Abstract

PURPOSE: There is an unmet need to identify women diagnosed with ductal carcinoma in situ (DCIS) with a low risk of in-breast recurrence (IBR) after breast conserving surgery (BCS), which could omit radiation therapy (RT), and also to identify those with elevated IBR risk remaining after BCS plus RT. We evaluated a novel biosignature for a residual risk subtype (RRt) to help identify patients with elevated IBR risk after BCS plus RT. METHODS AND MATERIALS: Women with DCIS treated with BCS with or without RT at centers in the US, Australia, and Sweden (n = 926) were evaluated. Patients were classified into 3 biosignature risk groups using the decision score (DS) and the RRt category: (1) Low Risk (DS ≤2.8 without RRt), (2) Elevated Risk (DS >2.8 without RRt), and (3) Residual Risk (DS >2.8 with RRt). Total and invasive IBR rates were assessed by risk group and treatment. RESULTS: In patients at low risk, there was no significant difference in IBR rates with or without RT (total, P = .8; invasive IBR, P = .7), and there were low overall 10-year rates (total, 5.1%; invasive, 2.7%). In patients with elevated risk, IBR rates were decreased with RT (total: hazard ratio [HR], 0.25; P < .001; invasive: HR, 0.28; P = .005); 10-year rates were 20.6% versus 4.9% (total) and 10.9% versus 3.1% (invasive). In patients with residual risk, although IBR rates decreased with RT after BCS (total: HR, 0.21; P < .001; invasive: HR, 0.29; P = .028), IBR rates remained significantly higher after RT compared with patients with elevated risk (HR, 2.5; 95% CI, 1.2-5.4; P = .018), with 10-year rates of 42.1% versus 14.7% (total) and 18.3% versus 6.5% (invasive). CONCLUSIONS: The novel biosignature identified patients with 3 distinct risk profiles: Low Risk patients with a low recurrence risk with or without adjuvant RT, Elevated Risk patients with excellent outcomes after BCS plus RT, and Residual Risk patients with an elevated recurrence risk remaining after BCS plus RT, warranting potential

Abstract

Many biological and physical objects derive their unique properties through an integrated multilength scale organization of their constituent nano and microscale structures. Such multiscaled structures are being exploited to engineer devices such as adhesives mimicking spatulae of a gecko, porous silicon drug delivery systems, to adaptive porous materials that mimick the multifunctionality of bone. A common feature in all these structures is that nanoscale units are all integrated into micron to macro scale structures and are accessible as individual modules for rapid response. Abstract

The activity of five 4-quinolone agents: norfloxacin, ciprofloxacin, ofloxacin, enoxacin and pefloxacin was tested against 121 distinct clinical bacterial isolates. Ciprofloxacin was the most active agent against Gram-negative bacteria and streptococci and pefloxacin was the most active against staphylococci. By serial sub-culture in sub-inhibitory concentrations of antibiotic we were able to select resistance in almost all Gram-positive and Gram-negative species, with the exception of Escherichia coli. Resistance to norfloxacin was more readily produced than resistance to the other four agents. There was almost complete cross-resistance among the five agents tested. This article describes the initiation of the Children's Hospice and Palliative Care Coalition, an organization that emerged from the concern of two women for the unique needs of children with life-threatening conditions. The Coalition, based in California, was inspired to improve care for children with life-threatening conditions and their families by the 2002 Institute of Medicine report When Children Die:... (which was printed in 2003: ). Its most recent work has been to advance legislative initiatives to develop, implement, and evaluate a pilot project waiver to provide a federally funded pediatric care benefit. 38. Inhibition of Aluminum Oxyhydroxide Precipitation with Citric Acid

Abstract

Citric acid has been shown to act as an agent for increasing the solubility of aluminum oxyhydroxides in aqueous solutions of high (>2.47 mol/mol) hydroxide-to-aluminum ratios. Conversely, citric acid also colloidally stabilizes particles in aqueous suspensions of aluminum-containing particles. Solutions of aluminum chloride, with and without citric acid added, were titrated with NaOH(aq). The presence and size of particles were determined using quasi-elastic light scattering. In solutions that contained no citric acid, particles formed instantaneously when NaOH(aq) was added but these were observed to rapidly diminish in size, disappearing at OH/Al ratios below 2.5 mol/mol. When the OH/Al ratio was raised beyond 2.5 by adding more NaOH(aq), suspensions of colloidally stable particles formed. Large polycations containing 13 aluminum atoms were detected by 27Al solution NMR in citric-acid-free solutions with OH/Al ratios slightly lower than 2.5. In comparison, adding citric acid to solutions of aluminum chloride inhibited the formation of large aluminum-containing polycations. The absence of the polycations prevents or retards the subsequent formation of particles, indicating that the polycations, when present, act as seeds to the formation of new particles. Particles did not form in solutions with a citric acid/aluminum ratio of 0.8 until sufficient NaOH(aq) was added to raise the OH/Al ratio to 3.29. By comparison, lower amounts of citric acid did not prevent particles from forming but did retard the rate of growth.

40. Silica Gels with Tunable Nanopores through Templating of the L<INF>3</INF> Phase

Abstract

We describe a detailed synthesis of a silicified inorganic/organic nanoporous monolithic composite conforming to the lyotropic liquid crystalline L3 phase. The pore dimensions of the silicified L3 phase scale with the solvent volume fraction in the synthesis reaction mixture. Changing the solvent fraction in the initial solution changes the ultimate pore diameter in the silicate, providing a simple method for tuning the diameter of the pores in the matrix. The resulting monolith is optically isotropic and transparent with a nonperiodic network. Accessible pores (which permeate the entire structure) in the silicified materials correlate with the solvent domain of the original liquid crystalline phase and therefore negate the need to remove the surfactant in order to gain access to the pore network. Measured characteristic dimensions are from 6 to well over 35 nm. X-ray scattering studies indicate a low polydispersity in the pore diameters for a given solvent fraction. Transmission electron and atomic force microscope images are consistent with a random morphology and measured surface areas exceed 960 m2 g-1 in extracted materials.

Abstract

This interim report describes the results of research conducted under Grant No. AFOSR-87-0114, which deals primarily with the processing and characterization of complex ceramic matrix composite systems. Particular emphasis was placed on developing processing schemes for whisker-reinforced ceramic matrix composites. Additional studies are being conducted on boron carbide-aluminum ceramic/metal composites. Further, theoretical studies have been made to provide the foundation for developing a more fundamental understanding of colloidal systems.
