Skeletal muscle is an important organ in our body and a dynamic composite of proteins. Citrus aurantium L. has been widely used in oriental medicine in Eastern Asia for a long time. It contains over 100 bioactive compounds and flavonoids that regulate the inflammatory response and tumorigenesis, through various mechanisms. In the present study, we investigated changes in the protein pattern using two-dimensional electrophoresis (2-DE) and matrix assisted laser desorption ionization time of flight mass spectroscopy (MALDI-TOF/MS) to assess the anti-inflammatory effect of flavonoids isolated from Korean C. aurantium L. in lipopolysaccharide (LPS)-induced L6 cells. L6 skeletal muscle cells were pretreated with flavonoids for 1 h and stimulated with LPS for 24 h. Proteins from the L6 cells of the control, LPS treated and flavonoid treated groups were extracted and resolved by 2-DE using pH 4–7 IPG strips loaded with 150 μg of protein. Forty-one differentially expressed protein spots were identified (more than two-fold was considered significant, p < 0.05), and 18 were detected by MALDI-TOF/MS. These results suggest that proteomics can be used to identify changes in the expression of marker proteins and the anti-inflammatory effect of flavonoids isolated from Korean C. aurantium L.

Citrus fruits are an abundant source of various flavonoids, which have been used as a traditional herbal medicine in Korea and China. Most flavonoids are known to have anti-oxidant, anti-bacterial and analgesic properties. In this study, it was examined whether flavonoids (nobiletin, naringin and hesperidin) isolated from Korea Citrus aurantium L. inhibited the pro-inflammatory mediators including cytokines by blocking nuclear factor-kappa B (NF-κB) and mitogen-activated protein kinase (MAPK) signalling in lipopolysaccharide (LPS)-stimulated RAW 264.7 macrophages. The flavonoids suppressed mRNA and protein expression of cyclooxygenase-2 (COX-2) and inducible nitric oxide synthase (iNOS) in LPS-induced macrophages. The molecular mechanism was associated with inhibition of the degradation/phosphorylation of I-κB-α and nuclear translocation of the NF-κB p-65 as well as phosphorylation of MAPK by flavonoids. These results suggest that flavonoids have anti-inflammatory effects by suppressing expression of COX-2, iNOS and cytokines by blocking the NF-κB and MAPK signalling in RAW 264.7 macrophages.

Aim of the study Scutellaria baicalensis Georgi is a widely used medicinal herb in several Asian countries including Korea. The various medicinal properties attributed to Scutellaria baicalensis include anti-bacterial, anti-viral, anti-inflammatory and anti-cancer effects. The present study investigated the cytotoxicity of Scutellaria baicalensis water extract (SBWE) on A549 non-small-cell-lung cancer cells and the A549 expression of cyclin D1, cyclin-dependent kinase 4 (CDK4) and matrix metalloproteinase-2 (MMP-2), and the effects of SBWE on cell cycle progression, especially the G1/S phase, and on cell motility. Materials and methods SBWE cytotoxicity was assessed by a standard colorimetric assay utilizing 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) and expression of cyclin D1 and CDK4 protein in SBWE-treated A549 cells was assessed by Western blot analysis. Flow cytometry analysis was performed to determine the effect of SBWE on A549 cell cycle progression. A549 cell MMP-2 activity was examined by zymography. Cell motility and migration was assessed by a scratch wound healing assay. Results SBWE was not cytotoxic. The production of Cyclin D1, CDK4 and MMP-2 activity were significantly decreased in a SBWE dose-dependent manner, with maximum inhibition occurring at SBWE concentrations of 250 μg/ml and 500 μg/ml. SBWE inhibited cell cycle progression in the G1/S phase and significantly inhibited the motility of A549 cells. Conclusions Cyclin D1 protein may be associated with MMP-2 activity and cell motility. Thus, SBWE promotes a strong protective effect against MMP-2 mediated metastasis and cell proliferation through the down-regulation of cyclin D1. SBWE may be a useful chemotherapeutic agent for lung cancer.

Thirty male pigs were infected orally with E. coli and Salmonella typhimurium, and divided into a control group and two additive groups to determine the effect of an additive mixture on the changes in protein expression. The pigs were given a food supplemented with a natural herbal additive containing immunoglobulin yolksac (IgY) at concentrations of 0.5% or 1%. On the 1st day and after eight weeks of feeding, the body weight gain, food intake and serum GOT/GPT levels were examined. The GOT/GPT levels on the 1st day were similar in the three groups. However, after eight weeks of feeding, the GOT level was significantly lower in the additive treatment groups (0.5% and 1.0%). In addition, the changes in the spleen proteome as a response to the herbal additive were examined using two-dimensional polyacrylamide gel electrophoresis. A total of 31 differentially expressed protein spots were identified by comparing the protein profiles of the control and additive treated porcine spleens. Finally, 19 proteins were detected by MALDI-TOF/MS. Overall, the proteins detected are involved in a range of biological process, such as metabolic processes, biological processes, transport, carbohydrate metabolic processes, generation of precursors and energy. In conclusion, these results support of the hypothesis that a natural herbal additive containing IgY can affect the immune regulation system and reduce the stress of microbial infections.

Polychlorinated biphenyls (PCBs) are environmental pollutants that are quite toxic to biological systems. This study examined the inhibitory effect of PCB126 and PCB114 on testicular steroidogenesis in male rats. Male Sprague Dawley rats received weekly intraperitoneal injections of PCB126 (0.2 mg/kg) or PCB114 (20 mg/kg) or vehicle (corn oil). Animals from each group were sacrificed at 2, 5 and 8 weeks after the injections. Blood and testis tissue samples were collected for the hormone assay, Western blotting and reverse transcriptase polymerase chain reaction (RT-PCR). The testosterone, luteinizing hormone (LH) and follicle-stimulating hormone (FSH) levels were assayed, and the expression levels of the mRNA and proteins associated with the testosterone biosynthesis pathway were measured to determine the effect of PCB126 and PCB114 on testicular steroidogenesis. The results showed that the testis weight was significantly higher in the PCB126-treated rats given eight shots. Moreover, the serum testosterone levels were significantly lower in the PCB126 and PCB114-treated groups than the control. The transcription and translation levels of P45017α and P450scc were significantly lower in the PCB126-treated groups than the control. These results suggest that PCB126 may affect testicular steroidogenesis by downregulating P45017α, P450 scc and have inhibitory effect on the testicular functions.

Polychlorinated biphenyls are environmental pollutants that are toxic to many biological systems. This study examined whether or not PCB126 and PCB114 have adverse effects on the serum thyroxine level and the serum proteome in rats. The results showed a lower serum total thyroxine level in the PCB126 and PCB114-treated groups than the control. Western blotting showed that the levels of transthyretin expression were significantly higher in the PCB-treated group than the control group. These results suggest that the PCB-mediated hypothyroidism is caused by the displacement of thyroxine from transthyretin.

Polychlorinated biphenyls (PCBs) are synthetic organic compounds with two phenyl groups well known environmental pollutants. This study examined the effect of persistent exposure to 2,3 � ,4,4 � ,5-pentachlorobiphenyl (PCB118) on serum cholesterol levels in male rats. Male Sprague Dawley rats were administered weekly intraperitoneal injections of either PCB118 (20 mg/kg) dissolved in corn oil or corn oil alone. One week after 2 and 5 administrations, the rats were sacrificed by a pentobarbital injection, and the effect of PCB118 on the serum levels of total cholesterol, high density lipoprotein-cholesterol (HDL-cholesterol), and low density lipoprotein-cholesterol (LDL-cholesterol) was investigated. The protein expression level of apolipoprotein A-I (apo A-I) and 3-hydroxy3-methylgulutaryl coenzyme A (HMG-CoA) reductase was also examined. In this study, the administration of PCB118 induced hyperalphacholesterolemia in rats. In addition, the protein expression level of HMG-CoA reductase and apo A-I was higher in the PCB118-treated rats than in the control. These results suggest that the hyperalphacholesterolemia induced by PCB118 in male rats may be associated with an increase in HMG-CoA reductase and apo A-I expression.

Salmonellosis is a major bacterial zoonosis that causes a variety of disease syndromes, from self-limiting enteritis to fatal infection in animals and food-borne infection and typhoid fever in humans. Recently, the emergence of multidrug-resistant strains of Salmonella sp. has caused more serious problems in public health. The present study investigated the antibacterial effects of Houttuynia cordata water extract (HCWE) against murine salmonellosis. In RAW 264.7 cells, there was no detectable cytotoxic effect of HCWE at any concentration between 25 and 100 microg/ml after 8-h incubation. The antibacterial activity of HCWE was then examined in a Salmonella enterica serovar (Salmonella typhimurium), and was found to increase in a dose-dependent manner at concentrations from 25 to 100 microg/ml during 8-h incubation. HCWE also affected RAW 264.7 cells including morphologic change and bacterial uptake, but there was no significant difference in bacterial replication in RAW 264.7 cells. With HCWE alone, nitric oxide (NO) production by RAW 264.7 cells did not increase, but when RAW 264.7 cells were infected by S. typhimurium, with or without HCWE, NO production with HCWE was 2-fold higher than that without HCWE. Treatment with HCWE did not affect inducible NO synthase (iNOS) mRNA expression by RAW 264.7 cells, but when RAW 264.7 cells with HCWE were infected by S. typhimurium, iNOS mRNA expression was increased during 8-h incubation. Furthermore, HCWE showed virulence reduction effects in S. typhimurium-infected BALB/c mice. After a lethal dose of S. typhimurium, the mortality rate in the HCWE untreated group was 100% at 7 d, but the HCWE 25, 50, and 100 microg/ml groups survived until 11, 17, and 23 d, respectively. These data suggest that HCWE is stable and beneficial in the treatment of bacterial infection including intracellularly replicating pathogens and may solve antimicrobial misuse and overuse.

We report retrospective and prospective studies to identify the causes of fracture of the femoral neck associated with femoral shaft nailing on the same side. Of a total of 14 neck fractures in a series of 152 shaft nailings, eight were not visible on the initial pelvic radiographs. We used CT scans before and after operation, and fluoroscopy during the procedure in our prospective series, and reviewed abdominal CT scans retrospectively with the window set to bone level. Six of the eight undisplaced fractures were shown to have been present before operation, but two were iatrogenic. We recommend the preoperative use of CT scans of the femoral neck in high-risk patients such as those with associated fractures of the acetabulum, the distal femur or the patella. Early diagnosis will allow better general management and early fixation of the neck fracture.

Pressureless sintered SiC specimens were joined using MgO-Al2O3-SiO2 (MAS) filler. MAS filler showed excellent behaviour of wetting on SiC substrate above 1480 °C, and the wettability was much influenced by the joining atmosphere. The joining was carried out at 1500 and 1600 °C for 30 min in Ar atmosphere. The flexural strength of the joined specimen showed 342–380 MPa up to 800 °C. However, the flexural strength of the joined specimen decreased to about 80 MPa at 900 °C due to softening of the joint interlayer. The results of the XRD and WDS showed that the reaction between SiC and the MAS filler produced the oxycarbide glass.

Signal-intensity characteristics of magnetic resonance (MR) images were assessed in five patients and in 10 rabbits with tuberculous arthritis. MR imaging findings were compared with histologic findings in the animal study. In both of clinical and experimental cases, tuberculous lesions showed both intermediate and high signal intensity on T2-weighted images, while they showed heterogeneously intermediate signal intensity on T1-weighted images. As T2-weighted images were compared with pathologic specimens, intermediate signal intensity corresponded to caseous necrosis, whereas high signal intensity related to granulomas or effusion. Postcontrast T1-weighted images showed enhancement at the peripheries rather than the centers of tuberculous lesions. These results indicate that tuberculous arthritis should be included in the differential diagnosis when intra-articular lesions with low or intermediate signal intensity are found on T2-weighted images.

Citrus fruits have been used as an edible fruit and a traditional medicine since ancient times. In particular, the peels of immature citrus fruits are used widely in traditional herbal medicine in Korea, as they are believed to contain bioactive components exerting anti-inflammatory activity. This study examined whether the crude methanol extract ofCitrus aurantium L.(CME) has a suppressive effect on inducible enzymes and proinflammatory cytokines by inhibiting the NF-κB pathway in LPS-stimulated macrophage RAW 264.7 cells. The cells were pretreated with the indicated concentrations of CME (5, 10, 20, and 50 μg/mL) and then treated with LPS (1 μg/mL). The results showed that CME (10, 20, and 50 μg/mL) inhibited the LPS- (1 μg/mL) induced mRNA and protein expression of iNOS in macrophage Raw 264.7 cells. In addition, the expression of COX-2 was inhibited at the mRNA and protein levels by CME in a dose-dependent manner. The mRNA expression of proinflammatory cytokines, such as TNF-αand IL-6, were markedly reduced by CME (10, 20, and 50 μg/mL). Moreover, CME clearly suppressed the nuclear translocation of the NF-κB p65 subunits, which was correlated with its inhibitory effect on I-κB phosphorylation. These results suggest that CME has anti-inflammatory properties by modulating the expression of COX-2, iNOS, and proinflammatory cytokines, such as TNF-αand IL-6, in macrophage RAW 264.7 cells via the NF-κB pathway.

The authors report a case of acetabular fracture of both columns that was associated with a rare nerve injury, entrapment of the obturator nerve in the hip joint.

Removal of the proximal interlocking screw (PIS), especially one that is deeply inserted in the greater trochanter or covered by heterotopic ossification, is troublesome and requires considerable effort because the search for its head usually requires removing bone from around the insertion site. We introduce a simple tip to remove this complicated PIS with the aid of a proximal drill guide, drill bit, and AO countersink.

We evaluated 200 patients who had a positive McMur ray test and found atypical McMurray test results in 24 patients (12%). These patients revealed pain or click ing or both either in the medial compartment of the knee when the leg was internally rotated or in the lateral compartment of the knee when the leg was externally rotated. The authors analyzed these para doxical findings at arthroscopic examination to identify the relationship between the type of meniscal tear and the direction of leg rotation that elicited the catching and displacement of the torn meniscal portion during the McMurray test. Contrary to conventional McMurray test findings, three different types of meniscal tears were found on the side of the knee where pain or a clicking sound occurred. The three types were 1) an teriorly based posterior oblique tears with anterior dis placement of the meniscus, 2) bucket-handle tears in the posterior half of the menisci, and 3) peripheral detachment of discoid menisci in the posterior half of the torn portions.

A corticocancellous core was removed from the lateral condyle of both femurs in 26 skeletally mature dogs. The cavity was treated with cryosurgery, phenol cautery or packing with bone cement. The animals were killed after 1, 4, and 12 weeks in the phenol and cement groups, and also after 24 weeks in the cryosurgery group. The extent of the bone necrosis and healing was assessed in each group. After cryosurgery the extent of necrosis was profound in that the area of damage was 365% compared to the area of the cavity; the depth of necrosis extended between 2.5 and 14 mm, beyond the cavity wall. The effect of phenol was negligible in that only microscopic areas of superficial focal necrosis were found around the cavity wall. Bone cement produced an area of necrosis of 153% compared with the cavity, with a depth of between 1.3 and 2.8 mm. Regeneration in the region of necrosis after cryosurgery was only scanty by 4 weeks, but by 12 weeks considerable areas of regeneration were identified and complete healing was observed by 24 weeks. Regeneration of the necrotized bone produced by bone cement packing was rapid and similar to that of the control specimens. These findings suggest that cryosurgery could play a significant role as a surgical adjunct to curettage in locally aggressive benign bone tumours and in some malignancies. Phenol cautery is not regarded as an adequate treatment after curettage of bone tumours. Although the extent of necrosis was relatively small, packing with bone cement is thought to be a useful choice in benign cases.

In repeat lumbar surgery for failed back syndrome, well organized fibrous scar tissue is often noted, binding together the dura, nerve roots, and paraspinal muscles. An animal experimental study was done to investigate the prevention of scar formation after lumbar laminectomy by using dacron and sodium hyaluronate. The experimental animals consisted of three groups: 1) control group, 2) D group (covering the laminectomy defect with dacron sheet), and 3) H group (covering the laminectomy defect with sodium hyaluronate gel). Animals were sacrificed at varying intervals (3-12 weeks) and the lumbar spines were evaluated with histologic preparations. Scar adhesion to the dura was most significantly suppressed in the D group, followed by the H group and the control group.

Purpose : The purpose of this study was to evaluate the radiological results of quadriceps-sparing total knee arthroplasty. Materials and Methods : 50 consecutive total knee arthroplasties with a quadriceps-sparing surgical technique and 50 consecutive total knee arthroplasties with conventional surgical techniques were performed. The same surgeon performed all the operations with the same type of prosthesis (Nexgen LPS-flex Total Knee System). We compared the differences between these groups in terms of preoperative and postoperative femorotibial angles, tibial component alignment angles, tibial component inclinations, and ratios of size of the tibia component to tibial cutting size. Results : There were no differences in preoperative femorotibal angles, tibial component alignment angles, tibial component inclinations, ratios of sizes of tibia tray to tibial cutting size, and blood loss between the two groups (p＞0.05). The average ranges of motion and HSS scores at 8 weeks postoperatively in the quadriceps-sparing total knee arthroplasty group were greater than those in the conventional surgical technique (p＜0.05). The mean postoperative femorotibial angle in the quadriceps-sparing total knee arthroplasty group was a valgus of 6.9±1.3˚ compared with a valgus of 7.7±1.4˚ in the standard arthroplasty group (p＜0.05). Conclusion : A quadriceps-sparing total knee arthroplasty offers predictable results in terms of radiological comparison with the conventional arthroplasty technique. But this technique has the tendency of producing a decreased valgus angle at the distal femur.

Purpose: To obtain spatial information on the acetabular fossa concerning ‘transacetabular screw’ insertion through the acetabular fossa parallel to the quadrilateral surface for fixing an acetabular fracture. Materials and Methods: Each hemipelvis harvested from 25 adult fresh frozen cadavers was sectioned into eight segments. This resulted in seven measuring points located along the outer margin of the acetabular fossa. At these points, we measured and compared the vertical thicknesses of cartilage and bone from the quadrilateral surface. Results: The average thickness of the thinnest portion of the acetabular fossa floor was 3.9±1.9 ㎜. The entire thickness of the bone and cartilage measured vertically from the quadrilateral surface at the 7 points ranged from 12.7±2.1 ㎜ to 17.8±3.8 ㎜. The thinnest safe space of exposed threads of the ‘transacetabular screw’ inserted through the acetabular fossa was consistently just below the posterior conus of the lunate surface, measuring 4.8±1.6 mm (p Conclusion: We suggest that ‘transacetabular screws’ can be inserted safely if due consideration is paid to the spatial characteristics of on the acetabular fossa. Screw insertion into the postero-inferior portion of the acetbular fossa should be done carefully.
