1. Outcomes and Predictors of Rapid Antiretroviral Therapy Initiation for People With Newly Diagnosed HIV in an Integrated Health Care System.

Abstract

Background Rapid antiretroviral therapy (ART) is the recommended treatment strategy for patients newly diagnosed with HIV, but the literature supporting this strategy has focused on short-term outcomes. We examined both long-term outcomes and predictors of rapid ART among patients newly diagnosed with HIV within an integrated health care system in Northern California. Methods This observational cohort study included adults newly diagnosed with HIV between January 2015 and December 2020 at Kaiser Permanente Northern California. Rapid ART was defined as ART initiation within 7 days of HIV diagnosis. We collected demographic and clinical data to determine short-term and long-term outcomes, including viral suppression, care retention, medication adherence, and cumulative viral burden. Logistic regression models were used to identify predictors of rapid ART initiation. Results We enrolled 1409 adults; 34.1% initiated rapid ART. The rapid ART group achieved viral suppression faster (48 vs 77 days; P <.001) and experienced lower cumulative viral burden (log10 viremia copy-years, 3.63 vs 3.82; P <.01) but had slightly reduced medication adherence (74.8% vs 75.2%; P <.01). There was no improvement in long-term viral suppression and care retention in the rapid group during follow-up. Patients were more likely to initiate rapid ART after 2017 and were less likely if they required an interpreter. Conclusions Patients who received rapid ART had an improved cumulative HIV burden but no long-term improvement in care retention and viral suppression. 4. Effects of femtocell deployment on interference to macrocell users in a cellular network.

Abstract

The infrastructure of cellular network has evolved over several decades and has penetrated even the remotest part of the earth. However, as bandwidth used for next generation cellular network keeps moving to higher frequency spectrum, weaker signals for indoor cellular users has attracted additional attention. Femtocell technology has become an emerging solution and has recently been extensively studied for indoor cellular signal coverage. But, it is important to categorically analyze pros and cons of incorporating new techniques in order to judge their compatibility with existing wireless networks. The existing cellular network primarily comprises of two elements; macro base stations (M-BSs) and macro user equipments (M-UEs). In this paper, we study the effect of large scale femtocell deployment on M — UEs. Femtocells devices are designed to be deployed by individual end user, thus distribution pattern of femtocell network is not controllable. However, due to the fact that cellular signal strength decreases as the distance to the M — BS increases, it is envisioned that more femtocell devices would be installed at the edge of macrocell than the center area. Before formation of large scale femtocell, it is meaningful to simulate this deployment scenario and discover the impacts on M-UEs. We utilize a Reverse Gaussian distribution to simulate the deployment of femtocells. Then, we provide a mathematical formulation of the interference caused by femtocells on M-UEs. Extensive simulations are also performed to indicate that the results of our mathematical analysis provide an excellent guideline for interference management in future large scale femtocell deployment. Abstract

This study looks at the post-purchase evaluation stage of consumers and what causes loyalty. The industry under investigation is Small Medium Sized Jewellery Retailers. The purpose of this study is to develop a deeper understanding of how customer loyalty is developed. To reach this aim the study focuses on the impact of loyalty schemes on repeat patronage. This paper aims to contribute to the existing literature on repeat patronage and factors that lead to customer loyalty. A wide body of existing literature focuses on large super markets or airlines that have loyalty schemes like plastic cards (to get discounts on purchases) by which customer data with the use of CRM tools to meet specific needs and wants. The aim of this paper is to attempt to understand how loyalty schemes in this industry differ in terms of its implementation and usage, and how loyally is developed. To address this purpose two research questions were approached to each the consumers and retailers. What extent do loyalty schemes build repeat patronage? What other factors contribute to repeat patronage? In order to reach our purpose semi-structured interviews were used, which comprised of interviews with both consumers and retailers. The conclusions in this study presented that loyalty schemes did not impact repeat patronage and were merely a differentiation technique of firms. Factors such as customer service, customization, and communication trust commitment were factors that had big contributions to repeat patronage. These efforts were built to create RM and in turn to foster trust and commitment to a store and in turn create customer loyalty. The main objectives of these activities are customer retention.

9. Interference Analysis and Mitigation in a Cellular Network with Femtocells

Abstract

Development of femtocells is the most recent step towards increasing the network capacity of a wireless network and improving the quality of service for cellular users. However, large scale femtocell deployment can significantly affect existing wireless network. We examine the effect of interference due to closed-access femtocell deployment in a wireless network on macro users. Numerical analysis and simulation are performed in order to understand the impact of femtocells in an existing wireless network. Another interference scenario arising in such networks is the interference amongst neighboring femtocells. This is due to the fact that all the femtocells use the same radio band, and thus it poses a serious interference issue amongst neighboring femtocells, especially when femtocells are densely deployed. We study the problem of downlink and uplink interference used in conjunction with the LTE technology and propose a solution to mitigate this interference using Fractional Frequency Reuse (FFR) technique. User equipments (UEs) report to their serving femto base stations (F-BS) periodically. If the report reveals high interference level, neighboring F-BSs locally adopt FFR in a coordinated but decentralized way. FFR divides the femtocell in cell-center and cell-edge areas where the cell-edge of interfering F-BSs use distinct set of sub carriers. The division takes place in a way such that the affected UE falls in the cell-edge area. The simulation results indicate that the proposed scheme decreases interference in the neighboring F-BSs, femto UEs (F-UEs) and macro UEs (M-UEs) and improves the overall throughput.
