Abstract Background Around 50% of gastric cancers are diagnosed at an advanced stage. Several chemotherapy regimens are now internationally validated. Few data are available on the routine daily management of advanced gastric or gastroesophageal junction cancers. We aimed to describe chemotherapy practices, tolerance, and efficacy overall survival (OS) and Progression free survival (PFS) in a prospective French cohort. Methods Patients starting palliative chemotherapy were prospectively enrolled in 49 French centres. The primary objective was to report and describe patients' characteristics and treatment strategies. Secondary objectives were OS, PFS, objective response rate, adverse events rate, performance status deterioration during the chemotherapy. Results A total of 182 patients were included; 179 were analysed. Most patients received platinium‐based chemotherapy as the first treatment and FOLFIRI as second; 62.0% of patients received a second line, and 32.4% a third line. More than two thirds of Her2‐positive patients were first treated with trastuzumab. The FOLFIRI regimen was the most frequently used second‐line therapy. Median OS was 13.3 months, similar whatever the chemotherapy or combinations used in the first line. Este artigo realiza um estudo polifônico e argumentativo de dois discursos publicados em dois portais online de notícias. Nosso objetivo é discutir o modo como os locutores constroem sentidos distintos em cada um dos textos a partir do mesmo fato: a divulgação, pela Organização das Nações Unidas, do relatório contendo os dados do Índice de Desenvolvimento Humano. Furthermore, many patients with cardiovascular disease and relatively normal levels of LDL-C have increased levels of low-density lipoprotein particles (LDL-P). The purpose of this case report is to illustrate the importance of LDL-P in the surprising presentation of vascular disease in a woman who had a high concentration of high-density lipoprotein cholesterol (HDL-C). El objetivo de este trabajo se centró en verificar el nivel de conocimiento de un grupo de adultos mayores con respecto al Síndrome de Inmunodeficiencia Adquirida (VIH/SIDA), para ello se utilizó el método de investigación cuantitativa-exploratoria. Los datos se recolectaron entre marzo y junio de 2008 a través de un cuestionario semiestructurado compuesto por preguntas abiertas y cerradas, aplicado a 23 adultos mayores, miembros del Grupo de la Tercera Edad Mexe Coração del municipio de Santa Maria, RS, Brasil. El rango etario de los participantes varió desde 58 a 96 años. El análisis de los datos revela la existencia de vacíos en el conocimiento de los adultos mayores sobre el concepto VIH/SIDA, grupos de riesgo, formas de transmisión, vulnerabilidad y otros factores que contribuyen al aumento de esta enfermedad durante este rango etario.This study aimed to verify the level of knowledge of agroup of elderly in relation to Acquired Inmunodeficiency Syndrome (HIV/AIDS). The method used was an exploratory quantitative research. Data were collected in 2008 between March and June, through a semistructured questionnaire with open and closed questions, applied in 23 elderly, members of the Group of Elderly Mexe Coração in Santa Maria, RS, Brazil. The ages of participants ranged from 58 to 96 years. Data analysis revealed the existence of gaps in knowledge of the elderly about the concept of HIV/AIDS, risk groups, ways of transmission and vulnerability and other factors that contribute to the rise of the disease in this age group.Este trabalho teve por objetivo verificar o nível de conhecimento de um grupo de idosos em relação ao Síndromeda Imunodeficiência Adquirida (HIV/AIDS). O método utilizado foi uma pesquisa quantitativo-exploratória. Os dados foram coletados entre março e junho de 2008 por meio de um questionário semi-estruturado com questões abertas e fechadas, aplicado a 23 idosos, integrantes do Grupo de Terceira Idade Mexe Coração do município de Santa Maria, RS, Brasil. A faixa etária dos participantes variou de 58 a 96 anos. A análise dos dados revela a existência de lacunas no conhecimento dos idosos em relação conceito de HIV/AIDS, grupos de risco, formas de transmissão, vulnerabilidade e outros fatores que contribuem para a ascensão da doença nesta faixa etária.

In 2010, under the guidance of the DGAI (German Society of Anaesthesiology and Intensive Care Medicine) and DIVI (German Interdisciplinary Association for Intensive Care and Emergency Medicine), twelve German medical societies published the “Evidence- and Consensus-based Guidelines on the Management of Analgesia, Sedation and Delirium in Intensive Care”. Since then, several new studies and publications have considerably increased the body of evidence, including the new recommendations from the American College of Critical Care Medicine (ACCM) in conjunction with Society of Critical Care Medicine (SCCM) and American Society of Health-System Pharmacists (ASHP) from 2013. For this update, a major restructuring and extension of the guidelines were needed in order to cover new aspects of treatment, such as sleep and anxiety management. The literature was systematically searched and evaluated using the criteria of the Oxford Center of Evidence Based Medicine. The body of evidence used to formulate these recommendations was reviewed and approved by representatives of 17 national societies. Three grades of recommendation were used as follows: Grade “A” (strong recommendation), Grade “B” (recommendation) and Grade “0” (open recommendation). The result is a comprehensive, interdisciplinary, evidence and consensus-based set of level 3 guidelines. This publication was designed for all ICU professionals, and takes into account all critically ill patient populations. It represents a guide to symptom-oriented prevention, diagnosis, and treatment of delirium, anxiety, stress, and protocol-based analgesia, sedation, and sleep-management in intensive care medicine.

A análise da alocação da biomassa e a correlação entre variáveis morfofisiológicas permitem maior entendimento do estabelecimento, produção e persistência de espécies perenes, em trabalhos de melhoramento genético, manejo e ecologia de plantas forrageiras. Este trabalho teve o objetivo de analisar a alocação de biomassa em leguminosas forrageiras (Adesmia latifolia, A. punctata, A. tristis, Lotus corniculatus, L. uliginosus). O experimento foi conduzido em casa de vegetação durante 210 dias (4000 graus-dia); as plantas foram cultivadas em caixas de 1 m². Nesse período, foram retiradas amostras periódicas para avaliação dos componentes morfológicos e descrição do hábito das espécies. A alocação da biomassa em raízes, caule e folha foi expressa em percentagem de massa seca. A maior alocação em folhas ocorreu nas espécies estoloníferas, A. latifolia (63,53%) e A. punctata (61,42%), que, por sua vez, mostraram a menor alocação em raízes, 10,90 a 14,77 %, respectivamente. Maior alocação de biomassa em caule (39,38%) e raízes (24,39%) foi verificada em L. uliginosus, espécie rizomatosa. Para todas as espécies, o índice de área foliar (IAF) esteve correlacionado positivamente com número de folhas das hastes secundárias e biomassa aérea. A. latifolia e Lotus spp. são exemplos extremos da relativa importância dos componentes morfológicos na formação do IAF e da biomassa. O padrão de acúmulo e alocação de biomassa nas leguminosas estoloníferas é caracterizado, principalmente, pela produção de folhas, indicando a importância do alongamento e enraizamento dos estolões, enquanto em A. tristis e Lotus spp., as frações caule e raízes apresentam igual importância.
