung mRNA expression of IL-1[beta] and IL-8, but not IL-6, was modestly increased in U. parvum-exposed fetuses. U. parvum exposure increased suffactant and improved lung gas volumes. The changes in lung inflammation and maturation were independent of serovar or dose. Exposure to U. parvum did not change multiple indices of air space or vascular development. Parenchymal elastin and collagen content were similar between groups. Expression of several endothelial proteins and pulmonary resistance arteriolar media thickness were also not different between groups. 41. Flow and Heat Transfer in a Ribbed Converging-Diverging U-Duct Under Rotating and Non-Rotating Conditions

Abstract

A numerical study based on steady Reynolds-averaged-Navier–Stokes (RANS) with the shear stress transport (SST) turbulence model was performed to investigate the flow and heat transfer in a ribbed U-duct with a rectangular cross section that converges linearly in the radially outward direction and diverges linearly in the radially inward direction under rotating and nonrotating conditions. Parameters examined include rotation number (Roi = 0, 0.0219, 0.0336, 0.0731), Reynolds number (Rei = 46,000, 100,000, and 154,000), and the duct's taper angle (α = 0 deg and 1.41 deg) under conditions relevant to gas turbines used for electric power generation. Results obtained show that increasing the taper angle from 0 deg to 1.41 deg, which appears negligibly small, significantly increases both the friction coefficient and the Nusselt number whether there is rotation or not. With rotation at Roi = 0.0336 and Rei = 100,000, the maximum increase in the average friction coefficient and Nusselt number was found to be 41.7% and 36.6%, respectively. Without rotation at Rei = 46,000, those are 11.5% and 14.7%, respectively.

Abstract

Abstract Background In the United Kingdom (UK), there is an extensive market for the class 'A' drug heroin. Many heroin users spend time in prison. People addicted to heroin often require prescribed medication when attempting to cease their drug use. The most commonly used detoxification agents in UK prisons are buprenorphine, dihydrocodeine and methadone. However, national guidelines do not state a detoxification drug of choice. Indeed, there is a paucity of research evaluating the most effective treatment for opiate detoxification in prisons. This study seeks to address the paucity by evaluating routinely used interventions amongst drug using prisoners within UK prisons. Methods/Design The Leeds Evaluation of Efficacy of Detoxification Study (LEEDS) Prisons Pilot Study will use randomised controlled trial methodology to compare the open use of buprenorphine and dihydrocodeine for opiate detoxification, given in the context of routine care, within HMP Leeds. Prisoners who are eligible and give informed consent will be entered into the trial. The primary outcome measure will be abstinence status at five days post detoxification, as determined by a urine test. Secondary outcomes during the detoxification and then at one, three and six months post detoxification will be recorded.
