Abstract

After outlining some general features of Hindu sacred places and their priesthoods, the present paper focus on the regional town of Pandharpur in Maharashtra, its Viṭhobā temple and priesthood. After a brief historical introduction to the town and its deity, and its special relation with the bhakti Vārakārī Sampradāya, a description of the scheme of daily temple rituals and darśana is given, followed by presentations of the two priesthoods of the Viṭhobā temple, the Baḍavas and Sevādhārīs, their organisation and income which seems to have been increasingly limited to the performance of the temple rituals. This leads naturally to the final chapter which gives an overview of the many conflicts and litigations around the management of the temple which have resulted in a long list of litigations in court both among the different priesthoods and among the priesthood and the government. After independence and the introduction of the modern state, the position of the local priesthood has been increasingly undermined by modern legislation which have, finally, led to the priesthood losing their inherited rights and privileges, as well as the management of the temple.

28. Gender Equality in Asia-Pacific: Reflections on the Beijing+25 Review

Abstract

This article reviews the 25-year progress made in implementing the ‘Beijing Platform for Action’ and the challenges that remain towards achieving gender equality in the Asia-Pacific region. Adopted in 1995 at the Fourth World Conference on Women, the ‘Beijing Platform for Action’ has been hailed as the most progressive policy blueprint for gender equality and women’s empowerment. In November 2019, over 600 participants from 54 countries, comprising representatives from Governments, international organisations and civil society organisations attended the Asia-Pacific Ministerial Conference on the Beijing+25 Review in Bangkok. The delegations reviewed the “achievements, challenges and priority areas for realizing gender equality and women’s empowerment” (UNESCAP, 2019a:1) in Asia-Pacific. For 25 years, since the adoption of the ‘Beijing Platform for Action’, Asia-Pacific has witnessed significant progress in girls’ education and women’s health. Unprecedented progress has been made in and reducing maternal deaths and enhancing women’s representation in national parliaments and local governments in several countries. However, there are some enduring challenges, including women’s economic empowerment and political participation, and violence against women. Whilst women play a pivotal role in protecting the environment and natural resources, they have been underrepresented in environment-related decision making and negotiations. Accordingly, the key actions outlined by the ‘Asia-Pacific Declaration on Advancing Gender Equality and Women’s Empowerment: Beijing+25 Review’ cover a wide range of issues, from women’s economic empowerment, political participation, to women’s full and effective participation in environment conservation, climate action and peace building process.

Abstract

Background The increasing burden of kidney failure (KF) in India necessitates provision of cost-effective kidney replacement therapy (KRT). We assessed the comparative cost-effectiveness of initiating KRT with peritoneal dialysis (PD) or haemodialysis (HD) in the Indian context. Methods The cost and clinical effectiveness of starting KRT with either PD or HD were measured in terms of life years (LYs) and quality-adjusted life years (QALYs) using a mathematical Markov model. Complications such as peritonitis, vascular access–related complications and blood-borne infections were considered. Health system costs, out-of-pocket expenditures borne by patients and indirect costs were included. Two scenarios were considered: Scenario 1 (real-world scenario)—as per the current cost and utilization patterns; Scenario 2 (public programme scenario)—use in the public sector as per Pradhan Mantri National Dialysis Programme (PMNDP) guidelines. The lifetime costs and health outcomes among KF patients were assessed. Results The mean QALYs lived per KF person with PD and HD were estimated to be 3.3 and 1.6, respectively. From a societal perspective, a PD-first policy is cost-saving as compared with an HD-first policy in both Scenarios 1 and 2. If only the costs directly attributable to patient care (direct costs) are considered, the PD-first treatment policy is estimated to be cost-effective only if the price of PD consumables can be brought down to INR70/U. Conclusions PD as initial treatment is a cost-saving option for management of KF in India as compared with HD first. The government should negotiate the price of PD consumables under the PMNDP.
